[Report on the 89th Scientific Assembly and Annual Meeting of the Radiological Society of North America--analysis of radiation dose by a change of a scan parameter of multi-slice computed tomography by a film method].

PURPOSE: It is for a purpose of this study to measure radiation dose by analyzing a dose profile of multi-slice computed tomography varying with helical pitch and a row slice thickness difference complicatedly. MATERIALS AND METHODS: We used multi-slice computed tomography, and helical pitch and row slice thickness change and scanned the helical scan. I used CTDI phantom of a diameter of 25 cm and I inserted roentgen diagnosis use film UR-2(new) which I put between my own phantom in center and 1 cm away from the outer surface and scanned it. And the provided level profile was converted into a dose profile with the dose-density curve which I made beforehand. I analyzed radiation dose than the dose profile. RESULT: In multi-slice computed tomography, radiation dose varied with assembly of row slice thickness and helical pitch. The change of a dose profile changed in a phantom surface part complicatedly. The maximum dose by the measurement of this time was 29 mGy in row slice thickness 0.5 mm, assembly of helical pitch 2.5. In addition, the minimum dose was 6.8 mGy in row slice thickness 3.0 mm, assembly of helical pitch 5.5. And, as for the difference of maximum dose in the same dose profile and the smallest dose, there were about 20 % in row slice thickness 1.0 mm, assembly of helical pitch 5.5. CONCLUSION: The dosimetry of multi-slice computed tomography by a film method enabled it to measure a change of a dose profile by a difference of a scan parameter by high interest solution ability. In addition, it is a method more superior in dosimetry of multi-slice computed tomography spreading through a Z-axis direction broadly than determination by computed tomography use ionization chamber dosimeter. Because radiation dose increases by a scan in thin row slice thickness and small helical pitch, care is necessary.

[Primary effect of preoperative intra-arterial infusion chemotherapy in cervical cancer].

In order to obtain better prognoses for cervical cancer, we conducted preoperative intra-arterial (i.a.) infusion chemotherapy of CDDP in combination with AT-II pressor. Two courses of i.a. chemotherapy were performed for 67 patients with cervical cancer in stage IIb containing 54 squamous cell carcinomas and 13 adenocarcinomas before operation every 3 weeks. A histologically desirable effect in cervical lesion was obtained. A group given preoperative i.a. chemotherapy had a significantly lower rate of infiltration beyond uterus in comparison with 156 patients with 145 squamous cell carcinomas and 11 adenocarcinomas treated by surgery alone as the control group. Further, the rate of histological infiltration to parametrial edges after i.a. chemotherapy tended to be lower than the control group. From the above, it was considered that preoperative i.a. infusion chemotherapy of CDDP in combination with AT-II pressor for cervical cancer was effective as a neo-adjuvant chemotherapy.

[A clinicopathological study on adenocarcinoma of the uterine cervix].

A total of 102 cases of adenocarcinoma of the uterine cervix were reviewed to define the clinical and pathological behavior. In the 18 years between 1970 and 1987 the incidence of cervical adenocarcinoma increased. The rate of lymph node involvement in stage Ib was 16.1% (5/31) and in stage II 26.2% (11/42). There was no difference between the rate of lymph node involvement in adenocarcinoma and squamous cell carcinoma. The 5-year survival rates were as follows: stage I 83.3% (25/30), stage II 58.1% (18/31), stage III (0/4), stage IV (2/5), and all stages 64.3% (45/70). The 5-year survival rate for patients with adenocarcinoma was poor in contrast to that for patients with squamous cell carcinoma (p less than 0.05). It was suggested that adenocarcinoma was less radiosensitive than squamous cell carcinoma.

[Pharmacokinetic and clinical studies on cefodizime in the field of obstetrics and gynecology].

We have conducted pharmacokinetic and clinical trials of a new cephem derivative, cefodizime (THR-221, CDZM), and obtained the following results. 1. We administered CDZM to 4 cases with abdominal simple hysterectomy due to myoma uteri at a dose level of 1 g by drip intravenous injection and studied average levels of transfer measured at various locations in the uterine tissues and adnexa at an average of 2 hours after administration. CDZM level was highest in the oviduct, 13.7 micrograms/g (ratio with respect to the uterine arterial blood: 91.3%), followed by the ovary, portio vaginalis, cervix uteri and endometrium, and was lowest in the myometrium, 8.3 micrograms/g (55.3%). CDZM concentrations were higher than 6.04 micrograms/g in any tissues. 2. To study CDZM transfer to pelvic cavity fluid, we administered CDZM to 5 cases with total hysterectomy due to cervical cancer of uteri at a dose level of 2 g using drip intravenous injection. The drug was transferred at high levels to the pelvic cavity fluid. A level of 11.7 micrograms/ml was observed at 3 hours after injection. The drug levels in the pelvic cavity fluid were maintained continuously higher levels than those of venous blood. These concentrations in the uterine tissues and pelvic cavity fluid were higher than the MIC against many strains of Gram-positive and Gram-negative bacteria, hence we considered them to be therapeutically effective concentrations. 3. Eleven cases of gynecological infections receiving in totals of 8 to 48 g of CDZM demonstrated "excellent" results in 3 cases, "good" in 8 cases. Eight strains of organisms were isolated from 10 cases.(ABSTRACT TRUNCATED AT 250 WORDS)

[Pharmacokinetics of FT-207 rectum suppository in squamous cell carcinoma of the uterine cervix. (I) Pharmacokinetics in single administration at a dose of 1 gram].

In order to use FT-207 rectum suppository for a long-term maintenance chemotherapy of squamous cell carcinoma of the uterine cervix, we examined its pharmacokinetics by single administration at a dose of 1 g employing chemical assay. The results were as follows: 1. The blood level of FT-207 showed its peak of 30 mcg/ml at 2 hours after administration, of which half-time was 9 hours. Inter-individual differences were few in changes in the curve. 2. The blood level of 5-FU showed two peaks, 0.25 mcg/ml at 4 hours and 0.017 mcg/ml at 36 hours after administration. There were large inter-individual differences in its alteration. 3. At the time of operation, both FT-207 and 5-FU showed low concentrations in blood. 4. The level of 5-FU in cervical focus was 0.068 mcg/g which was the highest among those in all genital organs, and 2.7 times higher than the level of 5-FU in blood. 5. There was a significantly positive correlation at r = 0.174 (p less than 0.1) between the level of 5-FU in the focus of cervical cancer and the level of FT-207 in blood.

[Pharmacokinetics of FT-207 rectum suppository in squamous cell carcinoma of the uterine cervix. (II) Pharmacokinetics in continuous administration at a daily dose of 1 gram].

FT-207 rectum suppository at a daily dose of 1 g was repeatedly administered to patients with squamous cell carcinoma of the uterine cervix. Total doses ranged from 7 to 41 g. The following pharmacokinetic results were obtained in comparison with single administration. The blood level of FT-207 showed its peak of 35 to 37 mcg/ml at 4 hours after administration, of which half-time was 11 to 12 hours. The blood level of 5-FU showed its peak of 0.038 to 0.041 mcg/ml at 2 to 4 hours after administration, of which half time was more than 24 hours. Inter-individual differences were less than those of single administration. The level of FT-207 in tumor tissue was 14.2 mcg/g which was 15 times higher than that single administration. The level in healthy uterine cervix was 18.1 mcg/g which was 12 times higher that of single administration. The level of 5-FU in tumor tissue was 0.359 mcg/g which was 5 times than that of single administration. The level in normal tissue was 0.15 mcg/g which was 3 times higher than that of single administration. The level of 5-FU in tumor tissue was 7.8 to 8.4 times higher than that in blood. There was no difference in concentrations of both FT-207 and 5-FU between metastatic and non-metastatic lymph nodes.

[Functional saving in the treatment of cervical cancer].

The radicality of Okabayashi's radical hysterectomy for treatment of cervical cancer has been well-known. However, its radicality also brought up the disturbance of fertility, endocrine and sexual function as well as the dysfunction of micturition and defecation. As early cervical cancer cases have increased yearly, and social rehabilitation became more necessary nowadays, functional saving is important and must be considered as the responsibility of the oncologists. This report denotes the preservation of ovarian function, sexual rehabilitation as well as the fertility in the treatment of cervical cancer.

[Treatment of cervical cancer in the aged].

The incidence of elderly patients in cervical cancer cases at The Department of Obstetrics & Gynecology, Okayama Medical School has been increases to 7.8% during the recent 5 years (1975-1979), comparing to that of the past period 1.9% (1934-1962). According to the statistics of The Ministry of Health and Welfare, the mortality of cervical cancer has been decreased in all the ages except the elderly patients, in which the mortality showed no marked fluctuation or somewhat elevation. According to the report of The Ministry of Health and Welfare, among the 2,014,843 cases of mass screening examination of cervical cancer, the incidence of elderly patient over 70 year old was 0.3% only, and the suspiciously malignant rate was 4.58% in this group. This means that in the elderly group, the incidence of mass screening examination was very low, and the morbidity was relatively high. Reviewing the elderly patients of the recent period, we found that about half of the patients were belong to the Stage III or IV, and even in the early cancer cases, the general diseases were complicated so much, that the main therapy was radiation rather than operation. Reviewing the prognosis and the irradiation dose in the radiation group, we found that the prognosis of the cases who received 4,000 rads external irradiation were better than that of the cases who received 5,000 approximately 6,000 rads. However, sometimes the complication of irradiation were too severe to continue, resulting in an incomplete treatment and bad prognosis. Considering about this, we select the cases to operate according to the physical state but not the age as indicated up to date, and the result was that, even in the aged group, the prognosis of the operable cases were comparable to that of the young aged. As a conclusion, the following can be said: 1) As the mean age has been prolonging, the incidence of the elderly cancer cases will increase progressively, and this will influence the treatment result of cervical cancer. 2) In the aged patients, the prognosis were influenced by the stage rather than the age, therefore, consideration of the treatment according to the stage is important.(ABSTRACT TRUNCATED AT 400 WORDS)

[Isolated aneurysm of the external iliac artery caused by non-specific angiitis].

Isolated aneurysm of the iliac artery is considered to be a rare disorder, although iliac aneurysm associated with abdominal aortic aneurysm is more common. Its clinical diagnosis is difficult, because symptoms are absent if an aneurysm is small, and because symptoms often mimic urogenital, neurological or gastrointestinal diseases if an aneurysm grows large without rupture. We reported a 40-year-old woman with isolated aneurysm of the left external iliac artery caused by giant cell arteritis, and surgically treated with success.

Patient dose calculated from exposure parameters on abdominal X-ray examination: phantom experiments and clinical application.

In the field of radiology, new modalities have been developed and expanded. However, X-ray-based diagnostic imaging remains the main stream. Therefore, radiological technologists must provide dosimetry and other physical data related to radiation exposure. In this report, the basic information presented in our previous reports is supplemented with data regarding the relationships between exposure parameters and dose for inverter-type X-ray generators, which have been widely adopted in recent years. This report also describes the measurement of percentage depth dose. We also conducted a survey of medical facilities in Okayama prefecture to gather data concerning the radiographic parameters used for abdominal examinations, and calculated the patient dose from these data. We tabulated and calculated the patient dose for each hospital. The results showed a wide range of variation. The overall mean values were 0.337 cGy for the surface dose and 0.061 cGy for the dose at a depth of 9 cm.
