[The usefulness of UFT administration in colorectal cancer with emphasis on postoperative administration (Part 3)].

To investigate the usefulness of the postoperative administration of UFT for colorectal cancer, 109 patients with a history of colorectal cancers from Nagasaki University First Department of Surgery and seven affiliated facilities were selected as subjects. After administering UFT 400 mg/day to both A and B groups one week prior to surgery, MMC 20 mg during surgery and 10 mg on the first day after surgery, the groups were divided into an A group, not administered UFT, and B group, administered UFT 400 mg/day. In addition, both groups were administered MMC 6 mg/m2 six times a month starting the first month after surgery. Although the A group consisted of 54 patients and the B group 55, 52 patients of the A group and 46 patient of the B group, for a total of 98, qualified as subjects for this investigation, and the following results were obtained. 1) Postoperative administration of UFT was useful in prolonging the survival period in non-curable resection cases, in Dukes C group cases, and in cases that exhibited a nuclear DNA aneuploid pattern. It was especially useful in improving the postoperative survival rate from the second year on. 2) When recurrent cases were examined, it was found to be effective in preventing remote metastasis in those cases administered UFT postoperatively. 3) There were no serious adverse effects and the majority of those that did occur were anorexia and diarrhea.

Bestatin in resected lung cancer. A randomized clinical trial.

A randomized study with and without bestatin, a new biological response modifier, was conducted in order to evaluate its survival effect on resected lung cancer. A total of 153 patients (72 with squamous cell carcinoma, 66 with adenocarcinoma, and 15 with other types of cancer) were evaluated. Among the patients with squamous cell carcinoma, the bestatin-treated group had significantly prolonged survival compared to the control group. No significant difference between the two groups was seen in adenocarcinoma.

Group-recognition ability of derivatized beta-cyclodextrin as studied by the ESR spin probing technique.

The effect of modifying the entrance face of beta-cyclodextrin on the inclusion of various functional groups in aminoxyl spin probes was determined by the use of heptakis (2,6-O-dimethyl)-beta-cyclodextrin and heptakis (2,3,6-O-triacetyl)-beta-cyclodextrin. The methylation of six of the secondary hydroxyl groups on the wider end of beta-cyclodextrin has a variable effect on the association constant of inclusion depending on the nature of the included group. The role of the methoxy group on the rim is discussed on the basis of the thermodynamic data derived from association constants. Contrary to the unmodified beta-cyclodextrin the pH of the solution does not influence the inclusion behaviour.

[Study on local recurrence of squamous cell carcinoma of the lung on hilar lesion].

The study population consisted of 62 patients with squamous cell carcinoma of the lung on hilar lesion who underwent curative or relative curative resection during the seven year period between January, 1980 and December, 1986. We studied the correlation between local recurrence and the distance from the surgical margin of the trachea or bronchus to the tumor. 1) As the classification of the T and N factors increased, the incidence of local recurrence became higher. 2) In order to prevent local relapse, we need to completely resect the hilar and mediastinal lymph nodes, and we must maintain a distance of 16 mm or more between the surgical margin of the trachea or bronchus and the tumor.

[Three cases of occult thyroid cancer with mediastinal lymph node metastasis manifesting as a mediastinal cyst].

We experienced three cases (one 38-year-old male, 58-year-old and 67-year-old females) whose resected mediastinal cysts proved to be metastatic lymph nodes from occult thyroid cancer. Primary lesions in thyroid gland were detected by echography in all three cases though they were detected in one case out of two by CT, in one out of three by 201T1 scintigraphy, in two out of three by 123I scintigraphy. After these examinations on thyroid gland, operations (one lobectomy, one hemithyroidectomy, one total thyroidectomy) were performed. Primary lesions in thyroid gland were all papillary carcinomas, and ranged 4 x 3 mm to 12 x 10 mm in size. Multiple foci were found in two cases, many intrathyroidal metastases in one case, cervical lymph node metastases in two cases. In cases of mediastinal cyst, thyroid gland should be examined by echography paying attention to the mediastinal lymph node metastasis from occult thyroid cancer. When occult thyroid cancer with mediastinal lymph node metastasis is diagnosed, total thyroidectomy followed by 131I radiation therapy is recommended as the treatment for a primary lesion, because of multiple foci in thyroid gland and intrathyroidal metastases.

[The discussion of the need of mediastinal lymph node dissection in the surgical treatment of lung cancer].

The need of lymph node dissection in the case of operative lung cancer with negative mediastinoscopy was discussed. As a preliminary study, the N-number obtained from the mediastinoscopy and that of postoperative examination was compared. The rate of agreement of N-number was as high as 96%. Secondary, 183 cases of lung cancer with squamous-or adenocarcinoma pathologically T-1 or 2. N-0 and P-0 or 1 were picked up and examined. In those cases, the result of 3 years survival rate was 80% in the cases of absolute curative operation and 60% in that of relative non-curative cases, and 5 years survival rate was 70% and 52% respectively, and the cases of absolute curative operation was statistically excellent than that of another group. However, the background of the two groups was different to some degree on account of the advancement of the cancer and the age of the patients. Therefore, the randomized study as for the lymph node dissection in the cases of negative mediastinoscopy was finally performed. In the 30 cases of squamous cell carcinoma where dissection was carried out in 13 cases and not done in 17 cases, the result of 3 years survival was 84% (dissecting group) and 89% (non-dissecting group), and 5 years survival was 73% and 63% respectively. In 23 cases of adenocarcinoma (13; dissecting, 10; non-dissecting), the result of 3 years survival was 100%, 63% and that of 5 years survival rate was 100%, 63% respectively.(ABSTRACT TRUNCATED AT 250 WORDS)

[Pre- and post-operative adjuvant chemotherapy of colorectal cancer. Part 1. Drug concentration in tissues following UFT administration].

The concentration of Tegafur, 5-FU and Uracil in tumor and normal tissue were measured in 47 colorectal cancer patients who had been administered UFT (400/mg) for seven days before operation. The concentration of Tegafur, 5-FU and Uracil in tumor was higher than in normal tissue (p less than 0.01), and the concentration of 5-FU was correlative to the concentration of Tegafur and Uracil in both tissues. The Dukes group and histological type were not related to the concentration of Tegafur, 5-FU and Uracil in either tissue. The uptake of each drug was good and high in tumor tissue.

Urinary excretion of xanthurenic acid and zinc in diabetes: 1) Separation of xanthurenic acid-Zn2+ complex by ion-exchange chromatography.

In view of the relevance of urinary xanthurenic acid (XA) and zinc in diabetes, a chromatographic procedure has been devised which allows separation of XA from its complex with Zn2+. The procedure involves chromatography on DEAE-Sepharose CL-6B at pH 7.2 with a stepwise Tris-HCl concentration gradient. The ultraviolet absorption difference spectra due to formation of the XA-Zn2+ complex allowed the estimate of the binding molar ratio in the XA-Zn2+ complex. This corresponds to a 1:1 as estimated on the basis of the Hill plot of the experimental data.

Studies on the urinary excretion of xanthurenic acid in diabetics.

Xanthurenic acid (XA), kynurenic acid (KA) and creatinine in fasting urine were determined by reversed phase high pressure liquid chromatography in order to investigate the distortion of tryptophan metabolites in diabetes mellitus. The results of ten patients with non-insulin dependent diabetes mellitus and ten normal healthy subjects were compared. No tryptophan load test was performed in this study, because tryptophan loading produces further latent shortage of active vitamin B6 which results in exacerbation of the disease. The ratios of XA to KA and to creatinine were 0.35 +/- 0.099 (mean +/- S.D.) and 0.99 +/- 0.321 in the diabetic patients. The corresponding figures in the normal subjects were 0.17 +/- 0.064 and 0.55 +/- 0.22. Both ratios were significantly higher in the diabetic patients than in normal subjects (p less than 0.001 and p less than 0.01, respectively). This means that XA was excessively excreted in diabetic patients resulting in distortion of tryptophan metabolism. Our findings indicated that the ratios are useful to monitor excess XA excretion and also for detection of diabetes.

Experimental studies of reflux esophagitis following total gastrectomy in rats-pathogenesis and treatment.

Reflux esophagitis was successfully produced in all rats operated on by total gastrectomy and esophagojejunostomy. Its histopathological features comprised erosion and ulceration associated with inflammatory cell infiltration, with hyperplasia of the mucosal epithelium in some animals. These rats were dosed with sodium polyacrylate for protecting the mucosal surface, underwent jejunal diversion for preventing the reflux of digestive juice, or were treated with both in combination, to observe the repairing status of the esophageal ulcer, using the ratio of the length of the regenerated epithelium in the ulcer to the length of the ulcer as the index for the degree of repair. The oral ingestion of 5% sodium polyacrylate solution in drinking water proved the most effective means of administration of this agent. The repair was more improved greater in the group undergoing jejunal diversion, and was greatest in the group receiving both in combination. From these findings, oral administration of a mucosal surface protector, sodium polyacrylate, is an effective therapeutic regiment in mild cases of reflux esophagitis following total gastrectomy, as well as elimination of the cause by diversion of the route in severe cases. The combination of the two therapies will probably by the most effective therapeutic means for this disease.

Primary malignant lymphoma of the trachea. Report of a case successfully treated by primary end-to-end anastomosis after circumferential resection of the trachea.

A 67-year-old Japanese man with exertional dyspnea was found by laminagraphy to have tracheal obstruction. The stricture was localized at the cervical and mediastinal trachea and was 5.0 cm long on an x-ray film. Eight tracheal rings were successfully resected and the ends of the trachea were anastomosed. The lumen of the removed trachea was narrowed by intramural and intraluminal polypoid growth originating from the tracheal wall. Histologically, the tumor was diagnosed as a mixed type of malignant lymphoma. The patient had serum hepatitis postoperatively, but now he is living normally without any difficulty in respiration. No signs of recurrence have been seen in the 5 years, 4 months since the operation. From this experience, the rarity and the possibility of curative operation of this tumor are discussed.

The differences of the metabolism related to vitamin B6-dependent enzymes among vitamin B6-deficient germ-free and conventional rats.

The differences of the metabolism related to vitamin B6-dependent enzymes were investigated using germ-free and conventional rats. There was a significant difference in the boyd weight gain between vitamin B6-deficient germ-free and conventional rats after about 30 days of the experiment, and the body weight gain was much less in the deficient germ-free rats than in the deficient conventional ones. Urinary excretion of xanthurenic acid was higher in the deficient germ-free rats than in the deficient conventional ones after 18 days. There was a significant difference in the activities of kynurenine aminotransferase in mitochondrial fractions of germ-free rats, but not in mitochondrial fractions of conventional ones. The activities of aspartate and alanine aminotransferases, with or without pyridoxal phosphate, significantly decreased in the deficient germ-free rats, but not in the deficient conventional ones. These findings indicate that the degree of vitamin B6 deficiency was more severe in the deficient germ-free rats than in the deficient conventional ones, and also suggest that intestinal microflora may have some effects on vitamin B6-deficient conventional rats.

Effect of vitamin B6 deficiency on the levels of several water-soluble vitamins in tissues of germ-free and conventional rats.

The influence of vitamin B6 deficiency on the levels of several water-soluble vitamins and on acetyl-coenzyme A carboxylase activity was investigated using of germ-free and conventional rats. Judging from the vitamin B6 levels in tissues and the percent of decrease, the degree of vitamin B6 deficiency was more severe in the tissues of deficient germ-free rats than in deficient conventional rats. Nicotine acid, pantothenic acid and biotin levels per wet weight significantly decreased in the liver of vitaminB6-deficient germ-free rats, and nicotine acid levels per wet weight significant decreased in the liver of deficient conventional rats. In the kidney of vitamin B6-deficient germ-free rats, a significant decrease in riboflavin and biotin levels was observed, although there was no observable difference in riboflavin, nicotinic acid, biotin and patothenic acid levels in the kidney of deficient conventional rats. From an enzymatic standpoint, acetyl-coenzyme A carboxylase activity was especially significantly decreased in both germ-free and conventional rats fed a vitamin B6-deficient diet, and the percent od decrease was more in germ-free rats than in conventional ones. These findings suggest that vitamin B6 deficiency had stronger effects on the levels of water-soluble vitamins in germ-free rats compared with conventional rats.

Observations of the concentration of zinc and iron in tissues of vitamin B6-deficient germ-free rats.

The transition of zinc and iron metabolism in vitamin B6 deficiency was investigated using germ-free and conventional rats. In contrast to previous reports, a decrease in zinc content was not observed in the liver, pancreas, kindney, spleen, lung or testes of vitamin B6-deficient conventional and germ-free rats, but we found an increase in zinc content in the kidney of conventional rats and in the liver and spleen of germ-free rats. Vitamin B6-deficient conventional and germ-free rats retained more iron in their tissues than the control animals did, except for the spleen of germ-free rats. The deposit of iron was more evident in vitamin B6-deficient germ-free rats than in vitamin B6-deficient conventional rats, and is possibly proportional to the degree of vitamin B6 deficiency. It is possible that the deposit of iron in the organs had some influence on metabolic disorders in vitamin B6-deficient rats.
