Autopsy case of primary choriocarcinoma of the urinary bladder.

Choriocarcinomas usually develop in the uterus and ovaries in the female, being extremely rare in the extragenital organs in the male. Extragenital choriocarcinomas in the male usually develop in the mediastinum or retroperitoneum. The frequency of choriocarcinoma in the urinary bladder is extremely low. The purpose of the present paper was to report an autopsy case of choriocarcinoma in the urinary bladder in the male. An 81-year-old male patient with macrohematuria was first diagnosed with transitional cell carcinoma (TCC). At autopsy a hemorrhagic necrotic tumor, which was found in the urinary bladder with metastatic lesions in the lungs, was diagnosed as choriocarcinoma microscopically. There was no evidence for choriocarcinoma derived from any other organs than the urinary bladder, although there were metastatic lesions in both lungs and the direct invasion into the prostate. From these findings it is concluded that the tumor was a primary choriocarcinoma in the urinary bladder in a male patient. Choriocarcinoma of the urinary bladder is very rare, but the prognosis is extremely poor in comparison with TCC even in the urinary bladder. Therefore, it is essential to clearly discriminate between choriocarcinomas and TCC.

Macrophage colony-stimulating factor (M-CSF), as well as granulocyte colony-stimulating factor (G-CSF), accelerates neovascularization.

It has been reported that bone marrow cells (BMCs) differentiate into endothelial cells of blood vessels, and that granulocyte colony-stimulating factor (G-CSF) mobilizes progenitors in the BMCs to the peripheral blood, while macrophage colony-stimulating factor (M-CSF) augments the production of monocytes. We examined whether M-CSF augments the differentiation of BMCs into endothelial cells of blood vessels using a hindlimb-ischemic model. Either G-CSF or M-CSF, or both, was administered to the hindlimb-ischemic mice for 3 days. Both M-CSF and G-CSF augmented the differentiation of BMCs into endothelial cells of blood vessels through vascular endothelial cell growth factor (VEGF), resulting in early recovery of blood flow in the ischemic limbs.

A new strategy for treatment of malignant tumor: intra-bone marrow-bone marrow transplantation plus CD4- donor lymphocyte infusion.

Donor lymphocyte infusion (DLI) is clinically used for the treatment of malignant tumors. We have found recently that intra-bone marrow-bone marrow transplantation (IBM-BMT) can be used to treat various autoimmune diseases, even when radiation doses are reduced. In addition, recently we have found that IBM-BMT can prevent not only graft failure but also graft-versus-host disease (GvHD). Based on these findings, we attempted to prevent and treat the progression of a tumor (Meth-A cell line: BALB/c-derived fibrosarcoma) by DLI plus IBM-BMT. When the tumors had grown to approximately 10 x 10 mm, the tumor-bearing BALB/c (H-2(d)) mice were irradiated with 5 Gy, and whole spleen cells from C57BL/6J (B6) (H-2(b)) mice (as DLI) were then intravenously injected into the BALB/c mice. Simultaneously, bone marrow cells (BMCs) from B6 mice were injected directly into the bone marrow cavity of the BALB/c mice (IBM-BMT). The tumors decreased in size, but the mice died of GvHD. However, when CD4(+) T-cell-depleted spleen cells were used for DLI, the recipients showed only mild GvHD and survived longer, due to the slow growth of the tumor. In contrast, when CD8(+) T-cell-depleted spleen cells were used for DLI, the recipients showed more severe GvHD than those injected with whole spleen cells. These results suggest that IBM-BMT plus DLI (the depletion or reduction of a certain cell population like CD4(+) T cells) could be helpful to suppress both GvHD and tumor growth.

Evaluation of a quantitative method for trace impurities in arsenous acids by synchrotron radiation X-ray fluorescence spectrometry and inductively coupled plasma-atomic emission spectrometry.

Thirteen arsenous acid samples of known origins and refining methods were collected. Each sample was subjected to quantitative analysis of any impurity elements present using synchrotron radiation X-ray fluorescence spectrometry (SR-XRF) and inductively coupled plasma-atomic emission spectrometry (ICP-AES). The trace elements selected were Sn, Sb and Bi for the reasons that they were considered not to be changed by their circumstances and that they showed high sensitivity to SR-XRF. These results obtained by both methods were compared and the correlation between these two methods was determined. The quantification of trace impurities obtained by SR-XRF using As as an internal standard showed good agreement with the results obtained by ICP-AES. The discrimination of refining method became possible by the comparison of these impurities' contents measured with non-destructive SR-XRF using several arsenous acid particles.

Nondestructive analysis of silver in gold foil using synchrotron radiation X-ray fluorescence spectrometry.

Small particles of gold foil detached from an indoor decoration might be important evidence to associate a suspect with a crime scene. We have investigated the application of elemental analysis using synchrotron radiation X-ray fluorescence spectrometry to discriminate small particles of gold foil. Eight kinds of gold foil samples collected in Japan were used in the experiments. As a result of synchrotron radiation X-ray fluorescence spectrometry, only two elements, gold and silver, were detected from all gold foil samples. The intensity ratios of AgKalpha/AuLalpha showed good correlation with the content ratios of Ag/Au. The variation of intensity ratio within a same sample was sufficiently small compared with those of different samples. Therefore the comparison of this intensity ratio can be an effective method to discriminate small particles originating from different types of gold foil.

Forensic discrimination of sheet glass by a refractive-index measurement and elemental analysis with synchrotron radiation X-ray fluorescence spectrometry.

Measurements of the refractive index (RI) and elemental analysis using synchrotron radiation X-ray fluorescence spectrometry (SR-XRF) were applied to the forensic discrimination of sheet-glass samples from different origins. The refractive index was calculated from the matching temperature at which the glass fragments became invisible in silicone oil. Fragments smaller than 1 mm in maximum diameter were taken from each of 11 sheet glasses and subjected to analysis by SR-XRF. The XRF spectrum of these samples indicated that a comparison of 6 elements (Ca, Fe, Sr, Zr, Ba and Ce) was useful for the discrimination of sheet glasses. Cluster analysis was performed using 33 sets of SR-XRF data obtained by triplicate measurements for the 11 glasses. Comparing 528 pairs among 33 samples, 515 pairs could be correctly discriminated. The number of indistinguishable pairs could be reduced from 36 to 4 by comparing the SR-XRF data. Elemental analysis by SR-XRF could provide small glass fragments with a more evidential value than the solely measurement of only RI, through a significant improvement of the discrimination capability.

Evaluation of hyperacute stroke using perfusion computed tomography.

Evaluation of the cerebral circulation dynamics in patients with stroke soon after onset is extremely important for planning the optimum treatment. Perfusion computed tomography (CT) was evaluated in 118 patients with stroke within 10 hours of onset in whom initial CT showed no ischemic change. The mean transit time (MTT), cerebral blood flow (CBF), and cerebral blood volume (CBV) perfusion CT maps were visually evaluated in three slices covering the body of the lateral ventricle, the basal ganglia, and the pons, and the ratios of the values in regions of interest (ROIs) in the ischemic lesion and the symmetrical location in the opposite hemisphere were calculated (ROI ratio of regional (r) MTT, rCBF, and rCBV). The location of occlusion was confirmed by angiography performed on the same day in 106 cases and the location of infarction by later magnetic resonance (MR) imaging. MTT maps correctly identified 44 of 46 cases of ischemia in the carotid system, 20 of 29 cases of ischemia in the vertebrobasilar system, and 11 of 35 cases of ischemia in perforator regions. Eight cases could not be identified by perfusion CT, angiography, or MR imaging. The ROI ratios at the upper boundary of infarction (n = 18) were: rCBF 0.574 +/- 0.220 (mean +/- SD) and rCBV 0.972 +/- 0.276, and at the lower boundary of non-infarction (n = 24) were: rCBF 0.504 +/- 0.247 and rCBV 0.815 +/- 0.169; showing a statistical significance of p = 0.348 for CBF and p = 0.026 for CBV (unpaired t-test). The perfusion CT MTT maps correlated well with the angiographical findings for the carotid system, but poorly for the vertebrobasilar system and the perforator regions. A rCBF ratio of 0.5 and rCBV ratio of 0.9 were established for the boundaries of ischemia.

Three-dimensional computed tomography angiography of the galenic system for the occipital transtentorial approach.

The venous variations of the galenic system were evaluated using three-dimensional computed tomography angiography (3D-CTA) to assess the influence on the occipital transtentorial approach in 150 patients who underwent 3D-CTA as a routine screening examination for cerebrovascular diseases. The variations of the vein of Galen with its tributaries, the tentorial sinus, and the veins around the tentorium were evaluated in multiple intensity projections and stereoscopic images. The angle between the vein of Galen and the straight sinus was 67.1 +/- 31.9 degrees (mean +/- SD). Observation of the pineal body from the direction of the approach tended to extend to the quadrigeminal bodies in acute angle cases, and to the third ventricle in obtuse angle cases. Bilateral internal cerebral veins (ICVs) joined in the anterior portion were associated with a long vein of Galen, or in the posterior portion with a short vein of Galen. The distance between the bilateral ICVs was 4.66 +/- 2.28 mm (mean +/- SD), and the shape of the space could be classified as spindle, parallel, hairpin, and round types. The basal vein could be classified into well-developed, hypoplastic, hardly recognized, and mimicking two basal veins because the tributary did not join but ran parallel to the basal vein. The drainage pathways lead to the anterior or posterior portion of the vein of Galen, the ICV, the tentorial sinus, and the superior petrosal sinus. The various types of the tentorial sinus and primitive tentorial sinus which might be sacrificed during section of the tentorium were confirmed. The inferior cerebral vein draining to the tentorial sinus could be seen. 3D-CTA could also demonstrate the presence, the course, and the drainage points of the internal occipital vein, the precentral cerebellar vein, the posterior pericallosal vein, and so on. 3D-CTA is useful to evaluate the variations of the venous system and the relationship with the tumor, and for preoperative simulation and intraoperative navigation of the occipital transtentorial approach.

Simulation of endovascular neurointervention using silicone models: imaging and manipulation.

Silicone models of cerebral aneurysm and stroke pathology were produced by copying the images of clinical cases to evaluate the simulation of endovascular procedures. Conventional silicone models, silicone suture models made from separate aneurysm sac and parent artery sutured together, and animal vascular models made from vessels obtained from pigs were made. The models were incorporated in the training system for simulation of endovascular procedures under digital subtraction angiography control using a portable angiography system. The conventional silicone models provided very close reproduction of the aneurysm and vessels, even the fine branches. However, greater resistance or impact was felt, especially when passing the device through curved vessels. Stroke models were insufficiently dilated by percutaneous transluminal angioplasty or stenting. The silicone suture model was useful to avoid damage to models of giant aneurysm or models with small orifices during the lost wax procedure, whereas the projection of domes was somewhat inaccurate. The animal vascular model could not provide accurate aneurysm shape and vessel diameter, although the feel during manipulation was much more realistic.

[A neonatal case of anterior spinal artery syndrome presenting with bilateral arm paresis].

Anterior spinal artery syndrome is rare in children, especially in neonates. We present a girl with hydrops fetalis and hypothyroidism who developed flaccid paresis of both arms in the neonatal period (around day 25). MRI of the spine performed on day 52 revealed atrophic changes at C5-Th1 without Gd-DTPA-induced enhancement. Nerve conduction studies were also helpful in the diagnosis;in the upper limbs, motor potential was not elicited, while sensory nerve conduction velocity was normal. These clinical and laboratory findings suggested an atypical case of anterior spinal artery syndrome.

Estrogen receptor-negative and human epidermal growth factor receptor 2-negative breast cancer tissue have the highest Ki-67 labeling index and EGFR expression: gene amplification does not contribute to EGFR expression.

Estrogen receptor (ER) and human epidermal growth factor 2 (HER2) are well-investigated molecules and the focus of many breast cancer therapies. There is a group of breast cancers lacking ER and HER2, but it is not fully understood. Treatment for these patients is limited to cytotoxic chemotherapy. The purpose of present study is to examine ER(-)/HER2(-) breast cancers, with a particular focus on epidermal growth factor receptor (EGFR). EGFR is a target molecule for which novel medicines have been recently developed for other organ cancers, however biological significance in breast cancer is not yet well demonstrated. Breast cancer specimens (n=58) were categorized into four groups: i) ER(+)/HER2(-) (51.7%); ii) ER(+)/HER2(+) (8.6%); iii) ER(-)/HER2(+) (20.7%); and iv) ER(-)/HER2(-) (19.0%). They were immunohistochemically (IHC) examined using antibodies for EGFR, platelet derived growth factor receptor (PDGFR)alpha, PDGFRbeta, parathyroid hormone (PTH) receptor, Ki-67, cyclinD1, p53, and vimentin. The Ki-67 labeling index (LI) was highest in ER(-)/HER2(-) (36.5%), and decreased in order from ER(-)/HER2(+) (31.4%), ER(+)/HER2(+) (17.7%), to (ER(+)/HER2(-) (15.9%) (p=0.001). EGFR, p53 and vimentin were highly expressed in ER(-)/HER2(-) breast cancer cells (p<0.01). CyclinD1 was inversely expressed to Ki-67 LI (p<0.001). Gene amplification of EGFR was examined by two in situ hybridization techniques, fluorescence in situ hybridization (FISH) and chromogenic in situ hybridization (CISH) in serial sections to IHC. Only 1 of 14 EGFR-positive breast cancers showed gene amplification at low levels by CISH. Overall, the ER(-)/HER2(-) breast cancer showed the highest Ki-67 LI, the most frequent expression of EGFR, p53 and vimentin, as well as the lowest expression of cyclinD1. It is unlikely that gene amplification contributes to EGFR expression. ER(-)/HER2(-) breast cancers have potential in the development of novel therapeutics, including targeted medicines.

Forensic discrimination of match heads by elemental analysis with inductively coupled plasma-atomic emission spectrometry.

In arson and bombing cases, matches are often used as the ignition method. We have investigated the use of elemental analysis by inductively coupled plasma-atomic emission spectrometry to discriminate match heads used in arson cases. Six elements, magnesium, aluminum, calcium, iron, zinc, and barium, in match heads were detected after the match heads were dissolved in HNO3, and these elements were quantified in 8 wood stick matches and 5 paper stick matches by means of calibration curves prepared from standard sample solutions. Using this method, we were able to distinguish all the matches from one another both before and after combustion. The method has the potential to be very useful for resolving arson cases.

Clinicopathological correlation between expression of PTHrP receptor and various prognostic factors in breast cancer without axillary lymph node metastasis.

PURPOSE: Expression of PTHrP is immunohistologically found in mammary tissues of all the normal cell, hyperplasia and malignant cell. In patients of breast cancer, positive ratio of PTHrP is reportedly 60-69%. Higher positive ratios of PTHrP are found in breast cancer showing bone metastasis than those without bone metastasis. And, breast cancer patients with PTHrP positive show poor prognosis suggesting a relationship to histological grade. Regarding PTHrP receptor, there is few report on its physiological role and relationship to other prognostic factors. The aim of this study was to determine the correlation between PTHrP receptor and various prognostic parameters of breast cancer patients with no axillary node metastasis. MATERIAL AND METHODS: We employed 26 breast cancer patients, who showed negative axillary node metastasis and did not receive postoperative therapy. We examined relationship between PTHrP receptor and various prognostic factors such as hormone receptors, MIB-1 index, proliferation of cell nuclear antigen (PCNA), DNA ploidy, S-phase fraction, age, tumor diameter, c-erbB-2, and Cathepsin D. RESULTS: No relationship between expression of PTHrP receptor and the cancer recurrence. PTHrP receptor correlated MIB-1 index alone (p < 0.044). CONCLUSIONS: This findings suggest PTHrP receptor is related to the tumor proliferation of breast cancer.

Microglia and macrophages as innate producers of interferon-gamma in the brain following infection with Toxoplasma gondii.

We previously reported the requirement of interferon-gamma (IFN-gamma) expression by cells other than T and natural killer (NK) cells in the brain, in addition to T cells, for prevention of toxoplasmic encephalitis following infection with Toxoplasma gondii. In the present study, we analysed the identity of the IFN-gamma-producing non-T, non-NK cells in the brain using infected athymic nude and SCID mice that lack T cells but express IFN-gamma in their brains. Intracellular staining for IFN-gamma followed by flow cytometry revealed that approximately 45-60% of the cells expressing IFN-gamma in their brains were positive for CD11b or F4/80 on their surfaces. Smaller portions of the cells were positive for pan-NK marker. Further smaller portions were positive for CD11c, and these cells were less than 5% of the IFN-gamma-expressing cells in brains of infected SCID mice. In addition to IFN-gamma proteins, large amounts of mRNA for IFN-gamma were detected in CD11b+ cells purified from brains of infected mice, but it was not the case in the cells obtained from uninfected animals. In infected SCID mice depleted of NK cells by treatment with anti-asialo-GM1 antibody, cells expressing IFN-gamma in their brains were all positive for CD11b, and the IFN-gamma-producing cells were detected in both CD45low and CD45high populations. These results suggest that CD11b+ CD45low microglia and CD11b+ CD45high blood-derived macrophages are the major non-T, non-NK cells which express IFN-gamma in the brain of mice infected with T. gondii.

Neuroprotective effects of ONO-1924H, an inhibitor of poly ADP-ribose polymerase (PARP), on cytotoxicity of PC12 cells and ischemic cerebral damage.

N-[3-(4-Oxo-3,4-dihydro-phthalazin-1-yl)phenyl]-4-(morpholin-4-yl) butanamide methanesulfonate monohydrate (ONO-1924H) is a novel inhibitor of poly ADP-ribose polymerase (PARP). In this study, we examined the effects of ONO-1924H on cytotoxicity induced by hydrogen peroxide in PC12 cells in vitro and cerebral damage and neurological deficits induced by middle cerebral artery (MCA) thrombus occlusion in vivo in rat. In the in vitro cytotoxicity assay, exposure to 0.5 mmol/L hydrogen peroxide induced cell death in differentiated PC12 cells. ONO-1924H, a PARP inhibitor (Ki=0.21 micromol/L), reduced cell death in a concentration-dependent manner that was correlated with inhibition of PARP activation. A 50% reduction in cell death (EC50) was achieved with 2.4 micromol/L ONO-1924H. In the MCA occlusion model, ONO-1924H was injected intravenously at doses of 3, 10 and 30 mg/kg/h for 3 h, and cerebral damage and neurological deficits were estimated 24 h after MCA occlusion. ONO-1924H treatment led to a significant decrease in cerebral damage in the 10 mg/kg/h-treated group (P < 0.05) and the 30 mg/kg/h-treated group (P < 0.01). Further, ONO-1924H at doses of 30 mg/kg/h significantly (P < 0.05) improved neurological deficits. These findings suggest that the novel PARP inhibitor, ONO-1924H, affords effective neuroprotection and is a useful therapeutic candidate for the treatment of ischemic stroke.

Rat cerebral ischemia induced with photochemical occlusion of proximal middle cerebral artery: a stroke model for MR imaging research.

A stroke model in rats with photochemically induced thrombosis (PIT) of proximal cerebral middle artery (MCA) is introduced for magnetic resonance imaging (MRI) study. Thirty-seven rats subjected to surgical and optical procedures for inducing the PIT models were scanned using a 1.5-T scanner with T1-weighted imaging (T1WI), T2-weighted imaging (T2WI), diffusion-weighted imaging (DWI), and contrast-enhanced perfusion-weighted imaging (PWI) at 1 h and 24 h after MCA occlusion. The penumbra evolution and PWI-derived parameters including relative cerebral blood volume (rCBV) and relative cerebral blood flow (rCBF) were monitored; and the relative lesion size (RLS) was compared with the final RLS on the gold standard triphenyl tetrazolium chloride (TTC) staining at 24 h. The results showed that the focal cerebral ischemic lesions were detectable in all rats with different MR approaches. The lesion on PWI at 1 h and on all MR images at 24 h was matched well with that seen on TTC staining; the peri-infarct area decreased from 6.2 +/- 7.2% of the brain volume at 1 h to 0.3 +/- 5.6% at 24 h. Compared to that in the contralateral hemisphere, rCBV in ischemic region was 52.6 +/- 21.4 and 40.0 +/- 15.8% (p > 0.05), and rCBF was 64.6 +/- 11.2 and 47.3 +/- 11.1% (p < 0.05) at 1 h and 24 h respectively. The present PIT model in rats has been successfully adopted for MRI research, which might be feasible for certain stroke studies and should be beneficial for the evaluation on effects of potential diagnostic and therapeutic approaches.

Visualization of stroke with clinical MR imagers in rats: a feasibility study.

This experiment was conducted in compliance with the guidelines of the International Committee on Thrombosis and Hemostasis and the current institutional regulations for use and care of laboratory animals. The purpose of the present study was to report the feasibility of using clinical magnetic resonance (MR) imaging devices for depiction of stroke in a rat model. Twenty-four rats with photochemically induced thrombosis of the middle cerebral artery were examined at superacute (1 hour, n = 24), acute (12 hours, n = 12), and subacute (24 hours, n = 12) phases with 1.5-T MR imaging weighted for T1, T2, diffusion, and gadopentetate dimeglumine-enhanced perfusion. With reasonable signal-to-noise ratio and imaging times, ischemic lesions were well distinguished on MR images as validated qualitatively and quantitatively with postmortem standard-of-reference techniques, including volume-rendered computed tomography, microangiography, and histochemistry. In the superacute phase, the perfusion defect at perfusion-weighted MR imaging was well matched with microangiographic and pathologic findings (P > .05). There was no difference in lesion size at perfusion-weighted MR imaging between superacute and subacute phases (P > .05). Performance of certain stroke-related research in rats is feasible with clinical MR imagers.
