[Clinico-pathological study of choriocarcinoma localized in the lung with emphasis on hCG monitoring systems].

We studied 15 cases of lung choriocarcinoma clinico-pathologically with emphasis on hCG monitoring systems. The results are as follows: Vivid tumor cells (VTC) were found to persist in 83.3% (5/6) of cases with a continued "LH level" of urinary hCG over 4 weeks. VTC were found to persist in all 4 cases with cellular response and in 6 of 8 cases with no cellular response. The syncytial cell element was observed in 75% (3/4) of cases with cellular response, but in only 25% (2/8) of cases with no cellular response. VTC were recognized in 83.3% of cases (5/6) with serum beta-hCG higher than the sensitivity of the beta-hCG-RIA system (0.2 ng/ml or 1.5 miu/ml). Even though serum beta-hCG was lower than 0.2 ng/ml or 1.5 miu/ml, VTC were found to persist in 2 of 3 cases. All of 5 cases with beta-hCG-CTP higher than the sensitivity of beta-hCG-CTP-EIA (Enzyme-Immuno-Assay) system (0.2 miu/ml) showed VTC in operative specimens. These results indicate that beta-hCG-CTP-EIA is a most useful means to employ in the monitoring of serum hCG.

[Newly developed enzyme immunoassay for hCG-beta CTP and its significance in follow-up practice of trophoblastic disease].

An enzyme immunoassay (EIA) for hCG-beta CTP was newly developed. Anti hCG-beta CTP antibody was prepared from antiserum to the synthetic peptide corresponding to the carboxyl terminal region by immunization of rabbits with hCG-beta subunit. Specimens to be tested were reacted with the antibody labelled with peroxidase and determined with a spectrophotometer after the addition of o-phenylenediamine. It was found that the sensitivity of the assay for hCG in sera was 0.2miu/ml and that values for the assay were significantly correlated with those for the RIA assay for hCG-beta (r = 0.916, Y = 1.27X + 1.8). Cross reactivity with LH, FSH, hCG-beta or hCG-alpha was observed to be 0.18%, 0.05%, 10.4% or less than 0.011% respectively. The assay has been applied in the management of hydatidiform mole, invasive mole and choriocarcinoma, indicating its usefulness because of the increase in sensitivity roughly 25 times or 10 times as great as hemagglutination or hCG-beta RIA assay.

Malignant fibrous histiocytoma of bone. A review of 13 cases and an ultrastructural study.

Observations on 13 patients with primary malignant fibrous histiocytoma of bone were reported. Included were nine male and four female patients, ranging in age from 6 to 81 years (mean, 44.8 years). Pain was the most common complaint. The interval from the first symptom to the initial treatment varied from 2 months to 20 years. Seven tumors arose in the knee region. Roentgenologically, most of the lesions presented with an osteolytic and destructive appearance. Histologically, highly variable morphologic features existed. The storiform-pleomorphic pattern was found in every tumor, although it was not necessarily pathognomonic for malignant fibrous histiocytoma of bone. Surgery, radical or incomplete, was the primary treatment for all but one patient. Lymph node metastasis was present in three. Five patients died of the disease from 3 to 79 months after the diagnosis (mean, 28 months), all exhibiting metastasis to the lung. Gaucher's body-like structure demonstrated in the electron microscopic study shows the histiocytic quality of malignant fibrous histiocytoma of bone.

Subterminal hydroxylation of fatty acids by a cytochrome P-450-dependent enzyme system from a fungus, Fusarium oxysporum.

The cell-free extract of a cytochrome P-450-producing fungus, Fusarium oxysporum, was found to catalyze the hydroxylation of fatty acids. Three product isomers were formed from a single fatty acid. The products from lauric acid were identified by mass-spectrometry as 9-, 10-, and 11-hydroxydodecanoic acids, and those from palmitic acid as 13-, 14-, and 15-hydroxyhexadecanoic acids. The ratio of the isomers formed was 50 : 36 : 14 in the case of laurate hydroxylation, and 37 : 47 : 16 in the case of palmitate. The reaction was dependent on both NADPH (or NADH) and molecular oxygen, and was strongly inhibited by carbon monoxide, menadione, or the antibody to purified Fusarium P-450. Further, lauric acid induced a type I spectral change in purified Fusarium P-450. Further, lauric acid induced a type I spectral change in purified Fusarium P-450 with an apparent Kd of 0.3 mM. The hydroxylase activity together with cytochrome P-450 could be detected in both the soluble and microsome fractions, and the activity was almost proportional to the amount of cytochrome P-450 reducible with NADPH. It can be concluded from these results that Fusarium P-450 reducible with NADPH. It can be concluded from these results that Fusarium P-450 is involved in the (omega-1)-, (omega-2)-, and (omega-3)-hydroxylation of fatty acids catalyzed by the cell-free extract of the fungus.

[Clinical evaluation of cefmenoxime in chronic complicated urinary tract infection].

The efficacy of cefmenoxime (CMX), which is a third generation, beta-lactamase-resistant cephem with a broad antibacterial spectrum, was examined in 43 patients with chronic complicated urinary tract infections. The usual dosage regimen was given 2 approximately 4 g/day of CMX by intravenous drip infusion over 1 hour. The duration of treatment was 5 days. Fifteen patients were cured and 21 improved, and the effective rate was 83.7%. Bacterial eradication rate in these cases was 88.2%, especially eradication of the original pathogens such as Serratia marcescens, Proteus species and Klebsiella species, occurred in high frequency. Laboratory abnormalities were slight elevation of serum GOT and GPT in 2 cases. From these findings, CMX was considered to be very effective in complicated urinary tract infections.

[Dynamics of various peptide hormone receptors in human placental cell membranes].

The action of peptide hormone receptors on the cell membranes and the cyclic-AMP in its post receptor system dynamics was studied. [Results] Receptors for hCG-LH, hPL-hGH, ACTH and Insulin were found in the placental cell membranes but receptors for FSH and PRL were not found. The possibility was suggested that the cyclic-AMP was the second messenger in the receptor system for hCG-LH, hPL and ACTH. The differences between normal placental cell membranes and hydatidiform mole were studied from the point of view of the receptor level. Receptors on molar trophoblast for hCG-LH, hPL-hGH and ACTH increased about two fold to three fold over those on normal trophoblast. The presence of receptors for hCG-LH, hPL and ACTH was confirmed also in GCH-1 (gestational choriocarcinoma cell line). The possibility is shown that the placenta has an auto regularity system in its own tissue throughout the various peptide hormones receptors such as hCG-LH and hPL.

Purification and properties of a cytochrome P-450 of a fungus, Fusarium oxysporum.

A cytochrome P-450 was isolated from a fungus, Fusarium oxysporum, which grew on a medium containing soybean oil as a sole carbon source. It was found as a heme protein that possess lipoxygenase activity, and seemed to exist in the soluble fraction of cell-free extracts. The cytochrome revealed multiplicity and could be separated into a least 3 fractions (A, B, and C). Two of them, termed Fusarium P-450A and -B, were highly purified. The complex of the ferrous Fusarium P-450 with carbon monoxide showed a Soret peak at 447 nm. The properties of the cytochromes (P-450A and -b) were closely similar to each other, the only detectable difference being in the pI (isoelectric point) value (5.2 and 5.0, respectively). The pI and molecular weight (48,000) values together with amino acid composition of Fusarium P-450 were similar to those of other cytochromes P-450 from various sources. Some other spectral properties as well as interactions with various ligands were also studied. Peroxidase or chloroperoxidase activity was not detected with Fusarium P-450.

Dupuytren's contracture: morphological and biochemical changes in palmar aponeurosis.

The palmar aponeurosis removed from ten patients with Dupuytren's contracture was studied using morphological and biochemical approaches. The histological characteristic of Dupuytren's contracture is the presence of numerous nodules among the lamellar structures of the collagen fibres. In the nodules, there are many active fibroblasts which are surrounded by immature fibres and metachromatic substances demonstrated by toluidine blue staining. Ultrastructurally, the active fibroblasts have the characteristics of myofibroblasts, as previously reported by Dr. Gabbiani. We found that some fibroblasts have intracellular collagen fibrils in the cytoplasm. When assayed by Siegel and Martin's method, lysyl oxidase activity of the palmar aponeurosis was significantly higher in Dupuytren's contracture than in normal hands. Biochemical studies such as electrophoretic analysis of mucopolysaccharides, determination of uronic acid and collagen contents were undertaken to compare the aponeurosis of Dupuytren's contracture with normal cases. The uronic acid contents were higher in Dupuytren's contracture than in the controls. However, no difference between the two groups was found in the collagen contents and in the composition of the mucopolysaccharides. These characteristic features; existence of myofibroblasts and intracellular collagen fibrils and increase in the activity of lysyl oxidase probably play a significant role in the establishment of flexion contracture of the fingers in Dupuytren's contracture.

A new microvascular surgical clip system.

A new microvascular clip system has been designed that consists of 3 sets of different sized clips, an applicator, and an approximator. The basic clip is a one-piece design consisting of two blades extending from an annular base. Its construction is simple and durable. The blade closing force is predetermined for each size clip and is set at pressures that are non-traumatic to blood vessels. The tips of the applicator allow the clip to rotate through a wide arc. Two basic clips can be used with the approximator to form a double clip.

[Variation of growth hormone and prolactin levels in umbilical vein (author's transl)].

Umbilical venous blood, obtained immediately after the delivery of 71 non-complicated pregnants between 38 and 42 weeks of gestation, was investigated for the variation in growth hormone and prolactin levels in the serum. Growth hormone and prolactin levels remained almost constant during the observed period of the gestation. Further, it was shown that while growth hormone levels did not significantly vary through the 24-hour period, prolactin levels significantly did. In prolactin levels, the maximum value at 2 approximately 4AM was significantly about 2 times higher than the minimum value at 6 approximately 8 and 8 approximately 10AM. The levels did not vary through 2 approximately 4 and 10 approximately 12 PM period. These suggest that circadian periodicity exists in the fetal prolactin secretion, but not in the growth hormone, and in addition, the periodicity is not affected by labor and delivery.

An experimental study on traumatic changes in microvessels produced by pressure clamping.

Acute and chronic changes produced in microvessels by clamping with serial clips were investigated by macroscopic observation and by light and scanning electron microscopy. The degree of damage of the artery was well correlated with the clamping pressure. The results of the present study revealed that a pressure of 131 g or less did not produce damage to the endothelium, while a pressure of 250 g or more brought about complete fracture or disruption of the intima and necrosis of the media.

[The clinical values fro chemical constituents of blood in normal miniature Shiba goats (author's transl)].

Using the both methods. RaBA-Super System (Rapid Blood Analyzer System), the clinical values for chemical constituents of blood in Shiba goats bred in Stock Farm, University of Tokyo, were determined on the following items: total protein, albumin, TTT, glucose, BUN, total bilirubin, total cholesterol, triglyceride, GOT, GPT, ALP, LDH, CPK, cholinesterase, calcium and inorganic phosphorus. The results obtained were summarized as follows: 1) All items determined were to be analyzed by means of the RaBA-Super System, although the values for total cholesterol, TTT, ALP and CPK varied considerably with individuals. 2) The values of adult female goats were significantly higher than those of young ones in total protein and albumin, and lower in glucose, cholinesterase, ALP and CPK. 3) The values of GPT and BUN of adult female goats in summer were significantly lower than those in winter and autumn. 4) The values for triglyceride and albumin of adult female goats using the RaBA-Super System were inconsistent with those analyzed simultaneously by the manual method, but a significant linear correlation was recognized between the both methods.
