A freeze-dried method for preparation of G6PD reagent tubes.

Mixed reagents for the Glucose-6-phosphate dehydrogenase (G6PD) deficiency fluorescent screening test were freeze-dried in plastic tubes. The reagents were then reconstituted with distilled water and the test was performed in the usual way. Initial testing with the freeze-dried mixed reagents gave consistent positive reaction to 12 normal blood samples and negative reaction to 9 G6PD deficient blood samples. This will enable a laboratory with freeze-drying facilities to prepare reagent tubes in bulk. As these tubes can be kept at 4 degrees C and do not require to be stored at -20 degrees C, a major laboratory can prepare these tubes and supply small laboratories for screening purposes.

Immunophenotyping of peroxidase-negative acute leukaemias using immunoalkaline phosphatase (APAAP) method.

Immunophenotyping of acute leukaemias has become an important diagnostic tool in haematology laboratories as it is now well recognised that the presence of certain surface markers has prognostic significance. In 1988, we experimented with the alkaline phosphatase anti-alkaline phosphatase (APAAP) method for immunophenotyping of leukaemic cells in our laboratory. 48 cases of peroxidase-negative acute leukaemias were studied. Our study showed that 2 peroxidase-negative cases carried myeloid surface markers, 44% were negative for the markers studied and 5% were unclassified due to technical problems. We concluded that the APAAP method is a useful technique for demonstrating cell markers in leukaemic cells as the reaction is reddish and usually intense. We failed to demonstrate surface markers in 44% of the cases probably because of the choice of a limited panel of monoclonal antibodies.

Frozen bone marrow trephine biopsy--a technical evaluation.

The routine study of bone marrow trephine biopsies involves fixation, decalcification, paraffin-embedment, sectioning and staining. However, this process creates artifacts, produces shrinkage of tissue, consumes time and can result in sections of unsatisfactory cytological quality. It also renders the tissue unsuitable for enzyme-histochemical and immunohistochemical analyses. Frozen section of bone marrow without decalcification was evaluated as an alternative method for the study of bone marrow. This method was found to give sections with comparable cytological quality to that of paraffin-embedment, yielded sections for interpretation within 24 hours, and allowed enzyme-histochemical and immunohistochemical analyses to be applied successfully.

Evaluation of the Contraves Autolyzer 801.

The Contraves Autolyzer 801 is a 20 parameter cell counter that operates on the principle of impedance. This evaluation study found that the within-batch precision for 5 of its parameters--WBC, RBC, Hb, Hct and Plt--had coefficients of variation below 5%, while the counts for red cell parameters were reproducible for up to 3 days. The analyser exhibited good linearity for all the parameters. When compared with 2 other counters--the Coulter S-Plus and the Coulter M350, some degree of proportional and constant bias due to unsuitable calibration were detected in the WBC, Hct and Plt parameters. Otherwise it showed generally good correlation in all the parameters and also with the manual PCV method. Carry over was negligible in the red cell parameters. The Contraves Autolyzer 801 is easy to operate and readily accepted by operators.

Left main coronary artery obstruction: surgical experience with 93 patients.

Among patients with ischemic heart disease, those with left main stem coronary artery stenosis has attracted most attention due to their ominous prognosis. During the period from August 1982 to February 1989, 93 patients underwent left main coronary artery revascularisation procedures at Singapore General Hospital. All the patients had greater than 75% obstruction of the left main coronary artery. Significant triple vessel disease occurred in 35 patients (37.6%), double vessel disease in 32 patients (34.4%), single vessel disease in 21 patients (22.6%) and left main coronary as an isolated lesion in 5 patients (5.4%). The overall hospital mortality rate is 6.5% and perioperative infarction rate is 8.6%. Since mid-1985, the hospital mortality rate has been reduced to 3.7% and the perioperative infarction rate to 4.9%. Average grafts per patient was 3.5. Intra-aortic balloon pump was used in 5 patients. Follow up period was from 5 months to 84 months with a mean of 36 months with the majority of patients in either New York Heart Association Class I or II and still alive.

Sterilisation patterns in dental practices in Singapore.

A survey to find out the standard of sterilisation of dental equipment was conducted via a questionnaire sent out to all Division I dental surgeons in Singapore. Questions asked determined the main methods of sterilisation/disinfection of hand instruments, air rotor and air motor handpieces, burs, salivary ejectors, ultrasonic scaling tips, and the use of disposable needles, local anesthetic, carpules, and gloves. Results indicated that while most respondents sterilised or disinfected various instruments, there was an apparent misconception that sterilisation and disinfection were the same thing. The majority used boiling with water or cold chemical disinfection. Most respondents were willing to treat patients with known histories of hepatitis B but would refer patients with histories of Acquired Immune Deficiency Syndrome (AIDS).

Total intracardiac repair for tetralogy of Fallot in adults.

The anatomic and clinical features of 47 patients who were 18 years of age or older at the time of total intracardiac repair for tetralogy of Fallot are reviewed. Twenty (43%) patients had had previous palliative surgery. Of 14 pulmonary-systemic shunts, 9 (64%) remained patent. The location of the ventricular septal defect was infracristal in 90% of patients. The predominant right ventricular outflow tract obstruction was at the infundibulum in 30%; another 64% of patients had combined valvular and infundibular obstruction. Total intracardiac repair was achieved; hospital mortality was 8.5%. Morbidity was minor, and hemorrhage was a significant problem in only 2 patients. Thirty-five patients have been followed from 11 months to 15 years after surgery. There were 4 late deaths; the actuarial 10-year survival rate was 82%.

Disseminated adenomyomas of the abdominal and pelvic cavities: a case report.

A case report of disseminated adenomyomas of the abdominal and pelvic cavities is presented. Although the pathogenesis of these benign tumors is still unresolved, the authors postulate that they develop from the differentiation into smooth muscle of a multi-potential cell that arises from endometrial stromal cells in an area of preexisting endometriosis. Based on the authors' experience, when multiple leiomyomas are found in the peritoneal cavity, it is wise to make serial sections to determine whether endometrial tissue is present. The clinician must be alert to the fact that on gross examination adenomyomas can mimic metastatic carcinoma. Differentiation is made only through histologic examination. In addition, since adenosarcoma arising from adenomyoma is a documented occurrence, careful examination of histologic sections of adenomyomas is essential to avoid making the wrong diagnosis.

Serum human placental lactogen (HPL) levels in patients with intact hydatidiform mole.

Serum human placental lactogen (HPL) levels in forty cases of intact hydatidiform mole were measured by radioimmunoassay. The HPL values were generally lower than normal pregnancies of the corresponding period of gestation. However, normal and occasionally higher than normal values were observed in a few cases. Serum HPL level alone is of some clinical use in the diagnosis of hydatidiform mole. When combined with human chorionic gonadotropin (HCG), a low HPL/HCG ratio for the corresponding period of amenorrhoea is a useful index in the diagnosis of hydatidiform mole.
