Evaluation of second cytoreductive surgery in the treatment of epithelial ovarian cancer.

OBJECTIVE: To evaluate the effectiveness of second cytoreductive surgery in the treatment of epithelial ovarian cancer. METHODS: From January 1989 to June 1994, second cytoreductive surgery was carried out on 33 patients with epithelial ovarian cancer who either underwent unsatisfactory primary debulking operation or had recurrence. According to FIGO staging (1987), there were 5 patients in stage I, 2 in stage II, 25 in stage III, and 1 in stage IV. Pathological grading was G1 in 2 cases, G2 in 9, G3 in 19 and uncertain in 3. The 33 patients can be divided into 3 categories: I, nine patients who had unsatisfactory primary debulking operation with macroscopic residual > 2 cm, and 1-2 courses of postoperative chemotherapy; II, 15 patients who had 6-8 courses of cisplatin-based postoperative chemotherapy and in whom recurrence was diagnosed after complete response for at least 3 months; and III, 9 patients who had the same treatment as category II and survived without cancer clinically for more than 6 months, and in whom recurrence was diagnosed during second-look laparotomy. All patients had been followed up for at least two years (27-168 months) dated from the primary debulking operation. RESULTS: Fifteen cases had no macroscopic residuals (group A), 5 had residuals < 2 cm (group B), and 13 had residuals > 2 cm (group C). The medium survival time and two-year survival rate in groups A, B and C were 59.09, 20.6 and 8.29 months, and 93.3%, 20% and 7.69% respectively (P < 0.001, A vs C; P < 0.05, A vs B and B vs C). CONCLUSIONS: The results suggest that second cytoreductive surgery is of value, and the key to success is to eliminate any macroscopic residual focus, or at most, to leave only minimal residuals < 2 cm. It is suggested that well-targeted multiple-route chemotherapy with sufficient courses before second cytoreductive surgery is important to achieving better results.

[The relation between nutrition and pulmonary ventilation in patients with chronic obstructive pulmonary disease during the relieved period].

To observe the effect of nutrition on pulmonary ventilation in aged patients with chronic obstructive pulmonary diseace (COPD) during the relieved period, the pulmonary function (MVV, PVC, FEV1, MMEF) of 261 aged patients with COPD was tested during the relieved period. The results showed that the MVV (48.75% +/- 19.9%) tested in the patients with malnutrition was significantly lower than that tested in the patients with normal nutrition (59.1% +/- 22.6%) (P < 0.01) while there was no marked difference in the ventilation in their large and small airway. It is conluded that malnutrition of the patients with COPD affects the constrictive function of respiratory muscle and thus results in a decrease in MVV.

[Risk of lung cancer among iron and steel workers in Anshan, China--case-control study].

A case-control interview study on 610 lung cancer patients and 959 controls was conducted among male workers in Anshan Iron-steel Complex. After adjusting for non-occupational risk factors, such as smoking, pulmonary disease, family history of cancer and the consumption of fruit, risks for lung cancer were significantly higher in workers engaged in smelting and rolling (OR = 1.5, 95% CI = 1.1-2.2), in the fire-resistant brick factory (OR = 2.9, 95% CI = 1.4-5.9), in general loading (OR = 2.5 95% CI = 1.0-6.1), and in coking (OR = 3.4, 95% CI = 1.4-8.5) for 15 or more years. Significant dose-response was observed for exposure to total dust and B[a]P, but not for specific chemical components of dust. The lung cancer risk increased 40% in iron and steel workers with long term occupational exposure.

Protective effects of dl-3-n-butylphthalide on changes of regional cerebral blood flow and blood-brain barrier damage following experimental subarachnoid hemorrhage.

OBJECTIVE: To investigate the therapeutic effects of dl-3-n-butylphthalide (dl-NBP) on regional cerebral blood flow impairment and blood-brain barrier damage induced by subarachnoid hemorrhage (SAH) in rats. METHODS: SAH was induced by an injection of 0.35 ml autologous blood into the lateral ventricle. The regional cerebral blood flow in caudate nucleus was determined by hydrogen clearance method. Permeability of blood-brain barrier was measured by extravasation of the protein bound Evans blue dye to brain tissue. RESULTS: SAH produced a marked decrease in both caudate blood flow and blood-brain barrier damage which showed increased extravasation of protein bound Evans blue dye to brain tissue. dl-NBP (5-20 mg/kg, given 5 min after the onset of SAH) and nimodipine (0.25 mg/kg) significantly increased the regional cerebral blood flow in caudate nucleus within 3 hours of SAH. Moreover, dl-NBP (10 mg/kg) and nimodipine (0.25 mg/kg, given 5 min and 3 h after SAH) significantly reduced the brain extravasation of Evans blue dye which increased after 6 hours of SAH. CONCLUSION: By improving the regional cerebral blood flow and blood-brain barrier, dl-NBP has therapeutic effects on experimental SAH.

Short-term effects of chemotherapy-surgery-chemotherapy regimen on clinically inoperable advanced ovarian cancer.

OBJECTIVE: To investigate the short-term effects of chemotherapy-surgery-chemotherapy regimen on clinically inoperable advanced ovarian cancer. METHODS: Retrospective analysis was made on 16 cases of stage III and 2 cases of stage IV epithelial ovarian cancer with fixed pelvic mass. Neoadjuvant chemotherapy with CP (cisplatin + cyclophosphamide) or CAP (CP + adriamycin) regimen was given by intraarterial, intraperitoneal, or systemic routes accordingly for an average of 2.8 courses. Sixteen of the 18 cases were assessed to be operable after preoperative chemotherapy. The uterus, adnexa, omentum, pelvic lymph nodes (14/18), and implants were included in the cytoreductive surgery. Pelvic masses were found to have almost disappeared in 9, and macroscopic residuals were found in 11 (residuals < 2 cm in 2, and > 2 cm in 5) of the 18 cases. Postoperative chemotherapy (CP, CAP, or CEP, E = VP16) was given for an average of 5.9 courses. RESULTS: Five out of 7 patients followed up for over 3 years have been surviving for 46, 44, 40, 38 and 36 months, respectively. CONCLUSION: Chemotherapy-surgery-chemotherapy regimen has beneficial effects on clinically inoperable advanced ovarian carcinoma.

[Preparation and evaluation of C60 bonded silica phase for high performance liquid chromatography].

C60 was chemically attached to gamma-(ethylenediamino) propylsilyl-bonded silica as a stationary phase for liquid chromatography. The new stationary phase was used to investigate the separations of polycyclic aromatic hydrocarbons (PAHs) and tert-butylcalix[n]arenes. The elution order of PAHs was found to be similar to that on octadecylsilica (ODS); the elution order of tert-butylcalix[n]arenes was found to be tert-butylcalix[8]arene, tert-butylcalix[4]arene and tert-butylcalix[6]arene with 1%(V/V) isopropanol/cyclohexane as a mobile phase.

[Chemical changes of compatibility for shaoyao decoction].

OBJECTIVE: To probe the regular patterns of interactions among chemical components in Chinese medicinal complex prescriptions. METHOD: The active components of Rhubarb, Coptis root and Baikal skullcap root in different patterns of compatibility were determined by TLC-scanner. RESULT: The berberine content in Coptis root can be decreased obviously by Rhubarb, Baikai skullcap root and Licorice root, especially by Rhubarb. Coptis root can also decrease the contents of acid components. CONCLUSION: The acid components can affect each other, thus reducing the contents of effective components.

[Resection of small primary liver cancer: clinical analysis of 82 cases].

OBJECTIVE: To determine which clinicopathologic features and surgical modalities are related to the prognosis of small primary liver cancer (SPLC). METHOD: Eighty-two patients who had under gone operations for SPLC (<or= 5.0 cm in diameter) from 1988 to 1995 were retrospectively studied. 60 patients were HBsAg positive, and 71 patients suffered from liver cirrhosis. Bisegmentectomy, segmentectomy and partial resections were performed in 13, 14 and 55 patients separately. RESULT: The overall operative mortality was 1.2%, and the 1-, 3-, 5-year survival rates were 92.7%, 72.5% and 54.3%, respectively. The 5-year survival rates of 78 patients with hepatocellular carcinoma according to histologic features were 79.6%, 49.9% and 41.1% in well-differentiated (15 patients), moderately (53), and poorly differentiated (10) groups. Recurrence and metastasis rates in 1, 3, 5 years were 12.8%, 40.7% and 71.5%, separately. CONCLUSION: SPLC has the same history of large PLC. Early diagnosis and limited hepatic resection are very important to decrease the mortality and increase the survival rate.

[Management of true extremity aneurysms: experience in 23 patients].

OBJECTIVE: To explore the etiologic feature and specificity of diagnosis and treatment in true aneurysms of the extremities. METHOD: 23 patients with true aneurysms of the extremities from January 1984 to September 1997 were retrospectively reviewed. RESULT: Of 30 true aneurysms of the extremities, multiple aneurysms were found in 7 patients (30.4%). The multiple aneurysms were diagnosed in the same time or in different time. 21 patients were treated by exclusion of aneurysm and replacement with autogenous saphenous vein or graft. CONCLUSION: Early diagnosis and treatment are recommended to prevent complications which can cause limb loss, and multiple aneurysms must be taken into account. Importance of arteriography should be stressed in the diagnosis of multiple aneurysms. Exclusion of aneurysm and autogenous saphenous vein reconstruction is the first choice of treatment, circulation of the operated extremities should be noticed and close follow-up is imperative to pay attention to the development of new aneurysms.

[A survey of the social and psychological factors of the heroin dependence].

The survey studied 100 dependence about their age, marriage, occupation, the way of heroin taking, history of heroin taking, and the reason of the first heroin taking. The data showed that most dependence were young or middle aged men, or people unable to graduate from junior middle school, self-employed laborers and unemployed. Dependence of cadres and people with better education were increasing. The heroin injection is becoming popular.

[Protective effects of dl-3-n-butylphthalide on changes of regional cerebral blood flow and blood-brain barrier after subarachnoid hemorrhage].

Effects of dl-3-n-butylphthalide(dl-NBP) on changes of regional cerebal blood flow (rCBF) and blood-brain barrier of rats subjected to subarachnoid hemorrhage (SAH) were investigated. Regional CBF was determined by hydrogen clearance method at 15 min before, 15 and 30 min thereafter once every 30 min up to 180 min after the beginning of SAH. The results showed that SAH produced a significant decrease in rCBF in caudate nucleus and blood-brain barrier damage which displayed increased extravasation of Evans blue to brain tissue. Both dl-NBP (5-20 mg.kg-1 given intraperitoneally 5 min after the onset of SAH) and nimodipine (0.25 mg.kg-1) were shown to increase rCBF in caudate nucleus through 3 h of SAH. Moreover, dl-NBP 10 mg.kg-1 and nimodipine 0.25 mg.kg-1, given 5 min and 3 h, respectively, after SAH, significantly reduced the brain extravasation of Evans blue 6 h after SAH, indicating that both dl-NBP and nimodipine has protective effects on blood-brain barrier. These results suggest that dl-NBP has therapeutic effects on SAH.

[Determination of flavonoids and quality evaluation of Chinese traditional drug "puhuang"].

This paper deals with the determination of flavonoids and quality evaluation of Chinese Typha. Four species, viz. Typha angustata, T. angustifolia, T. davidiana and T. latifolia and eight commercial samples of Chinese traditional drug "Puhuang" were collected. Five flavonoids in pollen, filaments, female flowers and leaves were determined by HPLC using mu-Bondapak C18 column, water--isopropanol--tetrahydrofuran = 83.5:14:2.5, as mokile phase and detected at 287 nm). The qualities of different Typha species and commercial "Puhuang" samples have been compared, and the criteria for their quality control were also discussed.

[Effects of butylphthalide on extracellular 6-keto-PGF1 alpha, TXB2 and 6-keto-PGF1 alpha/TXB2 ratio in cultured rat cortical neurons].

The effects of 3-n-butylphthalide(NBP) on the levels of 6-keto-PGF1 alpha, TXB2 and 6-keto-PGF1 alpha/TXB2 ratio were studied with methods of RIA. d-NBP and l-NBP(0.1-100 mumol.L-1) concentration-dependently increased 6-keto-PGF1 alpha release, decreased TXB2 release from neuronal cells, and significantly enhanced extracellular 6-keto-PGF1 alpha/TXB2 ratio in primary cultured rat cortical neurons exposed to hypoxic-hypoglycemic media for 5 h or hypoxic-hypoglycemic media for 5 h following normal media for 3 h. Aspirin(0.1-100 mumol.L-1) was also shown to inhibit TXB2 release from cortical neurons in a dose-dependent manner. However aspirin only increased 6-keto-PGF1 alpha/TXB2 ratio at low dose because aspirin inhibited both 6-keto-PGF1 alpha and TXB2 release simultaneously at large dose(10-100 mumol.L-1). This suggests that the action of l-NBP, d-NBP and dl-NBP on the increase of 6-keto-PGF1 alpha/TXB2 ratio might be one of the mechanisms in which NBP enhanced focal cerebral blood flow and improved ischemic brain damage.

[Protective effects of d-, l-, and dl-3-n-butylphthalide on neuronal damage induced by hypoxia/hypoglycemia in cultured rat cortical neurons].

The effects of l-3-n-butylphthalide(l-NBP) and d-3-n-butylphthalide(d-NBP) on hypoxia/hypoglycemia-induced cytotoxicity in primary cultured rat cortical neurons were studied. l-NBP and d-NBP(1-100 mumol.L-1) were shown to inhibit hypoxia/hypoglycemia-induced LDH release, decrease the percent of cell death and improve the damaged cellular morphology at 10 mumol.L-1 concentration. In addition, l-NBP, d-NBP and dl-NBP were also found to significantly reduce the liberation of polyribosomes from the neuronal rough endoplasmic reticulum and disaggregation of polyribosomes induced by hypoxia/hypoglycemia. These data suggest that l-NBP, d-NBP and dl-NBP can remarkably protect cultured neurons against hypoxia/hypoglycemia induced damage.

[Effect of dl-3-N-butylphthalide on the expression of hsp70 mRNA and c-fos in transient cerebral ischemic and reperfused rat brain].

Transient cerebral ischemia may cause striking changes in gene expression in rat brain. The induction of heat shock protein 70 (hsp70) mRNA is considered to be an important marker of cerebral ischemia injury, and c-fos may upregulate the expression of other genes related to the secondary injuries. dl-3-n-Butylphthaline(NBP) had been shown to have good anti-cerebral ischemic effect. Using the in situ hybridization and Northern blot technique, the effect of NBP on the expression of hsp70 mRNA and c-fos in transient middle cerebral artery occlusion (MCAo) rat caused by intraluminal thread was studied, and found that the expression of hsp70 mRNA was at the lesioned site at 1 h of reperfusion. It increased gradually with the duration of reperfusion time and peaked at 12 h at the lesioned site. With NBP treatment(i.p. 10 mg.kg-1 10 min before ischemia or 20 mg.kg-1 after ischemia), the expression of hsp70 mRNA attenuated significantly. For c-fos, the expression appeared at 0.5 h of reperfusion, peaked at 3 h, and decreased at 6 h. NBP pretreatment (10 mg.kg-1 10 min before ischemia) also decreased the c-fos expression. The same results were obtained with Northern blot technique. Since NBP had been shown to have good anti-cerebral ischemic effects, the attenuating effect on gene expression seemed to be the secondary effect after the alleviation of tissue injury.

[Effects of d-3-n-butylphthalide and l-3-n-butylphthalide on extracellular no level and intracellular cGMP level in primary cultured rat cortical neurons].

The effects of l-3-n-butylphthalide(l-NBP) and d-3-n-butylphthalide(d-NBP) on extracellular nitric oxide (NO) levels and intracellular cyclic GMP (cGMP) levels were studied in primary cultured rat cortical neuronal cells. Nitric oxide and cGMP levels were measured by using spectrometry and radioimmunological analysis(RIA), respectively. The results showed that d-NBP (0.1-100 mumol: L-1) markedly increased extracellular NO levels and intracellular cGMP levels in primary cultured neurons that were exposed for 10 h to hypoxic/hypoglycemic, N-methyl-D-aspartate (NMDA), or KCl media. On the contrary, l-NBP(0.1-100 mumol.L-1) significantly decreased extracellular NO levels and intracellular cGMP levels. It is suggested that there is a contrary effect of d-NBP and l-NBP on NO release and cGMP production induced by hypoxia/hypoglycemia, NMDA, or KCl.

[The development of medical infusion monitoring and controlling network system].

The paper provided a medical infusion monitoring and controlling network system, in which the mainframe and its extensions can realize communication with each other by means of asynchronous serial port. At the same time, every extension can test infusion speed and adjust it to the presetting infusion speed gradually. The extensions also can alarm and stop infusion automatically when infusion will be over or there is a infusion malfunction.
