[Microscopic measurement of the surgical resection margins of laryngeal carcinomas].

OBJECTIVE: To investigate the relationship between the primary sites of laryngeal carcinomas and the surgical resection margins, to obtain quantitative data of the safe distance of surgical margins for supraglottic and glottic carcinomas. METHODS: In this series, 33 cases of surgical specimens of laryngeal carcinomas were examined and measured microscopically by means of cutting strips of tissues adjacent to the tumor and longitudinal serial sections. RESULTS: The pattern and extent of invasion of supraglottic carcinoma were quite different from those of glottic carcinoma. Under microscopic measurement, the tumor invasion distance in 95% of the supraglottic carcinoma samples was within 10.52 mm, while that of 95% of the glottic carcinoma samples was within 4.41 mm. CONCLUSIONS: The local invasion of laryngeal carcinomas appears to be primary tumor site-dependent. The safe distance of surgical resection margins may be set at 12.52 mm and 6.41 mm for supraglottic and glottic carcinomas, respectively.

A multi-queue branch-and-bound algorithm for anytime optimal search with biological applications.

Many practical biological problems involve an intractable (NP-hard) search through a large space of possibilities. This paper describes preliminary results from a multi-queue variant of branch-and-bound search that combines anytime and optimal search behavior. The algorithm applies to problems whose solutions may be described by an N-dimensional vector. It produces an approximate solution quickly, then iteratively improves the result over time until a global optimum is produced. A global optimum may be produced before producing its proof of global optimality. Local minima are never revisited. We describe preliminary applications to ab initio protein backbone prediction, small drug-like molecule conformations, and protein-DNA binding motif discovery. The results are encouraging, although still quite preliminary.

[Pre- and post-surgical orthodontic treatment for skeletal open bite].

OBJECTIVE: To Study the principles and rules of pre- and post-surgical orthodontic treatment for skeletal open bite patients. METHODS: Thirty-two surgically treated open bite cases were analyzed, of which 9 were males, and 23 were females, aged from 16 to 38. Open bite was from 1 to 8.5 mm, average was 4 mm. 31 patients were Class III malocclusion, while 1 patient was Class II malocclusion. RESULTS: 1. Totally 21 patients were treated with orthodontics before and after orthognathic surgery, while 8 patients had pre-surgical orthodontics only, and other 3 had post-surgical orthodontics only. The duration for pre-surgical orthodontics was from 4 to 33 months, average was 12 months. The duration for post-surgical orthodontics was from 3 to 17 months, average was 8.5 months. 2. Presurgical orthodontic treatment included: Alignment of arches, decompensation of incisors, avoiding extrusion of incisors, and slight expansion of arches for coordination of arches. 3. Post-surgical orthodontic treatment included: Closure of residual spaces in the arches, realignment of arches, vertical elastics and Class II or III intermaxillary elastics. CONCLUSIONS: Skeletal open bites require combined orthodontic-orthognathic surgery for optimal and esthetical pleasing results.

Retrograde interlocking intramedullary nailing under arthroscopy for supracondylar femoral fracture.

OBJECTIVE: To evaluate the therapeutic effects of retrograde interlocking intramedullary nailing under arthroscopy on supracondylar femoral fractures. METHODS: From June 1999 to December 2000, 17 patients with supracondylar femoral fracture were treated with arthroscopically assisted implantation of retrograde interlocking intramedullary nail and close reduction. RESULTS: More than 6-month follow-up study after operation in 11 patients revealed that the average healing time was 3 months. Average range of the knee motion for all the patients was more than 90 degrees. There was no implant breakage and infection. CONCLUSIONS: This new method, combining the advantage of arthroscope and retrograde interlocking intramedullary nail, can provide a stable and reliable fixation, and meanwhile is less invasive to the soft tissue and knee, less operative time and blood loss, minimal disruption of the blood supply in fracture site. It is conducive to the fracture healing and the functional recovery of the knee joint and worthwhile to be recommended.

[Management of deep facial burn with early postburn debridement and delayed skin grafting].

OBJECTIVE: To explore optimal methods of the management of deep facial burn, so as to prevent the development of facial scar hypertrophy and deformity. METHODS: Partial tangential excision, delayed skin grafting, early relief of the eyelid eschar and immediate skin grafting were carried out in burn patients with deep facial burn during the early postburn stage. RESULTS: The wounds in 12 cases had healed completely within 3 postburn weeks(PBW). Scar hypertrophy and facial deformity were not observed during the follow-up. All the patients could express natural facial expressions. CONCLUSION: Facial deep burn wound could be managed as soon as possible with early partial tangential excision and delayed skin grafting. As a result, facial scar hypertrophy and deformity could be avoided.

[Mycobacterial spindle cell pseudotumor of lymph nodes].

OBJECTIVE: To study the clinicopathological of mycobacterial spindle cell pseudotumor in lymph nodes. METHODS: Two cases of spindle cell pseudotumor in the lymph nodes of infants were examined using light microscopy. The sections were stained with HE, Ziehl-Neelsen and studied immunohistochemically. RESULTS: Both infants were 1 year of age and had a history of Bacilli Calmette Guerin (BCG) vaccination before discovery of their lymph node lesions. Clinically, the patients presented fever and enlarged lymph nodes. Histopathologically, the lesions showed proliferation of spindle cells arranged in bundles, storiform patterns with abundant capillaries and lymphocyte, plasma cell and neutrophil infiltration forming a spindle cell pseudotumor. Most of the spindle cells were phagocytic cells which contained large amounts of mycobactrium as observed by Ziehl-Neelsen staining. Immunohistochemical staining showed strongly positive result for Mac387 and lysosome, but negative for S-100 in spindle cells and histocytes. CONCLUSIONS: Mycobacterial spindle cell pseudotumor of lymph nodes may occur not only in immunodeficient patients (such as AIDS) but also in infants after BCG vaccination. The lesions may mimic spindle cell tumor in lymph nodes, therefore it is very important to recognize these lesions in order to treat the patient properly.

[Effect of SP on intracellular free calcium concentration in cultured osteoblast].

OBJECTIVE: To investigate the effect of SP on intracellular free calcium concentration (Ca2+)i in cultured osteoblast. METHODS: Effect of SP on intracellular free calcium was measured using cell culturing and laser scanning confocal microscopy with fluo-3/AM as a fluorescence probe. RESULTS: After treated with SP(10(-4) mol/L), 70% of the cells responded to it; the intracellular (Ca2+)i increased markedly [fluorescence intensity increased by (90 +/- 18)%]. CONCLUSIONS: These results suggested that calcium may have a role as a intracellular secondary messenger in the action of SP in osteoblast.

[The effects of otitis media with effusion and hearing loss on the speech outcome after cleft palate surgery].

OBJECTIVE: To find out the exact effect of OME and hearing loss on the speech outcome post palatoplasty in cleft palate patients. METHODS: Otoscopy and tympanometry were performed in 18 cleft palate patients whose ages ranged from 5 to 23 years old and hearing thresholds were also tested before cleft palates repair. All patients were followed up from 3 to 18 months postoperatively. Articulation scores were evaluated and linear correlation analysis was performed between these results and their mean hearing thresholds for air conduction in speech frequencies before the palate closure. The incidences of the OME were compared among the groups with different articulation scores. RESULTS: The postoperative articulation scores correlate negatively (r = -0.504, P = 0.032 < 0.05) to the degrees of hearing loss. But the incidences of OME had no significant differences among the groups with different articulation scores. CONCLUSIONS: The hearing level preoperation is more important than tympanometric data on the foresight of the speech result postoperation in cleft palate patients.

[Orthodontic diagnosis and treatment of unilateral posterior cross-bite].

OBJECTIVE: The aim of this study is to investigate the clinical diagnosis and treatment of unilateral posterior cross-bite and to provide information for correct treatment of this disease. METHODS: The X-ray examination of patients' heads in the posterior-anterior position, analysis of pantomorphic films and model measurement were used for the diagnosis of unilateral cross-bite. According to these data, three types of unilateral posterior cross-bite would be classified, and different orthodontic appliances would be applied in the following treatment, including separate plate removable expansion appliances, Quad-helix appliances, narrowing dental arch and inter-arch elastics. RESULTS: A total of 78 cases of unilateral posterior cross-bite were treated by different techniques in a manner consistent with the classification. All of the clinical results of treatment were satisfactory and successful. CONCLUSION: If diagnosis is correct, and suitable treatment methods are chosen, unilateral cross-bite can be treated successful.

[Plasma cell granuloma of maxillary sinuses].

OBJECTIVE: To study the clinical features and treatment of plasma cell granuloma in maxillary sinuses. METHOD: The clinical information of 3 cases was reported and the literatures were also reviewed. RESULT: All of the 3 cases were cured after Caldwell-Luc operation combined with oral corticosteroids and antiallergic agent. Recurrence hasn't been found in half to 2 years follow-up period. CONCLUSION: Plasma cell granuloma of maxillary sinus is a chronic inflammatory lesion with unknown origin. In almost all of the cases, CT images showed some degree of involvement of the bony wall of the maxillary sinuses. The imaging findings mimicker malignant tumors. But the prognosis were quite well in most of the patients.

[Clinical and experimental study on multiple sclerosis with bushen gusui tablet].

OBJECTIVE: To observe the therapeutic effect of Bushen Gusui tablet (BSGS) in treating multiple sclerosis (MS) and its effects on experimental allergic encephalomyelitis (EAE) in guinea pigs. METHODS: Forty-three MS patients were treated with BSGS and their clinical symptoms, signs of nerve function, recurrent frequency, evoked potential and changes in magnetic resonance imaging (MRI) were observed. The EAE model of guinea pigs was induced by homogenate of rabbit spinal cord and complete Freund's adjuvant (CFA), the animals were treated with BSGS and compared with prednisone acetate, which was served as control. The mortality and pathomorphology of EAE animals were observed. The contents of serum interleukin-2 (IL-2), interleukin-6 (IL-6), tumor necrosis factor (TNF) as well as myelin basic protein (MBP) were determined. RESULTS: BSGS could improve symptoms and signs of MS patients and reduce recurrent frequency. The total effective rate was 88.37%. High dose BSGS could obviously reduce incidence of EAE, inhibit inflammatory reaction of brain and spinal cord as well as demyelination, and simultaneously inhibit the activity of serum IL-2, IL-6, TNF and MBP, in comparing with model group (P < 0.01). There were insignificant difference as compared with prednisone acetate group (P > 0.05). CONCLUSION: BSGS had certain effect on both MS patients and EAE model animals, which indicated that it was worth further studying and clinical application.

[Metabolism of ginsenoside Rb1 and panaxadiol saponins by fungi].

AIM: To study the metabolic process of ginsenoside Rb1 (G-Rb1) and panaxadiol saponins (PDS) by fungi. METHODS: Ten strains of fungi were incubated with G-Rb1 and PDS at a certain temperature with shaking. A portion was taken out at different time and mixed up with butanol. The butanol extract was analysed by thin layer chromatography (TLC), high performance liquid chromatography (HPLC) and electrospray ionazition mass spectrometry (ESI-MS). RESULTS: It was found that there were ginsenoside-Rd (G-Rd), ginsenoside-F2 (G-F2), compound K (CK) and 20(S) protopanaxadiol (Ppd) metabolites beside the prodrug G-Rb1 induced by fungi (No. 1, 2, 3, 6, 8, 9). CONCLUSION: The six strains of fungi have different degrees of ability to metabolize G-Rb1 and PDS. The possible metabolic process could be as follows: G-Rb1 (or PDS)-->G-Rd-->G-F2-->CK-->Ppd.

[Feature analysis for the envelope of liver's M-mode ultrasound images].

The liver moves in response to impulses arising from the main blood vessels. The hardness of the liver may be reflected by this kind of movement. The M-mode ultrasound technique is used in this paper to detect the information of livers' motion. The motion envelope of the liver excited by the abdominal aorta is extracted using the correlation technique. Several feature parameters, such as the amplitude, the standard deviation and Fourier harmonics, are extracted from this envelope. The values of parameters were calculated from 19 normal livers and 11 patients' livers, from which the criterion was obtained to decide whether the liver was normal.

[Glutamate induced modulation of free Ca2+ in isolated inner hair cells of the guinea pig cochlea].

OBJECTIVE: To investigate the modulation effect of glutamate (Glu) on intracellular free Ca2+ concentration([Ca2+]i) of inner hair cell (IHC). METHODS: Using the laser scanning confocal microscope (LSCM), the exogenous Glu-induced changes in [Ca2+]i of isolated 10 IHCs and 10 OHCs of guinea pig cochlea were observed with fluo-3, a fluorescent probe for [Ca2+]i. RESULTS: The IHCs were identified by their unique flask shape with a distinct neck and spherical base and a large spherical nucleus. In most cases, normal cell shapes could be maintained about two hours after isolation. The images of [Ca2+]i from LSCM were similar to those from inverted microscopy. Fluorescence of Fluo-3 distributed in the isolated IHCs with brighter staining in the nucleus. In the presence of low concentration of Glu (3.85 mumol/L), there was an increase of [Ca2+]i in IHCs, whereas no change in OHCs was found. Of the 10 IHCs, increases of [Ca2+]i were observed in 9 and no change in 1. Of the 10 OHCs, 7 showed no [Ca2+]i change and only 3 showed minor reduction of [Ca2+]i. An increase of the Glu concentration (21.88 mumol/L) induced a corresponding increase of [Ca2+]i in IHCs, but eventually resulted in a gradual decrease of [Ca2+]i with a distortion of the normal shape, which indicated that the IHCs were degenerated and swelling. CONCLUSION: These results suggested that exogenous Glu is capable of modulating [Ca2+]i of IHC and may be act on autoreceptor in a positive feedback manner. Excessive Glu induced the accumulation of IHC [Ca2+]i which finally resulted in the degeneration and edema of IHC and the reduction of IHC [Ca2+]i.

[Optical parameters of Tm3+ in oxyfluoride glass ceramic].

Optical-absorption spectrum of Tm3+ ions in oxyfluoride glass ceramic was measured, from which the measured oscillator strengths were obtained. The Judd-Ofelt intensity parameters omega t (t = 2, 4, 6) were determined by a best fit of the calculated and measured oscillator strengths, the rms deviation was 3.9 x 10(-7). Some predicted spectroscopic parameters of the excited states, like the spontaneous radiative transition rate, radiative lifetime, branching ratio and integrated emission cross section were given using the intensity parameters. The spectroscopic parameters were comparable with some laser materials, and some of them were better.

[New progress of surface enhanced Raman spectroscopic studies on a gold electrode in a nonaqueous system].

Based on a highly sensitive new generation confocal microprobe Raman system, the adsorption behavior of thiocyanate (SCN-) on a gold electrode in methanol solution was investigated by SERS for the first time. Preliminary results demonstrated that the interaction between SCN- and the electrode is so strong that its SERS signal can be detected in the whole research potential region (-0.1 V(-)-1.4 V). The orientation change may occur in the potential range -0.6 V-0.7 V. From -0.1 V to -0.6 V SCN- is adsorbed mainly through S end onto Au surface while in the potential range of -0.7 V to -1.2 V it is oriented mainly through N end. The potential dependence of the Raman frequency of nu CN resulted in two different values of electrochemical Stark effect, suggesting the existence of molecular orientation change.
