Leiomyosarcoma of the broad ligament--report of two cases.

Leiomyosarcoma of the broad ligament (LBL) is a very rare tumor. Up to 1989, there had been only 7 cases reported in the English literature. During recent 12 years, we have had 15 cases of leiomyosarcoma in our department. Among them, there were two cases of LBL. The others were leiomyosarcoma of the uterine corpus. The first patient was 36 years old. She received total abdominal hysterectomy, bilateral salpingo-oophorectomy and paraaortic lymph node sampling followed by chemotherapy with adriamycin. Intra-abdominal recurrence with hepatic metastasis occurred 18 months after operation and she received palliative radiotherapy. The second patient was 65 years old and received subtotal hysterectomy, bilateral salpingo-oophorectomy, omental biopsy and suprapubic cystostomy followed also by chemotherapy with adriamycin. She received segmental resection of ileum and ileostomy due to intestinal obstruction caused by metastasis 5 months after her initial operation. Both patients were still alive when reported with 33 and 26 months of survival, respectively. The nature and management for LBL are scarcely reported, accumulation of cases and clinical experiences are needed.

[Solitary squamous papilloma of the bronchus: report of a case].

A 30-year-old man had a history of smoking 1-2 packs per week for 10 years. He had suffered from a productive cough with whitish mucoid sputum for two months. Left anterior chest pain, palpitation and shortness of breath developed about two weeks before his admission. Chest radiographs showed collapse of the left lower lobe. Bronchoscopy revealed a strawberry-like tumor on the left main bronchus with nearly complete obstruction. Pathology showed bronchial squamous papilloma with surface dysplasia, but no evidence of malignancy. The obstructed lumen was completely reopened by bronchoscopic Nd-YAG laser therapy. Unfortunately, the tumor recurred two months later. Therefore a sleeve resection of the tumor was performed six months after the laser photoresection. After two years of follow-up, no evidence of tumor recurrence has been found.

[The pathogens of Taiwan mosquitoes--Coelomomyces species].

A brief mention was made of the history of world studies on the Coelomomyces fungi, the life cycle of the fungi, their importance as biological agents for mosquito control, and the known ecological information concerning the Taiwan mosquitoes parasitized by the fungi. Special accounts were made of the results of experiments infecting four mosquito species, Aedes aegypti, Ae. albopictus, Ae. triseriatus and Tripteroides aranoides with Coelomomyces stegomyiae var. chapmani, using the copepod, Phyllognathopus viguieri as an alternate host. The dead 4th instar larvae of Ae. albopictus containing sporangia were put together with healthy copepods for 10 days, then healthy 2nd instar mosquito larvae were added for a 1-day exposure. Then the larvae were transferred to clean water and subsequent examinations for signs of infection were made at 3-day intervals for a period of 21 days. The experiments demonstrated an infection rate of 59.7% and 90% respectively for Ae. albopictus and Ae. aegypti in the first trial, 18.5% and 23.3% in the 2nd trial, and none in the 3rd trial. The low infection rate in the 2nd trial and no infection in the 3rd trial were thought to be due to the extensive contamination of test water with algae. Therefore, in the 4th trial the rearing water was renewed with dechlorinated tap water and new copepods instroduced. The infection rate again rose to 41.1% and 56% respectively for the two species. Experiments with Ae. triseriatus and Tp. aranoides failed to produce signs of infection. Experimental infection of susceptible Ae. albopictus larvae with the sporangia stored at 8 degrees C showed that storage for one month produced an infection rate of 38.2%. However, storage for two months or longer produced no infection in the larvae of the same mosquito species.

Uterine malignancy developing after long term use of IUCD additional report of 2 cases: endometrial stromal sarcoma and leiomyosarcoma.

In our 2 previous papers published in this Journal, we had 5 cases of uterine malignancies developing after long term use of IUCD's. In the present paper 2 cases of such association are to be added. One case ATH with right adnexectomy was performed for persisting vaginal bleeding after 22 years of Lippes Loop application and recurrence 8 years after the first operation. Pathological diagnosis is endometrial stromal sarcoma. The other case is a leiomyosarcoma developing after 25 years of Lippes Loop application. Although no cause and effect relationship between IUCD use and uterine malignancy in human beings has been proved, the increase in such cases suggests to us that long term application should be avoided, particularly in view of the positive animal experiments. The successive detection of such cases in a relatively short period of time prompts us to make further studies to gain a deeper insight into such associations.

[Parathyroid adenoma associated with bilateral renal calculi. A case report].

Renal calculi is an uncommon clinical presentation of parathyroid adenoma today. Only 17% of parathyroid adenoma has renal calculi in Levin's report. The association of parathyroid adenoma is often overlooked in the patients of renal calculi. We report a case whose clinical presentation is renal calculi with renal colic Parathyroid adenoma was not discovered until the second admission. The patient's clinical presentation improved after parathyroidectomy. We report this case to remind the clinical physician regarding the possibility of coexistence of parathyroid adenoma in patients of renal calculi. Excision of the parathyroid adenoma is the only treatment for these patients.

A new method of inhalation challenge with propranolol: comparison with methacholine-induced bronchoconstriction and role of vagal nerve activity.

To establish a safe procedure for examining propranolol-induced bronchoconstriction, we have developed a new method for performing inhalation challenge with propranolol. Monitoring respiratory resistance during tidal breathing with continuous inhalation of propranolol in 1.5-fold increasing concentrations from 0.78 to 30 mg/ml for 1 minute at each concentration, we tested 43 subjects with stable asthma and 10 normal subjects. We also compared bronchial responsiveness with responsiveness to inhaled methacholine on separate days. In addition, to determine the role of vagal nerve activity in propranolol-induced bronchoconstriction, we studied the effect of atropine. Inhaled propranolol caused dose-related bronchoconstriction in all subjects with asthma but not in normal subjects. None of the subjects suffered severe asthmatic attack during the test, which was performed in 15 minutes or less. The minimum cumulative dose of methacholine and of propranolol, at the point where respiratory conductance began to decrease, was not significantly correlated. Increased respiratory resistance was reversed by atropine in 70% of the subjects with asthma with marked individual differences. These data suggest that, although in most subjects with asthma, vagal nerve activity contributes in varying degree to bronchoconstriction, other constricting factors may contribute in the remaining subjects. It is also suggested that the mechanism of bronchial response to propranolol differs from that of the nonspecific airway reactivity estimated by methacholine challenge.

Molecular species of phosphatidylcholine from rat gastric mucosa.

Phosphatidylcholine of rat gastric mucosa were found to constitute about half of the total phospholipids. The composition of 20 molecular species accounting for approx. 90% of the total phosphatidylcholine was determined by specific enzymic hydrolyses and AgNO3 thin-layer and gas-liquid chromatography. Disaturated (dipalmitoyl) phosphatidylcholine made up about 31% of the total phosphatidylcholines. Other species which occurred in significant concentrations included 16:0/18:1, 18:0/18:1, 16:0/18:2, 18:0/18:2, 16:0/18:3, 18:0/18:3, 16:0/20:4, and 18:0/20:4. These results indicate that rat gastric mucosa is similar to lung in that both contain elevated amounts of dipalmitoyl phosphatidylcholine. Other similarities between these two tissues are discussed.
