[An improved method for the isolation of rat hepatic stellate cells].

OBJECTIVE: To present an improved method to obtain pure, viable, freshly isolated hepatic stellate cells. METHODS: Adult male SD rats were used. All procedures were performed with the animals under sodium pentobarbital anesthesia. Three days after the single intravenous administration of 1 ml liposome-encapsulated CL2MDP, which has selective cytotoxicity of Kupffer cells, livers were perfused with D-Hank's solution containing 100 U/ml heparin for 10 to 15 minutes, and then with 0.05% collagenase dissolved in D-Hank's solution for 25 to 30 minutes. The liver was then gently homogenized and further incubated in 0.025% collagenase, and 0.005% DNAase I for 30 minutes at 37 degrees C under constant stirring. This suspension was filtered through stainless steel gauze and centrifuged for 2 minutes at 50 x g to remove parenchymal cells. Sinusoidal cells in the supernatant were recovered by centrifugation for 10 minutes at 300 x g. The cells were resuspended in the presence of 28.7% Nycodenz stock solution. The final concentration of Nycodenz at this stage was 11.5%. Following centrifugation for 17 minutes at 1400 x g, The cells at the top of this Nycodenz solution were collected. Cells were resuspended in Dulbecco's modified Eagle medium supplemented with 10% fetal calf serum, The cells were seeded in 50 ml culture flask at a density of 500,000 cells/ml, The cell viability was determined by trypan blue exclusion staining, the purity of hepatic stellate cells was identified by the expression of Desmin using immunocytochemistry method. Endogenous peroxidase staining was used to detect Kupffer cells. RESULTS: The yield rate of hepatic stellate cells was 3 x 10(7) per rat, the cell viability was more than 95%, the desmin positive cell rate was 90%, no endogenous peroxidase positive cells were detected. CONCLUSION: The method for the isolation of hepatic stellate cells was developed without Kupffer cells confusion. The availability of highly purified stellate cells will facilitate the investigation of their functions in primary culture.

Orthotopic liver transplantation for patients with hepatocellular carcinoma complicated by portal vein tumor thrombi.

BACKGROUND: Orthotopic liver transplantation (OLT) has been the valuable treatment of choice for patients with hepatocellular carcinoma (HCC). As the number of patients with portal vein tumor thrombi (PVTT) increases, most transplant centers become suspicious of the exact effect of the operation which has been accepted as a radical method. This study was designed to evaluate the clinical effects of OLT for patients with HCC associated with PVTT. METHODS: The follow-up of 24 patients with HCC complicated by PVTT who had received OLT (transplant group) from January 1999 to March 2003 was compared to that of 27 patients undergoing routine hepatic resection (resection group) and 59 patients without surgical treatment (non-surgical group). RESULTS: The perioperative mortality was zero for the transplant group. The 6-month, 1-year, and 2-year overall survival rates were 66.7%, 29.5% and 23.6% for the transplant group, 33.3%, 22.2% and 14.8% for the resection group (P=0.0335), and 42.1%, 24.4% and 4.1% for the non-surgical group, respectively (P=0.0316). The tumor free survival rates of recipients at 6-month, 1-year, and 2-year were 51.5%, 23.2% and zero, respectively. During the period of follow-up, the overall post-transplant intrahepatic recurrence or extrahepatic metastasis rate was 66.7% for the transplant group. CONCLUSION: OLT is an effective but palliative treatment modality for patients with HCC associated with PVTT followed by a prolonged survival but a poor tumor free survival rate.

Clinical experience in diagnosis and treatment of glucagonoma syndrome.

BACKGROUND: Pancreatic endocrine tumors are uncommon neoplasms and can lead to systemic disorder including glucagonoma syndrome, a very rare prototypical paraneoplastic phenomenon. The aim of this study was to assess the diagnosis and surgical strategy for the treatment of glucagonoma syndrome. METHODS: The clinical data of a case of pancreatic head tumor with typical glucagonoma syndrome of necrolytic migratory erythema (NME), diabetes mellitus (DM), anemia, and glossitis were retrospectively analyzed. RESULTS: Cutaneous eruption occurred mainly in the groin, extremities, thighs, buttocks, and perineum. A highly elevated level of serum glucagon was detected by radioimmunoassay. A tumor located in the head of the pancreas was well-defined by pre and intra-operative ultrasonography, contrast enhanced computed tomography, and magnetic resonance imaging. Tumor enucleation was performed, showing significantly improved symptoms. Near complete resolution of NME was shown one week after surgery. Surgical complications or recurrence was not found. CONCLUSIONS: The diagnosis of glucagonoma syndrome is established by marked clinical features such as NME as the hallmark clinical finding, hyperglucagonemia, and radiographically demonstrated neuroendocrine tumor. The topographic diagnosis of glucagonoma can be achieved by combined imaging methods. Enucleation of tumor is a valuable treatment for solitary pancreatic tumor without peripancreatic invasion, liver metastasis, and pancreatic duct compression.

Case-control study and transmission/disequilibrium tests of the genes encoding GABRA5 and GABRB3 in a Chinese population affected by childhood absence epilepsy.

BACKGROUND: Childhood absence epilepsy (CAE) is one of the most frequently recognized syndromes among the idiopathic generalized epilepsies (IGEs). CAE is considered to be a genetic disease, with a possible polygenic inheritance pattern. The genes responsible for CAE have not been identified yet. The object of this study was to investigate whether or not CAE is associated with the gene encoding the gamma-aminobutyric acid (GABA) type-A receptor subunits alpha5 (GABRA5) and beta3 (GABRB3) in a Chinese population. METHODS: Five microsatellite DNA repeats, 69CA, 85CA, 155CA1, 155CA2, and A55CA1, adjoining chromosome 15q11-q13, were used as genetic markers. Both case-control study and transmission/disequilibrium tests (TDTs), as well as fluorescence-based semi-automated genotyping techniques, were used in 90 CAE patient-mother-father trios and 100 normal controls of Han ethnicity to conduct association analysis. RESULTS: The frequencies of allele 5 of 69CA, alleles 2 and 8 of 85CA, alleles 6 and 7 of 155CA1, allele 2 of 155CA2, and alleles 1 and 11 of A55CA1 were significantly higher in CAE patients than in normal controls. To prevent spurious associations arising from population admixture, we further conducted TDT tests in the 90 CAE trios. The results of TDT analysis further suggested that microsatellite DNA repeats 85CA, 155CA1, and 155CA2 were associated with CAE. CONCLUSIONS: GABA type-A receptor subunit genes GABRA5 and GABRB3 may be either directly involved in the etiology of CAE in the Chinese population or in linkage disequilibrium with disease-predisposing sites.

[Spectrum diagnostic of arcjet].

Arcjet is a kind of propulsion device for mechanical operation and control of spacecraft. As its specific impulse is far greater than classical device using chemical propellant, arcjet is playing an increasing role in spacecraft propulsion. To improve our understanding of its working mechanics, the diagnostic method of arcjet is discussed and a set of spectrum diagnostic system is established in this paper. With this system, spectrum diagnostic was executed for Ar propellant at a setting value of flow rate and input current in a vacuum chamber. The result shows that the system has a high signal-to-noise ratio and the data collected can reflect the physical process objectively. Through transaction and analysis of these data, radial distribution of emission coefficient was obtained for different spectral lines, and radial distribution of temperature was also obtained through farther analysis of the emission coefficient. The result shows that under the experiment conditions of this paper, arcjet is in thermodynamic non-equilibrium state, therefore the temperatures obtained by different spectral lines are different.

[Study of genic mutations of androgen receptor in hypospadias].

OBJECTIVE: To study genic mutation of androgen receptor in hypospadias in Chinese. METHODS: Ninety-two patients with hypospadias were selected for the AR gene study (penile 33, scrotal 49, perineal 10; cryptorchidism 15, micropenis 34, indirect hernia 3, augmentation of breast 2, other deformities 8; posses family history 23) while 93 health men as the control. The DNA was collected from 5 ml venous blood by using the method of phenol/chloroform, and quantified with the agarose gel electrophoresis. The 2-8 exons of AR genes were directly sequenced with a PCR technique. The locus of mutation, the change of amino acid caused by genic mutation and the functional influence of target organ were also analyzed. RESULTS: There were 4 mutations found from the ligand binding domain of AR: exon 4[1 locus:664(ATT-->ACT] and exon 7[3 locuses: 840(CGT-->CAT),855(CGC-->CAC),859(CTC-->CTA)] in experimental group. The amino acid change of Iso664Thr may te the first findings in the world. The mutation of 859(CTC-->CTA) didn't cause the change of the code amino acid. The main clinical symptoms of exon 4[664(ATT-->ACT) were micropenis, slight hypospadias and augmentation of breasts. The exon 7[840(CGT-->CAT), 855(CGC-->CAC)]was showing micropenis and severe hypospadias. The last mutation [859(CTC-->CTA)] of exon 7 showed mild hypospadias. CONCLUSIONS: The rate of exon 2-7 mutations of the androgen receptor was 4.3%. The mutations were concentrated on ligand binding domain(exon 4-7), and the main phenotypes were hypospadias accompanying with micropenis found in our experimental group.

Identification and characterization of simple sequence repeats in the genomes of Shigella species.

A variety of simple sequence repeats (SSRs) have been identified in the genome of Shigella flexneri serotype 2a (strain Sf301), an enteric pathogen that causes bacillary dysentery in man. The distribution of SSRs, with unit length ranging from 1 to 9 nucleotides, was biased in different regions of the genome. The tri-, tetra- and hexanucleotide SSRs prevailed in the coding regions while the mono- and dinucleotide SSRs were more common in the noncoding regions. Many intergenic SSRs are less than 30 bp away from the downstream open reading frames (ORFs), suggesting a potential role in transcriptional regulation. To study polymorphism of SSRs, we compared 17 coding-region SSRs from strain Sf301 with the corresponding sequences from 23 other strains of four Shigella species. Five chromosomal loci were found to be polymorphic, of which those from S. flexneri strains were most variable. Particularly interesting is the C5-1 locus in the coding sequence of the hcaD gene encoding a subunit of ferredoxin reductase. Depending on the insertion of variable numbers of the unit sequence (CGCAG), the Shigella hcaD genes can encode truncated products due to premature stop codons or frame shifts, or products with extended core alpha helices that leads to radical alterations in the predicted tertiary structure. Hence, SSRs may serve as genotyping markers for epidemiological investigations, and may offer insights into evolutionary adaptation of the pathogens.

Haplotype structure and evidence for positive selection at the human IL13 locus.

Interleukin-13 (IL13) is believed to play an important role in the pathogenesis of atopy and allergic asthma. To better understand genetic variation at the IL13 locus, we resequenced a 5.1-kb genomic region spanning the entire locus and identified 26 single-nucleotide polymorphisms (SNPs) in 74 individuals from three major populations-Chinese, Caucasian, and African. Our survey suggests exceptionally high and significant geographic structure at the IL13 locus between African and outside Africa populations. This unusual pattern suggests that positive selection that acts in some local populations may have played a role on the IL13 locus. In support of this suggestion, we found a significant excess of high frequency-derived SNPs in the Chinese population and Caucasian population, respectively, as expected after a recent episode of positive selection. Further, the unusual haplotype structure indicates that different scenarios of the action of positive selection on the IL13 locus in different populations may exist. In the Caucasian population, the skewed haplotype distribution dominated by one common haplotype supports the hypothesis of simple directional selection. Whereas, in the Chinese population, the two-round hitchhiking hypothesis may explain the skewed haplotype structure with three dominant ones. These findings may provide insight into the likely relative roles of selection and population history in establishing present-day variation at the IL13 locus, and, motivate further studies of this locus as an important candidate in common diseases association studies.

[Association of child absence epilepsy with T-STAR gene].

OBJECTIVE: To investigate the Association of child absence epilepsy with T-STAR gene. METHODS: PCR was conducted on the DNA of peripheral blood white cells from 48 children with child absence epilepsy (CAE), 47 male and 49 female, aged 2.9 approximately 14, of Han nationality in Northern China and 48 healthy children in the same area to amplify the exons of T-STAR gene The PCR products underwent sequencing to identify the possible mutations. RESULTS: No mutation was found in the exons of the T-STAR gene, however, 3 single nucleotide polymorphisms (SNPs) were found. A case-control study was carried out, using SNP1 and SNP2. There was no significant difference in genotype frequency of the 2 SNPs between the CAE group and control group (SNA1: chi(2) = 2.965, df = 1, P = 0.085; SNP2: chi(2) = 2.965, df = 1, P = 0.085). There was no significant difference in allele frequency of the 2 SNPs between the CAE group and control group too (SNA1: chi(2) = 3.185, df = 2, P = 0.203; SNP2: chi(2) = 3.185, df = 2, P = 0.203). CONCLUSION: T-STAR may not be a susceptibility gene for CAE in Chinese populations.

The gene encoding GABBR1 is not associated with childhood absence epilepsy in the Chinese Han population.

Childhood absence epilepsy (CAE) is considered to be a genetic disease, but the genes responsible for it have not yet been identified. To investigate whether or not the GABBR1 gene is a susceptibility gene for CAE in the Chinese Han population, we systematically screened all the 22 exons and nearby intron regions of the gene and found 12 single nucleotide polymorphisms (SNPs). Using four SNPs as markers, we conducted a case-control study in 96 CAE patients and 96 normal controls. There were no significant discrepancies between the cases and controls in allele and phenotype frequencies of the four SNPs. There were still no significant differences in haplotype distributions between the cases and controls. We postulate that the GABBR1 gene might not be a susceptibility gene for CAE at least in the Chinese population.

[Infection of Coxsackievirus group B type 3 regulates the expression profile of chemokines in myocardial tissue/cells].

OBJECTIVE: To investigate the role of Coxsackievirus group B type 3 (CVB3) infection on the expression profile of chemokines (ChKs) in myocardial tissue/cells. METHODS: CVB3 was inoculated into male BALB/c intraperitoneally and primary neonatal myocardial cells of BALB/c to establish CVB3 infection models in vivo and in vitro, where the expression profile of ChKs was detected at different time points post-infection as well as under different loading of CVB3 qualitatively and quantitatively by RT-PCR. RESULTS: The expression of MIP-2 and IP-10 was induced post-infection, while SDF-1, MCP-1, MCP-2, MCP-3, MCP-5, MDC, FKN and Ltn were constitutively expressed in myocardial tissue. The expression of MCP-1, MCP-2, MCP-3, MCP-5, MDC and Ltn increased 1.8, 1.9, 3.7, 1.7, 1.3 and 1.2 folds post-infection higher than that of uninfected control (P < 0.01). There was not significant difference in the expression of SDF-1 and FKN between infected myocardial tissue and uninfected myocardial tissue (P > 0.05). The expression of Eot was not detected in infected and uninfected myocardial tissue. Every chemokine had different expression at different infection time points. For example, the expression of MIP-2 at the 4th day was 1.1 and 1.5 times than that of the 7th and 14th day (P < 0.01). IP-10 showed similar expression between the 4th and 7th days (P > 0.05), which is 2.47 and 2.54 times compared to that of the 9th day (P < 0.01). And the expression of MCP-1 at the 14th day post-infection was 1.3, 1.2 and 1.0 times comparing to that of the 4th, 7th, 9th day post-infection, which showed statistical meaning. The expression of MCP-2 at the 4th was 1.4, 1.5 and 2.2 times comparing to that of the 7th, 9th, 14th day, and moreover, the expression was lower than that of the basal expression. The expression of MCP-1 and MCP-3 was up-regulated significantly, which occurred at different time points post-infection in vitro. While the expression of MIP-2 and MCP-5 was down-regulated in vitro. The expression patterns of MCP-2, MCP-3, MCP-5 and MDC were consistent with CVB3 loading. But that of the others (FKN, SDF-1, et al) were inconsistent with CVB3 loading. There was a correlation between change patterns of MCP-3 and CVB3 loading post-infection (r = 0.881, P < 0.05) within 14 days after infection. The varied expression trend of MCP-1 and MCP-3 was similar to the titer of anti-CVB3 antibody (r = 0.913, P = 0.031), while the expression of MCP-2, MCP-5 and MDC shows contrary change. There was not a significant correlation between change patterns of other ChKs (IP-10, SDF-1, et al) and the titer of anti-CVB3 antibody. A positive correlation between anti-CVB3 antibody and MCP-1 (r = 0.976, P < 0.05) was showed. CONCLUSION: The expression level and kind of ChKs in vivo and in vitro was varied significantly in clusters after CVB3 infection. The ChKs changed in clusters consisted of the expression profiles of ChKs. There were complexity and unbalance in the change of the expression of ChKs in every expression profile. It suggested that CVB3 infection could regulate the expression of ChKs in myocardial tissue/cells likely in different ways.

T-type calcium channel gene alpha (1G) is not associated with childhood absence epilepsy in the Chinese Han population.

We investigated whether the T-type calcium channel gene alpha (1G) is associated with childhood absence epilepsy (CAE), a form of idiopathic generalized epilepsy. We carried out direct sequencing of exons 1-37 and the exon-intron boundaries of the alpha (1G) gene in 48 Han Chinese patients with CAE and 48 normal controls. We found no mutation in the exons of alpha (1G). However, we did identify six single nucleotide polymorphisms (SNPs). Using two of these as markers, we carried out a case-control study in 192 patients with CAE and 192 normal controls. The allele and genotype distributions of all the SNPs studied were not significantly different between cases and control groups, thus the alpha (1G) gene is not an important susceptibility gene for CAE, at least in the Chinese population.

Unique physiological and pathogenic features of Leptospira interrogans revealed by whole-genome sequencing.

Leptospirosis is a widely spread disease of global concern. Infection causes flu-like episodes with frequent severe renal and hepatic damage, such as haemorrhage and jaundice. In more severe cases, massive pulmonary haemorrhages, including fatal sudden haemoptysis, can occur. Here we report the complete genomic sequence of a representative virulent serovar type strain (Lai) of Leptospira interrogans serogroup Icterohaemorrhagiae consisting of a 4.33-megabase large chromosome and a 359-kilobase small chromosome, with a total of 4,768 predicted genes. In terms of the genetic determinants of physiological characteristics, the facultatively parasitic L. interrogans differs extensively from two other strictly parasitic pathogenic spirochaetes, Treponema pallidum and Borrelia burgdorferi, although similarities exist in the genes that govern their unique morphological features. A comprehensive analysis of the L. interrogans genes for chemotaxis/motility and lipopolysaccharide synthesis provides a basis for in-depth studies of virulence and pathogenesis. The discovery of a series of genes possibly related to adhesion, invasion and the haematological changes that characterize leptospirosis has provided clues about how an environmental organism might evolve into an important human pathogen.

Extensive association analysis between polymorphisms of PON gene cluster with coronary heart disease in Chinese Han population.

OBJECTIVE: An extensive association analysis of PON gene cluster (PONs) with coronary heart disease (CHD) was performed in Chinese Han population. METHODS AND RESULTS: Thirty polymorphisms of PON1, PON2, and PON3 gene were identified by direct sequencing of genomic DNA derived from 48 randomly selected patients. Twelve polymorphisms were additionally investigated for association with CHD in 474 male patients and 475 controls. Univariate analyses showed the cases had significantly higher frequencies of PON1 192Q allele, 160R allele, -162A allele, and PON2 311C allele than were seen in the controls. Logistic regression analyses revealed only the PON1 R160G and -162G/A polymorphisms remained significantly associated with CHD (P=0.0054 and P=0.0002). Haplotype analyses for various polymorphism combinations additionally confirmed the results of individual polymorphism analyses. Only the frequencies of haplotypes containing -162A allele were significantly higher, whereas only the frequencies of haplotypes containing 160G allele were significantly lower in cases than in controls in various polymorphism combinations. CONCLUSIONS: This extensive association study has identified the PON1 -162G/A and R160G polymorphisms to be independently associated with CHD in Chinese Han population and warrants additional study to elucidate the biological mechanism.
