[Symptomatic adrenoleukodystrophy heterozygote with fluctuated neurological symptoms--a case report].

We report a patient with symptomatic adrenoleukodystrophy (ALD) heterozygote, a 57-year-old female, who manifested fluctuated neurological symptoms. She is a mother of the patient with adrenoleukomyeloneuropathy associated with hypoparathyroidism and cerebral calcification. The heterozygote of ALD was diagnosed by elevated levels of very long chain fatty acids in her plasma and erythrocyte membranes. At age 51, she had a disturbance of consciousness, which subsided two months later. Neurologically, bilateral pyramidal tract signs were noted. CT revealed low-density areas in the white matter neighboring the bilateral anterior horns of lateral ventricles and small calcification in the basal ganglia. Four years later, her neurological manifestations including gait disturbance and mental deterioration subacutely progressed. MRI (T2-weighted image) showed diffuse high intensity areas in the cerebral white matter. Adrenal insufficiency, hypoparathyroidism and peripheral neuropathy were not detected. It has been reported that some ALD heterozygotes developed neurological symptoms, which resembled those of the patients with adrenomyeloneuropathy. The presented patient manifested neurological symptoms with fluctuation, which was not common clinical course in the heterozygotes of ALD. On the differential diagnosis of neurological diseases such as multiple sclerosis, cerebrovascular disease or other cerebral white matter diseases, it is important to consider the possibility of ALD heterozygotes.

Histologic findings on the condylar process of the mandible in rats with reduced bone-mineral density.

We studied the histology of the condylar process in rats on a low calcium diet. Female rats aged 15 weeks were given a low Ca diet for 14 weeks, which reduced bone-mineral density, i.e., induced osteoporosis. Histologic observation revealed tylosis of the fibrous layer of the condyle, shrinkage of the cartilaginous layer, enlargement of the marrow cavity, reduction in the number of osteoblasts in the condylar neck, and cellular disarrangement and other morphologic changes of both the fibrocartilaginous and cartilaginous layers.

An experimental study on mandibular movement and osteoporosis.

In order to clarify the relationship between mandibular movement and osteoporosis, the authors secured the temporo-mandibular joints of 8-week-old rats to immobilize their jaws; then conducted histologic studies chronologically, up to the fifth week. After three weeks, an expansion of the marrow cavity and a reduction of the trabecular were observed in the mandibular process and mandibular fossa. During the fifth week, an osteoporosis-like state was clearly observed. Further, an irregularity in the form of mandibular process was recognized, as well as a remarkable disturbance caused by cartilaginous ossification of the articular cartilage of the condyle.

[Orthostatic purpura: report of two cases].

Two patients with orthostatic purpura were reported. Case 1: A 47-year-old man was admitted to our institution because of multiple purpuric eruptions over the legs after the long periods of sitting or standing. Bleeding time was 4.0 min. Platelet aggregation induced by ADP was disaggregated and no aggregation was observed when induced by collagen and epinephrine. He was diagnosed as having release abnormality of the platelets caused by glutathione administered for treatment of liver damage for several years. Purpura, however, appeared even though platelet function became normal after discontinuing glutathione. Purpura proved to be induced by the long periods of sitting or standing. Case 2: A 37-year-old woman was admitted to our hospital because of purpuric eruptions over the legs for three years. Bleeding time was 2.0 min. Platelet aggregation induced by ADP, epinephrine, collagen and ristocetin was normal. Purpura was caused by long periods of standing without movement. Wearing elastic pantyhose showed reduced appearance of the purpura. Biopsy specimens from both patients revealed no abnormal findings. Both patients had no abnormal results in coagulation studies. Purpura seemed to be caused by the increased capillary pressure after the prolonged periods of standing. No case report concerning orthostatic purpura or mechanical purpura has been published.

[Progression from latent cardiomyopathy to dilated cardiomyopathy: a review of five cases].

We report 5 patients, who had no cardiac abnormalities at their initial evaluation but progressed to dilated cardiomyopathy (DCM) during a mean follow-up period of 8.2 years. Their echocardiograms revealed increases in left ventricular (LV) end-diastolic dimensions, deterioration of LV contractions, and reduction in the LV wall thickness. Electrocardiographic changes were developed of rhythm disturbances, intensified ST-segment and T-wave changes, and the prolonged QRS durations. These findings, especially those of electrocardiography, seemed to be important for an early detection and characterization of DCM.

[Pericardial effusion in acute myocardial infarction: clinical feature and significance].

To determine the incidence and clinical significance of pericardial effusion after acute myocardial infarction, two-dimensional echocardiography was serially performed in 137 consecutive patients. Pericardial effusion was observed in 45 patients (33%), of whom 22 were followed until they recovered and were discharged. Pericardial effusion was more frequent in patients with anterior acute infarction than those with inferior acute infarction, and so it was in non-recanalized patients than in recanalized ones. Patients with pericardial effusion had higher peak levels of creatine kinase, higher wall motion score indices, and higher defect scores of thallium imagings. The improvement of regional wall motion at an infarct zone in patients with pericardial effusion was less regardless of the successful early recanalization. These results show that pericardial effusion is a common event in patients with acute myocardial infarction and observation of transition of pericardial effusion is important for predicting prognosis.

[Utility of proton magnetic resonance spectroscopy in the diagnosis of human brain tumors].

Proton nuclear magnetic resonance spectroscopy (1H-MRS) was employed in vitro to obtain profiles of various metabolites contained in some of the representative human brain tumors. Extracted samples from each normal and tumoral tissue were used to obtain high-resolution spectra. While N-acetylaspartate was unequivocally demonstrated in normal brain tissues, it could not be detected in brain tumors. Creatine was variably reduced in brain tumors relative to that in normal brain tissue, and was undetectable in neurinomas and pituitary adenomas. Choline was present in all tumors and appeared as relatively prominent peaks in the spectra for meningiomas, pituitary adenomas and metastatic brain tumors. In contrast, levels of creatine and inositol in these tumors were low. Neurinoma showed the largest inositol peak among the tumors examined. Thus compared to normal brain tissue, creatine, choline and inositol in tumors exhibited various degrees of characteristic changes. Therefore we attempted to categorize the spectral pattern of normal and tumoral tissues based on creatine/inositol and/or choline/inositol ratios. There were statistically significant differences between either creatine/inositol ratios and choline/inositol ratios in the tumor groups and the ratios found in normal brain tissue. These ratios also varied relative to different tumor groups. These data suggested that nuclear magnetic resonance spectroscopy might yield clinically useful information which can not be provided by already widely used magnetic resonance imaging and might be applicable to the differential diagnosis of some brain tumors.

[Seven cases of eye involvement in adult with acute leukemia].

Seven acute leukemia cases with oculomotor paralysis and/or optic nerve paralysis were reported. Five cases were lymphoblastic and two were myelogenous leukemia. The average age was 30.7 years old and male to female ratio was 5:2. The eye involvement developed during hematological complete remission stage in 4 cases, and during relapsing stage in 2 cases. For the remaining case, eye symptom was an initial symptom of the disease. The computed tomography of ocular region showed tumorous involvement in 3 cases and no abnormality in other 4 cases. The white blood cell count on admission was over 30,000/microliters and leukemic cells occupied over 59% of bone marrow nucleated cells in 5 cases. Intrathecal administration, a whole brain irradiation and intra-arterial injection of Nimustin as well as standard systemic chemotherapy were performed against oculomotor and optic nerve involvement. Leucoencephalopathy occurred in 2 of 7 cases after intrathecal administration and irradiation. Since 1980, continuous treatment for prophylaxis of central nervous system leukemia have been done and this protocol was effective for meningeal involvement but not for ocular involvement.

Rest-injected thallium-201 imaging for assessing viability of severe asynergic regions.

To evaluate the utility of rest-injected 201Tl initial and delayed images for assessing the viability of severe asynergic regions, we studied 17 patients with apparently prior infarcted myocardium in combination with 99mTc ventriculography before and after revascularization. In 51 regions with severe asynergy, the percent 201Tl uptake was calculated as the ratio of counts on the segment with asynergy to the maximum counts on the normal segment. Eleven of 14 regions with resting 201Tl redistribution (Group 1) had improved wall motion after revascularization. However, 14 of 37 regions without redistribution also improved (Group 2). Twenty-three regions without redistribution or improved wall motion after revascularization (Group 3) had lower regional 201Tl uptake on their delayed images than those in Groups 1 and 2. Moreover, the initial regional uptake of Group 2 was higher than that of Group 3. These results suggest that redistribution on rest-injected 201Tl scans indicates reversibility of severely asynergic myocardium and that high 201Tl uptake in regions without redistribution may predict improvement in wall motion after revascularization. We conclude that 201Tl uptake may be useful as a marker of viability of severe asynergic regions before revascularization.

Development of an enzyme-linked immunosorbent assay of apolipoprotein E-AII complex in plasma.

Measurement of apolipoprotein (apo) E-AII complex in human plasma is important in determining the role of apoE in lipoprotein metabolism. In this paper, we demonstrate a new and simple method to determine apoE-All complex by using an enzyme-linked immunosorbent assay. Anti-apoE IgG (goat) was used as a capture antibody, and captured apoE-All complexes were detected by an anti-apoAll (rabbit) horseradish peroxidase-conjugated anti-rabbit IgG (goat) system. With this method, apoE-All complex was specifically determined without the interference of apoAll and was not affected by apoE monomer less than 250 mg/L. The content of the complex in reference serum, a normolipidemic serum pooled from five subjects with phenotype E3/E3, was arbitrarily defined as 100%. The coefficients of variation were 3.5%-6.3% within assay and 8.8%-11.6% between assays.

[A kinetic study on serum prolactin concentration in the thyrotropin-releasing hormone test].

In order to estimate the mechanism of prolactin secretion using the kinetic model, serum prolactin concentrations in normal young men were measured by an immunoradiometric assay before and after intravenous administration of 500 micrograms of thyrotropin-releasing hormone (TRH). The kinetic model was applied to evaluate the secretion rate both before and after the administration of TRH. A good correlation between the calculated values and measured values of serum concentration was observed (r = 0.995); and the adequacy of the model was confirmed. A few minutes (1.97 +/- 0.57 min) after the administration of TRH, the secretion of prolactin was stimulated, and serum prolactin concentration was maximum at 12.7 +/- 2.1 min. The total amounts of secreted prolactin was 75.4 +/- 18.4 ng/ml, which corresponded to the amount that was secreted in 4.3 hours at the normal rate. The half-life of prolactin was 40.8 +/- 13.8 min, which is in good agreement with the previously reported values. We have already reported on the kinetic study on the TSH secretion after TRH. Comparing the concentration change of prolactin and TSH, which were reported previously, it was found that the stimulated secretion started almost simultaneously, but lasted longer with respect to TSH.

Interferon gamma induces differentiation of cells from patients with myelodysplastic syndromes in vitro and in vivo.

A recombinant human interferon-gamma (IFN-gamma) was added to the clonal and suspension cultures of bone marrow (BM) cells from three patients with refractory anemia with excess of blasts (RAEB) before and after IFN-gamma therapy. IFN-gamma inhibited the hematopoietic progenitor cell growth from normal BM in one patient. In the remaining two patients with RAEB, IFN-gamma at lower concentrations (10-10(2) U/ml) consistently enhanced the growth of hematopoietic progenitors. The addition of IFN-gamma to suspension cultures promoted cellular differentiation in all three cases, as evidenced by persisting cytogenetic changes, Auer-rod containing maturing cells and cytochemical studies. Although IFN-gamma therapy resulted in partial or minor responses, these findings suggest the potential of IFN-gamma to act as an inducer of differentiation in myelodysplastic syndromes both in vitro and in vivo.

A topographical relationship between Helicobacter pylori and gastritis: quantitative assessment of Helicobacter pylori in the gastric mucosa.

A topographical relationship between the number of Helicobacter pylori in the gastric mucosa and the histological severity of gastritis was studied in 902 pairs of biopsy specimens taken from 314 patients. A pair of biopsies were taken from the antrum, the lesser curvature of the middle body, and the greater curvature of the upper body of the stomach. The quantitative assessment of H. pylori was made based on smear, culture, and tissue section. The histological severity of gastritis was assessed as to the degrees of mononuclear cell and polymorphonuclear leukocyte infiltration. A positive correlation was confirmed between the number of H. pylori and the severity of polymorphonuclear leukocyte infiltration. The degrees of inflammatory cells infiltration in the specimens with H. pylori colonization were significantly lower in the upper body than in the antrum.
