Blood pressures (BPs) were measured with standardized sphygmomanometers in 434 Japanese boys and girls living in the town of Hisayama. Simultaneously, data on pulse rate, weight and height were obtained. Out of the original 434 subjects, data were obtained repeatedly for 5 years in 280 subjects. BP levels were significantly correlated with weight in those aged 14-15 and also 19-20 years, but correlation coefficients were small. During the 5-year period, the mean systolic and diastolic blood pressure (SBP & DBP) increased significantly in both sexes, but the increments were greater in boys. Both SBP and DBP at 14-15 years of age were significantly correlated with data taken 5 years later for both sexes, and subjects with a higher initial BP (more than 90th percentile of the distribution) tended to have a higher BP after 5 years. SBPs after 5 years were independently correlated with initial SBP levels and changes in QI (D-QI) in both sexes. On the other hand, DBPs after 5 years were independently correlated with initial DBP levels and height for boys, and initial DBP levels and D-QI for girls.
Long-term prognosis of borderline hypertensives was studied in a prospective population survey carried on since 1961 in the town of Hisayama, Japan. Five consecutive BP recordings on 1621 subjects aged 40 and over were obtained at entry, and the variability in BP between the first and fifth readings was taken into account when classifying the subject into categorical groups. Even with an estimated variability in BP in several measurements on one occasion, a large fluctuation in BP was observed in both the borderline hypertensives and the normotensives. Borderline hypertensives with a transient elevation in BP more frequently died from cardiovascular disease than did those without BP elevation, as estimated by the long-term cumulative mortality. However, there was no difference in the frequency of hypertension-related organ damage between these two groups at entry.
Cefmetazole (CMZ) was administered to 102 infectious cases in the gynecoobstetric patients, and the basic and clinical studies have been performed. The main findings obtained in the present study are: Amont 78 cases of CMZ administration immediately after the initial infection (A group), 42 remarkably effective (53.8%), 36 effective (46.2%) cass were observed. When other drugs were not effective after the initial infection, CMZ was administered (B group) to 24 cases, and 15 remarkably effective (62.5%), 9 effective (37.5%) cases were observed. overall effectiveness in the A and B groups was 57 remarkably effective (55.9%) and 45 effective (44.1%) cases, which are very excellent clinical effects. Among 102 cases, the pathogenic bacteria were found in 55 cases, and 29 cases out of the 55 (52.7%) showed infections with E. coli and with other bacteria having mixed infections. The effect of CMZ to E. coli as judged by MIC was excellent, providing the excellent clinical results. E. coli and other Gram-negative pathogenic bacteria in the B group showed resistance to ABPC, CEX, CEZ and CET, and after administering CMZ, all cases showed disappearance of these bacteria, but the increase in the resistant bacteria for CEX, CET and CEZ was obviously shown. Subjective and objective adverse effects and clinical laboratory analysis showed no abnormal effect and values due to CMZ, and it was true of the of the case received 140 g in total of CMZ over 35 days, 4 g/day. It may be conducted from the above findings that CMZ is very effective and safe antibiotic agent in the infections in the gynecoobsterics .
A study involving the determination of total fibrinogen and high molecular weight (HMW) fibrinogen was performed with the glycine precipitation method. Ninety patients in different trimesters of pregnancy and 37 non-pregnant controls participated in the study. Total fibrinogen was found to increase during pregnancy until delivery, after which it declined gradually, peaking once more on the 3rd day of puerperium. HMW fibrinogen showed a similar pattern during pregnancy and puerperium, but markedly increased at the time of delivery. These elevated values during pregnancy indicate a significant change in the fibrinolytic system, which is believed to be depressed during pregnancy, enhanced during the postpartum period, and stabilized after the 3rd day of puerperium.

Six hundred and fifty-eight autopsy cases of both sexes in a general population of Hisayama town (autopsy rate 85%) were examined to determine the frequency and causes of sudden and unexpected death. Seventy-three sudden deaths (11.1%) were identified when sudden death was defined as death occurring within 24 hours after the onset of clinical symptoms. Thirty-nine deaths (54%) were due to intracranial hemorrhage (cerebral and subarachnoid) and 17 (23%) were due to coronary heart disease. An instantaneous death occurred within one hour was observed in 20 of the 73 cases (27%), of which 8 were attributed to coronary heart disease (40%) and one (0.5%) to subarachnoid hemorrhage. Pathologic evidence of recent myocardial infarcts or coronary thrombosis was found less frequently in cases who died within one hour than in those who died between one and 24 hours. An annual incidence of sudden death due to various etiologies, calculated from the population study on Hisayama residents who were aged 40 or over, was 1.7/1,000. Deaths from coronary heart disease in this community were much less common as compared with those reported in Western communities.

To elucidate the relation of hematocrit (Hct) to the incidence of cerebral infarction, a prospective follow-up study of 16 years (1965-81) was performed in a general population sample of 1220 Hisayama residents aged 44 and over, of both sexes. Most of the subjects who died during the follow-up period were autopsied, the rate being 89.0%. Hct decreased with advancing age in men, but not in women. The average value for Hct was significantly lower in women than in men. According to the mean value +/- 1 SD of Hct, the subjects were grouped into 3, in each sex as follows: low (less than 35%), normal (35-45%) and high (greater than or equal to 45%) for men, and, low (less than 30%), normal (30-40%) and high (greater than or equal to 40%) for women. During the follow-up period, cerebral infarction occurred in 117 patients. The cumulative incidence of cerebral infarction in the low Hct group for men was the lowest, even after adjustments for age and blood pressure. Conversely, the incidence in the low Hct group of women was significantly higher than that in the normal Hct group and was consistently increased with time during 2-5 years of the follow-up. After the 6th year or later, however, the incidence was gradually but significantly increased in the high Hct group, compared with the normal Hct group. Since Hct levels were related with other variables such as serum total cholesterol, serum total protein, Quetelet index and prevalence of hypertension in both sexes, heavy alcohol consumption in men, and glucose intolerance in women, such variables were taken into account using Cox's proportional hazards regression model.(ABSTRACT TRUNCATED AT 250 WORDS)
During a 20-year follow-up of 1,621 men and women aged 40 and over in Hisayama, Japan, 18 were found to have suffered transient cerebral ischemic attacks (TIAs) determined by clinical symptoms based on criteria proposed by the US Joint Committee for Stroke Facilities. The average annual incidence rate for the first TIA was 0.56 per 1,000 residents. Age and high blood pressure were strong determinants of TIAs for men. Nine of the 18 cases with TIAs (50%) subsequently developed cerebral infarction, an incidence significantly higher than that in the 1,603 subjects without TIAs (10.9%). Lacunar infarcts were most commonly found on pathologic examination of patients with TIAs who had had a subsequent stroke.
