[Contribution of self-surveillance of the personnel by electronic radiation dosemeters in invasive cardiology].

The methods of surveillance of coronary angiography or angioplasty operators vary from team to team. The choice of method implies evaluation of the level of exposition of unprotected areas. The usual methods of surveillance by dosimetry may be complemented by an electronic device used for industrial radiological protection against X and Gamma rays. This instrument emits a sound each time a micro sievert is detected: the operator perceives the photons of diffused rays. The value of protective screens is then directly audible. This instrument does not detect X rays with an energy of less than 50 KeV but there is a good correlation with results measured by thermoluminescent dosimetry. The measurement of irradiation were noted at three points on the body before and after the use of complementary protection which certain operators hesitated to use before this study. The results confirmed the value of these protections: the suspended lead screen reduced cervical irradiations by a factor of 15 and that of the left wrist by 8. Lower down, a flexible lead skirt reduced irradiation of the ankle by a factor of 19. These results underestimate the efficacy of the screen as low energy X rays were not measured and are even more easily absorbed by the screens.

[Alpha-1 inhibition. Clinical studies of the efficacy and tolerability of Alpress LP in mild to moderate essential hypertension].

Alpress LP allows a continuous flow of prazosin, thanks to its new galenic form. This implies a better efficacy and less side-effects in the treatment of hypertension. Besides, prazosin being known for its favourable effect on hypercholesterolemia, Alpress LP was studied in this respect. In each of the three studies detailed thereafter, concerning hypertensive patients, proof was made of the efficacy and the good tolerance of Alpress LP, as compared to reference therapeutics given in hypercholesterolemia.

[Acquired interventricular communications in bacterial endocarditis. Apropos of a case with review of the literature].

The case reported here concerns a female of 33 who presented, during the course of an acute bacterial aortitis which had been treated, with an inflammatory perforation of the membranous septum. The clinical picture consisted of an association of very severe heart failure with severe aortic incompetence, an apical pansystolic murmur, and atrioventricular conduction defects. Although it was carried out in the presence of active endocarditis (valve cultures positive for streptococci), operative intervention (Patch + Starr) allowed rapid healing and excellent cardiovascular function after a 30 month follow-up period. Careful study of this case and of the eleven others in the literature has enabled us to underline the following points:--the sudden appearance, in a case with infective aortic incompetence, of a gross pansystolic murmur associated with atrial or intraventricular conduction defects, should be taken as suggesting a septal perforation;--examination of the haemodynamic system in such a case runs certain risks, and should never be allowed to delay the operation, which must always be carried out urgently;--despite the unfavourable operative conditions, the results strongly support the need for surgical intervention in these patients in good time.
