Because pancreatic cancer often metastasizes to other sites, it may not be diagnosed until autopsy. Pulmonary metastases from the pancreas are not uncommon but unfortunately not often recognized. Dr Mishriki describes a patient with obstruction of the superior vena cava and metastatic pancreatic cancer that was misdiagnosed as primary lung cancer. Autopsy revealed the pancreas to be the primary tumor site.
Proper assessment of the preoperative patient is a blend of the art and science of medicine. The body of literature dealing with the various facets of this assessment has grown in the past few years. Unfortunately, this core of knowledge is neither well taught to residents-in-training nor well appreciated by many practicing physicians. Thus, evaluation of the surgical patient is often guided by personal anecdotes and unjustified assumptions. Seven common misconceptions are addressed in this article.
Tumors of the umbilical cord are extremely rare. Only two types are recognized: hemangioma and teratoma. A case of hemangioma of the cord prompted a review of the world literature that uncovered 17 other cases, bringing the total number of known cases up to 18. Our case is the first to be diagnosed by ultrasonography and was associated with polyhydramnios.
