Cytogenetic analysis of bone marrow from patients with primary myelodysplastic syndrome.

Of 34 patients with primary myelodysplastic syndrome (MDS), 26 (76.5%) were found to have chromosome defects using an improved bone marrow culture method and high-resolution G-banding technique induced by actinomycin D. The frequency of abnormalities varied among the subtypes: 1/2 in refractory anaemia with ringed sideroblasts; 18/24 in refractory anaemia; 5/6 in refractory anaemia with excess of blasts (RAEB); and 2/2 in refractory anaemia with excess of blasts in transformation. The most frequent abnormalities, either single, double or complex defects, were -5/5q-, -7, +8 and +21; 16q-/16p-, +19 and -X were also common. The percentage of aneuploid cells, in particular hypodiploid cells, was increased. The abnormalities were detected more frequently in complex aberrations associated with RAEB and RAEB in transformation. The presence of -5/5q- as a sole aberration was associated with longer mean survival time (greater than 18 months) but multiple (more than two) chromosomal abnormalities were associated with a poorer prognosis and a mean survival time of only 7.5 months. Chromosome follow-up studies indicated that patients with -7, +8, +21, -X and complex defects, increased hypodiploid cells and karyotpic evolution were likely to have a high risk of transformation to leukaemia or to a more severe subtype of MDS with a short overall mean survival time. These defects, mostly of the deleted type, are assumed to play a specific role in the pathogenesis of myelodysplasia. Repeated chromosomal analyses during the clinical course convey more accurate prognostic criteria for patients.

[Effects of PGE1 on penile blood flow].

From February to July in 1989, 47 patients came to our O.P.D. with the chief complaint of impotence. The average age was 48.3 +/- 10.7 y/o. We applied intracoporeal injection of prostaglandin PGE1 (20 mg); and evaluated its penile blood flow effect by color duplex scanning (Acuson 128). The erectile responses of the test showed that: 6 patients (12.8%) had normal response: and 16 patients (34%) had imperfect response. Altogether, the total positive response rate was 46.8%, and 25 patients (53.2%) showed impaired response. The onset of response was 9.1 +/- 3.6 minutes and the duration of erection was 59.2 +/- 24.7 minutes. The percentage of diameter change of both deep arteries after injection was Rt: 58.1 +/- 41.5%; left: 52.3 +/- 35.6%. The peak velocity of right cavernosal arterial flow after intracoporeal injection was 35.5 +/- 15.9 cm/sec; and that of the left side was 33.2 +/- 16.9 cm/sec. There was no correlation between the increment of peak velocity of the deep arterial flow and the erection grade. The same phenomenon was also found between the increased change in the diameter of the deep artery and the erection grade. 16 patients (34.1%) experienced tolerable pain during the procedure. Two patients (4.3%) experienced dizzines and discomfort due to venous leakage. No priapism was found. This study suggests PGE1 may be an excellent potential alternative to other vasoactive drug with less complication in the diagnosis and treatment of impotence. But the cost and stability were its shortcoming.(ABSTRACT TRUNCATED AT 250 WORDS)

A case of primary diffuse tracheobronchial amyloidosis treated by laser therapy.

We report a case of primary diffuse tracheobronchial amyloidosis in a 72-year-old lady who presented with a long history of recurrent cough, dyspnoea, wheezing, haemoptysis and chest infection. She was treated successfully with three sessions of laser therapy. There were improvements in both clinical symptoms and measurements of airway obstruction. Bronchodilators and oral prednisolone were not required after treatment.

Activation of NMDA receptors is necessary for fast information transfer at brainstem vagal motoneurons.

The involvement of N-methyl-D-aspartate (NMDA) excitatory amino acid subtype receptors in synaptically driven excitatory responses of ambigual motoneurons was investigated in vivo and in vitro. In urethane-anaesthetized rats, fictive oesophageal peristalsis evoked by topical application of muscarine (0.05-0.5 nmol) to the dorsal surface of the solitarial complex (NTS) was reversibly blocked by ipsilateral intraambigual injection of DL-2-amino-7-phosphonoheptanoic acid (AP-7, 0.5-1.5 nM) and (+-)-3-(2-carboxypiperazin-4-yl)-propyl-1-phosphonic acid (CPP; 0.5-1.5 nM). In brainstem sagittal slices, post-synaptic potentials were recorded from neurons of the compact formation of the nucleus ambiguus (AMBc). Stimulation of presumptive NTS afferents elicited a complex excitatory postsynaptic potential (EPSP) which usually consisted of both a high-threshold fast (HTF) and a low-threshold slow (LTS) component. Bath perfusion with AP-7 (30-50 microM) and CPP (50 microM) selectively blocked the HTF without affecting the LTS component, while kynurenate (1 mM) and 6-cyano-7-nitroquinoxaline-2,3-dione (CNQX, 5-10 microM) nonselectively suppressed both components. With sufficient stimulus strength, the EPSP generated a single spike arising from the HTF component. AP-7 (50 microM) either blocked the spike or increased the firing threshold. Furthermore, at the resting membrane potential, bath-applied NMDA induced a net inward current (269 +/- 189 pA) which had a negative slope in the range of -95 to -35 mV. In conclusion, NMDA receptors participate in solitario-ambigual synaptic transmission under physiological conditions and activation of these receptors is necessary for functional information transfer in this pathway.

Somatostatin inhibits nicotinic cholinoceptor mediated-excitation in rat ambigual motoneurons in vitro.

The interaction between somatostatin and acetylcholine, two putative transmitters in the nucleus ambiguus, was investigated on single ambigual neurons in a brainstem slice preparation. Somatostatin reversibly inhibited the nicotinic cholinoceptor-mediated depolarization and inward current induced by acetylcholine. This inhibition persisted in the presence of tetrodotoxin (TTX) or Mn2+. In contrast, somatostatin enhanced both the glutamate-evoked depolarization and spiking discharges generated by current injection. These results suggest that somatostatin exerts a differential action in modulating excitatory inputs to the nucleus ambiguus at the level of postsynaptic receptors.

Nicotinic cholinoceptor-mediated excitation in ambigual motoneurons of the rat.

The purpose of this study was to determine if ambigual oesophageal motoneurons of the rat possess functional nicotinic cholinoceptors. In urethane anaesthetized rats, acetylcholine (20-50 pmol) delivered micropneumophoretically from multibarrelled pipettes to the compact formation of the nucleus ambiguus produced either synchronous or propulsive oesophageal contractions which were fully and reversibly blocked by dihydro-beta-erythroidine (8-10 pmol) but were resistant to D-tubocurarine and hexamethonium (10-20 pmol). 1,1-Dimethyl-4-phenyl-piperazinium but not muscarine (8 pmol) exerted an analogous agonist action. Ejection of glutamate at the same sites produced similar oesophageal responses which were, however, resistant to dihydro-beta-erythroidine. Acetylcholine applied 5-15 s prior to glutamate transiently facilitated the glutamate-evoked response. The facilitatory effect of acetylcholine was replicated by 1,1-dimethyl-4-phenyl-piperazinium but not muscarine and inhibited by dihydro-beta-erythroidine. Physostigmine, applied either intra-ambigually (10-20 pmol) or by intravenous injection (0.15-0.3 mumol/kg), enhanced both acetylcholine and glutamate-evoked responses. In brainstem transverse slices, application of acetylcholine and glutamate to quiescent ambigual neurons of the compact formation resulted in a rapid membrane depolarization associated with an increased membrane conductance and spiking. Under voltage clamp, both acetylcholine and glutamate elicited a net inward current. The depolarizing response of these neurons to acetylcholine was blocked by dihydro-beta-erythroidine (0.5-2 pmol), hexamethonium (0.2 mM) and D-tubocurarine (10 microM) and persisted in the presence of tetrodotoxin (10(-6) M) or Mn2+ (5 mM) in the bathing medium.(ABSTRACT TRUNCATED AT 250 WORDS)

Role of solitarial GABAergic mechanisms in control of swallowing.

The role of solitarial gamma-aminobutyric acid (GABA)-ergic mechanisms in deglutition was investigated in urethane-anesthetized rats. When applied to the dorsal extraventricular surface of the nucleus tractus solitarii (NTS), muscimol reversibly inhibited 1) buccopharyngeal swallows evoked by either electrical or chemical stimulation of the NTS and 2) esophageal peristalsis evoked by muscarinic agonists. Bicuculline (5-1,000 pmol) applied to the NTS surface evoked rhythmic swallowing, which was reversibly blocked by DL-2-amino-7-phosphonoheptanoic acid (5-500 pmol). Methscopolamine (5-100 pmol) applied at the same site abolished the esophageal component of the response. Intrasolitarial application of bicuculline at s-glutamate-responsive loci in the intermediate and central subnuclei gave rise to buccopharyngeal and esophageal responses, respectively, and to a concomitant facilitation of glutamate-evoked responses. In subliminal doses ejected at esophageal loci, bicuculline induced deglutitive esophageal peristalsis during elicitation of buccopharyngeal swallowing by chemical (kainate or norepinephrine) or electrical stimulation of the NTS. We conclude that solitarial GABA neurons exert a tonic inhibition of the medullary deglutitive pattern generator and control buccopharyngeal-esophageal coupling.

Lymphangioleiomyomatosis--treatment with progesterone.

Lymphangioleiomyomatosis is a rare devastating disease affecting women mostly of child-bearing age. It presents with spontaneous pneumothorax, chylous effusions, hemoptysis and progressive breathlessness. Most patients die from respiratory failure within 10 years. There are no controlled studies on the efficacy of various treatment regimens. We report our experience with progesterone therapy in three patients. Two failed to respond, one died about 11 years after presentation and another after 5 1/2 years. The third patient has survived 11 years after onset of disease.

Occupational asthma in Singapore--a review of cases from 1983 to 1990.

One of the newer occupational diseases in Singapore is occupational asthma. As on 31 December 1990 there were 35 confirmed cases of occupational asthma in the official statistics on occupational disease. We report in this paper our observations and experience based on these cases. The problem of under-reporting and the importance of early diagnosis are discussed.

Occupational asthma due to ethylene diamine.

We report a case of bronchial asthma due to ethylene diamine in a chemical worker. The typical relationship with work exposure was elicited from the occupational history and serial peak flow measurements. A late dual asthmatic reaction was seen in bronchial provocation test with ethylene diamine. Bronchial hyper-responsiveness to isopropyly alcohol and histamine was observed and was likely to be the result rather than a predisposing factor for the development of occupational asthma. Amines are a powerful group of small molecular weight asthmatogens that are commonly used in various types of industries locally and should be recognised in the clinical evaluation of workers with asthma.

Effect on histamine responsiveness of reducing airway dimensions by altering posture.

Baseline airway geometry is thought to be an important determinant of the airway response to challenge; this geometry is altered by changing posture. The effect of changes in posture on airway calibre, midtidal lung volume, and the airway response to inhaled histamine was studied in eight healthy subjects (four female; mean (SD) age 29.8 (5.1) years, FEV1 3.54 (0.65) 1). Each subject was studied in both sitting and supine postures on two days; airway calibre was assessed by measuring total respiratory resistance (Rrs) at 6 Hz with a forced oscillation technique applied over 16 seconds of tidal breathing. Appropriate doses of histamine were selected by preliminary experiments and were always inhaled with the subject in the supine posture. Midtidal lung volume was larger in the sitting (2.9 (0.8) 1) than in the supine posture (2.4 (0.5) 1). Baseline Rrs was lower in the sitting than in the supine posture (2.03 (0.44) and 3.12 (0.76) cm H2O.1(-1).s*). The mean absolute increase in Rrs after the same dose of histamine was 1.22 cm H2O.1(-1).s in the sitting position (65.8% increase over baseline) and 1.39 cm H2O.1(-1).s (48.8% increase over baseline) in the supine position. The geometric mean provocation concentration of histamine causing a given percentage increase in Rrs was similar in the sitting (8.26 mg/ml) and supine (8.65 mg/ml) positions. Thus there was no significant increase in responsiveness after the reduction of airway dimensions and extra-airway distending forces that occurs in the supine posture.

Effects of increasing doses of beta-agonists on airway and parenchymal hysteresis.

We examined the effects of a deep inhalation on airway caliber before and after increasing doses of a beta-agonist in eight subjects, including one former and two current but mild asthmatics. With bronchodilation the increase in maximal flow on the partial flow-volume curve (P), initiated from functional residual capacity, exceeded that seen on the maximal curve (M), initiated from total lung capacity, such that isovolumic maximal flows diminished after a deep inhalation; i.e., M/P ratios fell with bronchodilation, as we and others have found. Five of eight reversed this downward trend in M/P ratios at higher cumulative doses. Quasistatic pressure-volume curves (QSPV) were simultaneously performed on two of these five and demonstrated a decrease in pressure-volume hysteresis (PVH) at the higher doses associated with a rising M/P ratio. Three of eight had continuing low and diminishing M/P ratio up to the highest dose given. QSPV were performed in two of these three and indicated no change in PVH at any of the doses. One of these two had a repeat study using a subcutaneous beta-agonist after the inhaled drug was given, and the M/P ratio rose as QSPV PVH fell. These data support the relative hysteresis analysis of airway and parenchyma as an explanation for volume history effects on airway caliber.

Occupational asthma due to isocyanates in Singapore.

Nine cases of occupational asthma due to isocyanates are described. The isocyanates involved included toluene diisocyanate (TDI), diphenyl-methane diisocyanate (MDI) and polyisocyanates. The importance of asking the occupational history in a patient with asthma is illustrated. The importance of early diagnosis and removal from further exposure is also discussed.
