The ANK device: a new approach in the treatment of the fractures of the lateral malleolus associated with the rupture of the syndesmosis.

The ANK device was developed for the treatment of fractures of the lateral malleolus occurring with rupture of the syndesmosis. While it provides the anatomic reduction of the fracture and the syndesmosis, it allows the physiologic movements of the fibula. It is not used for comminuted fractures of the lateral malleolus and in cases where fibular medullary canal is narrow. We included forty-nine patients who had the ANK device applied and at least 2 years follow-up. The mean follow-up was 41 months (range 24-124). The fractures were evaluated according to the Lauge-Hansen classification; 25 cases were evaluated as supination-external rotation, 11 cases were pronation-abduction, and 13 cases were pronation-external rotation type fractures. There were also 46 fractures of the medial malleolus and three ruptures of the deltoid ligament. Twenty-nine (59,2%) patients were evaluated as excellent, 12 (24,5%) as good, 5 (10.2%) as fair and 3 (6.1%) as poor. Arthrosis was observed in 3 (6.1%) of the patients.

Osteosarcoma of the pelvis.

Over a 25-year period we have treated 36 patients with osteosarcoma of the pelvis. Of the tumours, 24 (67%) were primary osteosarcomas and 12 (33%) arose either after irradiation or in association with Paget's disease. Six patients had a hindquarter amputation and 12 were treated by a limb-salvage procedure with intrapelvic excision. The five-year survival rate of all the patients with pelvic osteosarcoma was 18%, while for 17 treated by chemotherapy and surgery it was 41%. The prognosis for patients presenting with metastases or with secondary osteosarcoma was appalling and none survived after 29 months. No patient over the age of 50 years when seen initially survived for a year. Youth and a good response to chemotherapy along with complete surgical excision offer the best chance of cure.

Multiple diffuse fibrosarcoma of bone.

Multiple diffuse fibrosarcoma of bone is a very rare condition, with only 3 cases reported. Its presentation is like that of a multiple myeloma or a metastatic tumor, with multiple osteolytic lesions, and its extent is limited to the bones. Disseminated involvement of the vertebrae was detected in a 26-year-old man, suggesting a metastatic tumor; however, no primary organ or soft-tissue mass was identified. The histopathologic findings of the biopsy material were consistent with fibrosarcoma. Despite treatment, the patient died within 15 months.

Endoprosthetic replacement for primary malignant tumors of the proximal femur.

Fifty-four patients had endoprosthetic replacement of the proximal femur after resection of primary malignant or aggressive bone tumors. The mean age of the patients was 41 years. The average duration of followup was 9 years (range, 5-24 years). At the time of this review 35 patients are surviving. There were six revisions of the acetabulum and seven revisions of the femoral component. The survivorship of the prostheses without revision was 77% at 10 years falling to 57% at 20 years. There was a 13% risk of amputation and a 1.8% incidence of infection. The mean of the functional scores of the 35 living patients was 83% (range, 60%-97%). This study showed that successful long term results can be obtained with cemented massive endoprostheses despite the incidence of recognized complications including polyethylene wear and aseptic loosening.

Avascular necrosis of the femoral head in childhood: the results of treatment with articulated distraction method.

The treatment of avascular necrosis of the femoral head in children is still being debated. We performed articulated distraction in 11 children with avascular necrosis of the femoral head between March 1994 and February 1997. The reasons for avascular necrosis were: Perthes disease in eight patients, fracture of the femoral neck in two, and complication arising because of treatment of developmental hip dysplasia in one. Mean age was 7.9 years. Fixator duration time was 3.5 months and mean follow-up was 32 months. At follow-up, the femoral head was spherical and congruent in four patients, aspherical and congruent in five, and aspherical and incongruent in two according to Stulberg's criteria. The aim of the articulated distraction method is to neutralize muscle and weight-bearing forces, to prevent stress fractures of subchondral immature bone, to encourage synovial circulation, and to promote creeping substitution.
