Nature of enhanced mitochondrial oxidative metabolism by a calf blood extract.

The effect of calf blood extract (Solcoseryl, SS) on mitochondrial oxidative function in various states was studied polarographically in vitro. 1) Mitochondrial respiration in all 4 conventional study states (Estabrook, 1967) was enhanced by the addition of SS, including states 1 and 2 (endogenous substrates only). 2) The effect of SS on mitochondrial oxygen consumption was concentration dependent, while ADP/O ratio remained constant. The effect of added respiratory substrates varied with the particular substrate at optimally active concentrations. With suboptimal substrate levels, ADP/O ratios were concentration dependent, in contrast to the SS effect. Under oligomycin ATPase inhibition, SS was no longer active, in contrast to DNP, which remained active. 3) In states 3 (added ADP) and 4 (ADP exhausted), oxygen consumption and oxidative phosphorylation were enhanced by SS in the presence or absence of citrate, glutamate, pyruvate, lactate, or ascorbate. However, in the presence of succinate, SS had no effect. 4) ADP/O ratio was decreased by SS in the presence of added substrate, suggesting that SS activation of H(+)-ATPase enhances ATP hydrolysis as well as oxidative phosphorylation and ATP synthesis. 5) The enhancing effect of SS on mitochondrial function is due to hydrophilic components of SS. The lipidic components obtained by Folch fraction of SS have no effect. It is concluded that the effects of SS respiratory substrates and uncouplers on mitochondrial function are essentially different. SS enhances both ATP synthesis and oxygen consumption by mitochondria.

The time-space correlation method for measurement of erythrocyte velocity in microvessels using a CCD linear image sensor.

A new method for measuring erythrocyte velocity in the microvascular system was developed. A correlation function varying both time and distance, "time-space correlation method," was used in order to improve the time correlation and the space correlation methods. A CCD linear image sensor installed at the center of the focal plane of a camera was used to convert the passage of erythrocytes through the microvascular system to electrical signals via intravital microscopy. The method was applied to small vessels in the mesentery of rats. Using the present method, the time required for the measurement and the length of the vessel necessary for the measurement have been reduced, as compared with other methods.

Decreased cerebral metabolism in stroke-prone spontaneously hypertensive rats (SHRSP) with stroke and its possible improvement by Solcoseryl.

Local cerebral glucose utilization (LCGU) was decreased in SHRSP with stroke compared with normotensive Wistar rats. The decrement of LCGU was less in Solcoseryl-treated SHRSP with stroke than that in saline-treated SHRSP with stroke and these brain areas where LCGU was less damaged, in Solcoseryl-treated SHRSP were consistent with the important functioning sites of emotion, motor movement and memory. The result suggests that Solcoseryl may be useful for metabolic improvement of the brain damage after stroke.

[Study of satellite tumor nodules in renal cell carcinoma].

Topography and histological types of main tumor and satellite tumor nodules (STN) were investigated on 60 cases of renal cell carcinoma by gross and microscopic study by making whole area histological sections. 30-100 histological slides were made from the median sagittal section and multiple horizontal sections of renal cell carcinoma. STN were observed in 28 cases of the 60 cases (46.7%) and could be classified into 3 types according to their location relative to the main tumor. STN could not be observed in some cases despite the large main tumor of 10 cm or more, while STN were observed in some cases with the main tumor of only 2.5 cm or less. High grade and high stage cases showed a high incidence of STN. It is considered that, only in cases, with the tumors of low grade, low stage and has only 1 STN or less, conservative surgery on renal parenchyma is possible. When the case is of low grade and low stage and has a possibility of preserving normal renal parenchyma, conservative surgery on renal parenchyma may be justified even if the tumor is large.

Mechanism of anti-ulcer effect of calf blood extract.

The mechanism of anti-ulcer effect of Solcoseryl, an extract from calf blood, was studied. Solcoseryl (10 ml/kg, i. v.) increased basal and chemically reduced potential difference of the gastric mucosa in rats. Solcoseryl (1-10 ml/kg, i.v.) also increased gastric mucosal blood flow and gastroduodenal HCO3- secretion. These results indicate that Solcoseryl has a mucosal protective effect which may contribute to the mechanism of its anti-ulcer effect.

Klinefelter's syndrome with hypospadias and bilateral cryptorchidism.

A 3-year-old boy was found to have abnormality of the external genitalia at birth. Physical examination revealed hypospadias penis and bilateral cryptorchidism. Chromosomal analysis of peripheral blood showed the karyotype of 47,XXY, and the diagnosis of Klinefelter's syndrome associated with hypospadias and cryptorchidism was made. Klinefelter's syndrome is rare in infancy.

[Treatment of advanced cancer of the urinary tract with interleukin-2 (IL-2)].

Eight patients with advanced cancer of the urinary tract were treated with recombinant interleukin-2(IL-2) at our department between December, 1985 and September, 1987. Patients were given 5 x 10(5)-4 x 10(6) units of IL-2 by intravenous drip infusion daily. One of five patients who received IL-2 for over 28 days showed 62% regression in size of hepatic metastasis of bladder cancer on computed tomography and another showed marked improvement of Performance Status. The upward tendency of natural killer and lymphokine-activated killer activity of peripheral blood was observed during treatment. Fever, mental disturbance, hypotension and eosinophilia and others were recognized during administration of IL-2, and cardiac failure and disturbance of renal function were also recognized as severe side effect.

[Bilateral renal cell carcinoma associated with von Hippel-Lindau disease].

A 52-year-old man was referred to the department of neurosurgery of our hospital for evaluation of left occipital pain on September 27, 1982. Brain CT and arteriography showed midcerebellar tumor and an operation was performed on October 7, 1982. Histopathologically, the tumor was hemangioblastoma (solid type). His whole-body CT showed bilateral multiple renal tumors but no angioma retinae were found. Angiography revealed that the lesions were bilateral multiple (more than 20) renal tumors, bilateral adrenal tumors and left retroaortic renal vein. The patient underwent bilateral radical nephrectomy and lymphadenectomy on December 15, 1982. Renal cell carcinoma (grade II greater than III) with adrenal and left renal vein involvement were noted on the pathologic specimen. Postoperatively, he received supplement therapy with hydrocortisone and hemodialysis, but died of bleeding from the recurrent hemangioblastoma on July 7, 1983.

[Results of pulmonary resection for metastatic renal cell carcinoma].

Nine cases of metastatic renal cell carcinoma to the lung were studied to assess the efficacy of surgical management. Between January, 1965 and December 1981, 116 cases of metastatic renal cell carcinoma to the lung were treated. Nine of these cases (7.8%), were treated with surgical resection for the pulmonary metastases. The overall crude survival rate after pulmonary resection was 33.3% (3/9) at 3 years and 22.2% (2/9) at 5 years. Two patients are long-term survivors, one still being in good health 108 months, and the other 72 months after pulmonary resection. Pulmonary resection for metastatic renal cell carcinoma was considered effective in some selected slow-growing cases as protection against metastasis from a metastasis.

Undifferentiated renal cell carcinoma in infancy: report of a case and review of literature.

A case of rapidly progressed undifferentiated renal cell carcinoma in a 2 2/12-month-old boy is reported. The histology is characterized by sarcomalike spindle and pleomorphic cells and bizarre giant cells, thus creating many difficulties in the differential diagnosis. The diagnosis of renal cell carcinoma was established on the basis of tubular formation by clear and granular cells observed in a few discrete areas. The literature on renal cell carcinoma occurring under 10 years of age was reviewed, with special attention given to histological typing. It is generally believed that renal cell carcinoma in children is well differentiated, but 2 of 39 cases reviewed were undifferentiated. We conclude that renal cell carcinoma in infants or young children may have an undifferentiated, atypical histological appearance and can progress rapidly. The differential diagnosis from anaplastic Wilms' tumor and sarcomatous tumors of the kidney in children is discussed.

[Clinical evaluation of CMA in the treatment of prostatic cancer].

Forty five patients with previously untreated prostatic cancer were given chlormadinone acetate (CMA) at a dose of 100 mg/day (mean 12.6 months), and 42 of them were evaluated for effectiveness. Overall response evaluation revealed 19 complete responses (45.2%), 14 partial responses (33.3%), 4 minor responses (9.5%), and 5 were unresponsive (11.9%). Consequently CMA was judged to be effective in 33 patients (78.6%, complete and partial responses). Response rate was 100.0% for stage A, 88.9% for stage B, 77.8% for stage C, and 57.1% for stage D. The response rate for stage D was relatively high, possibly because the time of judgement of effectiveness was a little too early. As side effects, elevated triglyceride level, impaired liver function and impotence were noted in one patient each. All of them were slight. The testosterone level in both castrated and non-castrated patients were not higher than that in women, and no significant difference in the level was noted between the groups. Nevertheless, the clinical usefulness of castration is not denied. CMA is considered useful as an endocrine therapeutic drug, though its effect on the disease at stage D was insufficient. The administration method, such as sequential treatment and combination treatment of CMA and estrogen preparations, is a future subject.

[A case of embryonal rhabdomyosarcoma of the kidney].

A case of embryonal rhabdomyosarcoma of the left kidney is reported. A 16-year-old boy was admitted with the complaint of left abdominal pain and fever on January 6, 1983. Radiological examination showed a tumor of the left kidney; and, nephrectomy was performed. Histopathologically the entire tumor was composed of undifferentiated round cells. Diagnosis of embryonal rhabdomyosarcoma was made on the basis of special stains including immunohistochemical study with nervous tissue specific enolase. Although radiation and chemotherapy were performed postoperatively, the tumor recurred and the patient died on October 22, 1983. The problems of differential diagnosis of embryonal rhabdomyosarcoma from sarcomatous types of nephroblastoma, particularly rhabdoid tumor and other undifferentiated renal tumors were discussed. Fifteen rhabdomyosarcoma of the kidney including our case have been reported in the Japanese literature.
