[Cystic tumors of mediastinum].

Nine of twenty-four mediastinal tumors showed cystic lesions were operated at Kinki University Hospital. These nine mediastinal cysts included 5 of thymic cysts, 3 teratomas and one pericardial cyst. By the preoperative imagings, thymic cysts showed a smooth wall and homogenous content with water density. Teratoma consisted of cystic and solid parts, and the irregular thickness of the wall was found. One of these teratomas was ruptured into the pleural cavity, it has been proposed that digestive enzyme derived from pancreatic tissue with Langerhans island caused autolysis and necrosis with result in rupture. Pericardial cyst located at cardiophrenic angle mostly and which is useful on diagnosis.

[Influence of a fosfomycin combination on the anti-tumor effect of CDDP].

To elucidate the influence of fosfomycin (FOM) on the anti-cancer effect of cisplatin (CDDP), experimental chemotherapy of CDDP with or without FOM was performed. A human lung cancer cell line, HLC-1 serially transplanted into nude mice was treated with CDDP 10 mg/kg (i.p.). Tumor doubling time in CDDP group and CDDP + FOM group are 26.78 +/- 6.3 days and 33.90 +/- 11.82 days respectively, and they are significantly elongated compared to control group. Serum NAG level in CDDP + FOM group was significantly lowered. Therefore, we concluded that anti-tumor effect of CDDP was not diminished by FOM combination.

[A case of non-resectable thymoma which responded to the administration of adriamycin, vindesine and 5-FU].

Surgery and radiotherapy have been considered to be the primary therapies for malignant thymoma. However, the results of these therapies for invasive thymoma are poor. Recently, combination chemotherapy for invasive thymoma has been tried and good results have been reported. A 50-year-old female was admitted to our hospital. Her chest film showed a large anterior mediastinal tumor which was diagnosed as invasive thymoma. In order to remove the tumor, median sternotomy and 4th intercostal thoracotomy was done but the mass was found to have invaded to the adjacent tissues, and so only partial resection of the tumor was performed. Postoperatively, the patient was treated with Adriamycin, Vindesine and 5-FU, as a result of which the mass completely disappeared and CR was achieved. Our regimen, AVF, for non-resectable thymoma is therefore thought to be a potentially effective treatment.

Extraluminal air bubble on computed tomography.

We present two cases of gastrointestinal perforation associated with a tiny extraluminal air bubble on computed tomography (CT). The air bubble was located at the round ligament of the liver in a patient of perforated duodenal ulcer and at the intraperitoneal space adjacent to the perforated site of the anterior gastric wall in the other patient. A small amount of fluid was also seen at the round ligament in the second patient. The region of the round ligament of the liver should be carefully scrutinized for the presence of extraluminal air or fluid in patients with acute abdominal symptomatology.
