[Facial motor lesion after surgery of the parotid gland].

The authors report a retrospective study of 351 parotidectomies observed during a 28 years period. The average follow-up is 28 months. Preoperative and post-operative facial nerve function, type of parotidectomy, surgical management of facial nerve and histologic diagnosis according to the revised WHO classification (1990) are presented. Type of parotidectomy and degree of tumor malignancy are statistically analyzed. It seems that occurrence of post-operative facial nerve dysfunction depends on radical parotidectomy, whereas long term dysfunction is determined by tumor malignancy. Malignant epithelial tumors of the major salivary glands proved to be radiosensitive. Apart from preoperative dysfunction, clinical involvement and impossible facial nerve dissection, the authors insist upon sparing the facial nerve, event in the case of microscopic residual tumors, and prefer postoperative radiotherapy.

[Diagnostic perspectives in rhinology].

The physical evaluation of the nasal cavity and paranasal sinuses has been improved by recent advances in diagnostic endoscopic systems and computerized tomography. Today, nasal endoscopy is considered to be the only comprehensive diagnostic rhinoscopic examination and computerized tomography has replaced the standard plain film roentgenograms in the initial evaluation of the paranasal sinuses. Advances have also been made in the evaluation of nasal functions, particularly in the assessment of obstruction. Rhinomanometry is the only method of objective assessment of the patency of the nasal cavities and makes it possible to perform nasal provocation tests. Today, this method of exploration should not be overlooked. Secretory function may be assessed by various methods: nasal cytology, quantification of inflammatory mediators in nasal cavity washing, and biochemical analysis of nasal secretions (albumin/protein ratio). Mucociliary function can be evaluated by measuring the nasal mucociliary clearance, by determining the frequency of cilia strokes (after brushing of the nasal mucosa and microscopic examination under stroboscopic light) and by election microscopic ultra-structure studies. Other methods of exploration which do not yet have a practical application are currently being investigated: rheologic studies of the mucus, immunologic evaluation of nasal secretions and acoustic rhinometry.

[Vertigo and diving].

The appearance of a vertigo during scuba-diving result from different pathophysiological mechanism at which oppose specific therapeutic. Whether it's during barotrauma or decompression accident, the vestibular reach can cause drowning. It is possible to make the difference between barotrauma and decompression accident, with the good study of the scuba-diving and the time when the vertigo has came. So, the therapeutic will use recompression in a multiplace hyperbaric chamber or only hyperbaric oxygen.

[Medical treatment of nasosinusal polyposis].

The results of a computerized prospective study in 107 patients suffering from nasal polyposis and medically treated, are reported by the authors. They insist on necessary pretherapeutic investigations according to anatomic, physiologic and physio-pathologic facts, in order to try to achieve an etiologic and evolutive profile of the nasal polyposis. Local and general medical treatment was initially prescribed to all of our patients. The check's reasons analysis and the study of the various evolutions of nasal polyposis enable us to precise surgical indications.

[Cylindroma of the parotid gland. Apropos of 11 cases].

Few studies have been exclusively devoted to date to parotid cylindromas, essentially because of their rarity and their very slow progression. However, these tumours have a serious prognosis. Although local control appears to be better with complementary radiotherapy, the vital prognosis depends entirely on the presence or absence of distant metastases, essentially pulmonary. The analysis of 11 cases of parotid cylindroma leads the authors to propose a rational management. When the tumour is localised, particularly to the superficial lobe, careful total parotidectomy with preservation of the facial nerve is performed; neck dissection is only performed in the presence of obvious lymph node involvement. Postoperative irradiation is not systematically indicated. In the case of a voluminous tumour or in an obviously incomplete resection, deliberate sacrifice of the facial nerve is not essential, as radiotherapy allows good local control. Extensive mutilating operations do not appear to be justified. Chemotherapy requires the use of more effective drugs than those used at the present time. The complete remission rate in the present series was 60% at six years, but a much longer observation period is required, as the very delayed appearance of metastases is always possible.

[Carcinoma of the nasopharynx. Clinical signs and therapeutic problems].

Most of the carcinoma of the nasopharynx diagnosed in our country, are loco-regionally advanced disease. Thus 31/54 (57%) of our patients referred to the Centre Claudius Regaud between 1970 and 1980 were T2 N3, T3 N3, T4 N0 or N1 N2 N3. In spite of this poor initial presentation external radiotherapy can achieve a quite good rate of tumoral control inside the treated volume. In our series there were 17/47 (36%) tumoral and/or nodal recurrences and 7/47 (15%) metastases without local or regional failure. Improvement of initial évaluation of the tumoral volume is now obtained by the systematic use of C.A.T. and this procedure should reduce local failure rate. In the other hand, the association of chemotherapy and radiotherapy should be useful in treating advanced disease or poorly differentiated carcinomas with high metastatic risk.

[Venous hemangioma of the parotid in adults].

Venous malformation in the parotid region are rare lesions and may fail to be recognized. Their diagnosis is simple, however, and is suggested by clinical findings and confirmed by phlebography. Surgery is the treatment of choice but is always technically extremely difficult. However, excellent results are obtained even after incomplete excision.
