Cognitive failures after general anaesthesia for day-case surgery.

Patients are normally thought to have recovered from general anaesthesia within a few hours after day-case surgery. Previous recovery studies using objective psychometric tests showed a return to baseline values within this period. Nevertheless, patients are advised about what activities they should not undertake in the subsequent 24 h because it is feared that the residual anaesthetic effects may impair their ability. These studies did not follow patients out of hospital and their relevance to the real world is not known. Using the cognitive failures questionnaire, a standard measure of performance of everyday tasks, 54 patients were asked to report their own performance after general anaesthesia for the 3 days after discharge from hospital: 30 local anaesthetic patients were studied for comparison. The general anaesthesia group reported a highly significant greater incidence of cognitive failures after anaesthesia compared with the local anaesthesia group. This suggests that the residual effects of anaesthesia persist beyond 24 h.

Induction of anaesthesia with midazolam and a target-controlled propofol infusion.

Eighty patients premedicated with temazepam were allocated randomly to receive an intravenous bolus of midazolam (either 0, 1, 2 or 4 mg) 4 min before a propofol infusion using a system designed to maintain a target blood concentration of 3 micrograms.ml-1. Anaesthesia was successfully induced within 3 min of starting the infusion in 45% of patients receiving no midazolam. This rose to 70%, 85% and 95% in the groups receiving 1, 2 and 4 mg midazolam boluses respectively (p < 0.005). There were reductions in arterial blood pressure 3 min after starting the infusion in all groups but there was no significant difference between the groups. Apnoea was rare. Small doses of midazolam usefully enhance the effect of a propofol infusion administered at a dose which by itself would not induce anaesthesia in a majority of patients.

Intra-operative tracheal obstruction by tumour fragments.

A patient underwent oesophageal dilatation for invasive secondary carcinoma. During the course of the procedure, ventilation of the patient's lungs became impossible due to total tracheal obstruction. A rigid bronchoscopy was performed and the cause was found to be fragments of tumour that had broken off and were behaving like a ball-valve in the trachea and right main bronchus.

The population structure and transmission of Escherichia coli in an isolated human community; studies on an Antarctic base.

The population structure and transmission of Escherichia coli in a small group of individuals isolated for 26 weeks on an Antarctic base were studied by multilocus electrophoresis of eight enzymes and plasmid analysis. Two hundred and sixty-nine strains were isolated. They were grouped into 60 allozyme types (ETs). Half of these ETs were only isolated once; others were repeatedly isolated from single subjects. Eleven were found in more than one subject and the pattern of the occurrence of some of them was considered to provide evidence of their spread from subject to subject.
