Plasma nitrite/nitrate and endothelin-1 concentrations in neonatal sepsis.

AIM: To determine the changes in plasma nitrite/nitrate (NOx) and endothelin-1 (ET-1) concentrations during neonatal sepsis. METHODS: In a prospective study, 60 consecutive newborns meeting the criteria for sepsis and without receiving exogenous nitric oxide (25 haemoculture-positive [HC+] and 35 haemoculture-negative [HC-]) were compared with 68 healthy newborns (46 full-term and 22 preterm). NOx and ET-1 concentrations were measured in each newborn within 48 h of diagnosis of sepsis and then every third day up to three determinations. SNAP-II and SNAPPE-II severity scores were performed at the moment of highest clinical severity. RESULTS: At the beginning of the sepsis period, controls and septicaemic newborns had similar NOx and ET-1 levels, with the exception of infants with severe HC+ sepsis. Throughout the sepsis period, NOx increased in moderate HC+ sepsis and decreased in HC--sepsis, reaching a significant difference at the end of the study period (59.9 +/- 72.7 vs 33.9 15.3 micromol/L; p = 0.036). Meanwhile, ET-1 in newborns with severe HC+ sepsis remained higher than that in the moderate HC+ sepsis group and HC--group, reaching significant differences in all the periods. The highest ET-1 value was positively correlated with SNAP-II and SNAPPE-II scores. CONCLUSION: NOx concentrations increased throughout the neonatal HC+ sepsis period, reaching significant differences after 7-9 d. The highest ET-1 levels in neonatal HC+ sepsis emerged before the NOx peak, at 3-5 d, and later decreased. Only newborns with severe HC+ sepsis presented a significant increase in ET-1 concentrations from the beginning of the septicaemic process.

[Recurrent infection by Streptococcus agalactiae].

BACKGROUND: To study the factors implicated in the infectious process (host, microorganism and antibiotic) of a newborn early sepsis by S. agalactiae that suffered a reactivation at day five from discharge. METHODS: Description of two episodes of newborn sepsis by S. agalactiae corresponding to the same patient and microbiologic study of the isolated strain: typing by "genomic macrorestriction" and antibiotic tolerance by "timed killing curves". RESULTS: It was demonstrated that both strains of S. agalactiae type la/c belonged to the same clone as well as the tolerance to ampicillin of the strain. DISCUSSION: This sort of infections processes in the newborn are very serious and there is possibility of relapse. Thus, it is important to study the ethiologic agent and its relationship with antibiotics, in order to stablish the best treatment regimes, avoiding the possibility of relapses as the case we have described.

Assessing the effectiveness of X-ray contrast media for abdominal computed tomographic examinations: comparison of four low-osmolality agents.

The objective of this investigation was to assess the effectiveness of four iodinated X-ray contrast media for abdominal computed tomographic (CT) examinations. Fifty-three patients were prospectively randomized to receive iohexol 300 mgI/ml (100 ml, n = 17), ioversol 320 mgI/ml (100 ml, n = 13), iopromide 300 mgI/ml (75 ml, n = 12), or iopentol 300 mgI/ml (100 ml, n = 11) to perform a dynamic contrast-enhanced abdominal CT. Image-enhancement profiles for the liver, aorta, and vena cava were studied. The maximum liver enhancement, the time to maximum liver enhancement, and the area under the hepatic enhancement-time curve (AUC) were determined for each examination. Liver-enhancement profile showed significant differences between the four contrast agents, with lower values for iopromide towards the final part of the CT examination (P < 0.05). Hepatic peak values were attained earlier for iopromide, although these were lower than those produced by any other of the agents evaluated in this study. Iopentol produced fast and intense hepatic peaks. Consequently, high AUC values were obtained with iopentol, low values were obtained with iopromide (P < 0.05), although this can be explained by the lower amount of contrast medium contained in the commercial vial and administered to the patient (75 ml vs 100 ml). When normalized to a 100 ml dose, the AUC value for iopromide becomes even higher than the average of the other three agents (P = 0.05). Ioversol, although available and administered as a more concentrated solution (320 mg/ml), was comparable to the less concentrated iohexol and iopentol (300 mgI/ml).(ABSTRACT TRUNCATED AT 250 WORDS)

[Spontaneous dissection of the internal carotid artery. A report of two cases].

INTRODUCTION: Dissection of the internal carotid artery is a condition with a broad clinical spectrum. In absence of the classical triad of cervical pain, oculosympathetic ipsilateral paralysis and ischemic cerebral symptoms a considerable index of clinical suspicion is necessary to make a diagnosis. It is therefore considered to be probably underdiagnosed. The development of new techniques for neurovascular investigation, particularly ultrasound and magnetic angioresonance have improved the possibilities of diagnosis. CLINICAL CASES: In this paper we describe two cases of dissection of the internal carotid artery representing different clinical findings in the same condition. One patient presented with unilateral paralysis of the XI, X and XII cranial nerves and the other with transient ischemic cerebral accidents. In both cases the provisional diagnosis was made in function of the duplex findings of the supra-aortic trunks and confirmed by magnetic angioresonance studies. Both patients made satisfactory progress with complete recanalization of the vessels and no recurrence of symptoms after several months of follow-up. CONCLUSION: We discuss the different clinical characteristics and findings of the neurovascular examinations.

[Percutaneous nephrostomy. Review of 92 cases].

Presentation of 92 nephrostomies percutaneous in 70 patients. Nephrostomy was bilateral in 5 cases. Three patients had collections of renal origin (2 urinomas and 1 abscess) which were treated percutaneously. The indication for nephrostomy was in all cases obstruction of the collector system, with one exception where the indication was ureteral fistula. Origin was lithiasis in 53 cases, 15 cases vesical carcinoma, 3 post-surgical iatrogenic stenosis, 1 fistula and formation of urinoma after lithotrity, 5 prostate carcinoma infiltration, 8 infectious origin, 4 congenital and 1 due to glandular cystitis. 9 patients were monorenal. In 6 patients leucocytosis was present, 4 has fever, and in 11 coexisted fever and leucocytosis. High creatinine and urea levels were present in 55.7% and 41% patients, respectively.. Renal puncture through lower calyceal group was the preferred option. Purulent urine was obtained in 17 cases. With regard to nephrostomy complications haematuria occurred in 30 cases (only in 9 it lasted more than 1 day); and there was 1 case of pararenal haematoma. Pain was significant in 12% procedures and contrast extravasation resulted in 14% cases (in all of them proper positioning of nephrostomy catheter was possible). Spontaneous expulsion and the subsequent replacement of the catheter occurred in 12 cases. On average, residence time of catheter was 44.8 days. In all cases both the patient's signs and symptoms, and the laboratory results were improved.
