Recombinant human basic fibroblast growth factor (Rh-bFGF) in three different wound models in rabbits: corneal wound healing effect and pharmacology.

Prior to a clinical trial in humans, we studied the effect and pharmacological distribution of recombinant human basic fibroblast growth-factor (Rh-bFGF) in vivo. Healing experiments on de-epithelialized rabbits corneas (n = 24 animals) compared the efficacy of three bFGF doses to controls and revealed a significantly increased healing rate for both 200 ng and 500 ng per application Rh-bFGF treatment groups compared to the control groups. To assess possible side effects of Rh-bFGF (500 ng topically applied for up to 7 days, twice daily), ten rabbits were involved in a model of an anterior keratectomy wound (performed with Draeger's roto-keratome to a depth of 0.15 mm). Light microscopy of thin sections of treated corneas showed an increased fibrogenesis in the anterior stroma with a more pronounced activation of keratocytes. No evidence for abnormal neovascularization or inflammation was observed when compared to control corneas. Ocular penetration and systemic distribution of topically applied labelled 125I FGF was assessed in three models (iodine vapour epithelial burn, anterior keratectomy and penetrating autokeratoplasty) in 24 rabbits. No intraocular penetration of bFGF occurred as shown by direct gamma counting. Macroautoradiography showed a selective labelling of epithelial basement membrane when denuded and intact, as previously described. Evidence for systemic absorption of breakdown products was confirmed by heparin-sepharose chromatography of blood and urine samples. Under these conditions, we suggest that topical Rh-bFGF promotes corneal wound healing without morphological adverse reaction or intraocular and systemic penetration.

Lectin-binding sites in quick-frozen, freeze-substituted extracellular matrix of rabbit corneal stroma: a comparative electron microscopic study with different embedding procedures.

The efficiency of Epon, Lowicryl K4M and LR White resins for electron microscopy was tested on the reactivity of carbohydrate residues to lectins in quick-frozen, freeze-substituted extracellular matrix of rabbit corneal stroma, without the use of any chemical fixation. Three lectins conjugated to gold particles were used in a post-embedding method: Wheat germ agglutinin, Concanavalin A and Ricinus communis agglutinin 1. Best gold-labelling was obtained with Lowicryl K4M for Wheat germ agglutinin, and with both Lowicryl K4M and Epon for Concanavalin A and Ricinus communis agglutinin 1. Tissues embedded in LR White gave less intense labelling for the three lectins used in comparison with the two other resins. This study demonstrates the crucial role of resins in the optimal preservation of biological activity of carbohydrate residues in cryotechnically-processed cornea for electron microscopy.

Oral acyclovir for herpes zoster ophthalmicus.

BACKGROUND: Reports on the natural history of herpes zoster ophthalmicus stress its high morbidity related to vicious scars on eyelids, ocular complications, and post-herpetic neuralgia. Early treatment with oral acyclovir is effective, but the optimal duration of treatment has not been defined. METHODS: The authors performed a bicentric, prospective, randomized, double-masked study of 86 patients with acute herpes zoster ophthalmicus, within 72 hours of skin eruption, who received oral acyclovir (800 mg 5 times daily), either for 7 days (plus 7 days oral placebo) or for 14 days. All patients concomitantly received ophthalmic 3% acyclovir ointment; follow-up was at least 6 months. RESULTS: Statistical analyses of subjective symptoms, skin lesions, and ocular complications showed no significant differences between the groups, suggesting that a 7-day course of treatment was sufficient. Drug tolerance was good. Pooled data from both groups corroborated earlier reports that prompt treatment with oral acyclovir reduces the severity of the skin eruption, the incidence and severity of late ocular manifestations, and the intensity of postherpetic neuralgia. At 6 months, late ocular inflammatory complications were seen in 29.1% of our 86 patients, versus 50% to 71% of untreated patients described by others. Only 13% of our patients experienced post-herpetic neuralgia, which in no case required the use of analgesics. CONCLUSION: The authors believe it is not useful to prolong treatment with 800 mg of oral acyclovir 5 times daily for more than 7 days in herpes zoster ophthalmicus. This study confirms the efficacy of oral acyclovir not only against skin lesions and ocular complications, but also against postherpetic neuralgia in herpes zoster ophthalmicus.

A fish-eye disease-like familial condition with massive corneal clouding and dyslipoproteinemia. Report of clinical, histologic, electron microscopic, and biochemical features.

Fish-eye disease (FED) is a rare familial condition characterized by progressive bilateral corneal clouding and dyslipoproteinemia previously described in one family and an unrelated woman of Swedish descent. Biochemical studies have clearly demonstrated the existence of this entity as a unique dyslipoproteinemia. We present a non-Swedish family of Mediterranean ancestry afflicted with bilateral corneal clouding and lipoprotein analysis consistent with FED-like state. This family's biochemical profile, corneal button histology, and electron microscopy of one member are reviewed. Other dyslipoproteinemias causing corneal changes are considered. Corneal tissue and familial biochemical analyses differed significantly from previous descriptions. On the basis of these findings, explanation of pathologic deposition and disease mechanism is proposed.

Human recombinant bFGF stimulates corneal endothelial wound healing in rabbits.

We have previously shown that bovine, human placenta extracted and recombinant human basic Fibroblast Growth Factor (bFGF) are effective in enhancing corneal epithelial wound healing in vivo. In the present study, we investigated the effect of rh-bFGF on the regeneration of injured rabbit endothelium. A standardized wound was created by scraping of endothelial cells with a special device within the boundaries of a central epithelial trephine mark of 7 mm in diameter. A single dose of 1.5 micrograms rh-bFGF was injected into the anterior chamber immediately after wounding, while control eyes received the vehicle only (n = 27). Functional recovery and wound closure rates were assessed by means of ultrasonic pachymetry, corneal button wet weight, endothelial vital staining as well as direct computer assisted surface analysis of Janus green stained corneal buttons. Measurements were carried out 1, 2, 4, and 7 days after injury. Morphological evaluation and cell counts at D4 and D7 were also performed. Significant stimulation of endothelial regeneration in rh-bFGF treated eyes, was observed with all methodological approaches. These results demonstrate the effectiveness of rh-bFGF in enhancing experimental corneal endothelial wound healing and advocate for a possible clinical application of this growth factor in order to preserve endothelial cell function or to promote healing of this important monolayer in case of disease or injury.

[Bilateral hidrocystoma of the median canthus. Apropos of a case].

Cystic lesions of the eyelids are not uncommon and are variable in nature. A case report of a 42-year-old male patient is detailed. A bilateral cystic lesion was clearly visible at both medial canthuses. Surgical resection was performed under local anesthesia, followed by histopathological study of the specimens. This revealed that these lesions were hidrocystomas, of probable eccrine origin. As many different cystic lesions may be encountered on the eyelids (dermoid cysts, epidermic cysts, cystic basal cell carcinomas...), routine histopathological study of surgically removed cystic specimens is mandatory to obtain an accurate diagnosis.

[The keratocyte or fibroblast of the cornea: morphological and biochemical characteristics in normal stroma and a few cases of corneal dystrophies].

Keratocytes are differentiated mesenchymal fibroblast which are specialized in the production and maintenance of the transparent corneal stroma. These elongated cells with large nuclei and scant cytoplasm produce the collages, proteoglycans, and glycoproteins which make up the stroma, as well as the enzymes involved in post-translation modifications and catabolism of these macromolecules. The characteristic morphological features and specific metabolic activity of keratocytes undergo significant changes during tissue repair and in several corneal diseases. Alterations in keratocyte biosynthetic activity, regardless of their cause, ultimately lead to fibrillar alterations responsible for a loss of corneal transparency.

[Scleral erosion after surgery of retinal detachment disclosed by isolated corticodependent hyalitis. Apropos of 3 cases].

A chronic steroid-dependent hyalitis was the first sign of a transscleral erosion in three patients with severe myopia. This erosion was seen for more than 5 years after the procedure which consisted of a very tight scleral buckling with a 5 mm wide Lincoff sponge. Hyalitis disappeared in all three cases after removal of the material. In one case, the transscleral erosion was the cause of a recurrent retinal detachment. The sensitivity of hyalitis to oral steroid therapy suggests an immune process. Hyalitis in patients with a tight buckle must be cured by removal of the material.

[Pigmented seborrheic wart on the skin of the eyelid].

Pigmented tumors of the eyelid are often difficult to identify accurately, as different tumors may have the same clinical presentation. We report the case of a 84 year old female patient who noticed a black tumor of the lower eyelid, with a recent increase in size. Clinically, this lesion was an exophytic tumor of the medial part of the left lower eyelid, respecting the lacrimal punctum and involving the palpebral margin. A surgical excision was performed. Microscopic examination revealed a pigmented seborrheic keratosis, a benign tumor of the epidermis. Histopathology has a key role in the precise diagnosis of pigmented tumors of the eyelid, in which the differential diagnosis concludes sweat gland cysts, pigmented basal cell carcinoma, naevus and uncommon malignant melanoma.

[Palpebral form of mixed tumor of the lacrimal gland. Apropos of a case].

The clinico-pathological findings in a 45-year-old male patient with a lacrimal gland tumor are reported. This tumor had a spontaneous course of about nine years before it was resected. The clinical presentation was unusual as the tumor appeared as a nodular well-circumscribed swelling of the lateral part of the left superior eyelid. Imaging techniques did not reveal any involvement of the orbital part of the lacrimal gland. However a wide supero-lateral approach was chosen to achieve monobloc resection. Histopathology showed the typical features of an encapsulated pleomorphic adenoma, with no suspicious cytologic changes. Complete resection had a total curative effect for the patient. A wide lateral approach seems to be mandatory for tumors of the lacrimal gland to avoid partial resection and local recurrences.

Eyelid Neoplasms

[Use of perfluorocarbon solutions in the surgical treatment of posterior luxation of the lens].

Posterior lens dislocations represent 20 to 40% of all lens dislocations. It can occur after trauma. Visual prognosis can be good, but most often there are complications such as glaucoma, retinal detachment and uveitis. A few years ago, surgery was difficult, and surgery was delayed until the development of specific complications. A new technique, using perfluorocarbon liquids (PFCL), should be recommended because it is simple, and constantly effective. After vitrectomy, perfluorodecalin is injected into the vitreous cavity. As PFCL have a density twice the density of water, it allows the lens to float on the surface of the PFCL, behind the iris. Lens extraction can be associated with trabeculectomy, and anterior chamber lens implantation. We present a prospective study of 6 cases operated according to this technique. Postoperative results were good: visual acuity was preserved and ocular tension was controlled in cases of preoperative hypertonia. We believe this technique can be recommended after posterior lens dislocation, to prevent complications and to preserve visual acuity.

[Canalicular stenoses complicating the insertion of lacrimal plugs. Incidence and mechanisms].

Changes in canalicular permeability following the insertion of punctum plugs was studied. This occlusive method was used to treat dry eye syndrome in 43 patients over a period of 32 months. All had normal canalicular patency, confirmed on the day of implantation. One hundred and five large plugs were used (2.4 plugs per patient). Twenty seven (25.7%) impassable acquired canalicular stenoses were seen in 15 patients. The canalicular site of the 27 stenoses was the same: junction between the vertical and horizontal portions. Among the 27 stenoses: 4 plugs had disappeared at an unknown time, with follow-up probing taking place after a mean interval of 7.5 (months range: 1 to 15 months); 17 plugs had been expelled after being in place for an average of 5 months (range: 1 to 12 months; standard deviation 3.47), with follow-up examination taking place on average 12 months after insertion (range: 6 to 17 months; standard deviation 3.4); and 6 plugs were still in place after a mean of 7 months (range: 2 to 15 months; standard deviation 4.73) and the stenosis was discovered when they were removed. They were intact. These 6 Cases eliminate any possibility of intra-canalicular migration of the prosthesis--of all or part--of the prosthesis. Stenoses appeared to develop rapidly: 45% of cases were discovered during the first 3 months. 16/27 (59%) of stenoses were investigated a second time, 6 months later on average (range: 2 to 12 months): these 16 canaliculi all remained impassable.

[Bilateral actinic keratosis of the corneal limbus. Apropos of a case].

Keratinised tumors of the conjunctiva are not uncommon lesions. However, different pathological lesions may have a similar clinical. Actinic keratosis is one of these, it is an epithelial pre-cancerous lesion of the conjunctiva, which may become, if untreated, an invasive carcinoma. A case of bilateral lesions of this type is reported in a 61-year-old male patient who used to work outdoors, and the clinical diagnosis was followed by bilateral surgical removal of the two lesions. Histological examination study showed that the acquired modifications of the epithelium were typical of conjunctival actinic keratosis.

Conjunctival Neoplasms

[Pilomatrixoma of the eyelid. Apropos of a case].

Primary adnexal tumors of the eyelids are quite uncommon. This diagnosis should be considered in the case of a sub-epidermal tumor. We report the case of a 25-year-old female patient presenting with a left eyelid nodular tumor, which appeared clinically as a whitish swelling of the lateral part of the eyelid, covered by normal epidermis. Ultrasonography showed a heterogeneous structure probably containing calcified areas. Complete surgical removal was performed under local anesthesia. Macroscopically, the tumor was encapsulated, with an irregular surface, and it appeared stony under section. Microscopically, it was characteristic of a pilomatrixoma, with sheets of mummified cells devoid of nuclei, either clear or basophilic, and partially calcified. Neither granulomatous reaction nor ossification were observed. This clinicopathological report recalls that pilomatrixoma may appear as a single nodular tumor of the eyelid.

[Cataract surgery after injection of silicone].

Thirty eyes were studied retrospectively, after cataract surgery following intraocular silicone oil injection. Cataract surgery took place before silicone oil removal in 9 eyes, at the same time as silicone oil removal in 2 eyes, and after silicone oil removal in 19 eyes. Extracapsular extraction was performed in 27 eyes, and intracapsular extraction was performed in 3 eyes. A posterior chamber intraocular lens was placed in 14 cases, and an anterior chamber intraocular lens in one case. Capsular fibrosis was observed in 19 cases, and it was treated with YAG laser in 10 eyes during follow-up. Twenty five eyes had a postoperative follow up of 3 to 37 months. Total retinal detachment occurred in 2 eyes.

New method of ultrastructural localization of WGA-binding sites in the rabbit cornea using quick-frozen specimens followed by freeze-substitution.

Wheat germ agglutinin (WGA) conjugated with apo-horseradish peroxidase-gold was used as a probe for ultrastructural localization of N-acetyl-glucosamine and N-acetyl-neuraminic acid residues of glycoconjugates in posterior stroma and Descemet's membrane of rabbit corneas. Ultra-thin sections of quick-frozen, freeze-substituted, and Epon or Lowicryl K4M-embedded tissue were used. Numerous WGA-binding sites were detected in the stromal matrix and Descemet's membrane in both embedding procedures. Labelling was more intense with low-temperature embedding in Lowicryl K4M than with Epon. Gold particles preferentially labelled the major collagen fibrils of the stroma but otherwise appeared uniformly distributed over both extracellular matrices, except in some areas of the stromal matrix that were barely labelled. Frequently, the inter-particle distance of gold particles on collagen fibrils corresponded to the banding period of type I collagen fibrils, suggesting that they point out keratan sulphate proteoglycans which contain WGA-binding sites. The combination of cryotechniques and post-embedding procedures may be very effective for lectin histochemistry of the cornea and other eye tissues.
