Ovarian cortical transplantation may be an alternative to hormone therapy in patients with early climacterium.

OBJECTIVE: To discuss the 18-month follow-up results of fresh ovarian cortical tissue transplant. DESIGN: Case study. SETTING: Academic medical center. PATIENT(S): A 46-year-old patient who was operated on for uterine fibroids, and who then received an ovarian tissue transplant in July 2003. INTERVENTION(S): Serum hormone levels were measured at 3-month intervals. MAIN OUTCOME MEASURE(S): Follicular development evident by ultrasound examination; serum hormone levels (FSH, LH, E2). RESULT(S): Preoperative and 3rd-, 6th-, 9th-, 12th-, 15th-, and 18th-month hormone levels were, respectively, as follows: FSH: 9.06, 79.5, 13.7, 16.66, 51.91, 44.37, and 24.17 mIU/mL; LH: 5.91, 33.92, 8.78, 21.83, 38.31, 40.85, and 22.4 mIU/mL; E2: 166, 46, 48, 117, 31, 14.4, and 137.7 pg/mL. Folliculogenesis was confirmed by ultrasonography at the 6th, 9th, and 18th months during the follow-up period. CONCLUSION(S): Fresh ovarian autotransplantation may be a logical alternative for hormonal support for a specific patient group.

A transverse rectus abdominis myocutaneous (TRAM) flap for reconstruction of a large deformity in the buttock: a new indication for a TRAM flap.

A rectus abdominis myocutaneous flap has been used to repair the abdominal wall, chest wall, sternum, breast, and groin. We describe a patient in whom a large deformity in the buttock caused by a road crash was repaired with a transverse rectus abdominis myocutaneous (TRAM) flap. Distally-based TRAM flaps provide a good and reliable way of reconstructing the buttock. Its advantages are a long arc of rotation and well-vascularised bulky tissue that serve as a partition and promote quick healing of the defect. However, the flap is not the first choice for traumatic and infected wounds where fatty tissue is not desired.

Low-volume tumescent liposuction does not change plasma malondialdehyde levels: a preliminary study.

To evaluate the effects of low-volume tumescent liposuction on plasma malondialdehyde (MDA) levels, a controlled clinical study was conducted. Venous blood samples were taken just before and 10 min after the completion of tumescent suction-lipectomy for 15 patients. The means +/- standard deviations for patients' data were as follows: of patients' age (29.81 +/- 13.2 years) aspirate volume (1,250 +/- 450 ml), and body mass index (24.5 +/- 1.2). There was no significant difference between the preoperative and postoperative plasma malondialdehyde levels (p > 0.05). The results show no evidence of increased lipoperoxidation in the systemic circulation during low-volume tumescent liposuction.

Correction of buttock ptosis with anchoring deepithelialized skin flaps.

A new technique for correction of a buttock ptosis resulting from a degloving injury is presented. With this technique, the deepithelialized crescent-like zone in lateral thigh redundancy anchored to gluteal fascia through three separate incisions as used for creation of a new gluteal crease. The patient's satisfaction was good with long-term follow-up evaluation. Infragluteal fold reconstruction is achieved by reconstruction of the buttock's sustaining apparel through deepithelialized skin flaps anchored to deep gluteal fascia.

Fresh autologous transplantation of ovarian cortical strips to the anterior abdominal wall at the pfannenstiel incision site.

OBJECTIVE: To describe an open surgical technique for transplanting fresh ovarion tissue to the anterior abdominal wall at the incision site and to determine whether ovarian function would be restored after transplantation. DESIGN: Case study. SETTING: Academic medical center. PATIENT(S): A 44-year-old patient who underwent an operation for uterine fibroids. INTERVENTION(S): Microsurgical reconstruction of ovarian cortex and its transplantation to the anterior abdominal wall at incision site of Pfannenstiel. MAIN OUTCOME MEASURE(S): Follicular development evident by ultrasound examination; restoration of serum FSH and LH levels to nonmenopausal range; and disappearance of menopausal symptoms. RESULT(S): Early postoperative FSH, LH, and E(2) levels showed that menopause was confirmed. Postoperative hormone levels at months 2, 3, and 6 were as follows: FSH: 77.86, 79.50, and 13.70 mIU/mL; LH: 34.60, 33.92, and 8.78 mIU/mL; E(2): 29, 46, and 48 pg/mL. The patient is still followed-up for postmenopausal status. CONCLUSION(S): Autotransplantation of cortical strips to the anterior abdominal wall at the incision site without further incision can be a logical solution for the patients at early climacteric age.

Three new cases of disorganizationlike syndrome: one with accessory extrophia vesicalis.

The authors report 3 unrelated Turkish cases of disorganizationlike syndrome. All of these patients have accessory limbs, and 2 of them have accessory genitourinary structures. Interestingly, one of these patients has extrophia vesicalis of accessory bladder. None of them have chromosomal abnormality. Here the authors present distribution of findings of these cases.

Subglanular stricture: rare penile anomaly resulting from circumcision.

Circumcision is a commonly performed minor surgical procedure; however, it may lead to serious complications if performed by poorly trained health care providers. In this case report, a subglanular stricture causing a mushroom-like deformity resulting from circumcision is presented.

Pai syndrome: an adult patient with bifid nose and frontal hairline marker.

OBJECTIVE: A 17-year-old previously unreported patient with Pai syndrome is described. The boy had median cleft of upper lip, a polypoid skin mass over the columella, a minimal cleft of the upper central incisors, frontal alopecia of the anterior hairline, and bifid nose. Magnetic resonance imaging showed pericallosal lipoma. No mental retardation was present, and a chromosomal study showed normal male 46, XY karyotype.

Cutaneous anthrax of the hand and its reconstruction with a reverse-flow radial forearm flap.

Bacillus anthracis infection can lead to necrosis in tissues and may manifest as a fatal disease in human beings. The authors present a patient with a large area of skin necrosis on the dorsum of the hand that was reconstructed with a reverse flow-through radial forearm flap, and they discuss the relevant literature. To the authors' knowledge, this is the first published report of such extensive necrosis resulting from anthrax limited to the extensor retinaculum of the hand.

Psoriasis vulgaris and Poland's syndrome in the same patient.

Most common anomalies associated with Poland's syndrome are in the chest wall. This type of anomaly causes important psychological distress, especially in adolescent girls. A 19-year-old female with Poland's syndrome having psoriatic plaques on her trunk, knees, and elbows is reported in this paper. Such an association of Poland's syndrome with psoriasis has not been described previously.

Augmentation mammaplasty for mammarial hypotrophy and ptosis in a patient with ichthyosis.

A 30-year-old female with mammarial hypotrophy was diagnosed with non-bullous congenital ichthyosiform erythroderma (NBCIE) with dermatologic evaluation. Her mammarial hypotrophy and ptosis was corrected with a breast implant. Although there is a tendency to skin infections and increased skin fragility in the ichthyotic patients, breast augmentation with silicone prosthesis is well-tolerated by the patient.
