The authors report a case of unilateral moyamoya disease which developed into moyamoya disease 3 years later. Unilateral moyamoya disease is generally defined as moyamoya disease, but the exact relationship is unknown. In this case, occlusive changes developed in the stenotic carotid fork, and in a similar portion contralaterally which was intact. Follow-up 4-vessel angiography is strongly recommended even for unilateral moyamoya disease.
We synthesized two oligodeoxyribonucleotides bearing an isopropyl phosphotriester at defined positions using new phosphorobisamidite chemistry. Diastereomers were separated with HPLC and their template properties were analyzed. Priming oligodeoxyribonucleotides labeled with 32P at the 5'-end were annealed to the modified oligodeoxyribonucleotides and elongated with DNA polymerase I large fragment from Escherichia coli. Results show that the phosphotriester inhibits the DNA chain elongation partially and the extents of the inhibition are remarkably different between the two diastereomers.

Forty-six patients with adenocarcinoma of the prostate were given an intravenous infusion of the thymidine analogue, bromodeoxyuridine (BrdU), 200 mg/m2, at the time of needle biopsy or transurethral resection to label tumor cells in the DNA synthesis phase. The tumor specimens were stained by an indirect immunoperoxidase method with anti-BrdU monoclonal antibody. The BrdU labeling index, S-phase fraction, was determined by counting the number of BrdU-labeled cells in the tissue sections. S-phase fraction correlates with the results of histologic tumor grade, Gleason score, and growth patterns. The higher S-phase fraction may indicate biologic malignancy. Moreover, the degree of heterogeneity concerning S-phase fraction distribution within prostate cancer tissue could be evaluated and the findings compared with the morphologic appearance. The authors results suggest that the measurement of BrdU labeling index in prostate cancer may prove to be a new objective and quantitative assay for biologic potential of individual tumors.
A new bisphosphonate, 4-amino-1-hydroxybutylidene-1, 1-bisphosphonate (AHBuBP), was compared with 3-amino-1-hydroxypropylidene-1, 1-bisphosphonate (AHPrBP) and 1-hydroxyethylidene-1, 1-bisphosphonate (HEBP) assessing their effects on tumor induced osteolysis using human prostate adenocarcinoma cells in nude mice. The method consisted of inoculating transplantable human prostate cancer cells subcutaneously over the calvaria in nude mice resulting in a local tumor causing fragmentation of the bone. The parameters included assessing the extent of decreased osteolysis in bone as judged by X-ray and histological examination. The results showed the following sequence of potency: AHBuBP greater than AHPrBP greater than HEBP. The compounds were active not only when administered preventively before establishment of bone resorption, but also in an inhibitory fashion once the variables were already under the influence of the tumor. This inhibition was obtained with no apparent effect on the growth of the tumor. AHBuBP appears to be an interesting new bisphosphonate for future clinical use.
We report a case of nonseminomatous testicular tumor with lymph node involvement and multiple lung and liver metastases. The patient was considered to have a poor prognosis. He received systemic chemotherapy and cytoreductive surgery. Postoperatively a residual metastatic tumor in the liver was treated by intrahepatic arterial cisplatinum-diaminedichloride infusion. Follow-up observation has revealed no evidence of recurrence for 30 months since the last treatment.
A rare case of a type 2 proatlantal artery discovered following the rupture of a cerebral aneurysm in a 74-year-old female is reported. The aneurysm was clipped and the hematoma removed, but she died of severe vasospasm 9 days after surgery. The anomalous artery was thought to have been unrelated to rupture of the aneurysm.
A new experimental method to test the effect of drugs on tumor induced osteolysis using a transplantable bladder tumor (MBT-2) in mice has been designed. The method consists of inoculating tumor cells sc over the calvaria in mice, causing in a local tumor with fragmentation of the bone. This was accompanied by adjacent osteoblastic changes, which were evaluated by X-ray and histological examination. APD, a bisphosphonate derivative, at a dose of 3 mg/kg to 10 mg/kg sc protected the bone by decreasing the extent of osteolysis as judged by the same criteria. This inhibition was obtained without apparent effect on the growth of the bladder tumor in mice.
We report 31 cases of renal cell carcinoma treated with interferon. Indicators defined as bidimensionally measurable mass were present in 20 cases, which included 19 cases with metastatic lesions and one case with primary site who did not receive nephrectomy. Treatment were performed with interferon-gamma in 9 cases, and with interferon-alpha in 14 cases. Of them, 3 cases received alpha-interferon following gamma-interferon treatment. Remained 11 cases had no evidence of disease after radical nephrectomy or surgical removal of metastatic lesions, and received interferon-alpha as post operative adjuvant therapy. In 18 evaluable cases, one case (6%) showed minor response; 8 cases (44%) no change and 9 cases (50%) progressive disease. Survival rates (Kaplan-Meier's method) of minor response and no change cases at 1, 2 years were 89%, 59%. Those of progressive disease cases were 22% and 11%, respectively. Of 11 cases received post operative adjuvant therapy, recurrence was observed in four cases (36%) with the mean follow up period of 11 months. Frequent side effects were fever (62%), leukocytopenia (56%) anorexia (38%), fatigue (26%). Efficacy of interferon to metastatic renal cell carcinoma in this study is limited. Further studies are required to determine the benefit of post operative adjuvant therapy with interferon.
A clinico-statistical study was made on 92 patients with renal cell carcinoma who visited us from January 1978 to November 1988. The results were as follows: 1. Seventy-eight patients were treated by radical nephrectomy, two patients by partial nephrectomy, and three patients were treated at other hospitals initially. Nine patients were underwent only conservative therapy. 3. Overall 1, 3, 5, and 10-year survival rates by Kaplan-Meier method were 81.8%, 58.0%, 55.3%, and 50.2%, respectively. 3. The 3-year survival rates according to clinical T-stage were 71.3% for 56 patients in T2, 66.8% for 16 patients in T3a, and 24.1% for 16 patients in T3b. A significant difference existed between patients in T3a and patients in T3b. It appears that the new TNM classification system is more accurate for prognosis of patients in stage T3 than that currently in use.
Utilities of deoxyribonucleoside 3'-O-phosphorbisdiethylamidites in the synthesis of oligodeoxyribonucleotides and their analogues are described.
The purpose of this paper is to report the motor action potential (MAP) following transcranial stimulation and absolute refractory time (ART) in somatosensory evoked potential (SSEP) in normal rabbits. Twenty rabbits were used for this study. The MAP was recorded from a needle electrode that was inserted to the thoracic erector spinae muscle, quadriceps muscle and crural triceps muscle. The transcranial stimulation was applied on the motor area of the cerebrum by using a high voltage stimulator. ART was thereby investigated with the double stimuli method in SSEP. The waveform of the MAP was very similar to that of MAP evoked by the stimulation of peripheral nerve. A high degree of correlation existed between the latency of the MAP and distance from the stimulation site to the recording site. The ART was less than 40 ms and its mean value was 25.0 ms.
The motor action potential (MAP) following transcranial stimulation and absolute refractory time (ART) in somatosensory evoked potential (SSEP) were investigated after experimental spinal cord injuries in rabbits. The thoracic cords were injured at the level of the 11th vertebrae by Allen's method. The paralysis after the trauma was classified into 4 groups depend on its severity; severe, mild, transient and non palsy group. A single transcranial stimulation evoked the double MAPs (MAP1 and MAP2) which were characteristic for the palsy groups. The amplitude of the MAP1 was greater than MAP2 in the transient palsy group, while that of the MAP1 was lower than MAP2 in both severe and mild palsy groups. The amplitude of the MAP was significantly reduced in all injured animals. There was no correlation between the amplitude or latency of the SSEP and the severity of the palsy. However, the ART of the SSEP was considerably prolonged in all injured animals regardless of the severity of the palsy.
Distribution analyzing latex immunoassay (DALIA), a sensitive method based on the agglutination of latex particles coated with monoclonal antibody, was developed for determination of fibrin degradation products (FDP) in human plasma. The agglutination is measured by using an electric particle counter designed for medical research to determine the volume-ratio of agglutinated to non-agglutinated particles. Interference by serum constituents is avoided by coating the particles with the F(ab')2 fragments of monoclonal antibody and by using suitable latex particles. This assay enabled the determination of FDP in concentrations of 16 to 4000 ng/ml in less than 15 minutes. The mean concentrations of FDP in the plasma of 98 normal subjects and 142 patients with apparent disseminated intravascular coagulation (DIC) were 0.69 and 13.24 micrograms/ml, respectively. Analytical recovery of FDP in plasma samples averaged 104%. The correlation coefficient of DALIA with enzyme immunoassay using two monoclonal antibodies, was 0.809. The results obtained by analyzing the cross-reactivity with related components of FDP and the inhibitory effect of D monomer indicated that the present method can effectively detect the high molecular weight (HMW) fragments of FDP produced in the early stage of plasmin digestion.
A total of 18 patients with transitional cell bladder cancer was given a 0.5-hour intravenous infusion of bromodeoxyuridine at the time of endoscopic biopsy or transurethral resection to label tumor cells in the deoxyribonucleic acid synthesis phase (S phase). The tumor specimens were fixed with 70 per cent ethanol, embedded in paraffin, sectioned and stained by an indirect immunoperoxidase method with anti-bromodeoxyuridine monoclonal antibody as the first antibody. The bromodeoxyuridine labeling index, S phase fraction, was determined by counting the number of bromodeoxyuridine-labeled cells in the tissue sections. All grade 1 tumors had an S phase fraction of lower than 10 per cent. The average S phase fractions for noninvasive (11 cases) and invasive (7) tumors were 9.8 and 20.0 per cent, respectively. Two distant metastatic bladder tumors showed an average S phase fraction of 25.3 and 30.0 per cent. Thus, transitional cell bladder cancers with an S phase fraction of greater than 10 per cent appears to grow faster and be more invasive more often than those with an S phase fraction of less than 10 per cent. The higher S phase fraction may indicate greater biological malignancy. Our preliminary results suggest that measurement of the bromodeoxyuridine labeling index in bladder tumors may be a new objective and quantitative assay of biological potential of individual tumors.
