Spatial W-plasty full-thickness skin graft for neovaginal reconstruction.

Although neovaginal reconstruction has achieved satisfactory results, there is still room for refinement and improvement. Despite its popularity, McIndoe's inlay split-thickness skin graft method still has several significant drawbacks. Here a new approach to neovaginal reconstruction by using a full-thickness skin graft displayed in the fashion of spatial W-plasty is described. Our idea combines the advantages of a full-thickness skin graft and Z-plasty principle in clinical practice. Four patients who had been reconstructed with this method had satisfactory results with minimal secondary contracture and unilateral linear groin scar. Three of them have married and enjoy sexual life without difficulties. The fourth patient has not married but shows no contracture in follow-up examinations. Since the result is encouraging, this method deserves wider clinical application.

[A preliminary study on the red cell immune function in blood stasis syndrome in traditional Chinese medicine].

20 patients with blood stasis syndrome (BSS), 20 patients with non-blood stasis syndrome (NBSS), and 17 normal subjects were measured by tests of RBC-C3b receptor rosette and RBC immune complex rosette. The rates of RBC-C3b receptor rosette formation (RBC-C3b RR, %) in BSS group, NBSS group and normal subjects were 20.90 +/- 4.02, 12.88 +/- 3.29 and 16.74 +/- 4.13 (P less than 0.01) respectively. The rates of RBC immune complex rosette formation (RBC-IC R, %) in the three groups were respectively 8.28 +/- 3.68, 7.73 +/- 2.48 and 7.41 +/- 2.43 (P less than 0.05). RBC-C3b RR was correlated to RBC-IC R in the normal subjects (r = 0.695, P less than 0.001). In patients with the same disease, RBC-C3b RR between the two syndromes (BSS and NBSS) was also very significantly different (P less than 0.01). This study suggested that there are variations on red cell immune function in BSS patients, and also provided an evidence that different syndromes of traditional Chinese medicine exist in diseases.

Magnetic resonance imaging in hepatolenticular degeneration. Analysis of 9 cases.

Nine patients with biochemically proved Wilson's disease underwent magnetic resonance imaging (MRI) of the brain. Areas of abnormal signal, long T1 and long T2, caused by gliosis and edema were seen in the lenticula, thalami, caudatum, brain stem as well as in the dentate nuclei. The abnormalities were bilateral and symmetric. Asymmetric focal white matter lesions were noted in a few patients. Focal atrophies were seen in the head of caudatum. Symmetric cavitations were only seen in the lenticular nuclei. Acute progressive type (2 patients) was characterized by cavity formation, and chronic progressive type (7 patients) by gliosis, edema and focal atrophy.

Penile reconstruction for a victim of electrical injury with bilateral below-elbow amputations.

We report on a young man who had complete loss of the penis and bilateral below-elbow amputations following severe electrical injury. Reconstruction of the penis was completed by using a deliberately tailored dorsalis pedis flap to solve the problems encountered by a migrated skin tube. Construction of a neourethra with release of the contracture was done simultaneously and brought about successful results.

Chronobiology of Aging in Albizzia julibrissin: I. An Automated, Computerized System for Monitoring Leaflet Movement; The Rhythm in Constant Darkness.

We are using leaflet movements in Albizzia julibrissin as a model system for investigating the chronobiology of aging. To monitor leaflet movements during long dark periods with adequate temporal and spatial resolution, we designed an automated, computerized photoelectric monitoring system. Each of 12 leaflet pairs was positioned in an individual light-proof container, with one leaflet immobilized. The angle of the mobile leaflet was monitored by a photosensor array using a low intensity infrared beam. Leaflet position was determined by custom-developed software, using information on the shading patterns of the sensors. Data on leaflet angle as a function of time were collected and stored on a floppy disc and then printed in numerical and graphical form.Oscillations of young, middle-aged, and old leaflets persist during 7 d of darkness with a periodicity close to 24 h. Period length appears to be age-independent, but rhythmic wave form is age-dependent. The older the leaflet, the earlier and more completely it opens and the less completely it closes.

Periesophageal mediastinal fibromatosis.

We report two cases of aggressive fibromatosis of the mediastinum with esophageal involvement in adults. CT showed the posterior mediastinal mass involving the esophagus. In both cases, surgical resection was not feasible. Histopathologic examination showed a fibrous tissue. The patients died two years later.

Pyogenic spondylitis with Scheuermann's disease.

We present the case of a teenager suffering from dorsal and lumbar pain. Imaging modalities diagnosed a lumbar retro-marginal anterior herniation and thoracic spondylitis. Such association has been reported twice in the literature.
