[Technical modification of transaxillary resection of the first rib in surgery of thoraco-brachial outlet syndromes].

From April 1985 to September 1991, 88 patients were operated for a total of 100 thoracobrachial outlet syndromes. The patients were placed in the dorsal supine position, turned 30 degrees towards the contralateral side. The arm was suspended in slight abduction. The access obtained is identical to that obtained with the classical operation. No direct traction or compression is exerted on the pedicles during retraction and the operation requires a single assistant. The composition, indications and results of our series are similar to those reported in the recent literature. We did not observed any immediate or late neurological complications. We conclude that thoracobrachial outlet syndromes can be treated via an axillary approach using mechanical traction and a single assistant without increasing the risk of neurological complications.

Inhibitory effect of new imidazole derivatives on the proliferation and nucleic acid synthesis of leukemic cells.

New derivatives of imidazothiazole and imidazobenzothiazole were tested in vitro for their potential antiproliferative activity. Four imidazobenzothiazole derivatives exhibited a cytotoxic activity against two leukemic cell lines, compound I being the most effective. Cell cycle kinetics studies showed that this drug delays the progression of cells from G1 to S and G2 M phases. An inhibitory effect on DNA and RNA synthesis was also observed. The antiproliferative effect of this compound, analogue of immunosuppressive agents, suggested that it could be of interest for a therapeutic use and for the synthesis of new derivatives.

[Hydroxyiminoacetone-like reactivators of phosphorylated cholinesterases. Structure-activity relationships via molecular modelling].

A structure-activity relationships (S.A.R.) is performed on a series of iminocetones acting like reactivators of phosphorylated cholinesterases. The structural and electronic molecular characteristics are computed using a molecular modelling system (Moldesign). A multidimensional statistical analysis point out the role of MEP's which are representative of the whole molecular electronic distribution.

[Dorsal extradural hourglass lipoma in children].

The authors would like to recount and discuss the case of a 14 months old child which presented a dumbbell dorsal lipoma, purely extradural with no congenital anomalies. The warning signal was the discovery of paraparesis with the regression of walking ability. The hypothesis of medullary compression by a dumbbell dorsal tumor was quickly dismissed with the discovery of a mediastinal mass on the pulmonary X-Ray. A diagnosis of the pre-operational neuroblastoma was carried out because of detection of dorsal epidural medullary compression revealed by the myelography. The testing was completed with a vertebral computed tomography, which clearly showed the extent of medullar compression, and the mediastinal tumoral development under the paravertebral muscular masses. The child was operated on at 2 fronts, vertebrally and thoraxically for an encapsulated, perfectly separable and non hemorrhaging tumor. The anatomopathological examination confirmed the diagnosis of the lipoma. It is in the resumption of the computed tomographic examination that the tumoral density measures will confirm fatty tissue with numbers of density--50. In light of the literature, we are discussing the appearance of the lipoma, entirely exceptional because of its dorsal location, its extradural and certainly its dumbbell position, with large thoraxic development. It is necessary to stress the importance of the meticulous neurological examination of the child, the diagnostical necessity of the CT scan and the importance of the avoidance of the creation of an instable vertebral lesion at the time of laminotomy. In this case only the early double surgical operation permitted the recovery of the child.

In vivo methylation of guanine by the organophosphorus insecticide tetrachlorvinphos.

The in vivo methylating capability of the organophosphorus insecticide tetrachlorvinphos, assayed by the formation of 7-methyl-guanine in mouse liver, was investigated. Following intraperitoneal injection of male mice with different doses of the 14C-insecticide, labelled at the OCH3 groups, the total and specific radioactivity of nucleic acids and protein were determined. The 14C-labelling in the isolated macromolecules reached its maximum 24 hours following administration of the insecticide. Analysis of the acid hydrolysate of DNA and of RNA on Dowex-50 WX-12 revealed the presence of (7-14C) methylguanine. At maximum 14C-labelling, the amount of radioactive 7-MeGu, calculated as fraction of total dose, was around 9 X 10(-5) and 39 X 10(-5) for DNA and RNA, respectively.

[Granular cell tumor of a cerebral hemisphere: value of gliofibrillary protein acid assay].

A patient with a left parieto-occipital tumor presented generalized convulsions, and aphasia with alexia and agraphia. Diagnosis was confirmed by CT scan. Partial excision only was possible and was followed by radiotherapy. Histology showed the tumor to be composed of moderate and large sized cells with a granular PAS+ cytoplasm. Protoplasmic astrocytes were numerous at the periphery. Electron microscopy demonstrated typical features of Abrikossof's granular cell tumor: heterogeneous granulations constituted mainly of secondary lysosomes, autophagic vacuoles, multivesicular bodies, and rare angular bodies. The granular cells were interspersed with astrocytes, the feet of which were implanted on the capillary basal membrane. Immunoperoxidases assay of gliofibrillary protein acid demonstrated high levels in the intercellular prolongations, considered here to be astrocytic, but weak concentrations in the granular cells. This suggests uptake by the latter cells of adjacent astrocytic gliofibrillary protein acid, especially as the action of an anti-albumin serum on the tumor demonstrated that the granular cells can take up extracellular proteins by a passive mechanism. These findings stand against an astrocytic origin of the tumor. Comparison with other granular cell tumors, particularly pituicytomas, provides evidence of morphological unity, but cerebral granular cell tumors are rich in astrocytes, whereas these are absent in pituicytomas. The oncogenic unity of these tumors has not been established.

[Peritoneal pseudocyst, a rare complication of ventriculoperitoneal shunt (case report) (author's transl)].

The authors report an observation of peritoneal pseudocyst, complication of a ventriculoperitoneal shunt in a six month infant hydrocephalus, with spinal bifida. The literature review confirms the rarity of that complication (25 observations), always preceded by incidents of the shunt function, be it mechanical or infection. The pathogeny remains mysterious. It is probably a peritoneal reaction to infectious problems or stranger body reject. In therapeutic, except the evacuation of the pseudocyst by puncture or drainage by laparotomy, you must change the shunt utilizing the ventriculo-atrial shunt. It is necessary to keep obsession of an often latent infection that compels an external drain unit suitable antibiotics.
