[The health of adolescents: what do parents think about it?].

Parental care of adolescent children was studied in 501 families who attended the Centre of Preventive Medicine of Vandoeuvre-lès-Nancy. The health of their adolescent children was the parents' main concern and they considered morale, hygiene and alimentation as being the most important health determinants. Twenty-eight per cent of the parents indicated that they were anxious about their adolescent children. When they contacted the Centre of Preventive Medicine, parents were mostly seeking a regular check-up and information for their offspring. A correspondence analysis showed a breakdown of the parents into several groups, according to their responses and degree of concern for the health of their adolescents.

[Arterial pressure and puberty: relation to height-weight characteristics].

The study of 315 boys and 497 girls, aged 10 to 18 years, showed that blood pressure rises during puberty. Growth in height seems to explain this increase better than sexual maturation, at least in boys. These results confirm the significance of associating blood pressure and height in adolescence. The correlation coefficients between blood pressure, weight indices (weight for height, Quetelet's index, skinfold thickness), weight components (lean body mass, fat body mass) indicate that blood pressure is to a large extent determined by lean body mass (mainly in boys), according to the marked development of the osteomuscular system. We are of the opinion that this should be considered when interpreting blood pressure level in adolescents.

[Measurement of arterial pressure by an automatic method. Comparison of various devices for automatic measurement].

A comparative study of three Dinamap 845 instruments was carried out in order to find out whether the use of several automated blood pressure recorders constitutes a major source of variations in epidemiological studies. The three instruments were compared by means of a variant analysis model and proved to have virtually the same accuracy. Only one of them gave a slightly different diastolic pressure reading. No matter how useful are instruments for automated blood pressure measurement, comparisons between several recorders of the same type and regular testing for accuracy seem to be necessary in epidemiological studies.

[Self assessment of the stages of sex maturation in male adolescents].

Self-assessment of sexual maturation was evaluated in 319 adolescent boys aged 10 to 18 years. Sexual maturation rates were defined by physicians based on physical examination and by adolescents on the basis of five standard photographs corresponding to Tanner's five sexual maturity ratings [5]. For genital development, the agreement between physician and boys is poor. It is good for pubic hair development. The assessments were age-dependent. The use of adolescent self-staging appears of value in studying puberty and adolescence.

[Knowledge, behavior and attitude with respect to cardio-vascular diseases].

972 males and females more than 25 years old were asked by a self-questionnaire about cardiovascular disease (CVD): what do they know and how? What do they think they should do to prevent it? What do they personally do? What do they expect from physicians, society, people themselves? Findings indicate that CVD is regarded as an important problem by most of people. One in two subjects worries about heart disease. 70 p. 100 know at least one cardiovascular disease (infarct more often). 40 p. 100 can quote causes of CVD, emphasizing eating-habits, smoking and drinking, way of life. A correspondences analysis allows to establish a typology of responses: it makes appear clear differences in information according to social class, differences in concern about CVD linked namely with personal disease's experience, differences in psychological attitudes towards CVD's risk. Findings lead to set the problem of turning preventive knowledge into effective changes in health behaviour.

[Course during 5 years of weight in children and adolescents. Preliminary study].

A study of a weight related to height index (WRH) was performed in 5,984 children and adolescents. It showed a change in the mean value of the weight index between 11 and 18 years, particularly in girls. Over a 5 years interval there was a close correlation between weight indices (r around 0.7). However, the probability of remaining in the same index class varied greatly depending on the initial value of index and sex. Occurrence of overweight, as defined by a WRH value greater than or equal to 120, was greater in girls and more frequent during the second decade. This study confirmed the variability of weight index during childhood. Half of the overweight children remained so after 5 years. It also showed a high risk of overweight after 5 years in children with an initial index between 100 and 120. The weight index does not seem to be definitely established during childhood and is therefore inadequate to alone predict overweight 5 years later.

[Changes in the blood pressure during an exercise test in children and adolescents. Results with reference to height].

The variations of blood pressure (BP) during exercise are important when there is a doubt about the normality of the values or a possibility of future abnormality. This is particularly true in children and adolescents with borderline or variable values, whose capacity of cardiovascular adaptation to sport is to be evaluated or who have a family history of hypertension. This study was undertaken to establish the profile of blood pressure on exercise during bicycle ergometry with reference to the height of the child or adolescent. BP and heart rate were measured in 651 healthy children aged 5 to 18 years during stress testing by bicycle ergometry. The sample was divided according to sex and height (120-139, 140-159, 160-180 cm), and according to resting BP values. During simple progressive exercise, the work required is increased by 10 watts every 2 minutes for children less than 140 cm tall, and by 20 watts when over 140 cm tall. BP and heart rate were measured during the last 30 seconds of each stage. Heart rate and systolic BP increased progressively with work load; on the other hand, the diastolic pressure did not change on exercise. The regression equation and the slope of systolic BP with respect to heart rate are reported, and allow comparison of different groups according to height, sex and resting systolic BP. The increase in systolic BP was the same in all groups except for boys taller than 160 cm in whom it was significantly greater (p less than 0,01). Sex and resting BP had no significant effect on the slope of systolic BP on exercise. The same applied to exercise lasting more than 6 minutes. The mean values and standard deviation of systolic BP at a heart rate of 150/min were, for each height group in ascending order: 121.5 +/- 19, 129 +/- 22 and 151 +/- 26 mmHg for boys, and 118.5 +/- 26, 126 +/- 21 and 137 +/- 26 mmHg for girls. In order to simplify the interpretation of the results obtained individually, charts in the form of an ellipse were plotted containing the usual values (95% of a healthy population) of systolic BP on exercise at the heart rate of 150/min, for each sex. They were established with reference to resting systolic BP and height. The diagnostic and prognostic value of this test will have to be evaluated by comparison with values obtained in hypertensive children.

[Relationship with height- and weight-characteristics].

Blood pressure (BP) has been measured by one observer among 837 male healthy subjects, aged 18. As it has been shown in previous studies, there is a strong correlation between BP and height, weight, overweight indices, and arm-circumference. The correlations of height and arm-circumference with BP are not fully explained by the weight. It seems to be, at this age, an association of BP with body dimensions, and not only with fatness. The strong association of BP with the lean body mass confirms this fact. It seems finally that BP level at the beginning of adulthood is, in a large extent, determined by body dimensions and the development of the osteomuscular system, and much lesser by adiposity. On the other hand, evolution during adulthood depends much more upon variation of the weight, i.e of the fatness.

[Parent-child correlations for stature].

A study on the relationship between a child's expected height and his parents' attained adult height was made from data obtained from 15,381 healthy children, age 4 to 18 years, and of French descent. The correlation between child's and parents' height was significant and important. Correlation coefficients increase with the ages of children. The theoretical height which a child might expect to attain in adulthood, based on his parents' heights, could be calculated by diagrams or program cards used in conjunction with a manual calculator. This information might be of importance for pediatric or medical care in school.

Adiposity indices and clinical opinion.

The comparison of various adiposity indices in children and adolescents shows that there is a great similarity between the indices, and also between their ability to distinguish extreme groups. The adiposity indices used are highly correlated with weight and subscapular skinfold thickness, and are not generally independent of height. Differences in these correlations appear during the period of puberty. Discrimination of obese and non-obese subjects by the indices and by clinical opinion agrees in 65%-75% of instances, with no one index agreeing better than the others.

Family resemblance in body circumferences and their ratios: the Nancy family study.

The purpose of this study was to investigate the familial resemblance for individual body circumferences (suprailiac, waist, thigh and arm) and for the waist/suprailiac (as surrogate for the waist/hip) and the waist/thigh circumference ratios, in a sample of 216 unselected nuclear families with children aged 10-25 years. Familial correlations were jointly computed, using the maximum-likelihood method. The highest familial resemblance was observed for the waist circumference and for the waist/suprailiac and the waist/thigh circumference ratios. There was no significant difference between father-offspring and mother-offspring correlations when considering individual body circumferences. In contrast, when considering the two ratios there was a stronger mother-child than father-child similarity. Furthermore, there was a higher resemblance of the mother with her daughter than with her son, but no significant sib sex difference in the father-offspring relationship. There was also a heterogeneity of sibling correlations for the two ratios, the like-sex pairs exhibiting higher correlations than the unlike-sex pairs. Lastly, the similarity observed between spouses, of similar magnitude to the father-offspring correlation, suggests the contribution of environmental rather than genetic factors for explaining the familial resemblance of the two ratios. In conclusion, the great family resemblance for the waist/suprailiac and the waist/thigh circumference ratios (correlations ranging from 0.23 to 0.68) appears remarkable, and should be taken in consideration, given the predictive value of these indices for disease in adulthood.

[Strategy for serum ferritin measurement in 16 to 45 year old women in health screening centers].

Even in industrialized countries, the iron-deficiency anemia is frequent in menstruating women. However, the systematic measurement of serum ferritin is not justified. In this study, a strategy for ferritin measurement has been determined from data of centers for health screening, obtained in 6,098 menstruating women. This strategy is based on biological results (hemoglobin, MCV, RDW, GGT, ALAT) and on responses to the questions about blood donation, birth country and contraceptive habits. The measurement of serum ferritin is realized in 64% menstruating women and 23% have an hypoferritinemia (< 20 mg/l).

[Iron deficiency in patients over 60 years. Descriptive study in the consultant population of health screening centers].

AIM: The aim of the study was to evaluate the frequency of iron deficiency with serum ferritin in elderly population, and to appreciate the opportunity of early screening according to digestive diseases. SUBJECTS AND METHODS: Data were collected from 3524 men and 3120 women aged 60 to 75 years during a health screening examination. Evaluation of diagnosis and treatment were obtained through questionnaire completed by treating physician. RESULTS: The frequency of hypoferritinemia was about 2.3% in our population (hypoferritinemia was defined by serum ferritin<20 microg/L or between 20-40 microg/L if C reactive protein was > 12 mg/L). Anemia was found in 3.3% of patients. Logistic regression model adjusting for multiple variables was used to examine factors associated with hypoferritinemia. The probability was greater among non-anemic patients with chronic digestive bleeding (odds-ratio: 2.3), or with positive occult blood testing (odds-ratio: 2.3). Information about the medical follow-up was obtained in 81% of patients with hypoferritinemia. A digestive exploration was made in 38 cases. Digestive disease was found among 24.3% patients with hypoferritinemia, and three colorectal cancers were observed. CONCLUSION: The screening of hypoferritinemia in elderly population examined in health screening centres could not be recommended as its frequency was low in this population, despite a strong correlation between hypoferritinemia and digestive diseases.

[Nutrition of adolescent girls in Lorraine].

A survey concerning the nutritional habits, using the method of the dietary history, was undertaken among 273 girls aged 10 to 16 and examined at the Preventive Medicine Center in Nancy. The energy amount, on average 2,363 calories/day (9.88 MJ) and the distribution among the various nutriments differed little from the French recommendations. Energy intakes varied with bone ages and the socio-cultural level of parents. No intake changes were found with respect to the degree of overweight or to the growth peak. The study of the distribution of intakes according to the main groups of aliments showed the important place of dairy products (14.6%), breads (13.9%) as well as sweets and pastries (11.8%, 12.9%), distribution independent of age. Lunch represents the most important meal of the day (35.7%). Snacks provide as much energy as breakfast. The latter is insufficient in 20% of cases. However, this insufficiency is compensated during the day.
