Sleep apnea following bilateral excision of carotid body tumors.

A patient is described who developed apnea, hypertention, and tachycardia following the excision of bilateral carotid body tumors. Hypertension and tachycardia resulted, most probably, from bilateral ablation of the carotid sinus and/or the nerve of Hering. These phenomena have been described in the literature. The pathogenesis of the apneic attacks may be related to the ablation of the peripheral sensory organs (both carotid bodies), interference with the function of the respiratory automatic control system, and impaired control of ventilation during non-REM sleep. Temporary unilateral paralysis of the hypoglossal nerve with partial airway obstruction possibly represented a mechanical trigger. The apneic attacks subsided but bouts of hypertention and tachycardia persist.

Malignant melanoma of the head and neck in children. Review of the literature and report of a case.

Perusal of the literature revealed that until 1970, 13 children with malignant melanoma of the head and neck had been reported. This group includes two cases of melanoma that developed in a giant cell nevus, as well as one case of congenital melanoma. The biologic features of prepubertal melanoma appear basically analogous to those of the adult variety. Malignant melanoma of the auricle is described in a 2 1/2-year-old child. Surgical therapy was given, and BCG vaccination was used as an adjuvant. The patient is alive and free of signs of the disease 2 1/2 years following the operation. This appears to be the third reported case of malignant melanoma of the external ear in a child.

Variations in acoustic admittance related to type of ear tip. A plea for a standardized method.

In order to assess the validity of using various ear tips, tympanometry and acoustic admittance were studied in 20 young healthy subjects, and three different types of ear tip used to seal the external auditory canal. Significant variations in the admittance components and tympanometry curves were observed, resulting most probably from changes in the canal volume and the physical nature of the tipe. It appears advisable to develop and adopt an appropriate, standard ear tip for studies of admittance audiometry.
