[Postoperative outcome of residual pyelocaliceal stones].

The purpose of this paper is to investigate the outcome of postoperative residual stones and their influence to the renal function. We followed up 65 patients (73 kidneys) with residual stones after conventional operation. The follow-up period was on an average 11 years and 9 months. 50 of 73 kidneys were infected before surgery. The most frequently cultured organisms was Proteus mirabilis. Urinary infection persisted in 34 of 50 kidneys after surgery. The residual stones smaller than 5 mm existed in 10 kidneys, of which 7 became sterile. The stones between 5 and 10 mm existed in 18 kidneys, of which 7 (38.9%) became sterile, but in 22 kidneys with stones larger than 10 mm, the infection was not eliminated but in 2 kidneys. Conversely, postoperative infection was never found in 23 kidneys being sterile before operation. Residual stones increased in size in 39 kidneys, did not change in 19 kidneys, diminished in 3 kidneys, fell to the ureter requiring surgery in 7 kidneys and the spontaneous passage of stones was observed in 4 kidneys. In comparing the stone increase group with that of stone stable, the former had a high frequency of stones larger than 5 mm, of postoperative urinary infection and of many stone forming factors. We studied the change of renal function only by IVP in 71 kidneys. Thirty-two kidneys with residual stones and infection showed moderate or severe deterioration of renal function in a large majority of cases. Conversely, in 24 of 34 kidneys without infection, the renal function was stable and only 6 kidneys showed moderate or severe deterioration of renal function.

[Doxorubicin intra-arterial chemotherapy combined with low-dose irradiation in bladder cancer].

Fifteen patients with non-disseminated bladder cancer (T1:2, T2:2, T3:8, T4:3) were preoperatively treated with intra-arterial doxorubicin chemotherapy in combination with low-dose irradiation. The originally scheduled operations had been as follows; total cystectomy in 11, segmental cystectomy in 2, and TUR- or SVR-Bt in 2. The total dose of doxorubicin ranged from 120 to 540 mg/body, with an average of 312.7 mg/body and that of irradiation was from 4 to 34 Gy with an average of 24.0 Gy. Clinically, in the early phase of the study, complete remission (CR) was revealed in 10 (66.7%), and partial remission (PR) in 2 (13.3%). The bladder was preserved in 9 of 11 (81.8%) cases in which total cystectomy had been recommended. Pathological effects according to the criteria presented by Shimosato, Oboshi and Baba, were as follows; grade IV in 6 cases (40.0%), grade III in 2 (13.3%), grade II in 4 (26.7%) and grade I or 0 in 3 (20.0%). Therefore, viable tumor cells were not seen in 8 of 15 (53.3%) cases. Vesical non-recurrent rate in the cases with bladder preservation was 56.2% at 48 months by simple mode and 44.6% by accumulated mode. Survival in 13 cases (T2-T4) by pathological effects was 75.0% at 48 months in the grade IV and III group (n = 8), and 40.0% at 42 months in the grade II or less group (n = 5). The results obtained from this study seem encouraging, especially in the group where the pathological effects were grade IV or III, and leads to the conclusion that doxorubicin intra-arterial chemotherapy, combined with low-dose irradiation, could be a first-choice treatment for locally invasive bladder cancer.

[Uretero-caliceal anastomosis].

We report our experience of ureterocalycostomy in two patients with good results. Some technical points and the indications of this plastic surgery are discussed. If ureteropelvic anastomosis is impossible, this method of surgery must be tried before performing urinary diversion, such as nephrostomy.

[Experimental study of UFT in prostatic carcinoma (DU-145) in nude mice].

We evaluated the anticancer effect of UFT, which is composed of 1-(2-tetrahydrofuryl)-5-fluorouracil (Tegafur) and uracil in a molar ratio of 1:4, for prostatic carcinoma (DU-145) in nude mice, and the effect of 5-fluorouracil (5-FU) in vitro. Tumor growth of DU-145 in nude mice was inhibited in the group administered UFT (20 mg/kg), but was not in the Tegafur (100 mg/kg) group in comparison with the control group. The concentration of 5-FU in the tumor reached higher levels in the group administered UFT in comparison with the Tegafur group. Also, DU-145 cells were inhibited by about 70% in 4 gamma concentration of 5-FU compared with the control. These data suggest the clinical usefulness of UFT for prostatic carcinoma.

[Asymptomatic renal cell carcinoma incidentally detected by abdominal CT].

We reported two cases of renal cell carcinoma incidentally detected by abdominal CT during the evaluation of another clinical problem. They had no relevant symptoms of renal cell carcinoma. Case 1: The tumor was 32 X 33 X 34 mm. Histopathological diagnosis was clear cell subtype, tubular type greater than alveolar type, grade 1, INF alpha, pT2b, pV0, pNX, pM0. Case 2: The tumor was 20 X 20 X 16 mm. Histopathological diagnosis was clear cell subtype, alveolar type, grade 1, INF alpha, pT1, pV0, pNX, pM0.

[A case of multilocular renal cyst showing neoplastic features].

A 45-year-old woman was hospitalized with complaint of upper right abdominal pain. By serial examination, it was difficult to distinguish renal cyst from renal cancer. But, because of the frequency of the attacks of upper abdominal pain, nephrectomy was undertaken. The tumor measured 5 X 5 X 3 cm, was distinct from normal renal tissue, and consisted of a small cyst. Histologically, we diagnosed a multilocular renal cyst, and its lining cells showed small atypia and adenomatous proliferation. We concluded that this multilocular renal cyst showed neoplastic features.

Prophylactic long-term treatment of bladder tumors with oral chemotherapy (tegafur).

A total of 107 patients with bladder tumor (from Ta to T2) was studied. Patients were treated with or without prophylactic intravesical chemotherapy using mitomycin C or doxorubicin hydrochloride (Adriamycin) after the excision of tumor. After the preliminary treatment, 55 patients were given tegafur 600 mg p.o. daily (study group), and the other 52 patients (control group) were treated without tegafur. Both groups were followed up with periodic cystoscopy every three months. Actuarial nonrecurrence rate of the two groups were examined statistically. The results revealed that tegafur was a good prophylaxis against recurrence of the bladder tumor, significant differences at P from less than 0.001 to 0.025 were noted at every observation period except at six months. A significant difference at P less than 0.005 was also recognized from an overall comparison between two nonrecurrence curves.

[Clinical experience with cefmetazole in the urological field].

The clinical effectiveness of Cefmetazole, a new cephamycin antibiotic, was studied in 13 cases of complicated urinary tract infection and in 3 cases of acute simple pyelonephritis. The drug was administrated, in 2 or 4 g doses twice a day, by intravenous drip infusion, for 5 to 10 days. The overall clinical effectiveness rate, based on the UTI efficacy evaluation, was 92% for complicated urinary tract infection and 100% for acute simple pyelonephritis. Among 18 strains isolated, 14 strains were eradicated and 4 strains were replaced. The bacteriological efficacy was 100%. No adverse reaction was noted. Cefmetazole was suggested to be a useful antibiotic for the treatment of urinary tract infections.

Eosinophilic ureteritis? Regional ureteritis with marked infiltration of eosinophils.

The case of a thirty-seven-year-old man with eosinophilic granuloma of the ureter is discussed. Preoperative x-ray studies disclosed distal ureteric regional stenosis and moderate hydronephrosis with small stone shadows. A segmental ureterectomy and vesicoureteral neostomy were performed. Pathologic examination of the removed specimen revealed markedly infiltrated eosinophils of the ureteric wall. The literature is reviewed and the causation of eosinophilic granuloma discussed.
