[An unusual form of macrodactylia].

The authors report a rare case of macrodactyly of the thumb in a 36 year old woman. The hypertrophy predominates on the epiphyseal tissue with complete ankylosis of the trapezo-metacarpal metacarpo-phalangeal, and interphalangeal joints in an abnormal position. This results in total functional incapacity of the thumb. The cosmetic and psychological impact are important to consider. Treatment consisted of two operative procedures. First a silastic implant for the trapezo-metacarpal joint and an arthrodesis of the metacarpophalangeal joint in a normal position was done. Three months later, the volume of the thumb was reduced by a longitudinal bone resection. The functional and cosmetic result is assessed. The literature is reviewed and the different modes of treatment utilized in such a malformation are presented. A relation between macrodactyly and lipomatous macrodystrophy is considered.

[Complete fracture-dislocation of thoracic vertebrae 8 and 9 without neurological complications. Description of the injury. Therapeutic deductions].

The author has seen one case of complete fracture dislocation between the 8th and 9th thoracic vertebrae without paraplegia. An analysis of the lesion was made by tomodensitometry. It is concluded that the absence of a lesion of the spinal cord was due to an associated fracture of half of the posterior arch of the 8th thoracic vertebra. The treatment was based on conservative reduction and fixation by a halo cast. A review of the literature has shown only one similar case.

[Subcutaneous angioma of the hand].

The authors describe the specific features of hemangiomas of the hand in adults (three cases). The clinical syndrome is described. The main examination is by arteriography of the hand which shows three principal aspects: the vascular topography; the volume of the tumors; the extension of the neighbouring tissue. The treatment is analysed: surgical exeresis; in some cases, microvascular transplants are necessary (especially for the distal reconstruction of a finger); embolization is normally avoided because of the risk of necrosis of the finger.

Longitudinal pinning of fractures of the base of the first metacarpal.

From an experience of 120 fractures of the base of the first metacarpal bone, the authors evaluated the functional results of in 40 patients followed-up. Most of these patients had been treated by Bunden's method of longitudinal pinning. If one eliminates the comminuted fractures and the Bennett's fractures with a single large fragment treated by screw fixation the method of longitudinal pinning restores normal function to the first interdigitalspace Furthermore, the development of secondary arthrosis of the trapezo-metacarpal joint is rare.

[Parosteal chondrosarcoma. A report of two cases].

Parosteal chondrosarcoma is a tumour of the soft tissues different from juxta-cortical chondrosarcoma. It is extra-periosteal, has no attachment to bone and develops primary in the soft tissues. The authors review the literature and agree that the frequency of recurrence after local excision requires treatment by amputation.

[Treatment of rhizomelic arthrosis of the thumb by partial resection of the trapezium and rib cartilage autograft. Preliminary results].

The authors have studied the results of a personal technique concerning the treatment of trapezo-metacarpal arthritis: a partial trapezectomy is associated with a antogenous rib cartilage graft. 13 cases were treated using the procedure. 9 patients are reviewed with a follow up mean one year. 4 patients are free of pain, 5 are improved. Strength is improve in 4 cases (pinch), similar or inferior to the pre-operative status in 5 cases. Increase of motion concern Kapandji test and first web space. Follow up is insufficient to allow a conclusion about the long term behaviour of cartilage graft.

[Global exposure of the lower extremity of the radius by extended external approach].

With classical lateral access to the lower radius, only the styloid process is correctly exposed and many surgeons prefer elective anterior routes, or more rarely posterior access. Our experience with posterior compression-extension fractures of the lower radius using a posterior plate has led us to describe the modalities of progression to the bone without injury to the tendons and vasculo-nervous bundles. The technique described scrupulously avoids injury and also allows safe iterative access. Finally, and most importantly, this technique also exposes all the aspects of the bone. It can be successfully used for internal fixation in difficult cases and also for the treatment of former acquired or congenital deformations of the epiphysis. This route has been shown to be safe and successful in a prospective series of 187 applications of posterior plates for compression-extension fractures.
