Sexual behavior and smoking: risk factor for urethritis in men.

Two hundred men who had symptoms of urethritis and 207 controls were interviewed. Because 60% of the men were of foreign nationality, the analysis of risk factors for urethritis was undertaken separately for the French and foreign subpopulations. The risk factors for urethritis were different for the French and foreign men. For the French men who had smoked, the risk of infection was 2.9 times greater than that for men who had never smoked (95% confidence interval, 1.2-7.6), after taking type of sexual behavior into account. There is a significant increase in the risk of urethritis when the amount of tobacco consumed daily increases (P less than 0.02). This finding suggests that smoking is an independent risk factor.

Evaluation of a new chemiluminometric immunoassay, Magic Lite Chlamydia, for detecting Chlamydia trachomatis antigen from urogenital specimens.

The chemiluminometric sandwich immunoassay, Magic Lite Chlamydia by Ciba Corning (Medfield, MA), using one Chlamydia trachomatis-specific monoclonal antibody conjugated with acridinium ester and one polyclonal immobilized to magnetic particles, was compared to cell culture in 291 urogenital specimens from 92 men and 102 women tested both in the cervix and urethra. The proportion of patients with positive culture results ranged from 11.8% among the women to 15% among the men, using microculture plates and peroxidase-antiperoxidase staining procedure with genus-specific monoclonal antibody (Orthodiagnostics, Raritan, NJ). The chemiluminometric method for detecting Chlamydia trachomatis antigen showed an overall sensitivity of 72.4%, specificity of 97.7%, positive predictive value of 77.7%, and negative predictive value of 96.9%. A higher sensitivity of Magic Lite was found in clinical specimens yielding more than 10 inclusions by well in cell culture. The newly developed chemiluminometric test, easy to perform, had the same limits in terms of sensitivity and positive predictive value than the other widely-used existing tests for detecting Chlamydia trachomatis antigen from urogenital specimens.

[Monoclonal immunoglobulins, cytomegalovirus infection and malignant observations blood diseases].

Monoclonal immunoglobulins (Eg) are detected in the serum of 7/29 patients treated for leukemia (6 cases) or lymphoma (1 case). All of them developped cytomegalovirus (CMV) infection. The 7 patients were children or young adults; 6/7 were in complete remission of the hematological disease. They completed chemotherapy before the onset of the viral infection and had received blood components. The monoclonal Ig are IgG (6 cases) and IgM (1 case), In 2 sera there are two M components. The level of the peak was under 1 g/100 ml (5 cases) and reached 1.5 and 1.7 g/100 ml in the 2 other sera. Polyclonal Ig are normal except in one case where there is moderate hypo-Ig; Plasmocytic infiltration was observed in none of the 7 patients. Evidence of CMV infection was confirmed by viremia and/or significant rise of complement fixing antibodies. In four cases, the monoclonal Ig disappeared in less than 6 months. The significance of the monoclonal Ig associated with CMV infection is discussed.
