Cerebral arteriovenous malformations associated with aneurysms--a report of 10 cases and literature review.

We analysed 10 cases of arteriovenous malformations (AVMs) associated with aneurysms and reviewed the literature in order to confirm the clinical characteristics of AVMs associated with aneurysms. The mean incidence of aneurysms in our AVM population was 9.1% and the average age was 38 years. The ratio of male to female was 1.3 to 1. The aneurysms (40-96%) were often located on a feeding artery and seldom (23%) at an arterial bifurcation. The mortality rate was 0-26% in surgically managed patients and 38-60% in conservatively treated patients. The combination of intracranial vascular lesions is not rare. Most patients presented with symptoms referable to their AVMs. Treating both lesions in a single operation is the best option. The prognoses for most patients was good.

Human skeletal muscle HSP70 response to physical training depends on exercise intensity.

We have previously reported that HSP70 in human skeletal muscle could be induced by training. However, whether HSP70 induction is dependent upon exercise volume or exercise intensity remains unknown. The aim of the present study was to investigate the relationship between HSP70 and training intensity in rowers. Fourteen well-trained male rowers were divided into two groups (group A, n = 6; group B, n = 8). Group A performed higher intensity exercise during 1st phase, whereas group B performed higher intensity exercise during 2nd training phase. Training volume in 2nd phase increased in both groups. Both training intensity and volume were reduced in 3rd phase. Muscle samples were taken from m. vastus lateralis by fine needle biopsy before training, at the end of the 1st, 2nd and 3rd training phases. HSP70 was quantitatively determined using SDS-PAGE with silver stain. In group A, HSP70 increased significantly from 38 +/- 12 etag before training to 59 +/- 16 etag at the end of the lst training phase (loaded total protein 2.5microg), and decreased afterwards. In group B, HSP70 increase (from 36 +/- 11 etag to 50 +/- 13 etag) in the 1st phase was significantly smaller, there was a further increase of HSP70 in the 2nd phase (60 +/- 14 etag). At the end of the training, HSP70 decreased in both groups. Thus, HSP70 response to training seems to be dependent upon exercise intensity.

Lactate concentration in plasma and red blood cells during incremental exercise.

The purpose of this study was to investigate the distribution of lactate in plasma and red blood cells (RBC) in capillary blood during and after incremental exercise. We measured capillary plasma lactate and whole blood lactate of 10 subjects during incremental treadmill running and the first 20 min of recovery. To minimize lactate exchange from plasma to RBC between sampling and analysis, a recently developed rapid plasma separation method was used. RBC lactate was calculated. The RBC/plasma lactate concentration ratio decreased from 1.0 (0.85-1.28) before to 0.37 (0.25-0.45) after exhaustive exercise (plasma lactate 15.9 (12.2-19.5)mmol x I(-1), RBC lactate 4.8 (4.0-7.0) mmol x 1(-1)), thus showing that capillary plasma lactate increased much more rapidly than intracellular lactate during incremental exercise. In the first 5 minutes of recovery intracellular lactate still rose while plasma lactate already declined. Then both decreased while the concentration ratio as well as the absolute concentration gradient remained nearly constant (ratio 20 min after exercise termination: 0.43 (0.19-0.54).

Experimental carinal replacement with an Y-shaped collagen-conjugated prosthesis.

BACKGROUND: A new prosthesis was designed for reconstruction of the bifurcation of the trachea and used for experimental carinal replacement in dogs. METHODS: The main Frame of the new prosthesis consists of Y-shaped Marlex mesh tube reinforced with polypropylene spirals. It is coated with collagen extracted from porcine skin to provide biocompatibility and airtightness. Carinal replacement with omentopexy was performed in 17 dogs. RESULTS: Seven dogs survived the postoperative period, and 10 dogs died within 11 days after the operation. The main causes of early postoperative death were fistula and air leakage from the prosthesis. Causes of late postoperative death were obstruction of the main bronchus (two) and lung abscess (one). The four long-term survivors had no stenosis or dehiscence until they were sacrificed 15 months after the operation. Histological examination of these dogs revealed that the luminal surface was covered either with ciliated columnar epithelium or non-ciliated squamous epithelium. CONCLUSIONS: The study results suggest the possibility of successful prosthetic carinal reconstruction with epithelial regrowth using this Y-shaped prosthesis.

Early attachment of anaerobic bacteria may play an important role in biliary stent blockage.

BACKGROUND: In vitro studies have demonstrated that ciprofloxacin suppresses Escherichia coli attachment on stents, and ciprofloxacin has been shown to prolong stent patency in cats. However, clinical studies with antibiotic prophylaxis have produced conflicting results. The aim of this study was to isolate and identify the bacteria that attach early on unblocked stents removed from patients and to study their enzyme activities. METHODS: Eighteen unblocked biliary stents were removed from 17 patients (benign obstruction in 14 and malignant obstruction in 4). All patients received antibiotic prophylaxis (mean of 6 days). Stents were in place for a mean of 33 days. The inside of stents was scraped and sludge was cultured aerobically and anaerobically. Identification of isolated bacteria and measurement of beta-glucuronidase and phospholipase C activities were performed by using standard techniques. Gastric and duodenal juice from 18 patients with no biliary diseases was used as control samples. RESULTS: All stents were patent and only 6 had visible sludge. There were 19 anaerobes isolated from 16 stents (Clostridium perfringens 13, Clostridium bifermentans 4 and Bacteroides fragilis 2). Phospholipase C was detected in all Clostridium species. beta-Glucuronidase was produced only by 12 of 13 C perfringens isolates. Sixteen aerobes including Enterococcus species and Bacillus species were isolated but none produced beta-glucuronidase or phospholipase C. There were no aerobic gram-negative bacteria isolated from stents. Clostridium species and B fragilis were not recovered from the control samples. CONCLUSIONS: In patients who had received antibiotic prophylaxis against gram-negative bacterial infection, anaerobic bacteria may play a role in initiating stent blockage.

Physiologic concentrations of homocysteine inhibit the human plasma GSH peroxidase that reduces organic hydroperoxides.

The plasma reduced glutathione (GSH) selenoperoxidase is a highly conserved enzyme. Furthermore, a small clinical study reported that patients with severe atherosclerosis had low peroxidase activities. Together these observations suggest that the peroxidase is important in preventing atherosclerosis. Yet others have reported that when the assay was run in Tris buffer, it was inactive with the concentrations of GSH found in the plasma. Second, it is known that hyperhomocysteinemia increases the rate of atherogenesis. Because there is some homology between homocysteine and the cysteine in GSH, the question is whether the hyperhomocysteinemia effect may be due to inhibition of the peroxidase. We purified the peroxidase from human plasma and determined its activity by a coupled spectrophotometric assay and a substrate disappearance chemiluminescence assay. When the peroxidase activity was determined in phosphate-buffered saline solution (PBS), there was significant activity with the reported plasma GSH concentrations (5 to 20 micromol/L). The peroxidase was exclusively in the HDL fraction. There was no correlation between the peroxidase activity and the HDL or LDL cholesterol concentrations. Finally, at physiologic concentrations of GSH (9 micromol/L), the peroxidase was inhibited by physiologic, free homocysteine concentrations (1 to 5 micromol/L). These data suggest that the peroxidase is active in vivo and may be important in protecting the endothelium from atherosclerosis by preventing oxidant injury. The homocysteine inhibition of the peroxidase suggests a possible biochemical basis for the observed association between hyperhomocysteinemia and cardiovascular disease. Our studies imply that low concentrations of this peroxidase may be an independent risk factor for atherosclerosis.

Effects of a mandibular repositioner on obstructive sleep apnea.

The purpose of this study was to investigate the effects of a mandibular repositioner on airway, sleep, and respiratory variables in patients with obstructive sleep apnea. Twenty-two patients selected for this study were confirmed with a diagnosis of obstructive sleep apnea based on initial nocturnal polysomnography. The patients were fitted with a mandibular repositioner designed to hold the mandible anteroinferiorly. Six months later, an outcome polysomnographic study was undertaken for each patient with the appliance in place. Lateral cephalometric radiographs in the upright position were also obtained before and after 6 months of treatment. The respiratory disturbance index decreased in 21 of the 22 patients with the appliance in place. The mean respiratory disturbance index of the 22 patients decreased significantly from 40.3 to 11.7 events per hour (P <.01). Some 59.1% of subjects were considered a treatment success with follow-up respiratory disturbance index < 10 events per hour. The mean minimum blood oxygen saturation level during sleep also improved significantly from 73.4% to 81.3% (P <. 01). The mandibular repositioner was constructed to position the mandible at 75% of the maximal mandibular advancement and with a 7 mm opening between the upper and lower incisors, and no aberrant effect on temporomandibular joint was noted. The retropalatal airway space increased and the cross-sectional area of the soft palate and the vertical distance of the hyoid bone to the mandibular plane decreased significantly. The tongue posture became significantly flatter. A significant linear correlation was found between the reduction in apnea index and specific craniofacial skeletal structures (length of anterior cranial base, mandibular plane angle, and upper to lower facial height ratios, P <.05). Subjects with a smaller reduction in apnea index tended to have shorter anterior cranial bases, steeper mandibular planes, and smaller upper to lower facial height ratios. We conclude that a mandibular repositioner may be an effective treatment alternative for obstructive sleep apnea and that a reduction in the frequency of apneic episodes is mainly attributed to the effects of the appliance on oropharyngeal structures.

Carinal reconstruction with a Y-shaped collagen-conjugated prosthesis.

BACKGROUND: Carinal reconstruction by direct suturing is associated with a high mortality because high tension at the anastomosis can lead to tracheobronchial fistula. A new tracheal prosthesis was therefore designed for reconstruction of the tracheal bifurcation and applied for experimental carinal replacement in dogs. METHODS: The main frame of the new prosthesis consists of a Y-shaped Marlex mesh tube (C.R. Bard, Inc, Billerica, Mass) reinforced with polypropylene spirals, to which collagen extracted from porcine skin is chemically conjugated to provide biocompatibility and airtightness. This conjugated collagen is composed of amorphous and sponge collagen layers. The tracheobronchial bifurcation was replaced with the prosthesis in 10 beagle dogs. RESULTS: Eight dogs survived the postoperative period, and 2 dogs died within 4 days after the operation. Bronchoscopic examination revealed that the tracheal prosthesis was covered with smooth whitish tissue and that no stenosis or dehiscence was present in the 8 dogs even 6 months after the operation. Slight mesh exposure was recognized in 1 dog. Histologic examination revealed that approximately one half of the luminal surface was covered with ciliated columnar epithelium or nonciliated squamous epithelium. In the remaining lumen, especially in the middle portion of the prosthesis, connective tissue without epithelium was observed. CONCLUSIONS: These long-term results indicate that our bifurcated tracheal prosthesis can be used for reconstruction of the tracheobronchial bifurcation with long-term safety.

Immunosuppressant-free allotransplantation of the trachea: the antigenicity of tracheal grafts can be reduced by removing the epithelium and mixed glands from the graft by detergent treatment.

OBJECTIVE: To develop a method for eliminating the epithelium and mixed glands from tracheal grafts by detergent treatment and evaluate these grafts for immunosuppressant-free allotransplantation in dogs. METHODS: Fresh canine tracheal grafts were treated with a detergent (1% Triton X-100 t-octylphenoxypolyethoxyethanol; T-9284; Sigma Chemical Co, St Louis, Mo) at 4 degrees C for 48 hours. The grafts were then used for intrathoracic 5-ring tracheal replacement in other dogs without immunosuppressant treatment (n = 6, detergent treatment group). In the control group (n = 6) fresh untreated canine tracheal segments were implanted as allografts. All the implanted grafts were covered with an omental pedicle. RESULTS: In the detergent treatment group the chondrocytes in the graft had a similar appearance to those in the fresh trachea, indicating that the chondrocytes remained viable after the detergent treatment. In 5 of the 6 grafts, the epithelium and mixed glands had been removed completely. After transplantation, these 5 grafts were incorporated by the host trachea without stenosis. In the remaining treated tracheal graft, in which removal of the epithelium was incomplete, moderate stenosis was observed at the fourth week after implantation, although this was not progressive. In the control group, granulation tissue of the graft and significant stenosis were observed after transplantation. CONCLUSION: The antigenicity of tracheal grafts can be greatly reduced by removing the epithelium and mixed glands by the use of detergent treatment. The epithelium and mixed glands of the graft appear to be the determining elements involved in rejection after tracheal allotransplantation.

The pyruvate dehydrogenase E1 alpha gene is testosterone and prolactin regulated in prostate epithelial cells.

The prostate gland of humans and other animals has the unique function of accumulating and secreting extraordinarily high levels of citrate. The prostate secretory epithelial cells synthesize citrate which, due to a limiting mitochondrial (m-) aconitase, accumulates rather than being oxidized. Thus citrate is essentially an end product of metabolism in prostate. For continued net citrate production, a continual source of oxaloacetate (OAA) and acetyl CoA is required. Glucose via pyruvate oxidation provides the source of Acetyl CoA. In prostate cells, citrate production is regulated by testosterone and/or by prolactin. Both hormones selectively regulate the level and activity of pyruvate dehydrogenase E1 alpha (E1a) in animal prostate cells; thereby regulating the availability of acetyl CoA for citrate synthesis. Studies were conducted to determine if testosterone and prolactin might regulate the expression of the E1a gene in prostate epithelial cells. Prolactin treatment of rat ventral and lateral prostate cells and human PC3 cells increased the levels of E1a mRNA and the rates of transcription of the E1a gene. Testosterone also increased the mRNA level and transcription of E1a in rat ventral prostate cells, and in PC3 cells transfected with androgen receptor. However, testosterone treatment resulted in a repression of E1a gene expression in lateral prostate cells. Evidence is presented which supports the view that prolactin regulation of E1a is mediated via PKC. The rapidity of the effects of both hormones is representative of an immediate-early gene response. To our knowledge this represents the first report in any mammalian cells that, in addition to its constitutive expression in all mammalian cells, the E1a gene is a hormonally-regulated gene in specifically targeted prostate epithelial cells.

Do bimanual motor actions involve the dorsal premotor (PMd), cingulate (CMA) and posterior parietal (PPC) cortices? Comparison with primary and supplementary motor cortical areas.

Single neuronal activity was recorded from the dorsal premotor cortex (PMd), the cingulate motor area (CMA) and the posterior parietal cortex (PPC) in two Macaca fascicularis trained to perform a delayed conditional sequence of coordinated pull and grasp movements. The monkey had to perform three types of trials instructed in a random manner: (i) bimanually, using the two hands in a coordinated sequence of movements; (ii) unimanually, using the left hand only; (iii) unimanually, using the right hand only. The aim of this study was first to assess the bilateral relationships of the three cortical areas for unimanual motor control. Second, to establish whether the three cortical areas contain units reflecting bimanual synergy. A total of 255 task-related neurons were recorded from the PMd, CMA and PPC, where most neurons exhibited a significant modulation of activity in both contralateral and ipsilateral unimanual trials (bilateral neurons: 85, 77 and 61%, respectively). Lower proportions of neurons in PMd (7%), CMA (16%) and PPC (6%) were active in unimanual contralateral trials, but not in unimanual ipsilateral trials. The reverse (modulation of activity in ipsilateral but not contralateral unimanual trials) represented 5% of neurons in PMd, 7% in CMA and 3% in PPC. When comparing unimanual and bimanual trials to search evidence for bimanual coordination, 57% of PMd task-related neurons were classified as bimanual, defined as units in which the activity observed in bimanual trials could not be predicted from that associated with unimanual trials when comparing the same events related to the same arm. The proportion of bimanual neurons in CMA (56%) was comparable to that found in PMd (55%), whereas PPC exhibited a higher proportion of bimanual neurons (74%). Furthermore, comparison of the present data with our previous results regarding the supplementary (SMA) and primary (M1) motor cortical areas shows that there is no statistically significant difference between PMd, CMA, SMA and M1 with respect to the proportions of bimanual neurons. Altogether, these results suggest that the five cortical areas PMd, CMA, PPC, SMA and M1 are participating to the control of sequential bimanually coordinated movements. Inter-limb coordination may thus be controlled by a widely distributed network including several cortical and sub-cortical areas.

Selection of peptide ligands that bind to acid fibroblast growth factor.

Acid fibroblast growth factor (aFGF) binds to its cell-surface receptors in a heparin-dependent manner. In an attempt to define the aFGF recognition site on fibroblast growth factor receptor 1 (FGFR1), we developed a screening strategy for identifying FGF ligands that bind to the receptor-binding region of FGF. To retain the natural conformation of aFGF during screening, we used biotinylated heparin to immobilize aFGF on a streptavidin-coated dish. A 15-mer phage display peptide library was then screened in the dish and a group of related peptide sequences was identified. These peptide sequences contain two conserved motifs, SSG and VPS, corresponding to two protein sequences of the immunoglobulin-like (Ig-like) domain II of FGFR1 at amino acids 180-182 and 221-223 (CPSSG-VPSDKGNYTC). Further experiments demonstrate that the phage displaying these sequences can specifically bind to aFGF and that the synthesized peptide corresponding in sequence can inhibit mitogenic activity of aFGF. These sequences may thus constitute part of the aFGF-binding region on FGFR1, and the synthesized peptide has the potential to become a therapeutic agent as an aFGF antagonist.

Diurnal variation in the P300 component of human cognitive event-related potential.

Diurnal variation in the P300 component of the human cognitive event-related potential (ERP) was examined. The P300 component is considered to be a measure of neuroelectric activity related to cognitive functions such as attention allocation and information processing. Nine diurnally active healthy male subjects whose sleep-wake rhythms were synchronized prior to the experiment were studied. The P300 components oral temperature, heart rate, left- and right-hand grip strength, reaction time, subjectively rated sleepiness, attention level, and fatigue were measured at 08:00, 11:00, 14:00, 17:00, and 20:00. Significant diurnal variations in P300 latency, P300 amplitude, oral temperature, heart rate, left- and right-hand grip strength, subjectively rated sleepiness, and attention level were observed. The P300 latency at 08:00 was significantly longer than at 11:00, 17:00, and 20:00, while the P300 amplitude at 08:00 was significantly greater than at 17:00 and 20:00. The P300 latency was correlated positively with subjectively rated sleepiness and negatively correlated with subjectively rated attention level. These results suggest the existence of diurnal variation in human cognitive functions.

A preliminary study on intravenous infusion of sodium eicosapentaenoate.

Eicosapentaenoic acid (EPA) and arachidonic acid (AA), made into sodium salt solution (50 micrograms/ml), were used for intravenous infusion. In a preclinical study in dogs, Na-EPA lowered the activity of transminases (glutamic pyruvic transaminase [GPT], glytamic oxaloacetic transaminase [GOT]); however, Na-AA increased the activity of GPT and GOT. In the clinical study, the numbers of leukocytes and lymphocytes of volunteers increased and remained at that level for 3 to 5 days after intravenous infusion. The study indicated that an intravenous infusion of Na-EPA may have anti-inflammatory and immunomodulatory effects.

Transduction and utility of the granulocyte-macrophage colony-stimulating factor gene into monocytes and dendritic cells by adeno-associated virus.

The genetic manipulation of antigen-presenting dendritic cells (DC) offers promise for stimulating the immune response, in particular for anticancer and antiviral protocols. As adeno-associated virus (AAV) has shown promise as a gene delivery vector for transducing a variety of hematopoietic cell types, we have investigated AAV's ability to genetically alter DC. In this analysis, we modified the standard granulocyte-macrophage colony-stimulating factor (GM-CSF) and interleukin-4 (IL-4) treatment of adherent monocytes to generate DC. In our protocol, adherent monocytes were first infected with an AAV/GM-CSF/Neo vector, and the addition of IL-4 was delayed for 2 days to allow for a brief period of monocyte proliferation. AAV-mediated transduction of the GM-CSF and Neo genes into monocytes/DC precursors was demonstrated by G418 selection, GM-CSF secretion, GM-CSF RNA expression (reverse transcriptase-polymerase chain reaction amplification [RT-PCR]), and cell proliferation. Cells resulting from infection with AAV/GM-CSF/Neo virus, and subsequent IL-4 and tumor necrosis factor-alpha (TNF-alpha) treatment, displayed multiple classic markers consistent with mature DC. Finally, chromosomal integration of the AAV vector was also demonstrated in sorted CD83+ DC. These data strongly suggest that AAV vectors will be useful for the genetic manipulation of DC and suggest that the transduction of the GM-CSF gene was able to fully replace the need for exogenous GM-CSF in the production of mature DC.

Correlation of defective mitotic checkpoint with aberrantly reduced expression of MAD2 protein in nasopharyngeal carcinoma cells.

Nasopharyngeal carcinoma (NPC) occurs with a high incidence in many countries in south-eastern Asia. Chromosomal abnormalities have been commonly found in NPC, but the underlying mechanism is not well understood. We determined mitotic indices, the staining pattern of nuclear DNA and cell cycle profiles of NPC cells in response to treatment with microtubule-disrupting agents, and found that the mitotic checkpoint was defective in two out of five (40%) of the tested NPC cell lines. We also observed that an aberrantly reduced expression of MAD2, one of the key components of mitotic checkpoint, correlated with the loss of checkpoint control. Our findings suggest that a defective mitotic checkpoint characterized by reduced expression of MAD2 contributes to chromosomal instability in NPC.

Chemoprevention studies of the flavonoids quercetin and rutin in normal and azoxymethane-treated mouse colon.

In this study we investigated the chemopreventive effects of quercetin and rutin when added to standard AIN-76A diet and fed to normal and azoxymethane (AOM)-treated mice. Early changes in colonic mucosa were analyzed, including colonic cell proliferation, apoptotic cell death, cyclin D(1) expression and focal areas of dysplasia (FAD). The findings show that the number of colonic epithelial cells per crypt column increased (P: < 0.01) in each normal mouse group fed the flavonoids; AOM administration increased colonic crypt cell proliferation and resulted in a marked rise of bromodeoxyuridine-labeled cells in the lower proliferative zone of the crypt. Both supplementary dietary quercetin and rutin increased the apoptotic index and caused a redistribution of apoptotic cells along the crypt axis in normal mice fed a standard AIN-76A diet. The number of apoptotic cells/column and apoptotic indices markedly increased (P: < 0.01) in the AOM-treated group compared with untreated animals; apoptotic cells expanded throughout the colonic crypts after flavonoid supplementation and AOM administration. Positive cyclin D(1) expression was detected in mice on diets supplemented either with quercetin (P: < 0.01) or rutin (P: < 0.05). AOM administration resulted in the formation of FAD. Both the number of mice exhibiting FAD and the total numer of FAD observed were significantly reduced (P: < 0.01) in AOM-treated animals fed flavonoids compared with mice maintained on the standard AIN-76A diet. Surprisingly, however, quercetin alone was able to induce FAD in 22% of normal mice fed the standard AIN-76A diet.

Coordinated response of mammalian Rad51 and Rad52 to DNA damage.

Biochemical analysis has shown that mammalian Rad51 and Rad52 interact and synergize in DNA recombination reactions in vitro, but these proteins have not been shown to function together in response to DNA damage in vivo. By analysis of murine cells expressing murine Rad52 tagged with green fluorescent protein (GFP)-Rad52, we now show that DNA damage causes Rad51 and GFP-Rad52 to colocalize in distinct nuclear foci. Cells expressing GFP-Rad52 show both increased survival and an increased number of Rad51 foci, raising the possibility that Rad52 is limiting for repair. These observations provide evidence of coordinated function of Rad51 and Rad52 in vivo and support the hypothesis that Rad52 plays an important role in the DNA damage response in mammalian cells.
