Automatic analysis of serum lactate dehydrogenase isoenzymes by high-performance ion-exchange chromatography.

The repetitive analysis of serum lactate dehydrogenase (LDH) isoenzymes has been performed on a weak anion exchanger (TSKgel DEAE-5PW), which was developed by introducing diethylaminoethyl groups into TSKgel G5000PW (10 microns particle diameter)--a hydrophilic polymer-based material of large pore size--for high-performance gel chromatography. By use of this anion exchanger, a high-pH (greater than 8.0) solvent could be used and the albumin peak was completely separate from the LDH isoenzyme peaks. After 10 successive analyses with an autosampler, the coefficient of variation of the LDH isoenzyme elution times was less than or equal to 0.90%, and the coefficient of variation for peak areas was less than or equal to 3.85%. After 40 successive analyses, resolution between isoenzymes was generally greater than 1.25. This column can be used for more than 300 intermittent injections of human serum.

Unusual cases of presenile dementia with Fahr's syndrome.

Three patients with an unusual type of presenile dementia were studied. Atypical clinical pictures emerged from an evaluation of these cases. Their symptoms and signs were thought to be neither those of Alzheimer's disease nor those of Pick's disease but a partial mixture of those of both diseases. The neuropathological changes were characteristic and the common findings were as as follows: the absence of senile plaques, the widespread presence of numerous neurofibrillary tangles, a calcareous deposition of Fahr's type, a circumscribed cerebral atrophy in the temporal or/and frontal lobes, a moderate or severe demyelination and fibrous gliosis in the white matter of the atrophied areas, and a slight or moderate neuronal loss in the nucleus basalis of Meynert. Similar cases reported previously were reviewed.

An unusual case of Pick's disease.

A man aged 64, with a history of chronic trichloroethylene intoxication, presented early headache, impairment of memory, and "stehende Redensarten", later on mental deterioration with muteness and oral tendency. He died of bronchopneumonia. The brain was studied by light and electron microscopy. Numerous inflated cells, intraneuronal argentophil Pick bodies and central chromatolysis of neurons were found throughout the cerebral cortex. Widespread diffuse fibrillary gliosis was the finding bearing a striking resemblance to "progressive subcortical gliosis". Severe loss of nerve cells was observed in the temporal lobe (except the posterior of T 1) and moderate or mild loss in the gyrus rectus, insula, gyrus cinguli, and partial areas of the frontal, parietal, and occipital lobes. By electron microscopy, the argentophil Pick bodies consisted of a conglomeration of randomly arranged short 100-150 A filaments, ribosomes, vesicles, mitochondria, and scanty paired helical filaments. The present case was considered to represent a rare case of Pick's disease of the temporo-fronto-parieto-occipital type or panencephalic type.

Spectrophotometric method for selective assay of the five isoenzymes of human lactate dehydrogenase, based on their different stabilities at alkaline pH.

After preincubation of human lactate dehydrogenase (EC 1.1.1.27) isoenzymes 1 through 5 with lactate at pH 9.4, 9.8, 10.25, and 10.6 at 30 degrees C for 10 min, the reduction of NAD+ was measured at the same pH values and temperature during the interval 1-2 min after adding the coenzyme. Relative to the reference activities measured at pH 8.7 by the method of Buhl et al. (Clin Chem 23: 1289-1295, 1977), the respective activities of isoenzymes 1 through 5 thus measured were 111, 104, 96, 68, and 0% at pH 9.4; 123, 108, 88, 0, and 0% at pH 9.8; 140, 100, 0, 0, and 0% at pH 10.25; and 138, 0, 0, 0, and 0% at pH 10.6. These relations allow selective assay of the respective isoenzymes in samples containing mixtures of them. Optimal conditions for such selective assay at 37 degrees C are reported.
