Optimizaition of a low-frequency ultrasonic technique to monitor the change in physical states in viscoelastic media: gelation process

A new low-frequency ultrasonic device (50-100 kHz) in highly sharpened end sensors that behave as point sources were examined. The application of this new ultrasonic technique with two sensors coupled in the near field is to explore the relations between the physical properties measured through the evolution of the wave time of flight and structural changes during gel formation which is related to two factors: the ambient temperature and the mechanical resistance of the medium. The network evolution was interpreted by an approach based on the Flory model. The physical significance of this model was shown through a series of experiments using a low-frequency ultrasonic technique. Response curves demonstrate the different stages during gel formation.

Comparison of thermal injury zones in loop electrical and laser cervical excisional conization.

Thirty cervical conization specimens obtained by thin-loop electric excision were compared with 30 carbon dioxide laser excisional cone specimens. Zones of thermal artifact were measured with a precise stage micrometer technique calibrated to a Zeiss standard. The mean depth of coagulation at the ectocervical margin measured 0.187 and 0.164 mm for thin loop and laser, respectively. Thermal artifact at the endocervical margin was greater with electric loop (0.295 mm) than with superpulsed carbon dioxide laser (0.137 mm). The zones of thermal injury induced by both electric loop and laser made no significant impact on the interpretation of margin adequacy.

Electrosurgical thin loop conization by selective double excision.

Thin loop electrosurgical conization of the cervix was performed in 30 women, using a selective double excision (SDE) technique in order to conserve cervical tissue. The entire excisional cone was completed within 4 minutes, and blood loss was estimated at 5 ml or less. All cervices healed in patterns identical to CO 2 laser conizations. Critical tissue margins were easily identified by the pathologist. Compared to simple loop excisional cones, SDE saved greater than 2 cm3 of cervical stroma.

Aorto-femoral bypass with polytetrafluoroethylene prostheses: preliminary results in 363 cases.

From October 1977 to October 1982, 363 unilateral aorto-femoral bypasses using polytetrafluoroethylene (PTFE) prostheses were performed for predominantly unilateral aorto-iliac disease. The distal anastomosis was extended into the deep femoral artery in 57% of the patients. The postoperative mortality was 0.5%. The actuarial patency rate after 6 years was 87% in patients with claudication and 77% in those with critical ischemia. No false aneurysm developed. Thrombosis of the prosthesis was due to progression of distal disease, intimal hyperplasia and postural extrinsic compression. The latter seems to be characteristic of PTFE prostheses and can be treated by thrombectomy. In 21 cases a cross-over femoro-femoral bypass was done during the follow-up period because of contralateral progression of disease.
