Incidence of post-dural puncture headache with 25-gauge Quincke spinal needle.

A retrospective study was undertaken in patients who received spinal anesthesia in past two years in order to find out the incidence of post-spinal headache. There were 3729 cases, 1997 males and 1732 females. Seventy-two patients were noted to have post-spinal headache of whom twelve were male and sixty were female. The overall incidence was 1.93%. Incidence was 0.6% and 3.5% in male and female groups respectively. With respect to the type of surgical procedures in female group, patients undergoing Cesarean section had a higher incidence than those who received other surgical procedures, being 4.8% and 1.5% respectively. In conclusion, female patients were noted to have higher incidence of post-spinal headache, particularly in patients undergoing Cesarean section.

The laryngeal mask airway--clinical experience.

The laryngeal mask airway (LMA), a new type of airway, when it is inserted blindly into the hypopharynx forms a seal around the larynx and allows easy and convenient spontaneous or controlled positive pressure ventilation. A clinical application of the LMA to 70 surgical patients in our hospital have been evaluated. LMA was successfully applied to 68 (97%) patients who all received general anesthesia with unobstructed controlled ventilation. The patency of the airway did not change throughout the course of anesthesia. There were 2 patients whose airway was obstructed at the first attempt due to downfolding of epiglottis; in one of them the obstruction was confirmed by flexible fibreoptic laryngoscopy. Subsequent placement was successful in these 2 patients. Insertion failure was seen in 2 patients respectively due to small mouth and excessive salivation. The placement of LMA does not require laryngoscopy and there is no fear of misplacement in the esophagus. It becomes obvious that the LMA would substantially gain a place in the armamentarium in anesthesia and we think that its use would be of interest to anesthesiologists.

[Anesthesia for hepatoma resection].

Hepatocellular carcinoma is one of the leading cancers in Taiwan and is responsible for 20% of cancer deaths. Since long-term survival of hepatocellular carcinoma patients cannot be expected with any treatment other than surgery, the therapeutic value of hepatic resection has become more important than ever before. In Keelung Chang Gung Memorial Hospital, twenty-one patients with hepatoma received hepatic resection during the period of August 1985 to July 1989 were reviewed. These patients were induced for anesthesia with thiopental, succinylcholine and fentanyl; maintenance of anesthesia with isoflurane, N2O and O2. Four of them have abnormal coagulopathy preoperatively. Common intraoperative problems were metabolic acidosis and hypotension. Estimated blood loss showed great variety among these patients. Eighteen patients needed respiratory support and intensive care postoperatively. One patient was noted to have pulmonary edema. Otherwise, the overall procedure was smooth and satisfactory. In addition, there is no significantly difference in liver function test after a month of hepatoma resection. All the patients survived except one who died within two months after surgery.

Simple management of poor cervical progression during labor by continuous epidural analgesia.

A series of 112 primiparous parturients in labor with poor cervical progression were managed according to the double-lined labor nomogram. These patients all had labor progression beyond the alert line which were defined as poor cervical progression. Continuous low concentrated marcaine (0.125%) epidural analgesia were then offered to these indicated patients. These 112 poor labors were compared with 143 poor cervical progression labors without epidural analgesia in terms of mode of delivery and maternal morbidities. The results showed a decrease in cesarean section rate in cases with epidural analgesia (20.5% vs 34.3%, p less than 0.05) but no increase use of instrumentation was noted, also, maternal morbidities were much less in epidural cases compared to poor labors without epidural analgesia (6 in 112 vs 19 in 143, p less than 0.05). Continuous epidural analgesia performed on labors with poor progression improved the labor outcome and decrease maternal morbidities satisfactorily.

[Painless labor and instrumentation].

This report is a retrospective study of painless labor and its relation to the instrumentation. It consists of 200 cases of primigravida in labor. In the study group, 100 consecutive cases of primigravida receiving painless labor was chosen and compared to another 100 consecutive primigravida in labor without receiving painless labor. We found that painless labor with epidural analgesia would decrease the rate of cesarean section, but increase the frequency of using forceps and vacuum for labor. The rate of normal spontaneous delivery is low as compared with those not receiving epidural analgesia for painless labor. However, painless labor bears no relation to the labor course nor discernible significance to the Apgar Score of the newborn babies. We strongly recommend painless labor to those poor pain tolerated primigravida.

[P6 acupoint injection reduced postoperative nausea and vomiting].

Despite improvements in anesthetic techniques postoperative vomiting still remains a problem, particularly in ambulant outpatients, and when an opioid is given before or during anesthesia. Anti-emetic are of limited value and often cause drowsiness or other side effects. We report a study in which a simple acupuncture procedure was used in patients undergoing fraction D & C under standard intravenous anesthesia. Sixty-four patients were included and allocated randomly into one of the two groups: control group receiving standard fentanyl-valium-thiopentone anesthesia alone, and the other group receiving injection of 3 mL normal saline into the P6 acupuncture point after the anesthesia. Ten out of the 32 control patients experienced nausea or vomiting compared with only 2 out of 32 patients receiving acupoint injection. The reduction in nausea was significant. These findings cannot be well explained, but it is recommended that the use of acupuncture as an anti-emetic should be explored further.

Experience with major hepatic resections for hepatocellular carcinoma.

Nine major hepatic resections for hepatocellular carcinoma were performed during a period of 2 years and 4 months. HBsAg was positive in six patients, preoperative serum alpha-fetoprotein was more than 20 ng/ml in 5 patients, and liver cirrhosis was present in four patients. Two patients presented with spontaneous rupture of hepatocellular carcinoma. The great value of ultrasonography is stressed as an emergency diagnostic tool in any patient with an acute abdomen without obvious cause. There was no operative mortality or morbidity among this series of patients. All but one patient with a ruptured hepatoma are alive without evidence of disease 3-31 months after liver resection. These encouraging operative results made the authors willing to adopt an aggressive policy toward the surgical treatment of hepatocellular carcinoma.

Analysis of headache in a Chinese patient population.

One hundred and fifty patients with headache were studied by initial interview, before beginning treatment in the pain clinic. The McGill Pain Questionnaire (MPQ) for Headache and a Bakal Topographic Chart were translated into Chinese and employed to assess the characteristics of the headache. The series included significantly more women than men. The elderly were in low proportion, but teenagers were a significant proportion. Women tended to report a longer headache history than men. Topographically, 50% of patients had bilateral temporal headache, and 18% had unilateral headache, more on the right than on the left. Most of the patients were considered to belong to tension headache and mixed headache types (40% and 26%, respectively). Major aggravators resulting in headache hypersensitivity were overwork, fatigue, insomnia, poor sleep, stress, and tension. Analgesics were the primary means of relief reported in 46% of the patients, while resting and sleeping (44% and 27% respectively) were also found to be significant relief factors. In the McGill Pain Questionnaire, specific subclasses of miscellaneous dimension and sensory description. Chinese patients used different classes and far fewer MPQ words than patients reported in the western literature.

A preliminary study of long-term epidural morphine for cancer pain via a subcutaneously implanted reservoir.

A series of 252 patients with terminal cancer (mostly with bony metastases) were treated with epidural morphine. Results were good to excellent pain relief in 85% of patients, but those with malignant growths above the neck showed relatively poor response. For those who survived more than 3 months, the daily morphine requirement increased progressively from 3.5 +/- 0.6 mg to 19.5 +/- 5.3 mg. Drug tolerance developed but no signs of addiction were noted. Respiratory depression was detected in 2 cases due to negligence but resolved uneventfully. No infection of the central nervous system was seen. Systemic reactions were mild and transient. The major drawbacks were catheter failure which required reinsertion, and sharp pain during injection in the later stages of therapy. Despite the side-effects, percutaneous epidural morphine is a useful treatment modality of pain control in cancer patients because it is readily available, safe and not too expensive.
