Percutaneous transluminal coronary angioplasty for culprit lesions in patients with post myocardial infarction angina based on dextrocardia and anomalous coronary arteries. Case reports and methods.

Four cases of successful coronary angioplasty for anomalous coronary arteries, including dextrocardia associated with three-vessel disease, single left coronary artery with proximal left anterior descending lesion, anomalous right coronary artery (RCA) from adjacent left coronary sinus of Valsalva associated with proximal RCA lesion, and anomalous left circumflex angulated lesion bifurcated from the RCA, were encountered. Four cases with 8 target lesions who had a mean age of 63.5 +/- 11.5 years old are presented. All the targets lesions were completely dilated through balloon angioplasty, including use of a newly developed support device for cases with large jeopardized myocardium. The factors for complete revascularization were appropriate selection of catheters and originality and ingenuity of procedural technique based on the anatomic characteristics.

Efficacy and issues of emergent percutaneous transluminal coronary angioplasty. Comparison of clinical results in younger and elderly patients.

The authors investigated the efficacy and prognosis of emergency percutaneous transluminal coronary angioplasty (PTCA) in elderly patients. A study was conducted on the early and late prognosis of the study group composed of 66 younger patients < sixty-five years in Group A and 46 elderly patients > or = seventy years in Group B who underwent reperfusion therapy, including PTCA, for acute myocardial infarction (AMI). No difference was seen in the initial success rate of 82% for Group A and 85% for Group B, or in the vessel patency in the predischarge coronary arteriogram (CAG) with 84% for Group A and 87% for Group B. However, in-hospital mortality was 3% for Group A and 11% for Group B. The patient restenosis rate in the 4.2 months follow-up CAG was 28% for Group A and 50% for Group B. The lesion restenosis rate was 30% for Group A and 53% for Group B (P < 0.05), and the late period mortality rate was 3% for Group A and 11% for Group B. Although there was no difference in the initial success rate of reperfusion through PTCA for AMI in the elderly as compared with the younger patient group, poorer results were seen in the restenosis rate and mortality rate.

[A study of the efficacy of thrombolytic therapy for acute myocardial infarction in elderly patients].

We compared the short and long term outcome between an elderly group (E; aged 70 year more, 17 cases) and a younger group (Y; less than 70 years, 95 cases). Reperfusion rates were similar in both groups (E; 70.6% vs Y; 67.3%). Reocclusion rates at predischarge CAG were similar in both groups (E; 70.6% vs Y; 67.3%). Reocclusion rates at predischarge CAG were similar in both groups (E; 7.2% vs Y; 6.8%). Hospital cardiac deaths in E were higher than Y (E; 5.9% vs Y; 4.2%). Intestinal bleeding in E was more frequent than in Y (E; 5.9% vs Y; 3.2%). We concluded that thrombolytic therapy for acute myocardial infarction in elderly patients was useful, however bleeding complication in elderly patients were higher than in younger patients.

Human papillomavirus type 16 DNA detected by the polymerase chain reaction in non-cancer tissues of the head and neck.

Cancer-free tissues from various anatomical subsites in the head and neck were examined by the polymerase chain reaction (PCR) for the incidence of human papillomavirus (HPV) types 16 and 18. We detected HPV-16 DNA in 9 of 103 samples (8.7%), including specimens from the paranasal sinuses, tonsil, hypopharynx and larynx. However, no HPV-16/18 DNA was detected by Southern hybridization in these 9 samples. The significance of the presence of HPV-16 DNA in non-cancer tissues is still unknown, but PCR detection only of high-risk HPV DNA in head and neck cancer should be evaluated cautiously because of its ubiquity in this region.

Structure-activity relationships of HIV-1 PR inhibitors containing AHPBA.

A series of Human Immunodeficiency Virus type-1 protease (HIV-1 PR) inhibitors that contain 3-amino-2-hydroxy-4-phenylbutanoic acid (AHPBA) at the scission site of the substrate were prepared and evaluated for their inhibitory activity. Preliminary studies on the chain length of inhibitors and the hydroxyl configuration of AHPBA indicated that small (2S,3S)-derivatives, composed of the regions between the P3 and P2' sites, showed enough inhibitory activity toward HIV-1 PR to become prototypes for further structural modification. Systematic replacement at the sites from P3 to P2' revealed that some bicyclic heteroarylcarbonyl derivatives possessed strong potency and good enzyme selectivity.

Morphologic and metabolic study of the effect of oxygenated perfluorochemical perfusion on amputated rabbit limbs.

The purpose of this investigation was to examine the effect of oxygenated perfluorochemical perfusion on ischemia injury to skeletal muscle in rabbits. Forty-two hindlimbs of white rabbits were divided into six groups: 3-hr perfusion; 6-hr perfusion; 3-hr hypothermia; 6-hr hypothermia; sham operation without perfusion or hypothermia preservation; and biopsy. Endothelial cells in these muscles were evaluated using electron microphotography. The areas of both the inner and outer sides of cross-sectioned endothelial cells without cell nuclei were measured, and a new "capillary index" was calculated: (outer area--inner area)/outer area. The values of the capillary index in the perfusion groups were similar to those in the biopsy group. However, the values in the hypothermia groups were significantly greater than those in the biopsy group. These measurements suggested that the perfluorochemical perfusion method preserved the capillary endothelial cells in a nearly normal condition. The adenine nucleotides of these muscles were also evaluated. ATP levels in the 6-hr hypothermia group showed a substantial decrease to 57 percent of those in the sham-operated group. The 6-hr perfusion group levels, however, decreased only to 80 percent of those in the sham-operated group. Although application of the oxygenated perfluorochemical perfusion method did not maintain normal ATP levels, the maintenance of ATP levels was clearly higher than levels maintained in the hypothermia-preservation group. These findings suggest that oxygenated perfluorochemical perfusion is a satisfactory method for preserving amputated limbs.

Basic fibroblast growth factor has a beneficial effect on the viability of random skin flaps in rats.

We investigated the effect of exogenous basic fibroblast growth factor (bFGF) on the viability of random skin flaps in rats. Thirty-six rats were divided into three groups of 12 animals: a saline control group and two bFGF-treated groups. Caudally based random skin flaps were raised on the backs of the rats. In the control group, saline was instilled under the flap after skin closure. Twelve other rats were treated with bFGF (20 micrograms) just after skin closure. The remaining 12 rats were treated twice with bFGF (20 micrograms), both just after the operation and 48 hours later. There was a significant improvement in viability in the bFGF-treated groups on postoperative day 7 compared with the control group. Total adenine nucleotide levels in the flaps on postoperative day 10 were significantly higher in the group receiving two doses of bFGF than in the other groups. Thus, bFGF can improve the viability of endangered skin flaps.

Studies of human immunodeficiency virus type 1 (HIV-1) protease inhibitors. III. Structure-activity relationship of HIV-1 protease inhibitors containing cyclohexylalanylalanine hydroxyethylene dipeptide isostere.

Systematic replacement of the P4-P2 subsites of substrate-based human immunodeficiency virus type 1 protease (HIV-1 PR) inhibitors containing cyclohexylalanylalanine hydroxyethylene dipeptide isostere (Cha-psi [H.E.]-Ala) at positions corresponding to the scissile sites of substrates was carried out. The structure-activity relationship revealed that compounds with the combination of hydrophilic P3 and beta-branched hydrophobic P2 amino acids generally showed strong inhibitory activity against HIV-1 PR. In particular, compounds 4 (Boc-Orn-Val-Cha-psi [H.E.]-Ala-NHBun; Bu(n) = n-butyl, Ki = 11 nM) and 6 (Z-Orn-Val-Cha-psi [H.E.]-Ala-NHBun, Ki = 8 nM) exhibited good enzyme selectivity, possessing no significant inhibitory activities toward closely related aspartic proteases, pepsin, cathepsin D, and renin. As a possible model system for (anti-Mo-MSV/MLV complex (Mo-MSV = Moloney murine sarcoma virus; MLV = murine leukemia virus)) activity was investigated. Both compounds were found to inhibit moderately the focus formation of Mo-MSV/MLV complex in NIH3T3 cells (compound 4, IC50 = 1.8 microM; compound 6, IC50 = 1.0 microM).

Evaluation of the efficacy of nicorandil in patients with ischemic heart disease by exercise Tl-201 myocardial SPECT.

The effect of nicorandil on myocardial perfusion in ischemic heart disease was studied using exercise-load Tl-201 myocardial SPECT (Ex-SPECT). ExSPECT was carried out in 12 patients with previous myocardial infarction (OMI) and 9 with angina pectoris (AP) before and after administration of nicorandil 15 mg/day, for three or more weeks; %Tl uptake and the washout rate in infarcted or ischemic areas were calculated from short axial images using the Bull's eye method. In the OMI group, %Tl uptake and washout rates in the infarction areas improved significantly from 52.4% and 0.25 before nicorandil to 60.4% and 0.38 after nicorandil. In the AP group, too, %Tl uptake and washout rates showed a significant improvement from 56.9% and 0.10 before to 69.1% and 0.33 after administration of nicorandil. Six subjects who had not received the drug, and who showed negative washout rates, had high improvement rates when nicorandil was administered, suggesting that the drug could increase myocardial perfusion during exercise loading as well as suppressing coronary spasm. Ex-SPECT was carried out in 4 subjects before and after the administration of nicarandil and after subsequent surgical treatment (PTCA or CABG) and the effects of the two therapies were compared. The washout rate was improved from 0.01 to 0.34 by the administration of nicorandil, and a notable increase in coronary artery blood flow was achieved compared to the level after surgical treatment, i.e. 0.50. It is concluded that normal dosages of nicorandil have a powerful direct effect of dilating the coronary arteries without any influence on preload or afterload.

[A biomechanical analysis of pronation-supination of the forearm using magnetic resonance imaging: dynamic changes of the interosseous membrane of the forearm during pronation-supination].

A magnetic resonance (MR) study was performed using a 0.5 tesla system to investigate the behavior of the interosseous membrane of the forearm during pronation-supination and to evaluate the influence of pronation-supination loading in the neutral position. The right forearm was examined in twenty volunteers at the proximal fourth part, middle part and distal fourth part of the forearm. Slices were examined at maximum pronation, 45 degrees pronation, neutral, 45 degrees supination and at maximum supination. A 0.1 Nm torque in both rotational directions was added in the neutral position. The MR image of the interosseous membrane of the forearm was a thin line with low contrast in the neutral position. The tendinous portion and membranous portions of the interosseous membrane could be differentiated. At maximum pronation and at maximum supination, the interosseous membrane was flexed, caused mainly by the relaxation in the membranous portion. The radius shifted slightly volarly to the ulna at maximum pronation, caused by the incongruity of the distal radioulnar joint. The radius shifted dorsally with pronation loading, and shifted volarly with supination loading. The inelasticity of the membranous portion of the interosseous membrane may be responsible for pronation-supination contracture, while rotational loading may be a cause of the distal radioulnar joint dislocation. These studies suggest that this technique is useful for further in vivo studies of kinesiology.

Comparison of Virapap filter hybridization with polymerase chain reaction and Southern blot hybridization methods for detection of human papillomavirus in tonsillar and pharyngeal cancers.

A method using a commercial dot filter hybridization kit, Virapap, was compared with Southern blot hybridization and polymerase chain reaction (PCR) for the detection of human papillomavirus (HPV) types 16 and 18 in pharyngeal and tonsillar cancers of 12 patients as well as tonsillar biopsies from 28 patients with chronic tonsillitis. Concordant results between Virapap and PCR, Virapap and Southern hybridization, and PCR and Southern hybridization methods were obtained respectively in 41.7%, 58.3% and 83.3% of the cancer cases, and 67.9%, 67.9% and 85.7% of the control (tonsillitis) cases. Virapap false-positive results were found in 5 cancer cases and 5 control cases. Although the Virapap method is reported to be useful for detecting HPV in gynecological tissues, this method cannot be recommended for the detection of HPV in pharyngeal and tonsillar cancers.

Studies of HIV-1 protease inhibitors. I. Incorporation of a reduced peptide, simple aminoalcohol, and statine analog at the scissile site of substrate sequences.

Inhibitors of the protease of human immunodeficiency virus type-1 (HIV-1) were designed and synthesized. A reduced peptide, simple aminoalcohol, and statine analog, 4-amino-3-hydroxy-5-phenylpentanoic acid (AHPPA), were inserted at the scissile site of substrate sequences of HIV-1 protease. While both reduced peptides and simple aminoalcohol derivatives were weak inhibitors, the peptides containing AHPPA demonstrated moderate inhibitory activity. The more potent alcohol configuration of AHPPA is (R), which is opposite to the configuration in potent inhibitors of other aspartic proteases. In particular, compound 28 ((3R,4S)-4-(N-tert-butoxycarbonyl- L-glutaminyl-L-asparaginyl)amino-3-hydroxy-5-phenylpentanoic acid 2'-methylbutylamide) had a Ki of 0.36 microM and exhibited excellent enzyme specificity.

[Evaluation of myocardial viability in the infarcted areas by the improved myocardial perfusion after percutaneous transluminal coronary angioplasty].

Muscle viability in infarcted areas was evaluated based on the relationship between Tl myocardial scintigraphic findings and improvements in myocardial perfusion after percutaneous transluminal coronary angioplasty (PTCA) in 110 patients with old myocardial infarction (OMI). 1. Myocardial perfusion was improved in 95% of patients with redistribution in infarcted areas before PTCA, but in only 29% of patients without redistribution before PTCA. 2. The %Tl-uptake and the washout rate were significantly improved after PTCA in all patients with redistribution before PTCA with normal to dyskinesia of segmental wall motion. The %Tl-uptake of infarcted area after PTCA was greater than 50% in all patients. 3. The %Tl-uptake and the washout rate were significantly improved after PTCA in patients without redistribution before PTCA. However, the %Tl-uptake was not changed after PTCA in such patients with akinesia or dyskinesia of segmental wall motion. 4. Myocardial perfusion was improved late during prolonged follow-up after PTCA in 40% of patients without restenosis. Stunning of the myocardium is likely in these patients. 5. %Tl-uptake of more over 50% in patients with or without redistribution before PTCA indicates the presence of muscle viability. Muscle viability cannot be expected in the infarcted areas of patients without redistribution showing akinesia or dyskinesia of segmental wall motion. Patients without redistribution before PTCA include cases with underestimated ischemic lesion. The diagnosis of muscle viability by single photon emission computed tomography will be improved by including evaluation of segmental wall motion by echocardiography.

Persistence of human papillomavirus type 6e in adult multiple laryngeal papilloma and the counterpart false cord of an interferon-treated patient.

Human papillomavirus (HPV) type 6e DNA was detected in a laryngeal papilloma, in a papilloma recurring after an interval of six years and in the counterpart clinically-normal false cord of the same patient. No detectable subgroup change of HPV DNA occurred during the clinical course, which included therapy by CO2 laser followed by intramuscular alpha-interferon injection. The data demonstrated a single subtype of HPV to have persisted, and the prevention of a recurrence of laryngeal papilloma during our patient's long clinical course to have failed, in spite of the interferon treatment.

Human papillomavirus type 18 DNA in so-called HEP-2, KB and FL cells--further evidence that these cells are HeLa cell derivatives.

The contamination of HeLa cells into many culture cells has been suspected. We detected HPV type 18 DNA in two HeLa cell sublines, Hep-2 laryngeal cancer and KB oral cancer cell lines as well as FL cell line derived from normal amniotic membrane which have been extensively used so far in experimental virology. On the other hand, we detected HPV type 18 DNA neither in other epithelial cell lines nor in fibroblastic human embryonic cells. The digestion analysis with several restriction enzymes of the HPV type 18 DNA in HeLa, Hep-2, KB and FL cell lines were almost the same. The molecular size of episomal DNA could not be demonstrated by digestion of these DNAs with XhoI, a non-cut enzyme for HPV type 18 DNA, suggesting all HPV types 18 DNA were integrated. Interestingly enough, the HPV type 18 specific DNA in these cells was cleaved into two fragments with another non-cut enzymes, B g/II and HindIII, suggesting the presence of a single cutting site for each enzymes presumably generated by rearrangements or mutations. These results provide further evidence that the so-called Hep-2, KB and FL cells are all HeLa cell derivatives.
