[Complications of laparoscopic cholecystectomy in China: analysis of 39,238 cases].

All Chinese articles published from November 1995 through April 1994 about laparoscopic cholecystectomy were identified through CMCC. From 600 titles, 105 were read and left for analysis. Other 21 articles were from the 6th Biliary Surgical Congress and this hospital. Severe complications of laparoscopic cholecystectomy were identified in 409 patients (0.4%), bile duct injury accounted for 0.32%, postoperative cystic duct leak and bile leak for 0.11% and 0.20% respectively. Peritoneal abscess and bowel injury occurred in 0.07% and 0.06% of cases, and postoperative hemorrhages were identified in 0.1% of the cases. Fourteen (0.04%) postoperative deaths resulted from operative injury. The data demonstrate that laparoscopic cholecystectomy is an operation which associated with low morbidity and mortality. But bile duct injury is still a problem, and complications of laparoscopic cholecystetomy can be minimized by improving operative procedure.

[Content determination of baicalin and baicalein in shuanghuanglian powdery injection by the method of HPLC-ECD].

The contents of baicalin and baicalein in Shuanghuanglian Powdery Injection have been determined by the method of HPLC-ECD under following conditions: column-ERC-ODS-1161, 6 mm x 100 mm; temperature-55 degrees C; mobile phase-0.1 mol/L KH2PO4, acetic acid and tetrahydrofuran (1000:100:190); flow rate-0.8 ml/min; detector voltage-+650 mV. The result shows the limit of detection for baicalin and baicalein to be 25 ng and 50 ng respectively, with an average recovery of 100.1% and 99.7% respectively. Highly reproducible, rapid, simple, convenient and accurate, this method is good for the content determination of baicalin and baicalein in Chinese medicinal herbs and preparations.

[Ultrastructural morphological observation on the plasma cells in periodontitis-affected human gingival tissue].

The transmission electron microscope was used to demonstrate the mechanism of immunoreaction in periodontitis-affected gingival tissue. The result showed that the number of plasma cells (PC) was the highest in infiltrated inflammatory cells and with degeneration. According to the variation of ultrastructural features of nuclei; plasma membrane (PM) and cisternae (Ci) of rough endoplasmic reticulum (RER), PC were classified into 4 stages. The chromatin of their nuclei varied from normal, to marginal concentrated, pyknosis, and karyorrhexis. The PM appeared intact, partial loss, to complete loss. The Cisternae showed varying degree of dilatation and the RER was broken into numerous discrete vesicles. The electron density of cisternae varied from low to high, and again low. The number of Russell's bodies varied from rare to increase, and then rare again. At last degenerated PC underwent apoptosis. The results show that the adult periodontitis is related to humoral immunity.

[Fermentation conditions of engineering strain utilizing starch for production of alkaline proteinase].

The highest activity of alkaline proteinase produced by Bacillus pumilus c172(pBX 96) transformant was 9,000 U/ml in shaking flask where corn meal and bean cake meal were carbon and nitrogen sources, respectively. The enzyme activity was raised under the conditions of initial pH 7.0, MgCl2 instead of MgSO4, and glucose (0.1%) in the substrate. The fermentation of c172(pBx 96) transformant was carried out with parameters of pH, reducing sugar, total sugar and enzyme activity.

Effect of dl-3-n-butylphthalide on brain edema in rats subjected to focal cerebral ischemia.

The present study evaluated the effect of dl-3-n-butylphthalide(NBP), a novel brain protective agent, on brain edema in rats following focal ischemia. Edema was induced by occluding the right middle cerebral artery (MCAO), producing permanent focal ischemia in the right cerebral hemisphere, which developed ipsilateral brain edema reproducibly. Edema was assessed 24 h after MCA occlusion by determining the brain water content from wet and dry weight measurements, and the sodium, potassium concentrations with ion-selective electrodes. In this model, NBP at the dose of 80, 160 and 240 mg/kg p.o. 15 min after MCAO prevented from brain edema in a dose-dependent manner. A significant reduction of sodium content and an increase in potassium level were observed in all drug-treated groups. It showed that NBP strongly attenuated brain water entry, sodium accumulation and potassium loss. Nimodipine treatment (5 mg/kg s.c.) also reduced brain edema (P < 0.05). The results suggest that a strong anti-edema activity of NBP may play an important role to contribute to the treatment of ischemic damage.

[Determination of organic acids and inorganic anions by gradient ion chromatography].

The chromatographic conditions for separation and detection of organic acids and inorganic anions by gradient ion chromatography with suppressed conductivity detection were studied. The optimized gradient programs were established. Ion chromatography were performed with a DX-100 chromatograph (DIONEX). The separation column is IonPac-AS11. Compared with NaHCO3/Na2CO3 and Na2B4O7, NaOH was the optimal eluent. The effect of organic modifier was also studied. Among methanol, 2-propanol and acetonitrile, methanol can make ion pairs such as malate and succinate, malonate and tartrate gaining baseline resolution. By using ion exchange separation, Cl-, NO3-, malate, succinate, malonate, tartrate, SO4(2-), oxalate were eluted between 5 mmol/L NaOH-16% CH3OH and 10 mmol/L NaOH-16% CH3OH in 25 min. A mobile phase composed of 30 mmol/L NaOH, 50% CH3OH and D.I. water was chosen to elute two groups of organic acids and inorganic anions: (1) quinate, formate, Cl-, malate, malonate, oxalate, citrate, isocitrate, aconitate; (2) lactate, Cl-, SO4(2-), tartrate, PO4(3-), citrate, isocitrate, aconitate. The detection limits (S/N = 3) were 0.1625 (quinate), 0.0691 (formate), 0.0115 (Cl-), 0.0886 (malate), 0.0591 (malonate), 0.0263 (oxalate), 0.1147 (citrate), 0.2017 (isocitrate), 0.3656 (cis-aconitate), 0.1045 (trans-aconitate), 0.1950 (lactate), 0.0729 (tartrate), 0.0224 (SO4(2-)) and 0.0692 (PO4(3-)) mg/L. The relative standard deviations were lower than 11.9% (n = 7) and the correlation coefficients ranged from 0.9212 for Cl- to 0.9999 for formate. The method was applied to determine the organic acids and inorganic anions of beverages and citric acids fermenting-medium. The results were satisfactory.

[Environmental pollution, human exposure and its health effect of sodium pentachlorophenate in schistosomiasis prevalent area].

Sodium pentachlorophenate (Na-PCP) has been used in China for years as an molluscacide to kill oncomelania, which is an intermediate host of Schistosome. To evaluate the effects of its long-term successive usage on environment, human exposure and health, studies were carried out in Sichuan, Jiangxi, Jiangsu and Fujian provinces, with a time gap of more than one month between sample collection and last spray of Na-PCP. Results indicated that PCP contents in surface water, soil, sediment, animals and plants were significantly higher in studied areas than in control areas. The daily intake and the content in urine of PCP were also sigificantify higher in studied areas. But, there was no difference on physical and biochemical examinations except that a 22%-28% decrease of blood cholinesterase activity was found in studied areas. The health effect of impurities in Na-PCP, dioxins and furans, was assessed and discussed.

In situ hybridization analysis of Girk2 expression in the developing central nervous system in normal and weaver mice.

A mutation in the gene Girk2 that encodes an inwardly rectifying potassium channel is the genetic defect causing the behavioral and pathologic abnormalities of the weaver mutant mouse. Of the pathologic abnormalities, the best studied is the neuronal degeneration that occurs in the cerebellar cortex and in the midbrain dopaminergic neurons. A detailed characterization of the topographic and temporal expression of Girk2 is fundamental to elucidate the mechanisms underlying neurodegeneration in these mutant mice. In this study we utilized in situ hybridization to determine the expression of Girk2 mRNA during prenatal and postnatal development in the murine central nervous system (CNS). Girk2 expression was seen in multiple regions of embryonic CNS including the cerebellum and midbrain. During postnatal development, the highest expression was seen in the cerebellum, midbrain and hippocampus. However, since the developing cerebellum undergoes significant neuronal loss due to the degeneration of granule cell precursors, Girk2 mRNA expression in this area decreases progressively.

[Studies on Fas ligand expression in patients with systemic lupus erythematosus].

The Fas/Fas ligand (FasL)-mediated apoptosis may play a role in the induction and maintenance of T cell tolerance. To investigate the role of FasL in the apoptosis of lymphocytes in autoimmune diseases, gene and protein expression of FasL were examined in peripheral blood mononuclear cells (PBMC) from patients with systemic lupus erythematosus (SLE) or rheumatoid arthritis (RA), and from healthy donors by a newly-designed semiquantitative reverse transcription (RT)-PCR and flow cytometry. Although no significant difference in FasL gene expression was obtained among three groups, SLE patients exhibited a wide distribution of the values. In SLE patients, there was a significant correlation between FasL gene expression by PBMC and some clinical parameters including SLE Disease Activity Index (SLEDAI) score, anti-DNA antibody titer and complement titer (CH50). More interestingly, a marked increase in FasL gene expression was observed in untreated SLE patients, whereas a significant decrease was observed in prednisolone-treated SLE patients. Flow cytometric analysis revealed the expression of FasL on T cell subsets from SLE patients and on anti-CD3 mAb-stimulated T cells from healthy donors. In vitro experiments, dexamethasone inhibited FasL gene expression by PBMC from healthy donors in a dose-dependent manner and with time of incubation. These results clearly indicate that FasL is up-regulated in active SLE patients, reflecting in vivo T cell activation, and that corticosteroids directly down-regulate FasL gene expression by human PBMC.

Nor-binaltorphimine precipitates withdrawal and excitatory amino acid release in the locus ceruleus of butorphanol--but not morphine-dependent rats.

The relative involvement of kappa opioid receptors in the mediation of behavioral and neurochemical responses to withdrawal from chronic drug treatment with the opioid analgesic butorphanol was studied using in vivo microdialysis to detail extracellular fluid concentrations of glutamate and aspartate within the locus ceruleus. Sprague-Dawley rats were rendered opioid dependent after 3 days of intracerebroventricular (i.c.v.) infusion of butorphanol (26 nmol/microl/hr) or morphine (26 nmol/microl/hr) and after i.c.v. infusion of saline vehicle (1 microl/hr). Acute withdrawal was precipitated by i.c.v. injection of the selective kappa opioid receptor antagonist nor-binaltorphimine (48 nmol/5 microl) after the 3-day period of infusion. Behavioral signs of withdrawal were detected after nor-binaltorphimine only in butorphanol-dependent rats. Basal levels of glutamate and aspartate were not different between treatment groups. Nor-binaltorphimine in the butorphanol-dependent rats increased glutamate to 227% and aspartate to 158% in the initial 15-min sample (P < 0.01). Nor-binaltorphimine did not increase glutamate or aspartate concentrations in the morphine-dependent or saline-treated groups. These results indicate a significantly greater participation of kappa opioid receptors in the development of butorphanol, rather than morphine, dependence and identify a differential neurochemical response to butorphanol withdrawal within a defined brain region, the locus ceruleus.

Expression of antimetastatic gene nm23-H1 in epithelial ovarian cancer.

OBJECTIVE: To investigate the expression of nm23-H1 in epithelial ovarian cancer. METHODS: A total of 55 specimens from patients with epithelial ovarian cancers, 15 patients with serous cancers, 7 with mucinous cancers, 17 with endometrioid cancers and 16 with adenocarcinomas, were examined for the expression of nm23-H1 by means of the immunohistochemical technique. RESULTS: The total positive rate was 23.64% (13/55). The tissular nm23-H1 immunostaining had significantly higher expression in mucinous cancer (57.14%) than in other histologic types of epithelial ovarian cancer (P < 0.05). Compared to that in the primary tumor, nm23-H1 expression in the omental metastatic tumor in the same patient was reduced. CONCLUSIONS: The expression rate of nm23-H1 is higher in the primary tumors than in the metastatic tumors and is different in various histologic subtypes of epithelial ovarian cancer. The expression of nm23-H1 may participate in the inhibitory process of tumor invasion.

[The role of TNF alpha in the allograft rejection of orthotopic rat liver transplantation].

OBJECTIVES: To measure tumor necrosis factors-alpha (TNF alpha) in rats on the 3rd, 64d days after orthotopic rat liver transplantation, and detect the expression of TNF alpha mRNA in the grafts. METHOD: Reverse transcription polymerase chain reaction (RT-PCR) was used. RESULTS: The TNF alpha serum levels on postoperative day 3, 6 were significantly higher in allograft group than in isogeneic graft group. RT-PCR analysis revealed that TNF alpha mRNA was present in allografts, while no TNF alpha mRNA expression occurred in isogeneic grafts. Our findings supported that TNF alpha plays an important role in the pathogenesis of acute allograft rejection, and suggested that sequential determinations of TNF alpha after operation may be of value in the prediction and diagnosis of acute rejection.

Complications of laparoscopic cholecystectomy in China: an analysis of 39,238 cases.

OBJECTIVE: To investigate the complications of laparoscopic cholecystectomy in China. METHODS: All Chinese articles about laparoscopic cholecystectomy published between April 1994 and November 1995 were identified through CMCC (Chinese Medical Computerized Contents). From more than 600 titles, 105 articles were screened for analysis. Another 21 articles from the 6th Biliary Surgical Congress and 300 cases from the General Hospital of PLA were added. A total of 39,238 cases of laparoscopic cholecystectomy from 91 hospitals were studied. RESULTS: Severe complications of laparoscopic cholecystectomy were identified in 409 (1.04%) patients, including bile duct injury (in 0.32% of patients), postoperative cystic duct leak (0.11%), postoperative bile leak (0.20%), peritoneal abscess (0.07%), bowel injury (0.06%) and postoperative hemorrhage (0.1%). Fourteen postoperative deaths (0.04%) resulted from operative injury. CONCLUSIONS: The data demonstrate that laparoscopic cholecystectomy is an operation associated with low morbidity and mortality rate, but bile duct injury is still a major problem. Complications of laparoscopic cholecystectomy can be minimized by improving operative procedure.
