Azathioprine and methylprednisolone: prevention of chronic cerebral vasospasm in dogs.

OBJECTIVE: To study the preventive effects of single use of azathioprine and its combination with methylprednisolone on chronic cerebral vasospasm (CVS). METHODS: A canine model of the "double subarachnoid hemorrhage (SAH)" was established to study the angiogram characteristics of chronic CVS. The effects of azathioprine and its combination with methylprednisolone on the prevention of chronic CVS were observed in a randomized study. The content of malondialdehyde (MDA) and pathological changes in the basilar artery (BA) wall were also analyzed. RESULTS: The results were obtained on the 7th day after subcarachnoid hemorrhage. In the azathioprine group, the BA diameter was 87% +/- 26% of the original BA diameter, and the MDA content of BA wall was 0.03 +/- 0.01 nmol/mg tissue. In the azathioprine plus methylprednisolone group, it was 93% +/- 20% (BA diameter) and 0.02 +/- 0.01 nmol/mg tissue (MDA content). In the control group, the BA diameter was 53% +/- 19% and MDA content was 0.11 +/- 0.05 nmol/mg tissue. Pathological examination confirmed the reduction in BA wall damage in the experimental groups. CONCLUSION: Azathioprine has a preventive effect on chronic CVS following SAH. It may suppress the production of free radicals and reduce damage to the BA wall. Combined use of azathioprine with methylprednisolone allows lower dosage and less complication compared with a single agent, and results in a better outcome.

Development of cytotoxic cerebral edema in rats following intracaudatum injection of tACPD, an agonist of metabotropic glutamate receptors.

OBJECTIVE: To explore the involvement of metabotropic glutamate receptors (mGluRs) in the formation of cerebral edema. METHODS: Male Wistar rats weighing from 250 g to 300 g were used. Trans-1-aminocyclopentane-1, 3-diacarboxylic acid (tACPD), an agonist of mGluRs, was microinjected into the right caudatum. Brain water content was determined by a wet weight/dry weight technique and Na+, K+ and Ca2+ contents were measured by inductive Couple Plasma-9000 at 6 h, 24 h and 48 h post-injection. Extravasation of Evan's blue (EB) into the brain was determined as an indicator of disturbance in the blood-brain barrier (BBB) and endothelial cells. Histologic studies were performed under a Leitz microscope and a Philips EM208s electron microscope. RESULTS: Dose-dependent and time-related increase of brain water was induced after tACPD (10, 50, 500 and 1000 nmol) injection. A significant increase in Na+ and K+ content but not in Ca2+ content was observed. EB extravasation showed no blue stain, indicating no increase in BBB permeability induced by tACPD-injection. Electron microscope study confirmed this finding and revealed remarkable swelling of astrocytes especially endfoot processes of astrocytes around capillaries at 6 h after tACPD-injection. In addition, all changes mentioned above occurred in both caudatum. CONCLUSION: These results indicate that activation of mGluRs by tACPD injected into the caudatum induced cytotoxic brain edema and interfered with astrocyte K+ buffering. This may provide new clues for therapeutic intervention.

[Study on the relationship between methylenetetrahydrofolate reductase gene polymorphism and plasma homocysteine level in pregnancy induced hypertension patients].

OBJECTIVE: To investigate the methylenetetrahydrofolate reductase (MTHFR) gene polymorphism and plasma homocysteine level in pregnancy induced hypertension (PIH) patients. METHODS: The MTHFR genotypes were determined in 62 PIH patients and 90 normal pregnant women by the restriction fragment length polymorphism method, plasma homocysteine level was measured by high performance liquid chromatography and fluorescence detection. RESULTS: The frequencies of T677 allele (0.52) and the T677/T677 genotype (0.27) in the PIH group were markedly higher than those in the control group (0.31 and 0.13, P < 0.05, P < 0.01 respectively), mean plasma homocysteine level in the PIH group (9.62 +/- 2.11) mumol/L was also significantly higher than that in the control group (6.85 +/- 1.98) mumol/L (P < 0.01). CONCLUSION: The MTHFR gene T677 variant is the genetic high risk factor for PIH. It can induce the hyperhomocysteinemia, potentially contributing to the pathogeny of PIH.

[Study on the relationship between CD44v6, p53 gene mutation and ovarian carcinoma metastasis].

OBJECTIVE: To explore the relationship between CD44v6 expression, p53 gene mutation, cell ploidy and ovarian metastasis. METHODS: Eighty-five ovarian specimens (20 normal specimens, 20 benign tumors, 20 non-metastatic carcinomas and 25 metastatic carcinomas) were collected. Cell ploidy and the distribution of cell population in each stage of cell cycle were detected by flow cytometry. By using techniques such as reverse transcription-polymerase chain reaction, southern blot and image analysis, the quantitative and qualitative expression of CD44v6 were determined; the mutation of p53 gene in the ovarian specimens was also determined by polymerase chain reaction-single strand conformation polymorphism with silver staining. RESULTS: In the normal ovarian specimens, benign tumors, non-metastatic and metastatic carcinomas, the positive rates of CD44v6 expression and p53 gene mutation were 0%, 10%, 75%, 88% and 0%, 5%, 40%, 60%, respectively, both increased gradually. The mean dark density in the non-metastatic (3,820 +/- 289) and metastatic (10,132 +/- 1,521) groups was significantly different (P < 0.01). In the positive and negative CD44v6 ovarian carcinoma groups, the mean carcinoma cell percentage at the G2M stage was 5.90% and 5.06% (P > 0.05), mean aneuploidy carcinoma cell percentage was 57% and 50% (P > 0.05) respectively. In the mutated and non-mutated p53 gene groups, their mean cell population at the G2M stage was 11.15% and 5.85% (P < 0.05), mean aneuploidy carcinoma cell percentage was 74% and 36% (P < 0.05), respectively. CONCLUSIONS: The expression of CD44v6 is closely related to carcinoma metastasis. The relationship between CD44v6 expression, p53 gene mutation and carcinoma metastasis can be involved in the different mechanism. As compared to mutated p53 gene, CD44v6 appears to be more suitable as a metastatic marker.

[The significance of CD3/MIB1 and CD3/CD25 in B cell lymphomas].

OBJECTIVE: To identify the distribution and significance of proliferating T cells (PTC) in normal tonsil and a group of B cell lymphomas. METHODS: Double immunohistochemical staining was used to detect PTC with MIB1 expression. Giemsa staining was used to detect Helicobacter pylori (Hp) in gastric MALT-type lymphoma. Cell culture was applied to investigate the relationship among tumor cells, Hp and T cells. RESULTS: Very few T cells expressed MIB1 in the follicular centre of normal tonsil and in that of follicular lymphoma. PTC were mainly distributed to the extrafollicular and T cell zones. So did CD3+ T cells expressed CD25. In low grade primary B cell gastric MALT-type lymphomas, tumor cells proliferated in vitro in response to Hp but died in the absence of T cells. PTC increased with increase in the degree of malignancy in different kinds of B cell lymphomas. CONCLUSION: T cells in follicular centres of follicular lymphoma are not in cell cycle. Proliferation of neoplastic B cells in low grade gastric MALT-type lymphomas requires Hp-specific infiltrating T cells. Transformation from low to high grade lymphoma is associated with an increase in T cell activation. T cells expressing MIB1 might be related to prognosis.

[Classification of secondary nasal deformities associated with unilateral cleft lip].

OBJECTIVE: To classify secondary deformities of cleft lip noses. METHODS: 56 patients with secondary nasal deformities of cleft lip were chosen at random. Cartilages of those noses were dissected and measured when correcting nasal deformity in order to find the relationship between anatomical anomaly of nasal cartilages and nasal deformity. RESULTS: Three grades of anatomical anomalies of the nasal framework were found. Grade I, the morphological anomaly occurs at lower third of the diseased side only. Grade II, the lower third of the another side is involved in addition. Grade III, the dorsum nasi is also deviated. CONCLUSIONS: This classification may be employed as a practical guide for surgical planning.

[Treatment of Class II division 1 extraction cases by use of edgewise technique].

OBJECTIVE: To treat class II division 1 extraction cases by means of Edgewise technique. METHODS: 27 skeletal class II division 1 malocclusion patients were extracted 2 upper first premolars and 2 lower second premolars, and treated by Edgewise principles and sequential force system. They aged from 11 to 20. The duration of orthodontic treatment was 25.5 months on average(22-28 months). RESULTS: The patients' profile was much improved; Molar relation changed from class II to class I relationship; Anterior overjet and overbite are normal; Good occlusal intercuspation is also achieved. The ANB angle was significantly changed from 5.6 degrees +/- 2.2 degrees to 3.2 degrees +/- 1.4 degrees; Z angle was altered from 60.8 degrees +/- 5.8 degrees to 76.4 degrees +/- 6.6 degrees; Distance of AO-BO was reduced from 6.8 +/- 2.4 mm to 3.2 +/- 1.1 mm. CONCLUSIONS: Edgewise technique is an effective method for treatment of class II division 1 extraction cases.

[Clinical effect of Xinqingning combined low dose continuous gastrointestinal dialysis in treating uremia].

OBJECTIVE: To investigate the clinical effect of Xinqingning (XQN), a preparation of rhubarb, combined with low dose continuous gastrointestinal dialysis in treating uremia. METHODS: Patients of uremia were divided into three groups at random, eighteen patients in the group A were treated with 1000 ml gastrointestinal dialysate (non-absorbed mannitol solution) orally 2-3 times a day, twenty patients in the group B treated with the same therapy as that in the group A, but combined with XQN 5-10 tablets 3 times per day and 19 patients in the group C treated by orally taken coated aldehyde oxystarch 5-10 g, 3 times a day. The therapeutic course for the three groups were 11 months. The changes in clinical manifestation, renal function, nutritional condition, and electrolytes before and after treatment were observed. RESULTS: After treatment, significant improvement was revealed in aspects of uremic symptoms, quality of life, nutritional condition, serum creatinine, urea nitrogen, serum phosphate, uric acid, CO2 combining power, creatine clearance, body weight and arm muscular circumference in the group A and B, as compared with those in the group C. In comparison of the group A and B, the group B showed a lower serum triglyceride and slower progression of chronic renal failure (CRF). All the criteria were not improved in the group C and with serum creatinine raised significantly. CONCLUSION: XQN combined low dose continuous gastrointestinal dialysis therapy was effective in definitely treating uremia, it provides a new therapeutic means of non-replacement therapy for CRF with uremia.

[Intraperitoneal lymphatic chemotherapy for colonic cancer].

OBJECTIVES: To evaluate the effect of intraperitoneal lymphatic chemotherapy with carboplatin and sandostatin on colonic carcinoma and abdominal lymphatic metastasis in patients after radical resection. METHODS: Patients with colonic cancer were divided into two group: abdominal lymphatic chemotherapy group and control group. The lymphatic chemotherapy group was treated at the introduction stage of anesthesia with carboplatin (200 mg) injected intraperitoneally and sandostatin (0.1 mg x 2) injected subcutaneously, whereas the control group was just treated with carboplatin (200 mg) injected intraperitoneally. The platinum levels in abdominal lymphatic nodes were monitored during operation. The two groups were uninterruptedly given intraperitoneal lymphatic chemotherapy or intraperitoneal chemotherapy. Retroperitoneal lymphatic metastasis was evaluated in each patient by abdominal CT scan after operation. RESULTS: The platinum levels in abdominal lymphatic nodes increased more significantly at 2, 3 hours in the lymphatic chemotherapy group than in the control group (P < 0.01). There was no difference in the platinum levels of the portal vein between the two groups, and also no difference in the platinum levels between the lymphatic nodes with or without metastasis. The rates of retroperitoneal lymphatic metastasis after resection were significantly lower in the lymphatic chemotherapy group control group (P < 0.01). CONCLUSION: Intraperitoneal administration of carboplatin combined with sandostatin may elevate the platinum levels in the abdominal lymphatic nodes and reduce the abdominal lymphatic metastasis after radical resection of colonic cancer. It is an effective method for intraperitoneal lymphatic chemotherapy.

[Clinical results of transmyocardial laser revascularization for 77 patients with coronary artery disease].

OBJECTIVE: To analyses the clinical results of transmyocardial laser revascularization (TMLR) for 77 patients with coronary artery diseases (CAD). METHODS: The mean age of the patients was (65 +/- 7) years. Previous medical record included CABG (6 patients), PTCA (9), AMI (66.2%), hypertension (70.1%), and diabetes mellitus (45.5%). TMLR was performed on the beating heart via a left anterolateral thoracotomy at the fifth intercostal space. Transesophageal echocardiography showed transmyocardial penetration of 23 +/- 6 channels. RESULTS: The hospital mortality was 3.8%, and postoperative complications were AMI (3.8%), left ventricular failure (2.6%), PVC (5.2%). After operation, the mean CCS angina class was improved from the baseline 3.5 +/- 0.7 to 2.1 +/- 0.3 at 3 months, 1.7 +/- 0.3 at 6 months, 1.7 +/- 0.3 at 12 months and 1.8 +/- 0.4 at 24 months. One patient died of AMI and two died not due to cardiac events during a follow-up of 3 to approximately 24 months. Echocardiography showed that the average of left ventricular ejection fractions was improved significantly at 6 months after operation compared with the preoperative value (P = 0.0457). (201)TI-SPECT showed a remarkable improvement in reversible ischemia in 70% patients followed up. Metabolic stress test for 20 patients followed up patients demonstrated an average increase in exercise tolerance from 7.1 +/- 3.2 min at the baseline to 9.6 +/- 1.3 min at 12 months (P = 0.021). Similarly the METs increased from 4.3 +/- 2.1 at the baseline to 5.4 +/- 2.0 at 12 months. CONCLUSIONS: TMLR is a safe and effective procedure for the treatment of end stage coronary artery diseases not amenable to PTCA or CABG. The effect of TMLR is associated with indication, correct evaluation of myocardial ischemia, and management for postoperative complications.

[Effects of p16 and GM-CSF combined gene therapy on immune response of tumor-bearing mice].

OBJECTIVE: To investigate the effects of p16 and GM-CSF combined gene therapy on immune response of tumor-bearing mice (Renca). METHODS: The model of renal tumor-bearing mice was constructed by subcutaneous injection of Renca cells. The tumor-bearing mice were divided into 5 groups at random three days after injection of Renca cells. Different groups of mice were given different treatments. The antitumor immune responses and the survival time of mice were tested. Histopathologic analysis of tumor tissue was done. RESULTS: Enhancement of antitumor immune responses and prolongation of survival period were observed more obviously in tumor-bearing mice after combined therapy than in those of the other four groups. CONCLUSIONS: p16 and GM-CSF combined gene therapy can enhance the antitumor immune responses of mice and inhibit the growth of pre-established renal cell carcinoma.

[Evaluation on the effects of community-based comprehensive prevention and control of hypertension in the rural areas in China].

OBJECTIVE: To explore the strategies and measures of community-based comprehensive prevention and control of hypertension in the rural areas. METHODS: Community-based comprehensive prevention and control of hypertension was put in practice in 120,000 people in Fangshan District of Beijing during the period of "The Ninth Five-Year Plan (1996 - 2000)". RESULTS: After intervention, blood pressure of the whole subjects were under control and kept the level of baseline data; the rate of increase of prevalence in the intervention areas was below the rate of that from 1979 to 1991 in China (the former was 14.7%, while the latter was 53.7%); the incidence rate of the subjects was 1.4%, close to the average rate of age 15 and above in China; the rates of awareness, treatment and control were above the levels of baseline data; SBP and DBP of the patients decreased by 2.9% and 5.6%, respectively; while the rate of having high risk factors reduced by 13.4% on average. CONCLUSION: The community-based comprehensive strategies and measures played an important and effective role in prevention and control of hypertension in rural areas.

[The susceptible alleles on HLA-DRB 1 of type I diabetes in children in Harbin].

OBJECTIVE: To evaluate association between type I diabetes in children and HLA-DRB1 alleles in Harbin. METHODS: Case-control method was employed, and the PCR-SSP technique was used to identify HLA-DRB1 alleles. RESULTS: The frequencies of DRB1 * 0301 and DR3/4 heterozygosity were significantly higher but the frequency of DR2 was significantly lower in cases than that in controls. In DR4 subtypes, the frequency of DRB1 * 0405 was higher and the frequency of DRB1 * 0406 was lower in cases than that in the controls. The difference between cases and controls in DRB1 * 0901 was not significant. CONCLUSION: DRB1 * 0301 allele and DR3/4 heterozygosity were the important susceptibility and heterozygosity genes while DR2 was the most important protective gene in Harbin. The DR4 possessed double-tendency. DRB1 * 0405 conferred susceptibility and DRB1 * 0406 conferred little protection. The susceptibility conferred by DRB1 * 0901 was not certain in type I diabetes in childhood in China. This study provided useful genetic data on the study of etiology of type I diabetes mellitus.

[Preliminary study on relationship between antibody against heat stress protein 70 and hypertension].

OBJECTIVE: To preliminarily explore the relationship between main antibody against heat stress protein 70 (anti-HSP70) and hypertension. METHODS: Serum anti-HSP70 was determined with Western blotting technique in hypertensive and normotensive groups. Logistic regression model was used to analyze the relationship between anti-HSP70 and hypertension. RESULTS: It was found that level of antibody against the main heat shock protein 70 was significantly higher in patients with hypertension (based on criteria of 160/95 mm Hg, 1 mm Hg = 0.133 kPa) than in controls, after adjustment for the generally recognized risk factors for hypertension (such as age, sex, length of employment, body mass index, job, et al.). There still was stronger association between anti-HSP70 and hypertension (based on criteria of 140/90 mm Hg), but its strength of association was weaker than the former. CONCLUSIONS: Anti-HSP70 may potentially serve as a serum biomarker to assess hypertension, or anti-HSP70 itself may be involved in the development of hypertension.

[The effects of batroxobin on the healing of and microcirculatory blood flow volume in deep partial thickness burn wounds in rats].

OBJECTIVE: To investigate the effects of batroxobin on the healing of and microcirculatory blood flow in deep partial thickness burn wound in rats. METHODS: Wistar rat inflicted by 4 cmx 4 cm deep partial thickness scalding on the back was taken as the model. Twenty male rats were randomly divided into scalding burn group and batroxobin treatment group. Cutaneous blood flow volume was measured before and at 0.5, 2, 4, 6, 12, 24 and 72 postburn hours (PBH). Scalding and residual burn wound areas were measured immediately and on the 14th and 18th postburn day (PBD). The rats were sacrificed on 30th PBD and the cutaneous samples were harvested for hair follicle counting and histological examination with LM. RESULTS: The cutaneous blood flow volume of rats in single burn group decreased progressively during 2-72 PBHs. But in batroxobin treating group, cutaneous blood flow volume improved evidently and the wound healing accelerated obviously with increased cutaneous appendages formation. CONCLUSION: Batroxobin might restore the blood circulation of stasis band of burn wound, so as to accelerate wound healing and to improve the quality of healed skin.

[Clinical observation on effect of yixin yidu decoction in treating acute viral myocarditis].

OBJECTIVE: To observe the effect of Yixin Yidu Decoction (YXYDD, self-formulated according to the TCM therapeutic principle of supplementing Qi and Yin, clearing away evil Heat and toxic substances) in treating acute viral myocarditis (AVM). METHODS: Seventy-six patients of AVM were randomly divided into 2 groups, the 40 patients in the treated group were treated with YXYDD and the 36 patients in the control group treated with Qidong Yixin Oral Liquid (QDYX), the therapeutic course for both groups was 4 weeks. RESULTS: YXYDD was more effective than QDYX in improving clinical symptoms, adjusting the abnormal ECG, lowering the serum level of myocardial enzymes, improving the cardiac function of left ventricle and regulating the T-cell immunity (elevation of CD3, CD4 and CD4/CD8 ratio). CONCLUSION: YXYDD is an effective medicine in treating AVM.

[Determination of fenfluramine in corpse using GC/NPD and GC/MS].

A method was described for the determination of fenfluramine by GC/MS in blood, urine, liver, lung, kidney and heart tissues. After liquid-liquid extraction of fenfluramine in the specimens obtained from autopsy, the samples were qualitatively identified by GC/MS, and quantitatively determined by GC/NPD. 4-phenyl butylamine was used as internal standard. The concentrations of fenfluramine in blood, urine and liver were 7.8 micrograms/ml, 64.2 micrograms/ml and 31.3 micrograms/g respectively. The findings of autopsy and the results of toxicological analysis were also discussed.
