[A case of acute lupus pneumonitis followed by high-resolution CT].

A 43-year-old woman was admitted to our hospital because of fever and dyspnea. She had facial rash, hair-loss, arthralgia, urinary casts, thrombocytopenia, lymphocytopenia, and deposits of IgG, IgM, C3 in the epidermis. SLE was diagnosed. She also had severe dyspnea, which gradually progressed. In spite of steroid pulse therapy and cyclophosphamide, she eventually died about two months after admission. Chest X-ray film showed bilateral ground-glass shadow and volume-loss. High-resolution CT of the lung showed marked high density area with airbronchogram in the posterior part and a gradually enlarging slight high density area in the anterior area. Pathological findings of the posterior area were atelectasis and dense fibrosis. The findings of the anterior area were collapsed alveoli and dilated alveolar ducts. These findings might suggest that alveolar collapse play a major role in the course of pulmonary fibrosis of this patient.

Intraperitoneal seeding of ruptured hepatocellular carcinoma: case report.

We report a solitary intraperitoneal recurrence in a long-term survivor after rupture of hepatocellular carcinoma. Ultrasonography demonstrated the finding of a lobulated mass with a "mosaic" appearance. The patient underwent resection of this implant and is currently disease free. We emphasize the importance of periodic surveillance of disseminated metastases of this condition.

Use of disposable stapler in operative cystogastrostomy for pancreatic pseudocyst.

Surgical stapling techniques are widely used in gastrointestinal surgery. These procedures are excellent in convenience and safety. We describe here a new practical application of the surgical disposable stapler, Auto Suture Premium Plus CEEA 34 circular stapler, for the operative drainage of a large symptomatic pancreatic pseudocyst. A 68 year-old man underwent an operative cystogastrostomy using this instrument. His post-operative recovery was uneventful. He is free from symptoms, and abdominal tomography shows complete disappearance of the cystic cavity. We believe that this is the first clinical paper that reports on the stapled cystogastrostomy. This instrument is very useful for creating a stapled cystogastrostomy, similar to one created in the standard open approach.

Effects of ulinastatin on polymorphonuclear leukocyte elastase and interleukin 6 levels after hepatectomy.

Eight patients undergoing hepatectomy received 15 x 10(4) U of ulinastatin during and after surgery and then twice daily for 4 days, and 11 hepatectomized patients served as untreated controls. Between-group differences in age, duration of surgery, volume of blood loss and transfusion, weight of resected liver, and results of preoperative liver function tests were not significant. On postoperative day 1, mean (+/- SD) polymorphonuclear leukocyte elastase levels were significantly lower in the treated patients than in the controls (310 +/- 69 micrograms/L vs 519 +/- 382 micrograms/L; P < 0.05); interleukin 6 levels also tended to be lower in the treated patients than in controls (42.4 +/- 34.0 pg/mL vs 63.3 +/- 43.1 pg/mL). The results indicate that ulinastatin has beneficial effects on host defenses after surgery.
