[Extracorporeal cholelithotripsy, is it still justified as treatment of cholecystolithiasis?].

This prospective study evaluates the results of extracorporal shock-wave piezoelectric lithotripsy in 52 patients, 41 with radiolucent and 11 with calcified shell gallstones. The fragmentation rate was above 90% and the complication rate 31% particularly with pains or intolerance to the adjuvant dissolution therapy. At the end of the treatment, seven cholecystectomies were carried out among the 31 patients for who lithotripsy failed. After 18 months, the success rate was 23% and 12 patients only remained stone free. Nowadays, thanks to the new laparoscopic therapy, extracorporal shock-wave lithotripsy is no longer judged appropriate, especially because of its poor cost-effectiveness and high recurrence ratio.

[Permanent cardiac stimulation: evaluation of a 10-year experience].

This 10-year prospective follow-up study (1978-1988) covers 430 patients who received 463 permanent pacemakers in a regional hospital. One cardiologist established the indication for implantation and a general surgeons' team performed the operation. Mortality was very low (0.5%). Among the local complications (6.2%), infection was the most dangerous and often required repeated surgery or even removal of the entire pacing system.

[E. Delorme correction of rectal prolapse].

Rectal prolapse is a distressing condition. Among the multiple corrections described, the perineal approach has generally been abandoned. However, Delorme's procedure is a good, simple and efficient intervention with a low morbidity for certain elderly selected patients. The purpose of this paper is to recall the details of this technique and review its results.

[Reconstructive methods of the antero-lateral thoracic wall after extensive surgery of invasive tumors].

Local recurrence after conservative or enlarged surgery for breast carcinoma, or primary chest wall's tumors should first be approached surgically, chimio- and radiotherapy being used only later on. A precise local and general evaluation has to be made first, trying to determine whether there is or not an invasion of bony structures. When they are free of tumor, regional transfer of musculocutaneous flaps can usually cover the defect (latissimus dorsi, 1-2 rectus abdomini, pectoralis major); great omentum is used when this defect is too large. When ribs, sternum or deep structures are invaded, reconstruction uses successively: a mersilene mesh, bone cement, an omental flap covered with mesh skin grafts 2 days later. Surgical management of these difficult situations is most of the time only a palliative measure which gives these patients a better quality of life for the short time they still have got.

Mallet fractures.

Contrary to our observations, reports in the literature maintain that only a third of mallet finger deformities are caused by intra-articular fractures of the posterior margin of the distal phalanx. As functional impairment sometimes remains after treatment, there is still much discussion as to the relative merits of conservative and surgical management of this lesion. This article presents 21 cases of mallet finger deformities with fracture of the distal phalanx, of which 17 received surgical treatment. The operative technique is described, followed by a discussion of the results.

[[Clodronate therapy of hypercalcemia caused by hyperparathyroidism].

The diphosphonates, powerful agents which inhibit osteolysis, have been used parenterally with success to treat malignant hypercalcemia. 4 cases of anatomically confirmed primary hyperparathyroidism (age of patients 58-78 years) were admitted with a serum calcium of over 4 mmol/l in one case, over 3,5 mmol/l in a second case and over 3 mmol/l in the other two. The patients received an infusion of isotonic saline and dichloromethylene-diphosphonate (clodronate) parenterally in a dose of 500 mg a day. Serum calcium began to fall in all four patients 48 hours after the treatment was started. Intravenous clodronate therefore appears to be the treatment of choice for parathyroid crises and allows surgery of the adenomas in better metabolic conditions.

[Hernia recurrence--study of 100 case history].

One hundred cases of recurrent inguinal or femoral hernia are reviewed in order to find out some common causes of this important problem. The delay of recurrence is usually short and frequently technical errors are responsible for this surgical failure. Considering their own results, the authors propose a few easy solutions in order to lower the recurrence rate. A prospective study now in progress will help us to appreciate our present results.

The migrating spilled stone.

Two cases of late complications of laparoscopic cholecystectomy following parietal breach are reported. The escaped unretrievable infected lost gallstones behave subsequently like foreign bodies and migrate out of the peritoneal cavity. Two patients presented seven months and two years later with abscesses containing stones. Early recognition and effective surgical treatment are the only guarantee of success.
