Venous disease and ergonomics of female employment.

AIM: The aim of this study was to describe the employment conditions of women with chronic venous disorders of the lower limbs. METHODS: Cross sectional study conducted by general practitioners who describe the first 3 women, between 18 and 65 years of age, who were employed and who presented with at least CEAP stage I venous disorders. RESULTS: Occupations held by these women indicate significant departures from the general population with an over representation of industrial workers (18.6% vs 11.9%) and an under representation of intermediate professions (12.5% vs 26.6%), (P<0.001). At work, 78.2% (n=4,143) of the women remained standing for 6.2+/-2.4 hours per day and/or 52.3% (n=2,771) were seated for prolonged periods, 28.9% (n=1 503) were exposed to sources of high heat on the legs and 18.2% (n=947) wore garments that compressed the abdomen Conditions favorable to the ergonomic evolution of their workstation are limited: only 9.2% (n=397) thought it possible to reduce the time they spend standing; 10.1% (n=319) the time they spend sitting; 12.9% (n=189) their exposure to heat. Combating these factors appears difficult: 74.3% (n=3 883) state that they do not have sufficient breaks to rest their legs, 38.9% (n=2,053) that they do not have the opportunity to stretch their legs and 42.5% (n=1,395) that compression stockings would be permitted, but would be a hindrance in their work (85.6%, n=4,503). For 27% (n=1,424) of respondents, these problems significantly increase the arduousness of their work and 73.7% (n=3,870) think their working conditions have worsened their venous distress. CONCLUSIONS: Women who consult for venous problems are employed in work which are characterized by unavoidable conditions constituting undeniable venous risk factors for venous disease and occupational medicine does not pay enough attention to the ''ladies legs'' at work.

[Traumatic luxation of the posterior tibialis tendon].

Anterior dislocation of the tibialis posterior tendon is a traumatic lesion, which is rarely suspected following ankle trauma. It seems to be promoted by hypoplasia of the retromalleolar sulcus. Surgical treatment is easy and reportedly gives excellent results, but the diagnosis is difficult and absence of diagnosis may result in functional instability of the foot requiring more invasive surgical treatment.

[Reconnaissance: theoretical construction of social change].

Analysing individual reactions--omnipotence or helplessness--to the question put forward in this symposium, the author observes that the same reactions are present in the mental health field. Investigation of these reactions shows that they alternate in time and are based on a quest for recognition. This quest follows a pattern: First, a group proclaims new knowledge which is based on a fantasy. Then, when this group obtains recognition with its corresponding power, making the transition from a position of impotence to one of power, the fantasy becomes an illusion of omnipotence which excludes others, triggering the cycle again in the excluded groups. This pattern could explain the evolution of the mental health field over the last four decades. Over the last few years, however, there have been some failures in this process, and the author suggests possible solutions to move beyond the impasse.

[Diagnostic problems of circumscribed myositis ossificans: presentation of an atypical case].

The authors report the case of a 22-year-old female suffering from thigh pain for 5 months. The first diagnosis considered was circumscribed myositis ossificans but the duration of symptoms and the radiological aspect mimicking some malignant features led to consider parosteal sarcoma as a possible diagnosis. Surgical resection was therefore requested and histological analysis confirmed the diagnosis of circumscribed myositis ossificans. This case emphasizes the difficulty in differentiating circumscribed myositis ossificans from a slowly growing tumor as both can be associated with increasing pain and a malignant radiological aspect. A surgical approach is therefore frequently necessary for definitive diagnosis and treatment.

[Adaptation mechanisms of chronically mentally ill patients in daily life].

In this article, the author presents the main results of an exploratory clinical study aimed at identifying the adaptation mechanisms used by chronic psychiatric patients. These patients had been treated and followed by an external clinic located in an underprivileged urban environment. Upon the analysis of social worker's observations, the author was able to pinpoint 39 adaptation mechanisms within five areas of daily life: economical, residential, temporal, interpersonal and therapeutic. In conclusion, the author emphasizes on the six operating parameters of these adaptation mechanisms.

[The deinstitutionalization process in the United States (Part 1).].

In this first article, the author analyses the socio-e-conomico-political conditions that have led to the closing of 78.5 % of beds in American asylums in the last 30 years. The author goes on to show how the U.S. government used legislative and fiscal measures to accomplish its deinstitutionalization policy, a policy also influenced by court battles spearheaded by psychiatric patients and their supporters.

[The deinstitutionalization process in the USA (2nd part).].

In this second article, the author analyses the impact of American deinstitutionalization on patients, their families, the communities and the health care system (medical clinics, general hospitals, community mental health centres and residential concerns). Following his analysis, the author develops the various hypotheses produced to solve the problems created by deinstitutionalization. These hypotheses follow the dominant political theme of the 80s in the area of health and social services : the integration of services.

[Crisis intervention.].

This article is an extensive review of literature on crisis intervention. The authors explore the theoretical grounds of this approach and rate them by means of a thorough analysis of the research done on the subject. More precisely they elaborate a definition of a crisis, provide a classification of same, explain its process and describe how the intervention develops in a crisis situation. They examine, in the second part, the dependence phenomenon, and the determining factors encountered. Then, they study the notion of the triggering factors that lead them to the necessity of taking into account the mediatory factors and explanatory variables in the resolution of the crisis.

[The process of psychosis in young adults.].

The experience accumulated over the years with psychotic indi-viduals risks being lost in confusion if care is not taken to introduce certain reference questions and to respond to them, at least partially. In this text, we will ask ourselves the following questions while attempting to communicate, through them, our experience and the elements of response that we have discovered there : 1. What is the dynamic of the encounter with the psychotic in the acute phase? What response should we choose among the three which are possible : sealing-over, integration or accompaniment? 2. What are the stages in the process of equilibration in the young schizophrenic? 3. Does an effective model of accompaniment exist? We will end this article with several questions which might serve as directions for eventual research efforts.

[Young adults in the path of chronicity.].

For the first time, a generation of people (young adults of 18-35) with serious psychiatric problems can no longer use asylums as a refuge from the stress of daily life. These people have to rely on their own experience with the help of a caring team, intermediary structures and medication to face the vicissitudes of life. But because they are very vulnerable to stress and suffer from psychosis or personality disorders, this is not easily done. They represent 10 % of all psychiatric patients and monopolize 40 % of the therapist's time and therefore become a serious challenge to the caring team. Considering the acuteness of the problem, we thought it would be interesting to know these young people better. Therefore, with the help of a review of literature, we will try to answer these questions : who are these people ? What is their future? What is the underlying dynamics of their behavior? What programs do we offer for their care?

[Therapeutic community. Part 1 : Definition, characteristics and evolution.].

In this article, the authors analyze the evolution and development of the therapeutic community concept in the Anglo-Saxon world. In an initial section, they define this concept and its characteristics. Then they make explicit the key concepts which underlie therapeutic intervention in a typical therapeutic community : that of Maxwell Jones at Belmont Hospital. Finally, on the basis of the work of Mosher and Menn, of Berke, and of Sassolas, they elucidate the criticisms made by the above mentionned authors with regard to the treatment of psychosis practised in the institutional treatment system, and they illustrate the approach proposed by these therapist-theoreticians.

[Therapeutic community. Part 2 : La Chrysalide.].

In this article, the authors make explicit the conception of psychosis underlying their approach in a therapeutic community called "La Chrysalide", founded in January 1981. They view psychosis as a defence system against relational and intrapsychic conflicts. Subsequently they describe, in concrete terms, the structure and functioning of this community and then analyze its first sixteen months of operation which followed the social approach of Jones. In particular, they analyze the types of crisis, the roles, and retrace the cyclical evolution of the group of residents during these several months.

Exploring cognitive complaints in schizophrenia: the subjective scale to investigate cognition in schizophrenia.

While it has become commonplace to test the various components of memory in schizophrenia with paper-and-pencil or in-lab tasks, very little data exist on the subjective complaints of patients regarding their memory. Few instruments have been designed to collect systematically the complaints of patients with schizophrenia. We present a work in progress on the Subjective Scale to Investigate Cognition in Schizophrenia (SSTICS), a 21-item, Likert-type scale that is simple and easy to use. It allows a quantitative approach to the subjective and cognitive dimensions of schizophrenia. Stip constructed the scale based on a questionnaire covering several cognitive domains: memory (working memory, explicit long-term memory), attention (divided, distractibility, alertness, sustained), language, and praxia. We evaluated the psychometric properties of the SSTICS in a population of 114 French-speaking patients in Montreal. Patients were recruited in the community and assessed with the Structured Clinical Interview for DSM-III-R (SCID), the Positive and Negative Syndrome Scale (PANSS), and the Extrapyramidal Symptoms Rating Scale (ESRS). Cognition was measured using the Rey Auditory Verbal Learning Test (RAVLT) (long-term memory), Controlled Oral Word Association Test (verbal fluency), and Trails A and B. Preliminary analyses showed very good internal consistency for the global score (alpha=0.88), and alphas varying from 0.57 to 0.72 for the subscales. Stability over time was very good. The principal components analysis accounted for a multiple structure. Correlations between subjective scores and objective cognitive assessment were significant for several domains. Validation of the SSTICS needs to be completed through further exploration of the factorial structure and testing of the English version.
