[Acute epidural hematoma clearly demonstrated by fat suppression MR image].

We report a case of acute cervical epidural hematoma clearly demonstrated by fat suppression MR image. A 64-year-old woman was admitted because of quadriparesis with neck pain, both occurred suddenly. She had been healthy except for mild hypertension. No drug was administrated previously. Neurological examination suggested cervical myelopathy. MRI of the cervical spine was performed fifteen hours after the onset. Although no significant intensity change was observed on T1 or T2 image, fat suppression image clearly demonstrated epidural hematoma at the level of C4 to C5 cervical spine. Her symptoms were disappeared spontaneously within seven days. No vascular abnormality was observed by MR angiography. Although MRI is useful to detect epidural hematoma, signal intensity of the hematoma on T1 and T2 image may be unclear within 24 hours, which was shown in our case. We recommend that fat suppression image is helpful to detect epidural hematoma clearly, especially in acute phase.

[Intravascular ultrasound guided polytetrafluoroethylene coronary stent graft implantation in a patient with a large coronary pseudoaneurysm at eight months after coronary intervention: a case report].

A 67-year-old man with exertional angina presented with a large pseudoaneurysm of the right coronary artery at eight months after balloon angioplasty. Intravascular ultrasound revealed a large saccular pseudoaneurysm of 10.9 x 5.7 mm diameter at the proximal segment of the right coronary artery. Complete closure of the coronary pseudoaneurysm was successfully obtained using a coronary stent graft consisting of a thin flexible polytetrafluoroethylene (PTFE) membrane placed between two stents using a unique sandwich technique. Follow-up angiography and intravascular ultrasound revealed no significant restenosis in the PTFE stent graft segment. PTFE coronary stent graft implantation is a feasible, safe and useful method to treat a large coronary pseudoaneurysm after coronary intervention.

[A case of subacute combined degeneration with normal serum vitamin B12 level].

A 40-year-old woman was admitted to our hospital because of pancytopenia with megaloblastic anemia. Two months later she complained of rapidly progressive gait disturbance and numbness in the distal part of limbs. She also told that her hair had turned totally gray in the third decade. Neurologically, mental state, cranial nerves and cerebellar functions were normal. Superficial sensations were impaired below the lower thoracic level and deep sensations were completely lost in the lower limbs. Moderate weakness was found in the lower limbs, symmetrically. Deep tendon reflexes were diminished in the upper limbs and absent in the lower limbs. Babinski's reflex was positive bilaterally. MR images of the spinal cord showed hyperintensity in the posterior column below the thoracic cord. Although the serum level of vitamin B12 was within normal range, serum homocysteine level was elevated markedly. Under the diagnosis of subacute combined degeneration (SCD) due to possible vitamin B12 deficiency, the treatment with intravenous injections of 500 micrograms/day of mecobalamin was undertaken. Muscle strength and sensory impairment improved progressively and she became able to walk with a cane. The coloration of her gray hair was also noted. After treatment, pancytopenia and megaloblastic anemia also markedly improved. Vitamin B12 became high in serum concentration and the serum level of homocysteine became normal. These clinical and laboratory findings support the diagnosis of SCD with normal serum level of vitamin B12 in our case, suggesting that the level of vitamin B12 in serum does not always correlate with that in tissue and, therefore, SCD should not be excluded just only by the reason of normal serum vitamin B12 level.

Association between interleukin-6 gene polymorphism and human T-cell leukemia virus type I associated myelopathy.

We studied cytokine gene polymorphisms in the promoter region, including interleukin (IL)-6, IL-1beta, and IL-10, in Japanese patients with human T-cell leukemia virus type I (HTLV-I) associated myelopathy (HAM) (n = 65), asymptomatic HTLV-I carriers (n = 143), and HTLV-I seronegative, normal controls (n = 160). There was a significant difference between HAM patients and HTLV-I carriers in the distribution of IL-6 promoter polymorphism at position -634 (chi(2) = 9.90, p = 0.0071). The IL-6 genotype was also significantly different between HAM patients and normal controls (chi(2) = 11.53, p = 0.0033), while a similar distribution was observed in IL-1beta and IL-10 polymorphisms among HAM patients, carriers, and normal controls. The results suggest that IL-6 gene region may contribute to susceptibility to HAM, and that aberrant cytokine productions could be involved in the development of HAM.

The constitutive and inducible expression of Nurr1, a key regulator of dopaminergic neuronal differentiation, in human neural and non-neural cell lines.

Nur-related factor 1 (Nurr1), nerve growth factor-induced gene B (NGFI-B) and neuron-derived orphan receptor-1 (NOR-1) constitute the orphan nuclear receptor subfamily of transcription factors. Previous studies showed that midbrain dopaminergic neuronal precursor cells failed to differentiate in Nurr1-deficient mice. To investigate a role of Nurr1 in human neuronal function, Nurr1 mRNA expression was studied in human neural cell lines by RT-PCR and northern blot analysis. Nurr1, NGFI-B and NOR-1 mRNA were coexpressed in all human neural and nonneural cell lines under the serum-containing culture condition, except for SK-N-SH neuroblastoma, in which Nurr1 mRNA was undetectable. The levels of Nurr1, NGFI-B and NOR-1 mRNA were elevated markedly in NTera2 teratocarcinoma-derived neurons (NTera2-N), a model of differentiated human neurons, following a 1.5 or 3 h-exposure to 1 mM dibutyryl cyclic AMP or 100 nm phorbol 12-myristate 13-acetate. NGFI-B mRNA levels were also elevated in NTera2-N cells by exposure to 100 ng/mL brain-derived neurotrophic factor (BDNF). To identify Nurr1-target genes, the mRNA expression of 27 genes potentially involved in dopaminergic neuronal differentiation and survival, including BDNF, glia-derived neurotrophic factor, their receptors, tyrosine hydroxylase and alpha-synuclein, were studied in HEK293 cells following overexpression of Nurr1. None of these genes examined, however, showed significant changes. These results indicate that Nurr1, NGFI-B and NOR-1 mRNA are expressed constitutively in various human neural and non-neural cell lines under the serum-containing culture condition, and their levels are up-regulated in human neurons by activation of protein kinase A or protein kinase C pathway, although putative coactivators expressed in dopaminergic neuronal precursor cells might be required for efficient transcriptional activation of Nurr1-target genes.

[A case of hypertrophic spinal pachymeningitis associated with MPO-ANCA].

A 71-year-old man was admitted to our hospital, because of subacute progressive back pain and thoracic transverse myelopathy. Magnetic resonance imaging showed thickening with gadolinium enhancement of dura mater at the T1-T6 vertebrate levels. A dura biopsy specimen revealed fibrous thickening of the dura with the inflammatory changes, and diagnosis of hypertrophic spinal pachymeningitis was made. The cerebrospinal fluid (CSF) contained high titer of myeloperoxydase anti-neutrophil cytoplasmic antidody (MPO-ANCA), indicating that the antibody synthesized intrathecally. After treatment with oral prednisolone, 60 mg daily for a month, thickness of the dura and the CSF MPO-ANCA improved dramatically. This is the first report of hypertrophic pachymeningitis associated with MPO-ANCA localized exclusively in the spine. This report suggests that intrathecal MPO-ANCA synthesis is an index of disease activity as well as a diagnostic hallmark. Early corticosteroid therapy is recommended in this disorder.

Bilateral anterior cerebral artery territory infarction associated with unilateral hypoplasia of the A1 segment: report of two cases.

We report two cases of bilateral anterior cerebral artery (ACA) territory infarction. On magnetic resonance (MR) angiograms, the A1 segment of the ACA was unilaterally hypoplastic in both cases, suggesting that unilateral hypoplasia of A1 is a significant predisposing factor for this rare type of cerebral infarction. When the contralateral A1 is dominant, embolic materials may enter into it more easily.
