Retroperitoneal bleeding due to a ruptured aneurysm of the middle colic artery.

In a 51-year-old man the middle colic artery ruptured. The patient had abdominal pain for several days, and then suddenly, a considerable intraperitoneal bleeding occurred. The aneurysm was extirpated and in the postoperative angiogram, numerous aneurysms were detected in the colic artery. Splanchnic artery aneurysm mainly occurs in renal artery, splenic artery and hepatic artery, but rarely in the mesenteric artery. The pathogenesis of mesenteric artery aneurysms is usually mycotic false aneurysm from prior aortic endoarteritis. In the present case, the etiology of the aneurysm was not confirmed.

Recent trend on the prevalence of tardive dyskinesia in Japan.

An epidemiological study was undertaken, taking a survey of 2,274 chronic schizophrenic patients hospitalized for 2 years or more in 17 mental institutions in Japan. The overall prevalence of tardive dyskinesia (T.D.) was 19.1%. The prevalence of TD differs considerably from one institution to the other, with a range of 0 to 36.1%. The longer the history of institution was, the higher the prevalence of TD. The prevalence showed a tendency to increase with increasing age. Long-term hospitalization concomitant with continuous neuroleptic medication seemed to play a role in the prevalence of TD. TD occurred with higher frequency in patients with somatic complications than in those without complications. Past drug history, including duration of treatment and responsible neuroleptics, has been considered to play an important role in the prevalence of TD. But, results of the present investigation showed that the present drug status might also influence on the appearance of TD. The facts obtained from the present study are of value to establish regimens mainly based on medical treatment for prediction and prevention of TD.

Monitoring of haloperidol serum levels and it's clinical significance.

Haloperidol serum levels were determined by the recently developed radioimmunoassay technique to elucidate the relationship between serum levels and clinical effects and that between serum levels and adverse effects. The serum levels of haloperidol in divided daily doses schedule reached to the steady state more rapidly (within 7 days) than in single dose schedule. The single daily dose schedule showed significantly wider diurnal variation than the divided daily doses schedule. No remarkable differences in serum concentration was noted between different types of oral haloperidol preparation (liquid, granule, tablet). No definite relationship was established between the serum concentration of haloperidol and incidence of tardive dyskinesia. No definite positive relationship between clinical effects and serum concentration of haloperidol could obtained.

The relationship between haloperidol blood levels and clinical responses.

Haloperidol blood levels in patients were determined to elucidate the relationship between blood levels and clinical effects. Determinations of haloperidol serum were performed by radioimmunoassay and the clinical status was estimated using BPRS. The relationship between haloperidol serum levels and BPRS total score was studied in 20 chronic schizophrenics currently receiving haloperidol treatment. The results did not show positive relationship between these two. Another 11 schizophrenic patients were treated with daily doses of average 6 mg of haloperidol. The patients were classified into improved and unchanged group according to the changes of BPRS total score. All 7 cases classified into the improved group had short duration of the illness less than one year. In this group, the levels were lower initially but gradually rised in paralled with the increase of extent of recovery on BPRS. Furthermore, improvement of psychopathology by means of decrease of BPRS total score was still found after haloperidol serum levels reached a plateau. The results did not support the view that high blood concentration of haloperidol might contribute to the clinical effects, but still suggested utility of measuring blood levels in acute or subacute patients being treated with haloperidol medications.

Myasthenia gravis with thymoma: analysis of and postoperative prognosis for 65 patients with thymomatous myasthenia gravis.

Sixty-five patients with thymomatous myasthenia gravis were investigated. Thymomas were present in 44% of the male patients and 19% of the female patients with myasthenia gravis. The incidence of thymomatous disease in male patients was higher than in female patients in all age groups. Eighty percent of men more than 50 years old and women more than 60 years old had myasthenia gravis with thymoma. Germinal center formation in the thymus of patients with thymomatous myasthenia gravis was positive in 91% and was high grade. The prognosis for patients undergoing extended thymectomy of thymomatous myasthenia gravis was significantly better than in those having transsternal simple thymectomy, but it was worse than the prognosis for patients with nonthymomatous myasthenia gravis. No increase in the rate of remission or palliation was seen one year after thymectomy. It is concluded that early thymectomy is effective in control of myasthenia gravis in thymomatous myasthenia gravis.

Effects of preoperative duration of symptoms on patients with myasthenia gravis.

In 80 patients with type II nonthymomatous myasthenia gravis who underwent extended thymectomy, we investigated preoperative duration of symptoms, prognosis after thymectomy, immunological findings, and germinal center formation in the thymus. Our findings included the following. First, the palliation rate after thymectomy ranged from 73 to 100% and was independent of the preoperative duration of symptoms. The remission rate was high in patients with a short preoperative duration. Second, the lymphocyte count of peripheral blood decreased as preoperative duration increased. Third, the percentage of positive reactions to purified protein derivative of tuberculin decreased as preoperative duration increased: 100% in the one-year group, 78% in the two-year group, 75% in the three-year group, and 56% in the four-year group. Fourth, the degree of germinal center formation in the thymus was higher in patients with a longer preoperative duration. The correlation between germinal center formation and preoperative duration was significant. Finally, the T-cell population in peripheral blood and immunoglobulin, and antibody to acetylcholine receptor in serum, had no significant relationship with the preoperative duration of myasthenia gravis.

Trans-FBS-channel ventilation during the flexible bronchoscope (FBS) examination in infant.

The most important problem during FBS examination in infants is respiratory care. A new ventilation method using the channel of the FBS is proposed for respiratory care during FBS examination in infants. An FBS with a channel of more than 1.2 mm in diameter was directly inserted into the trachea under general anaesthesia without an orotracheal tube. The first step of this ventilation is the flushing of pure O2 into the lung as inspiration through the channel of the FBS. The second step is the aspiration of expiratory gas through the channel by a vacuum pump. Ventilation during the FBS examination is maintained by the repetition of these two steps by the operation of a stop-cock. Experiments in dogs, and clinical use showed that this method of respiratory care is effective and safe with certain limitations. Clinically, inspection with the FBS could be performed continuously for five minutes in most cases.

Photo-oxidation of histidine residues in rat M1- and L-type pyruvate kinases.

Rat M1- and L-type pyruvate kinases were inactivated by photo-oxidation mediated by methylene blue at pH 8.0 and O degrees C according to first-order kinetics. The pH profiles of the inactivation rates of these isozymes showed that amino acid residues having a pK value of 7.0-7.5 were involved in the inactivation. Three histidine residues per subunit of M1- or L-type enzyme were destroyed by the photo-oxidation with complete loss of the enzyme activities. However, two of the three photo-oxidized histidine residues in the L-type enzyme were more important in the inactivation reaction. The kinetics of the partially inactivated L-type enzyme suggests that complete inactivation is achieved via an intermediate form having low affinity for phosphoenolpyruvate (PEP). These observations revealed the involvement of essential histidine residues of two different kinds in the catalytic mechanism of the L-type enzyme. In the photo-oxidation of M1-type enzyme, no intermediate form was observed. Addition of PEP or pyruvate to the reaction mixture markedly prevented the photo-oxidative inactivation of only the M1-type enzyme in the presence of K+ and Mg2+; the addition of ADP or ATP was ineffective even in the presence of both metal ions. This protective effect of PEP was counteracted by further addition of ATP but not by ADP. However, photo-oxidative inactivation of the L-type enzyme was not prevented even by the addition of PEP in the presence of both metal ions, owing to the low affinity for PEP at 0 degrees C, in spite of the presence of fructose 1,6-bisphosphate (Fru-1,6-P2).(ABSTRACT TRUNCATED AT 250 WORDS)

Effect of Clostridium perfringens beta toxin on blood pressure of rats.

Guanethidine treatment or adrenal medullectomy significantly inhibited the elevation in blood pressure induced by Clostridium perfringens beta toxin, and the combination of the two drastically reduced the pressure rise, to less than 19% of that in control rats. When rats were pretreated with tetrodotoxin or hexamethonium, the toxin-evoked rise was significantly inhibited. Elevation in blood pressure induced by the toxin in spinal rats tended to be less than that in control rats. When investigated by a microscopical technique, arteriolar constriction in the mesenteric vasculature was observed after the blood pressure elevation induced by the toxin reached a maximum. Blood flow in the skin decreased with an increase in blood pressure following intravenous injection of the toxin. It is concluded that beta toxin acts on the autonomic nervous system and produces arterial constriction.

Clinical efficacy, extrapyramidal symptoms and serum levels: influence of administration schedules and concomitant drugs on serum bromperidol concentrations.

Bromperidol (4 or 12 mg per day) was administered to 18 newly admitted schizophrenics and 29 chronic schizophrenic inpatients once or four times a day and the two dose schedules were compared. The bromperidol levels in the four-times-a-day group were significantly higher than those in the once-a-day group and the daily variation in the serum level of the agent was markedly wider in the latter than in the former patients. The incidence and severity of extrapyramidal symptoms were not significantly different between the two. A clear relationship was not present between the serum levels and the development of extrapyramidal symptoms. The bromperidol serum values were not correlated with the therapeutic response but, at more than 5 ng per ml, there might be a positive correlation between the bromperidol levels and clinical efficacy in the newly admitted schizophrenics. As for concomitant medication, levomepromazine may raise the bromperidol serum level. An anti-parkinson drug failed to depress the bromperidol level.

Antibody to acetylcholine receptor in myasthenia gravis: production by lymphocytes from thymus or thymoma.

In 13 of 17 myasthenic patients without thymoma and all 7 patients with thymoma, thymic lymphocytes produced antibody to acetylcholine receptor (AChR). Lymphocytes from the thymoma itself did not produce AChR antibody except in one patient. The rate of antibody production was higher in patients without thymoma than in patients with thymoma (32.5 and 3.9 fmol/10(6) cells/week, respectively). The rate of antibody production was related to the serum titer of AChR antibody (r = 0.7, p less than 0.001). Enrichment of B cells using a nylon wool column increased the rate of antibody production by thymic lymphocytes 1.3- to 8.0-fold.

Interrelation between plasma amino acid composition and growth hormone secretion in patients with liver cirrhosis.

We examined the possibility that abnormalities of growth hormone (GH) release in cirrhotic patients were related to a reduction in the ratio of branched chain amino acids (BCAAs) to aromatic amino acids (AAAs) in plasma. The intravenous infusion of 250 micrograms of thyrotropin-releasing hormone (TRH) caused a significant rise in plasma GH greater than 5 ng/ml and more than twice as much as the basal levels in 7 out of 15 patients (responders) but an insignificant rise in the remaining patients (non-responders). The difference in the basal GH level was not significant. The oral glucose load suppressed plasma GH in all of the normal subjects and 6 of 7 non-responders, while it was elevated in 6 of 7 responders and one of the non-responders. The ratio of BCAAs to AAAs in the plasma of cirrhotic patients was 1.21 +/- 0.38, which was significantly lower than that of normal subjects (3.31 +/- 0.42, p less than 0.01). In addition, there was a significant difference between responders and non-responders in the ratios (0.96 +/- 0.22 vs 1.42 +/- 0.36, p less than 0.05). An inversely significant correlation (p less than 0.05) between the ratios of BCAAs to AAAs in plasma and the peak levels of GH after TRH injection was observed when all subjects were combined, but no correlation was found between the ratios and the peak levels of GH after oral glucose loading. There were also significant correlations (p less than 0.01) between the ratios and various parameters including the serum albumin, cholinesterase and indocyanine green disappearance rate constant (KICG).(ABSTRACT TRUNCATED AT 250 WORDS)

Effects of 2-hydroxyoestradiol and 4-hydroxyoestradiol on gonadotrophin and prolactin secretion in women.

In order to investigate the effects of 2-hydroxyoestradiol (2-OHE2) and 4-hydroxyoestradiol (4-OHE2) on the release of human pituitary hormones, each steroid (500 micrograms single bolus) was injected to young women during the follicular phase. Both steroids resulted in a transient suppression followed by a small rise of LH and FSH. 4-OHE2 appeared to be more potent than 2-OHE2. Injection of only the vehicle caused no change in gonadotrophin levels. The morning fall of plasma Prl was common in young women as an expression of circadian variation. Although the Prl fall tended to be greater in the 4-OHE2 group, analyses failed to reach statistical significance.

Changes of platelets, serum lactic dehydrogenase, gamma-glutamyltranspeptidase, choline esterase and creatine phosphokinase levels in patients with Cushing's syndrome.

In order to identify non-endocrine laboratory tests of diagnostic value in Cushing's syndrome, we measured platelet counts and serum myogenic and hepatic enzyme levels in 10 patients with Cushing's syndrome and compared the findings with those of 15 obese patients without Cushing's syndrome. Patients with Cushing's syndrome had increased numbers of platelets, moderately elevated serum lactic dehydrogenase and gamma-glutamyltranspeptidase levels, and significantly lower creatine phosphokinase and choline esterase activities compared with those of obese control patients. We concluded that when several of these abnormal values were seen in obese patients the levels of suspicion for Cushing's syndrome should be high.

A case of chronic pancreatitis successfully treated by endoscopic removal of protein plugs.

A 56 years old male with chronic pancreatitis complained of intractable abdominal pain, anorexia, emaciation and peripheral edema. Medical treatment initiated only partial improvement in the general condition and hypoproteinemia. Endoscopic retrograde cholangiopancreatography revealed multiple filling defects in the dilated main pancreatic duct. Endoscopic aspiration of pure pancreatic juice yielded numerous protein plugs. The endoscopic removal of protein plugs from the pancreatic duct resulted in remarkable improvement in symptoms, laboratory findings and ERCP findings. We consider this procedure to be an important new treatment of chronic pancreatitis.
