Aspergillus brain abscess in a patient with acute myeloid leukaemia successfully treated with voriconazole.

Cerebral aspergillosis is associated with very high mortality in immunocompromised hosts. Conventional antifungal agents like amphotericin-B and itraconazole are almost ineffective in cerebral aspergillosis. Newer azoles have been shown to penetrate the blood, cerebrospinal fluid barrier and achieve effective fungicidal concentrations. These newer azoles may change the outlook of this fatal condition. We report here a patient with cerebral aspergillosis who was successfully treated with voriconazole.

Studies on oil palm trunks as sources of infection in the field.

Diseases of oil palm caused by Ganoderma boninense are of major economic importance in much of South-East Asia. This paper describes results from an ongoing field trial concerning the spread of the pathogen from artificially inoculated trunks used to simulate spread from windrowed trunks. Three planting distances for bait seedlings revealed that the closer the seedling was planted to the source of inoculum the sooner it succumbed to the disease. However, infection only occurred when the trunks were mounded (covered with soil), and seedlings planted around uncovered trunks (at any distance) have showed no symptoms of disease to date. Isolates are being collected from infected plants and molecular analysis is being undertaken to give more information on the spread of the pathogen.

Investigations on the causes of upper stem rot (USR) on standing mature oil palms.

Three different trials to examine the cause of upper stem rot (USR) infection in oil palm failed to achieve any infection. In the first experiment, inoculum was applied as colonised rubber wood blocks or as spore suspensions. In the second experiment, particular attention was given to ensure that the Ganoderma spores were freshly collected to maintain viability but no infection was observed around the inoculation sites of any of the different oil palm tissues treated. Lastly in the third experiment, both monokaryotic and dikaryotic mycelial cultures were applied directly to cut fronds, which were protected with a moist covering, but no infection was detected after more than two years. Failure to achieve infection by direct inoculation would indicate that USR does not arise from direct infection of living tissues by Ganoderma spores or mycelium, this is probably because of insufficient inoculum potential to cause infection. It is suggested that USR infection is achieved only when a sufficiently large source of inoculum has built up in dead material, probably in frond axils, and this allows invasion of the living tissues.

Dexpanthenol enemas in ulcerative colitis: a pilot study.

OBJECTIVE: To test the hypothesis that topical administration of pantothenic acid, a precursor of coenzyme A, might result in increased tissue levels of coenzyme A, improvement of fatty acid oxidation, and amelioration of ulcerative colitis. MATERIAL AND METHODS: In an open-label pilot study, three patients with active left-sided ulcerative colitis received nightly enemas that contained 1,000 mg of dexpanthenol for 4 weeks. Before and after the study, patients submitted stool specimens for short-chain fatty acid analysis and urine collections for measurement of pantothenic acid and dicarboxylic acids; they also underwent flexible sigmoidoscopy for procurement of biopsy specimens for histologic examination and measurement of colonic coenzyme A activity. A clinical disease activity index and histologic disease activity index were used to assess response. RESULTS: Despite increases in urinary pantothenic acid, no significant changes were found in colonic tissue coenzyme A concentrations, fecal short-chain fatty acid concentrations, or urinary dicarboxylic acid concentrations. Moreover, no significant changes in clinical or histologic disease activity were noted. Although stool frequency and rectal bleeding remained unchanged, all patients noted increased abdominal cramping, and one patient had an increased extent of disease. CONCLUSION: Topically administered dexpanthenol seems to be absorbed, but at the dose used in this study, it did not influence concentrations of colonic coenzyme A activity, fecal short-chain fatty acids, or clinical response in patients with active left-sided ulcerative colitis.

Biochemical analysis and electron microscopic study on intracellular virions in NIH/3T3 mouse cells chronically infected with Moloney murine leukaemia virus: effect of interferon.

Radioactively labelled virus particles of intracellular origin were isolated from the cytoplasmic fraction of disrupted NIH/3T3 cells chronically infected with Moloney murine leukaemia virus [NIH/3T3 (MLV)]. Interferon (IFN) treatment for 48 h, which arrested more than 90% of virus release, resulted in a remarkable accumulation of these intracellular virions. However, no major effect of such treatment was apparent on their structural properties. Transmission electron microscopic examination revealed that these intracellular virions were located within cytoplasmic vacuoles. IFN treatment resulted in a considerable increase in the number of virus-containing vacuoles, as well as the total number of vacuolar virions. It seems that IFN inhibits the final release of vacuolar virions from the cells, thus leading to their intracellular accumulation.
