Loss of accommodation produced by peristriate lesion in man?

We observed a bilateral accommodative paresis associated with a spontaneous parieto-occipital hematoma in a 37-year old patient. There was no clinical or computerized tomography (CT) evidence of transtentorial herniation or upper brainstem pathology. With resolution of the hematoma, accommodation returned to normal.

Giant cerebral Echinococcus cyst with galactorrhea and amenorrhea.

A case of giant left-sided frontal cerebral Echinococcus cyst causing headaches, galactorrhea--amenorrhea, secondary sterility, and gain in weight in an adult female patient is reported. The operative removal of this huge cyst led to complete neurological and endocrinologic recovery.

CT pneumomeatography in the diagnosis of small acoustic neuromas.

The introduction of a small quantity of air by lumbar puncture in the lateral decubitus position followed by high-resolution computerized tomography (CT) scanning is a simple method allowing optimal visualization of the cerebellopontine angle cistern and the internal auditory meatus. Our experience with 37 CT pneumomeatographies in 22 patients who had clinical and audiometric findings suggestive of retrocochlear lesions is presented. Thirty-two examinations were normal, and 5 revealed small acoustic neuromas. In all cases, conventional X-rays, multidirectional tomography and contrast CT were nonconclusive. We advocate that CT pneumomeatography be the procedure of choice in the diagnosis of purely intracanalicular or very small extracanalicular acoustic neuromas.

Subdural empyema of sinus origin in children.

Two children with subdural empyema of paranasal sinus origin presented as neurological emergencies. Both had severe acute pansinusitis with complicating subdural empyema. The need for a high index of suspicion in the proper clinical setting is mandatory even in the absence of confirmatory contrast studies. Surgical proof should be immediately followed by adequate treatment. The high mortality rate even in the antibiotic area calls for this approach.

Carbon dioxide laser surgery of basal meningiomas.

This preliminary report summarizes our results with laser surgery in patients with basal meningiomas (8 basilar and 1 intraventricular extending also into the third ventricle). Illustrative preoperative and postoperative CT scans are included. The advantages and limitations of this surgical technique are discussed briefly.

CT scan of retroperitoneal hematoma with neuropathy.

A hematoma in the left iliacus muscle induced by anticoagulant therapy and causing a femoral neuropathy was demonstrated by CT scan. The scan returned to normal after the surgical evacuation of the hematoma. Other possible uses of the CT scan to demonstrate neurological lesions in this region are discussed.

Anterior interbody fuson for cervical osteomyelitis. Reversal of quadriplegia after evacuation of epidural spinal abscess.

Interbody fusion for stabilization of the cervical spine after osteomyelitic destruction of the body of C5 vertebra is reported in a patient with quadriplegia and sphincter disturbances secondary to an epidural abscess. The successful union of the bone graft along with complete neurological recovery after anterior decompression and evacuation of the epidural mass seem to justify the procedure.

Transverse cervical myelopathy: an unusual complication of retropharyngeal abscess.

Retropharyngeal abscess complicated by suppuration in the central nervous system, intra- or extraaxial, is rare. Acute transverse myelopathy complicating a retropharyngeal abscess may be more frequent than epidural spinal abscess and usually affects the thoracic cord. Therefore, the occurrence of acute high cervical transverse myelopathy is rarely encountered and warrants reporting. The need for neuroradiologic diagnostic evaluation is emphasized in order to rule out a surgically treatable lesion.
