Pepsin acid correlation in patients with Zollinger-Ellison syndrome.

In 3 patients with Zollinger-Ellison syndrome, gastric pepsin and acid secretion, and pepsin acid correlation were studied. The results obtained are as follows. In nocturnal secretion, and histamine-, insulin- and gastrin-stimulated secretion, pepsin hypersecretion was not as conspicuous as acid hypersecretion, and so pepsin acid correlation showed low levels of less than 1.0. Not only before operation but also after vagotomy or distal gastrectomy, the same results were obtained. These results suggest that both before and after operation Zollinger-Ellison syndrome should be suspected in patients with pepsin acid correlation of less than 1.0 in nocturnal secretion or stimulated secretion.

Influence of obstructive jaundice on gastric mucosal barrier in dogs.

The influence of obstructive jaundice on the gastric mucosal barrier was studied before and after ligation of the common bile duct using 21 mongrel dogs with a Heidenhain pouch. The gastric mucosal barrier was examined using gastric mucosal permeability of ionic net fluxes of Na+ and H+ (delta Na+, delta H+), and transmucosal electrical potential difference (PD) during the irrigation of the pouch with various test solutions. No significant differences in delta Na+ or delta H+ during the irrigation with 100 mN HCl were found between before and after obstructive jaundice. Almost the same results were observed during the irrigation with 150 mN HCl. Obvious increases in delta Na+ and delta H+ were found, when 20 mM sodium taurocholate (taurocholate) was used in addition to 100 mN HCl. No differences were, however, found between before and after obstructive jaundice. The PD during the irrigation with 100 and 150 mN HCl showed about the same levels of -50 mV. The PD decreased early during the irrigation with 20 mM taurocholate in addition to 100 mN HCl, but no differences were found in these values between before and after obstructive jaundice. Dose-response relationships were found between the concentration of taurocholate in addition to 100 mN HCl, and delta Na+, delta H+ and sigma PD/15 min, while no differences in ED50 of taurocholate for delta Na+, delta H+ or sigma PD/15 min were observed between before and after obstructive jaundice. These results suggest that obstructive jaundice itself exerts no influence on the gastric mucosal barrier with respect to gastric mucosal permeability and PD in dogs.

[Gastric mucosal defensive factor in various gastric diseases--with special reference to the influence of the operative insult and obstructive jaundice].

Gastric mucosal defensive factor plays an important role in the pathogenesis of various gastric diseases as well as aggressive factor such as acid. Gastric mucosal defensive factor of various gastric diseases and the relation to acid secretion were studied. The influence of operative insult and obstructive jaundice on gastric aggressive and defensive factors was also studied. Defensive factor was investigated using gastric transmucosal potential difference (PD). The results obtained were as follows: PD showed low value in the gastric diseases with the mucosal lesion such as gastric ulcer, stress ulcer and bleeding gastric ulcer. Particularly the last two diseases, which had active mucosal bleeding, showed remarkably low values. PD tended to increase following acid secretion but had no significance. Operative insult seemed to cause transient increase of intragastric pH and deterioration of gastric defensive factor, and the change was enhanced by obstructive jaundice.

The effect of ursodeoxycholic acid on biliary bile acid composition in patients with cholesterol gallstone.

To elucidate the role of conjugated biliary bile acids in gallstone dissolution, the acids in bile were determined by high-performance liquid chromatography before and after the treatment with ursodeoxycholic acid for 3-26 months in patients with gallstone. The stone-dissolving effect of ursodeoxycholic acid was confirmed in 7 of 10 patients and the lithogenic index lowered significantly after the treatment. The compositions of cholate, chenodeoxycholate and ursodeoxycholate were about 33, 45 and 4%, respectively, in the control and pre-treatment groups. In the post-treatment group, a markedly low value was observed in primary bile acids both glycine-conjugates and taurine-conjugates, especially in cholate, with a significantly high value of ursodeoxycholate (p less than 0.001) of both conjugates. On the other hand, no difference was observed in the composition of deoxycholate with significantly low percentage of taurine-conjugates compared with that in the pre-treatment group. The ratio of glycine- to taurine-conjugated bile acids showed a significantly higher value in the post-treatment group than in the pre-treatment group (p less than 0.001) and the control group (p less than 0.005). The bile specimens were measured concomitantly by gas-liquid chromatography and the results were compared with those of high-performance liquid chromatography. The mean value of total bile acids, the ratio of cholate to chenodeoxycholate and the ratio of glycine- to taurine-conjugated bile acids obtained by the former analysis procedure represented about 57, 80 and 115% of those of the latter. It is concluded that the high G/T value seems to have a role in the dissolution mechanism.

Studies on muscular strength-myogram (MSMG). Part 1. A new device of apparatus for measuring muscular strength-myogram.

1. A new apparatus, consisting of a strain gauge and a carrier amplifier, was devised for measuring back muscular and shoulder arms strength by the author. This apparatus was applied for analysing myogenic disorder related to muscular fatigue or pain. 2. Muscular strength with load and its myogram were recorded simultaneously, and their changes during the exertion were analysed. 3. Muscular strength-myograms of sternocleidomastoid muscle, upper trapezius muscle, deltoid muscle, rhomboid muscle and lumbodorsal muscle were observed from the back muscular strength-myogram, and those of teres minor muscle and major pectoral muscle from the shoulder arm strength-myogram. 4. It seemed paradoxically that the amplitude of action myogram increased in proportion to the grade of lowering of muscular strength caused by muscular fatigue or pain.
