[Latrodectism in a child].

BACKGROUND: The malmignatte Latrodectus mactans tredecimguttatus, commonly known as a black widow spider, can be found in the Mediterranean region. Its bite is a cause of a rarely seen syndrome called latrodectism. CASE REPORT: During a visit to Corsica, a 13-year-old boy developed abrupt severe abdominal pain and spasmodic muscular contractions, headache and vomiting. The patient was restless and experienced hallucinations including distressing visions of death. His high blood pressure (154/100 mmHg) returned to normal within 3 days. Clinical examination revealed dyspnea and facial edema associated with blepharoconjunctivitis and hyperreflexia, together with a scattered erythema and pruritus. These changes took place within minutes, after a probable black widow spider bite. The abdominal and neuropsychiatric symptoms disappeared after 5 days. Treatment with calcium gluconate, paracetamol, phloroglucinol and hydroxyzine had no effect, but diazepam decreased the acuteness of the symptoms. Anti-venom serum was not used. CONCLUSION: Diagnosis of latrodectism must be based on clinical and epidemiological data. Erroneously diagnosing surgical acute abdomen, renal colic, meningitis, tetanus or opioid withdrawal would entail incorrect treatment.

[Management of asymptomatic small tumor of the kidney of incidental discovery].

OBJECTIVE: The incidental discovery of renal tumours is increasingly frequent. All small renal tumours are not inevitably cancers. The objective of our study was to compare radiological and histological data and to establish a diagnostic and therapeutic approach to the incidental discovery of a small renal tumour. MATERIAL AND METHODS: Retrospective study of 22 incidentally discovered asymptomatic renal tumours, with a long axis of less than 5 cm on the pathology specimen. RESULTS: The tumour was discovered on ultrasound in 68% of cases. All patients were operated, allowing histological examination of the operative specimen, revealing 14 cancers, 4 angiomyolipomas, 2 adenomas and 2 haemorrhagic cysts (with thick contents). Ultrasound demonstrated a hypoechoic tumour in 13 cases, 11 of which were cancers, and a hyperechoic tumour in the other 7 cases, corresponding to 4 angiomyolipomas, 2 cancers, and 1 adenoma. Computed tomography showed a heterogeneous tumour in 12 cases, corresponding to a cancer in all but one case. The tumour was homogeneous in the other 10 cases, including 4 cancers. Arteriography showed a hypervascular tumour in 3 cases, all cancers, and a hypovascular tumour in 7 cases: 2 cancers, 2 adenomas, 2 very small angiomyolipomas and 1 haemorrhagic cyst. CONCLUSION: Despite the development of medical imaging, further progress is required to resolve the diagnostic problems raised by certain incidentally discovered asymptomatic renal tumours, particularly tumours with a homogeneous appearance on CT scan and a hypovascular appearance on arteriography.

[Neurofibroma of the recurrent nerve: apropos of a case].

A case of solitary benign neurofibroma of the recurrent laryngeal nerve is presented. The preoperative diagnosis was in favour of a thyroid cancer. The discovery of this benign tumour leads to discuss the malignancy criteria of a cold nodule. The recurrential paralysis is an evidence of malignancy in only about 80% of cases. The surgical operation is the best treatment because it allows the histology that asserts the benignancy of the lesion. The review of the literature confirms the exceptional nature of this tumour. The respective histology of neurofibromas is finally briefly recalled.

[Neurological complications of zona. Apropos of a case].

A 70 year old woman had left sixth nerve palsy, following an ophthalmic zona. Later she had controlateral hemiplegia with expression aphasia. Corticosteroid therapy improved the situation. The pathogenesis of this complication is discussed in this article. Ischaemic vasculitis is now a well known complication of herpes zoster which has already appeared in the literature. The case presented was related to ischaemia of the anterior artery of the troncus infero-lateral of Heubner's artery confirmed by further exploration.

[Latrodectism in Corsica. Study of 15 cases].

Malmignate bites are rare in Corsica nowadays. The authors report 15 cases of latrodectism in patients hospitalized between 1986 and 1993 at the Ajaccio hospital in southern Corsica. The disease has a polymorphous expression. We investigated the frequency and the specificity of the symptoms. Eleven clinical signs and 2 biological signs enabled us to describe 3 categories: specific, moderately specific, non-specific. Most of the signs described in the literature were found in this study, as well as a concordant frequency. We notice, though, that the latrodectesmica facies was not often observed in this series. No serious complication occurred.

[Uretero-uterine fistula. Diagnostic and therapeutic management].

A case of uretero-uterine fistula leads the authors to analyse the possible causes and review the diagnosis approach, which is above all clinical:incontinence of urine with persistence of normal micturition; leakage of urine via the cervix and cystoscopically normal bladder, most often following cesarean section. Treatment possibilities are considered, with uretero-vesical reimplantation having a place of choice.
