Alport syndrome with neurofibromatosis type-I: a case report.

We report a 9-year-old boy with repeated fractures of the tibia from age 6 months and microscopic hematuria from age 2 years. His maternal family has a history of nephritis and his paternal family has neurofibromatosis type-I (NF-I). The boy's renal biopsy revealed an irregular attenuation and splitting of the glomerular basement membrane. The skin biopsy was stained with monoclonal antibody against the alpha 5 chain of type IV collagen; the epidermal basement membrane was negative in the boy and segmentally positive in the boy's mother. We conclude that the patient inherited Alport syndrome from his mother and NF-I from his father. We postulate this was a chance association and that this case does not suggest any relationship between the two diseases.

Labeling of amoeboid microglial cells and intraventricular macrophages in fetal rats following a maternal injection of a fluorescent dye.

Amoeboid microglial cells (AMC) in fetal brains were labeled by rhodamine B isothiocyanate (RhIc) when injected intravenously or intraperitoneally into mother rats at late state of pregnancy. The fluorescent cells were immunostained with antibodies OX-42 and OX-18 that recognize complement type 3 (CR3) receptors and major histocompatibility complex class I (MHC-I) surface antigen, respectively. RhIc-labeled AMC were first observed in the cavum septum pellucidum and subependymal cysts associated with the cerebral aqueduct as well as the fourth ventricle, and subsequently at other sites including the corpus callosum and other subcortical white matter. The fluorescence intensity increased with time after RhIc administration so that after 1 day the cells were brightly labeled. The majority of the labeled cells were round, with some elongated ones bearing two or three processes. Besides AMC, macrophages in the ventricular system were also labeled. All fluorescent cells were double labeled with OX-42 and OX-18 antibodies. Present results suggest that when introduced into the maternal circulation, RhIc could readily gain access into the fetal brain through the inefficient placental, blood-brain and blood-cerebrospinal-fluid (blood-CSF) barriers. The avid uptake of RhIc in circulation by brain macrophages indicates an active scavenging role of these cells in fetal brain. The labeling of cells by maternal route offers a rapid method for study of distribution of brain macrophages in fetuses.

Treatment of bilateral intrahepatic stones with high duct strictures through selective central hepatic resection.

Bilateral intrahepatic duct stones with strictures, more common in the Orient than elsewhere, are difficult to access surgically. The results of surgical procedures have not been generally successful and are attended by a high postoperative complication and recurrence rate. We report the treatment of 30 patients with intrahepatic calculi and high duct strictures by means of selective central hepatic resection without dissection of the major vessels at the hilum. Central hepatic resection provided satisfactory access to the primary and secondary confluences of the intrahepatic ducts, allowed removal of residual stones and ascariasis, and permitted correction of multiple strictures. Twenty-nine patients so treated were followed for a mean of 32 months after operation. No patients developed recurrent fever, biliary colic, or jaundice after the operation. The technique is therefore recommended as an effective alternative to extensive hepatic lobectomy in the treatment of the intrahepatic calculi with multiple strictures.

Immunoglobulin G subclasses in serum and circulating immune complexes in patients with Kawasaki syndrome.

IgG subclass concentrations in sera of 17 patients with Kawasaki syndrome were determined. Significantly increased IgG1 (P less than 0.001) and IgG3 (P less than 0.001) were found in the patients compared with 22 age-matched healthy children, whereas IgG2 and IgG4 were normal or slightly decreased. IgG immune complexes were measured by protein A-enzyme-linked immunosorbent assay (ELISA) combined polyethylene glycol precipitations. Eight of 17 patients (47.1%) were found to have circulating immune complexes (CIC) values above the normal control range (geometric mean +2 SD). IgG subclass composition in CIC was analyzed. The subclasses in CIC were predominantly IgG1 and IgG3. Because the antibody responses to different antigens exhibit IgG subclass restriction, it would suggest that the change of serum and CIC IgG subclasses in Kawasaki syndrome may have relevance to the pathogenesis of the disease.

Near-maximal exercise responses in health and disease in middle-aged Chinese men.

Cardiovascular, aerobic, and ventilatory responses to a multistage treadmill test of near-maximal exercise were determined in 208 middle-aged Chinese men to provide standards in health, and to identify differences in relation to hypertension, coronary heart disease, chronic pulmonary disease and stroke. Except in patients with hypertension, exercise duration was shortened and symptom-limited oxygen uptake was reduced in all disease groups. Unexpectedly, stroke patients exhibited lower heart rate responses to exercise than did patients with coronary heart disease.

Polarcardiographic criteria for myocardial infarction in Chinese men.

Polarcardiograms (PCG) derived from xyz leads of the Frank electrocardiogram (ECG) were recorded in the supine position in 1264 initially healthy middle-aged Chinese men who had been under continuing medical surveillance and were re-examined seven years later. Polarcardiographic criteria for myocardial infarction (MI) were demonstrated in 97 men (7.7%), but only 15 of them showed diagnostic Q waves in the electrocardiogram (ECG). Polarcardiographic criteria for MI were independent of age, but ranged from 4.3% in 464 apparently normal men to 38% in 21 men with manifestations of ischemic heart disease. The possible association of polarcardiographic criteria and a history of smoking was limited to those with clinical evidence of heart disease. The "false positive" rate of 4.3% in clinically normal men was similar to that reported in younger Canadians and in Cretan population samples.

Seven-year follow-up of cardiovascular study and maximal exercise of Chinese men.

A seven-year follow-up in 1973 of a prospective cardiovascular study of 1820 initially, healthy, middle-aged Chinese men of 40-59 years of age identified 1745 (95.9%) known survivors, 49 (2.7%) interim deaths, and 26 (1.4) who could not be traced. Of the survivors, 1462 (83.8%) were re-examined, 292 (16.7%) had another treadmill test of maximal exercise, and 283 (16.2%) failed to return for re-examination. On the basis of interim surveillance of hospital admissions, questionnaires and re-examination, a greater incidence of noncardiovascular events (338 or 18.6%) than evidence of cardiovascular disease (220 or 12.1%) was found while the majority (1021 or 56.1%) remained healthy. Total mortality was 0.29 for men under 50 and 0.76 per 100 person-years for men of 50 or more years of age. Only nine, or 18.4% of the deaths were due to cardiovascular causes, and unexpectedly for this population sample, only three were attributed to stroke. When cardiovascular morbidity was related to presence of ST depression after maximal exercise, to hypertension at rest by WHO criteria, to both findings, or to absence of either on initial intake examination, incidence increased from 2.3% for NEITHER group, to 5.7% for ST group, to 11.9% for HT group, and to 25.0% for BOTH groups. Re-examination revealed more evidence of cardiovascular disease than did surveillance of hospital admissions. Additional to effects of aging and mild adiposity, longitudinal changes in blood pressure and ST depression, increasing in the NEITHER group, but less frequent in the other groups, showed some evidence of regression toward the mean, as well as emerging disease and the confounding effects of uncontrolled treatment of hypertension in many. The potential for prediction of subsequent cardiovascular morbidity or mortality appeared stronger for hypertension than for postexertional ST depression, although the two were additive in this population, which is more prone to hypertension and stroke but now is developing clinical manifestations of coronary heart disease more frequently.
