The efficacy of fluoxetine in the treatment of premature ejaculation: a double-blind placebo controlled study.

PURPOSE: The efficacy of the selective serotonin re-uptake inhibitor fluoxetine in the treatment of premature ejaculation was examined. MATERIALS AND METHODS: The study comprised 17 patients with premature ejaculation who presented to the urology clinic of our medical school. In this double-blind study the patients were randomized into treatment groups receiving 20 mg. fluoxetine daily for 1 week and 40 mg. daily afterward (group 1) or 1 capsule placebo daily for 1 week and 2 capsules daily afterward (group 2). The groups were evaluated according to the latent period of intravaginal ejaculation. RESULTS: The latent period of intravaginal ejaculation in group 1 was significantly longer than that in group 2. Nausea, headache and insomnia were reported side effects. CONCLUSIONS: Fluoxetine may be regarded as a safe and effective alternative in the treatment of premature ejaculation.

Reference values of umbilical cord and third-day cystatin C levels for determining glomerular filtration rates in newborns.

The aim of this study was to determine reference values for serum cystatin C at, and 3 days after, birth, and to determine if the concentration was influenced by gender, gestational age or bilirubin level. Umbilical cord and peripheral venous blood was taken, and serum cystatin C, creatinine, and total and direct bilirubin levels were measured. The mean concentration of cystatin C was not significantly different between cord blood and blood taken on day 3 (1.36 +/- 0.35 mg/l and 1.35 +/- 0.33 mg/l, respectively). Comparison of subgroups, divided by gender, duration of gestation and bilirubin levels, using the Mann-Whitney U-test and Wilcoxon analysis, showed no effect of these parameters on cystatin C levels.

Late infantile acid maltase deficiency: a case report.

A five-year-old boy with late-infantile (juvenile) form of acid maltase deficiency is presented. His symptoms were restricted to skeletal muscle. There is commonly a correlation between the amount of residual acid maltase activity and the severity of the clinical picture. Although the residual enzyme level was very low in our patient, no progression of his neurological findings have been observed during the follow-up period of two years.

Clinical features of 21 patients with lissencephaly type I (agyria-pachygyria).

Lissencephaly (agyria-pachygyria) is the most severe neuronal migration disorder, characterized by total or partial absence of gyri. In this study, 21 patients with lissencephaly type I (9 girls, 12 boys) with a mean age of 19 +/- 21 months (2 weeks-8 years) were evaluated clinically and graded according to neuroradiological findings (19 patients by magnetic resonance imaging MRI and 2 by computed tomography CT). Three patients were classified as lissencephaly grade 2 and 18 patients as grade 3 or 4. Clinically, 12 patients (57%) had microcephaly, and eight (38%) had facial dysmorphism. All the patients had prominent psychomotor retardation, moderate to severe; the most frequent neurological findings were spastic guadriplegia (36.4%) and hypotonia with exaggerated tendon reflexes (27.3%). Seventy-eight percent of the patients had epileptic seizures resistant to conventional treatment. Lissencephaly is a cerebral cortical malformation that should be considered in children with developmental delay with or without microcephaly and facial dysmorphism. In addition, it should be investigated in the etiology of early-onset childhood epilepsy.

The effect of adjunctive Mitomycin C in Ahmed glaucoma valve implantation.

PURPOSE: To evaluate the effectiveness and safety of adjunctive mitomycin C (MMC) in Ahmed glaucoma valve (AGV) implantation for refractory glaucoma. METHODS: Twenty-two eyes of 22 patients who underwent AGV implantation with adjunctive MMC (0.5 mg/ml) for 3 minutes (Group A) were compared to a control group of 26 eyes of 26 patients (Group B) who received AGV implantation without MMC. Nine patients were female and 13 patients were male in Group A and 11 patients were female and 15 patients were male in Group B. The mean age was 56 years ranging from 13 to 77 in Group A and 58 ranging from 14 to 71 years in Group B. Success was defined as an intraocular pressure (IOP) between 4 and 21 mmHg with or without glaucoma medication and with no additional glaucoma surgery, phthisis, or loss of light perception. RESULTS: The probability of success at 1 year was 86.36% and 80.76% in Group A and Group B, respectively. The number of glaucoma medications decreased from 2.82 to 0.56 in Group A and from 2.65 to 0.73 in Group B. Postoperative hypotony as an early complication was higher in Group A (31.81% in Group A and 15.38% in Group B). As a late postoperative complication, tube exposure developed in 3 patients (13.63%) in Group A and no such complication was seen in Group B. There was no statistically significant difference in success and complication rates between the two groups (p>0.05). CONCLUSIONS: Although adjunctive MMC in AGV implantation is safe and effective, it may not offer a better chance of surgical success compared with AGV implantation without MMC.

Defective serum opsonization activity in children aged 6-48 months having acute purulent otitis media.

Serum opsonization of yeast (Saccharomyces) was investigated in 51 patients whose ages were between six and 48 months (median 15 months) with acute purulent otitis media and in an age-matched control group (median 13 months). Opsonization was assessed by measuring yeast particle uptake in an assay based on an electronic count of the unphagocytosed particles in serum by polymorphonuclear leukocytes. Despite normal levels of CH50 and serum immunoglobulins, a defective opsonization was determined in 13.7 percent of the patients (7 in 51). The corresponding figure was 2.9 percent in 103 healthy controls (p < 0.001). On the other hand, 218 percent (5 in 23) of the children having a history of recurrent purulent otitis media showed defective opsonization (p < 0.001). Previously, the presence of an opsonization defect has been linked to low levels of mannan binding lectin (MBL), a calcium dependent serum lectin that acts as an opsonin. Therefore, our findings indirectly support the idea that MBL has an important role as host defense, particularly in the earlier period of life when the antibody repertoire is restricted.

Free hydatid cyst only covered with germinative membrane disrupted from fibrotic capsule in the peritoneal cavity: a case report.

Echinococcosis, an endemic disease on the Asian continent, is caused by ecinococcus granulosus and rarely ecinococcus alveolaris. Although it occurs frequently in the liver and the lung, it can be localized in all tissues and organs. The purpose of this study is to report a rare localization and the complication of hydatid cyst in the pelvis : it pushed forward and upward the urine bladder and there was bilateral hydroureteronephrosis causing uraemia because of outflow obstruction. To our knowledge, a similar case has not been published before in English literature.
