Leprosy occurs rarely in women of reproductive age. Until this report, the treatment of leprosy with dapsone has not been associated with any adverse fetal or neonatal side effects. We have reported what we believe to be the first case of neonatal hyperbilirubinemia after maternal dapsone therapy for leprosy.
Myositis ossificans progressiva is a rare autosomal dominant disorder characterized by a progressive ectopic ossification of the connective tissue of the voluntary muscles and ligaments with associated debilitation and characteristic skeletal malformations. There have been less than 600 cases in the literature. This is the first reported case of a viable pregnancy in a patient with myositis ossificans progressiva.
Chorionic villus sampling (CVS) is a recent advance in prenatal diagnosis in the first trimester. maternal serum alpha fetoprotein (AFP) screening for neural tube defects is done from 14-17 weeks gestation. Previous studies have shown an elevation of AFP levels following amniocentesis. The aim of this study was to determine the effect of CVS on AFP levels immediately following this procedure. CVS was performed under sonographic guidance on 22 patients between 6 and 12 weeks gestation undergoing elective termination of pregnancy. A Portex catheter was used for obtaining villi. Maternal serum AFP levels were ascertained before and after CVS by radioimmunoassay. In the group of patients 8 weeks or less, no change in AFP levels was seen; in patients greater than 8 weeks gestation, a significant rise in post-biopsy AFP level was noted in 7 of 14 patients. Further studies are planned to clarify the effect of CVS on AFP levels in patients with ongoing pregnancies. Clarification of this issue is important to the role of CVS in prenatal screening.

Postdate pregnancy presents a difficult problem in both the antepartum and intrapartum periods. Because of the various complications associated with prolonged gestation, the following precautions should be taken during intrapartum monitoring and delivery: 1. Strict criteria must be established for elective induction should it be needed. Elective induction should be limited to those postdate patients with inducible cervices. 2. Extensive intrapartum fetal heart rate and uterine activity monitoring should be done on all patients. Physicians should be alert and ready for intervention and for any possible abnormal finding. Fetal biochemical assessment should be done liberally, especially when meconium is passed. 3. For patients with thick meconium passage, close intrapartum surveillance should be done by both biophysical and biochemical means. During delivery, attention should be given to minimizing the possibility of meconium aspiration. Good neonatal resuscitation is essential. 4. When the estimated fetal weight is 4500 gm or greater, the possibility of shoulder dystocia should be considered. Unless the patient's previous obstetric history or the progress of labor suggests a possible successful vaginal delivery, cesarean section should be considered.

Rhesus monkeys immunized with the beta-subunit of ovine luteinizing hormone (oLHbeta) developed circulating antibodies which cross-reacted strongly with rhesus monkey chorionic gonadotropin. Normal ovulatory cycles continued, but the fertility of immunized monkeys as compared with that of controls was significantly reduced. Thus, the rhesus monkey represented a useful animal model for the study of certain aspects of an "antifertility vaccine." We investigated the effects of circulating antibodies to oLHbeta on corpus luteum function by measuring production rates (PRs) and peripheral concentrations of progesterone during the luteal phase of the menstrual cycle. Both parameters were significantly lower in immunized animals than in control animals. The length of the menstrual cycle was also significantly reduced. Progesterone PRs were also determined on days 10 and 15 after mating to test the assumption that the antifertility vaccine prevents pregnancy by interfering with "corpus luteum rescue" (the increase in PRs of progesterone usually occurring on day 15 after mating in fertilized animals). PRs increased from days 10 to 15 in pregnant controls, were unchanged in nonpregnant controls, and were significantly lower on day 15 in immunized monkeys as compared with nonpregnant and pregnant controls. These results suggest that "corpus luteum rescue" is suppressed in immunized animals.
