Change in oxygenation status in intratumour total and quiescent cells following gamma-ray irradiation, tirapazamine administration, cisplatin injection and bleomycin treatment.

C3H/He mice bearing SCC VII tumours received 5-bromo-2'-deoxyuridine (BrdU) continuously for 5 days via implanted mini-osmotic pumps to label all proliferating (P) cells. The mice then received gamma-ray irradiation, or administration of tirapazamine (TPZ), cisplatin or bleomycin. At various time points after each treatment, tumour-bearing mice were irradiated with a series of test doses of gamma-rays, while alive or after being killed, to obtain hypoxic fractions (HFs) in the tumours. Immediately after gamma-ray test irradiation, the tumours were excised, minced and trypsinized. Tumour cell suspensions obtained were incubated with cytochalasin-B, a cytokinesis blocker, and the micronucleus (MN) frequency in cells without BrdU labelling (i.e. quiescent (Q) cells) was determined using immunofluorescence staining for BrdU. MN frequency in the total (P + Q) tumour cells was determined from the tumours that were not pre-treated with BrdU. MN frequency of BrdU-unlabelled cells was then used to calculate the surviving fraction of the unlabelled cells from the regression line for the relationship between the MN frequency and the surviving fraction of total tumour cells. TPZ and cisplatin reduced the HF after treatment, especially in Q cells, and this tendency was particularly marked with TPZ. In contrast, bleomycin increased the HF after treatment. Both reoxygenation following gamma-ray irradiation or bleomycin treatment and a subsequent return to pre-treatment levels of HF following TPZ or cisplatin treatment (rehypoxiation) occurred more rapidly in total (P + Q) cells than in Q cells. Based on our previous report that total (P + Q) and Q cells within this tumour have large acutely and chronically HFs, respectively, we conclude that acute hypoxic cells play a major role in reoxygenation and rehypoxiation in SCC VII tumours.

Association of postprandial hypertriglyceridemia and carotid intima-media thickness in patients with type 2 diabetes.

OBJECTIVE: Serum triglyceride levels are important in the development of atherosclerosis. Although triglyceride levels are generally increased in the postprandial periods, the association between postprandial triglyceride (pTG) levels and atherosclerosis has not been investigated in diabetic patients. To investigate the role of pTG levels in atherosclerosis, we examined the correlation between pTG levels and carotid intimal-medial thickness (IMT). RESEARCH DESIGN AND METHODS: Carotid IMT was measured by ultrasonography in 61 patients with type 2 diabetes. Plasma glucose (PG), insulin, total cholesterol, triglycerides, and HDL cholesterol levels were measured after overnight fasting and 4 h after a meal. RESULTS: Carotid IMT of the patients with fasting hypertriglyceridemia was greater than that of the patients with normal fasting triglyceride (fTG) levels (0.85+/-0.12 vs. 0.76+/-0.14 mm; P = 0.02). The carotid IMT was increased in the patients with pTG levels >2.27 mmol/l. The normo-normo (NN) and normo-hyper (NH) groups consisted of patients with normal fTG levels but with pTG levels <2.27 and >2.27 mmol/l, respectively. Patients with both hypertriglyceridemia and pTG levels >2.27 mmol/l formed the hyper-hyper (HH) group. Carotid IMT was significantly increased in the NH (0.86+/-0.13 mm) and HH (0.85+/-0.12 mm) groups compared with the NN group (0.73+/-0.13 mm; P<0.01). Although postprandial PG, pTG, and fasting LDL cholesterol levels were all independently correlated with carotid IMT, pTG levels had the strongest statistical influence (P = 0.002). CONCLUSIONS: Postprandial hypertriglyceridemia despite normal fTG levels may be an independent risk factor for early atherosclerosis in type 2 diabetes.

Electrochemically active DNA probes: detection of target DNA sequences at femtomole level by high-performance liquid chromatography with electrochemical detection.

Electrochemically active DNA probes were prepared by linking a ferrocene unit with 5'-aminohexyl-terminated oligonucleotides. The DNA sequences of probes 5a, 5b, and 5c were 5'-T12-3', 5'-T20-3', and 5'-TGCAG TTCCG GTGGC TGATC-3', respectively. Probe 5a could form a complex selectively with a single-strand poly(A) and a double-strand DNA fragment containing an A13 sequence and these complexes could be detected at femtomole levels by an electrochemical detector (ECD) on HPLC. The observed ECD response was proportional to the amount of the complex over the range 20-100 fmol. Probe 5c was capable of detecting femtomole levels of a restriction DNA fragment having oncogene v-myc. Moreover, probe 5b was able to detect picogram levels of mRNA taken from rat brain or yeast total cellular RNA. This proves that the electrochemically active DNA probes are useful in analyzing traces of DNA and RNA carrying the complementary sequence.

Development of total medical material distribution management system.

Since September 1992, attempts have been made at Kagoshima University Hospital to develop the Medical Material Distribution Management System which helps to realize optimal hospital management as a subsystem of the Total Hospital Information System of Kagoshima University (THINK). As this system has been established, it has become possible for us to have an accurate grasp of the flow and stock of medical materials at our hospital. Furthermore, since September 1993, the Medical Material Distribution Management System has been improved and the Total Medical Material Distribution Management System has been smoothly introduced into the site of clinical practice. This system enables automatic demands for fees for treatment with specific instruments and materials covered by health insurance. It was difficult to predict the effect of this system, because no similar system had been developed in Japan. However, more satisfactory results than expected have been obtained since its introduction.

The management of medical records.

An optical disk filing system is an efficient approach to storing medical records; however, this system has not yet been put to practical use because it is usually a "stand-alone" type indirectly connected to a hospital information system. We have developed a medical record management system with an optical disk filing system connected to the host computer in the hospital information system. We can retrieve and display the medical records through the CRT (Cathode Ray Tube) terminals of the hospital information system at every ward and outpatient clinic. The patient's clinical information can be sent to several areas in the hospital using the hospital information system.

Development of a hospital management system to analyze nursing information using a microcomputer.

We have developed a hospital management system to analyze the large amount of the nursing information using a microcomputer system. In this system, we have combined the information from the patient database and the nursing database in order to evaluate the quality of medical care in the hospital. The utilization of a microcomputer is useful for filing and analyzing it and presenting the results in a variety of formats, including graphic representations.

A total hospital information system at Kagoshima University Hospital.

At Kagoshima University Hospital, we have been developing the total hospital information system named THINK (total hospital information system of Kagoshima University), using a large-size computer system. In this system, the entry of data is done only once at the point of origin. The mutual use of data is done by various subsystems, and the user interface is uniformity. As a result of the speed, accuracy, and flexibility of the system's software, we have achieved efficient medical services and hospital management by a unified database; furthermore, we have improved medical care to patients and we can use this database for supporting not only medical education but also clinical research.

Computerized nursing system in hospital information system.

We have developed a computerized nursing system including a nursing information system, a nursing administration system and a nursing support system. This on-line nursing system is linked to the total hospital information system. This paper describes the purpose and concept of the system development. The nursing system can be utilized in the individualized planning, delivery and evaluation of patient care.

[Studies of the manifold of amylase].

alpha-Amylase (1.4-alpha-D-glucan-glucanohydrolase, E.C 3.2.1.1) is distributed widely in animal and plant kingdoms. A number of properties of this enzyme have been recognized molecular biologically using animal organs. On the other hand, physiological roles and specificities of serum amylase are not known. The source organs of serum amylase have not been confirmed in every animal. The purposes of the experiments are to find out the specificities and varieties of amylase in some kinds of animals. The following results were obtained. 1. Amylase activities in the sera (body fluids) of some animals (Mammals, Birds, Amphibians, Fishes, Insects and Shellfish, 25 kinds altogether) were quite different from each other. The highest amylase activity except insects was observed in the serum of hamster (400 units) and the lowest was in the serum of horse (0.4 units). The activity of locust and oriental longheaded locust, eating grain mainly, was high (locust, 1965 units). 2. Five isoamylases were detected in the serum of rat. Four of them migrated to anode. 2-5 isoamylases were observed in other subjects and the mobility of isoamylases was different from each other. This seemed to be caused by the differences of isoamylase proteins. 3. Amylase activities in the brain, parotid gland, submandibular gland, sublingual gland, tongue, lung, heart, liver, stomach, spleen, pancreas, adrenal, serum and urine of hamster, rat, mouse and rabbit were measured. The activity was especially high in the parotid gland and pancreas. In rabbit, however, amylase activity in these organs was lower than that of other animals. 4. Isoamylases in some organs of four kinds of animals (hamster, rat, mouse and rabbit) were separated electrophoretically. Isoamylases in the serum of rat, hamster and mouse were similar to those in the parotid, submandibular and sublingual glands, respectively. In rabbit, it was difficult to separate isoamylases in the parotid gland and pancreas. 5. Amylases in the parotid gland and locus body were purified by column chromatography (potato starch).
